A Gastroesophageal Cyst in the Posterior Mediastinum; A Report of Case (후종격동에 발생한 위식도 낭종)

  • The gastroesophageal cyst is rare variety of benign developmental cysts in the mediastinum and it arises from sequestrations of nodules of forgut in the developing embryo.The patient was 23 year old man with complaint of right chest pain. Simple chest X-ray and chest CT scan showed a huge homogeneous cystic mass in the posterior mediastinum. The resected cystic mass showed combining of portion of esophagus and stomach. The cyst was confirmed as gastroesophageal cyst.

Thymolipoma - One Case Report - (흉선 지방종[1례 보고])

  • The thymolipoma is rather rare benign tumor of the thymus. One case of huge thymolipoma, seen in a 11-year-old boy, is presented. It is about 2.16kg. He had some chest discomfort. The chest film showed homogeneous haziness fills the left hemithorax, On chest CT scan, multiple small amorphous soft tissue densities were recognized as islands within a large fatty mass. Tumor resection was performed through left anterolateral thoracotomy. The mass was yellowish soft, measured 29x19Xllcm, 12X7.5x3.5cm, 7.0X3.0X1.0cm. Microscopically, the tumor was comprised of abundant mature adipose tissue and normal thymic tissue.

  • Sclerosing hemangiomas of lung are benign neoplasms of uncertain histogenesis. They have variegated histologic appearance characterized by an admixture of solid, hemorrhagic, papillary and sclerotic lesions. We have experienced a case of sclerosing hemangioma of lung recently. She was 43 year-old woman and suffered only from mild vague chest pain. Well circumscribed round mass was placed at the right hilum. Mass enucleation was done and she was recovered and discharged with event free.

Leiomyoma of the Trachea (기도평활근종 치험 1례)

  • Leiomyoma is extremely rare benign tumor of the trachea. A case of leiomyoma of the trachea is described in a 36-year-old man. The patient had a brief history of asthma-like symptoms. Computed tomography of the chest revealed the ovoid lower trachea mass, which was occupying about 80% of the tracheal lumen. At fiberoptic bronchoscopy highly vascularized round mass was seen on the membranous portion of the lower trachea. The tumor was excised by segmental sleeve resection and end-to-end anastomosis of the trachea was performed. Postoperative course was uneventful and bronchoscopic findings showed clear healing of the anastomotic site.

A Large Schwannoma of the External Auditory Canal with Adjacent Bony Erosion

  • Schwannomas originating from the external auditory canal are relatively uncommon whereas they are commonly found in head and neck region. The authors recently experienced a 18-year-old male with a large mass in his right side external auditory canal. The mass was reported as a schwannoma and after the local excision, no symptom or sign of recurrence was identified. We report the case with literatures.

  • Renal inflammatory pseudotumor is a very rare benign condition of unknown etiology characterized by proliferative myofibroblasts, fibroblasts, histiocytes, and plasma cells. In the case we report, the lesion appeared on contrast-enhanced power Doppler US images as a well-defined hypoechoic mass with intratumoral vascularity, and on CT as a low-attenuated mass. Differentiation from malignant renal neoplasms was not possible.

  • Cystic lymphangioma is an uncommon benign developmental tumor of the lymphatic system, seen rarely in adults. A 23-year-old man visited DKUDH complaining of the painless swelling in the right submandibular area. It was reported that swelling had increased since 5 months ago. Clinically, fltriangle uctuant and mobile mass was palpated at the right submandibular area. CT scan showed a large, well-defined, homogenous low density mass, measuring 5.0 × 2.5 × 4.0 cm. T1W MRI scan demonstrated a large, multiseptated homogenous low signal mass with septum and rim enhancement. Proton-density and T2W MRI scan showed mass of high signal intensity. MRI scan was able to delineate better the enhanced multi septation and extent of the lesion. Histopathologically, a lobulated cystic mass lined by a layer of flattened endothelium was observed.

  • Tongue tumors of pediatric patients are the most common soft tissue tumor of pediatric oral and maxillofacial tumors. There are many kind of pediatric tongue lesions such as hemangioma, lymphangioma, papilloma, and cyst. Most of these lesions are benign, but malignant tumors of tongue may be occurred. Therefore, malignancy should be ruled out. Sometimes, tongue lesions are present with dyspnea, dysphagia, dysarthria, bleeding, or cosmetic problem. We experienced a case of chronic inflammatory mass on posterior 1/3 of the tongue in 4 month old female patient. Swallowing difficulty and respiratory distress symptom occurred because of tongue mass effect. The tongue mass was confirmed as granulation tissue by microscopic examination. After excision of tongue mass, she had no problem with swallowing and breathing.

  • A 52-year-old male complained of a painless, firm, and slow-growing mass in his right breast outer portion. The chest CT revealed a 3.3 cm-sized oval shaped, microlobulated, mild enhancing mass. Ultrasound showed a microlobulated marginated heterogeneous hypoechoic mass with internal vascularity and calcifications in the mass. On the ultrasound-guided core needle biopsy, the mass was confirmed as a benign granular cell tumor (GCT). The patient transferred to another hospital and underwent surgical removal of the lesion. GCT of the breast is uncommon and mostly benign neoplasm to originate from Schwann cell. Clinical and radiologic features of GCTs, including CT and ultrasound images, mimic malignancy and make diagnosis of GCT more difficult. The CT images of GCTs are much rarely reported. Physicians and radiologists must be aware of radiologic characteristics of this rare benign tumor for male breast, to avoid misdiagnosis this tumor for breast malignancy and overtreat.

Is $^{99m}Tc$-MDP Mammoscintigraphy Useful in Patients with Breast Mass Lesions? ($^{99m}Tc$-MDP 유방 스캔은 유방 종괴의 감별진단에 임상적인 유용성이 있는가?)

  • Purpose: Piccolo et al. advocated the usefulness of $^{99m}Tc$-MDP breast scan in differential diagnosis of breast mass with high accuracy. But there are little or no follow-up studies about it as we know. We studied $^{99m}Tc$-MDP uptake pattern and lesion/background ratio in patients complaining palpable breast lesions to evaluate the clinical usefulness of $^{99m}Tc$-MDP scan. Materials and Methods: Total 34 patients were studied with physical examination, mammo-gram and $^{99m}Tc$-MDP scan prospectively. Anteroposterior and both lateral view of breast were obtained 5 minutes after iv injection of 740 MBq $^{99m}Tc$-MDP. Breast uptake pattern of $^{99m}Tc$-MDP was analyzed by a grade system: 0=no uptake, grade 1=bilateral diffuse uptake, grade 2=asymmetric faint uptake, grade 3=focal hot uptake. 20 cases were pathologically confirmed by excision biopsy or aspiration biopsy. 14 cases were normal in physical examination and mammogram. Results: Pathologic results showed 7 carcinomas, 6 benign solid tumors, and 7 fibrocystic changes. Grade 3 pattern of $^{99m}Tc$-MDP uptake was noted in 4/7 carcinomas, 3/6 benign solid tumors, and 1/7 fibrocystic changes. Grade 2 pattern was 217, 0/7, 3/7 respectively. The average L/B ratio was 1.66 in carcinomas, 1.68 in benign solid masses, 1.20 in fibrocystic diseases, 1.05 in normal patients. L/B ratio was higher in carcinoma and benign mass groups than in fibrocystic change and normal control groups(p=0.005). But there was no statistical difference between L/B ratio of malignant mass group and benign mass group. Conclusion: $^{99m}Tc$-MDP scan is not suitable to routine clinical use for breast mass diagnosis. It might be used in limited conditions when whole body bone scan is planned.
