Purpose: The purpose of this study was to evaluate the effect of hyperbaric oxygen therapy(HBOT) and the resultant amputation rate in the treatment of diabetic foot ulcer. Material and methods: From 2000 Jan. to 2002 April, thirty two diabetic foot patients were admitted for treatment of foot ulcers and infection. Of the thirty two cases, thirteen patients received HBOT and nineteen did not. The results were analyzed by amputation rate and healing time. All were classified according to the Wagner classification. Results: Of the HBOT treated group, three(23%) patients underwent amputation: two below knee and one metatarsophalangeal disarticulations. Of the non-treated group, eleven(58%) patients underwent amputation, : four below knee and seven metatarsophalangeal disarticulations. The healing times, based on hospital days were average 16, 38, 43 days in the HBOT treated group and average 20, 50, 35 days in the non treated group respectively in Wagner grade II, III, IV. Conclusions: HBOT might be effective in decreasing amputation rate and hospital stay in diabetic patients with severe foot ulcers.
하지동맥 질환의 유병률은 고연령 군에서 증가한다. 최근의 기술적 진보로 혈관 내 치료가 점점 더 많은 빈도로 시행되고 있다. 간헐적 파행의 치료 목표는 보행 능력을 향상시키고 증상을 완화시키는 것이다. 이를 위해 해부학적 내구성을 높이는 것이 중요한 전략이며 개통률이 치료 평가 기준이 된다. 임계 하지허혈을 가진 환자에서는 병변이 광범위하고 특히 무릎아래동맥을 심하게 침범한다. 임계 하지허혈의 치료 목적은 상처 회복을 촉진하고 주요 절단을 예방하는데 있으며 사지 보존율이 평가의 기준이 된다. 장골동맥 협착의 치료에는 피복 혹은 비피복 스텐트 삽입술이 일차적 치료로 인정된다. 대퇴슬와동맥 질환은 죽종제거술과 함께 약물방출풍선 및 스텐트가 자주 사용되는 반면 무릎아래동맥 질환에서는 풍선확장술이 주요 치료 방법이다. 컴퓨터단층 혈관조영술은 절대적 금기증이 없는 환자에서 혈관 내 치료계획 수립을 하는데 로드맵을 제공한다.
Purpose: To evaluate the effectiveness of percutaneous transluminal angioplasty (PTA) below the knee as a treatment in diabetic foot gangrene. Materials and Methods: Between May 2003 and May 2006, angiography was performed in 35 diabetic foot gangrene classified as either Wagner grade IV or V. Infrapopliteal PTA was performed in 10 patients among them. Clinical success was defined as prevention of major amputation. Results: Among 25 patients who did not receive infrapopliteal PTA, the major amputation rate is 22% (in one arterial occlusion cases), 50% (in two arterial occlusion cases), 63% (in three arterial occlusion cases), respectively. Infrapopliteal PTA was successfully performed in 8 among 10 patients. Two patients were failed and undergone below-knee amputation. Toe amputation were performed in 2 patients with one arterial occlusion. Out of 6 patients with three arterial occlusions, toe amputations were performed in 4 patients and the other 2 patients were healed through debridement. Conclusion: As a first choice revascularization procedure for limb salvage in diabetic foot gangrene, infrapopliteal PTA can be one of treatment options.
This research examines the attributes of the Korean costume culture inherent in the expression of "-like" by focusing on the professional roles implied in the style of clothes. Based on a random sampling, the sample was comprised of 315 male and female college students who reside in Seoul and Gyeonggi area. The results were as followed: 1. Subjects preferred the blouse as the most "female student-like" upper wear. The female students recognized the Y-shirts as the "male teacher-like" upper wear while the male picked the Y-shirts for this category. For the "student-like" sleeve length, the female selected the cap sleeves and the three-quarter sleeves for the female and selected the short sleeves for the male. Subjects preferred the Peter Pan and the sailor collars for the "female student-like" category and selected the shirt collars as the most "male teacher-like" and "male student-like" style. 2 Subjects perceived the A-line and the flared skirts as the "female student-like" dresses. For the lengths of skirts, subjects agreed that the above-knee. the below-knee and the midi length were all proper for female teachers. For the length of pants, subjects selected the 9/10 as the most "female teacher-like" style. 3. Subjects selected black, white, light purple and beige as the preferred colors for female teachers. Subjects chose white and yellow as the most "female student-like" colors and picked the colors, navy blue, light blue and blue, as the "male-like" and "male teacher-like" colors.
This study was designed to apply the stimulation system developed in our laboratory to investigate how the stimulation conditions affect the muscle contractile characteristics in the isometric condition as well as during the FES standing/walking. Four paraplegic and ten healthy subjects participated in this study, and their knee extensors were voluntary contracted or electrically stimulated to measure the muscle force and the fatigue index for different waveforms of the pulse train. We also investigated different combinations of the electrode positions during standing/walking. It was confirmed that continuous and high-frequency stimulation causes faster fatigue than intermittent and low-frequency stimulation. Fatigue resistance was higher around the optimal muscle length than at a stretched position in healthy subjects, whereas the opposite was observed in paralyzed subjects. The paired t-test results with the level of significance at 0.01 indicated that the sinusoidal waveform generated the largest torque among the four typical waveforms. Although statistically not very significant, the sinusoidal waveform also generated, in general, the highest fatigue resistance at an intensity level below the supramaximal stimulation. One of the paraplegic subject who participated in the standing/walking program can now stand up for 1 minute and 50 seconds with the knee extensors, and walk for about 5 minutes at the speed of 12m/sec.
Unlike production line works, construction work is non-repetitive in nature. In this study, a checklist was developed to characterize the ergonomic hazards of construction work. Eight existing checklists including OWAS, RULA, and REBA were integrated, and a new version was designed to include more postures of lower extremities. The analysis results obtained using the new checklist showed that manual materials handling (MMH) is the most common risk factor. About 62% of the MMH was done below the knee level or above the elbow level, and 22% was carried out with the trunk twisted more than $60^{\circ}$. The next risk factors were the works done with the knee bent or squatted, or with the shoulders abducted. It can be concluded that the new checklist is successful as a quick and easy tool for screening risk factors of construction works, although it is not capable of determining action levels. Further studies on validation of the checklist are expected.
목적: KT-2000 Knee Ligament Arthrometer를 이용하여 정상 슬관절의 전방 전위를 측정하고, 결과에 영향을 미치는 인자들을 알아보며, 전방 십자 인대 손상 환자 슬관절의 전방 전위를 측정하여 이를 바탕으로 정상인의 전방전위 정도를 전방 십자 인대 손상 환자와 비교하여 이 기기의 유용성을 알아 보고자 한다. 대상 및 방법: 슬관절 손상의 과거력이 없는 성인 남자 30명을 대상으로 2명의 검사자가 30lb의 힘으로 슬관절30$^{\circ}$굴곡에서 근육 이완 및 수축, 25$^{\circ}$내회전, 25$^{\circ}$외회전 자세로 전방 전위를 측정하여 두 검사자간의 전방 전위 및 좌우 차이를 비교하였고. 신체 조건에 따라 두 군으로 나누어 각각의 차이에 따르는 전방 전위를 비교하였다 전방십자 인대 손상 환자 30명에서 슬관절을 이완시킨 상태로 3차례 이상 전방 전위 검사를 시행하였다. 결과: 검사자 1의 슬관절 전방 전위는 30$^{\circ}$굴곡 상태로 근육 완전 이완, 근육 수축 상태. 25$^{\circ}$내회전 25$^{\circ}$외회전위치에서 우측 슬관절은 6.5${\pm}$1.5 mm. 2.5${\pm}$0.9 mm, 4.8${\pm}$1.2 mm, 6.4${\pm}$1.3 mm였고 좌측 슬관절은 5.6${\pm}$1.3 mm, 2.1${\pm}$0.8 mm, 4.5${\pm}$1.2 mm, 5.2${\pm}$1.3 mm였으며. 검사자 2는 각각 6.9${\pm}$1.2 mm, 2.9${\pm}$1.1 mm,5.6${\pm}$1.6 mm, 6.9${\pm}$1.5 mm였고, 5.5${\pm}$1.7 mm, 1.9${\pm}$0.9 mm, 5.1${\pm}$1 9 mm, 5.7${\pm}$1.6 mm였으며, 검사자1의 전방 전위 좌우 차는 근육 이완 상태에서 0.9${\pm}$1.0 mm였다. 전방 십자 인대 손상으로 수술을 받은 환자의 전방 전위는 평균 11${\pm}$2.93 mm쳐고, 건측과의 차이는 평균 6.5${\pm}$2.31 mm였다 좌우 슬관절의 비교에서 검사자 1과 2는 30${\pm}$굴곡, 근육 이완시 통계학적으로 좌우차는 있었으나 각각 25례(83%), 21례(70%)에서 좌우 차이가 2mm미만이었고 3 mm이상은 모두 1예에 불과하였으며 근육 이완시 정상인과 전방 십자 인대 파열 환자의 전방 전위 좌우차는 통계학적으로 의미 있는 차이를 보였다. (<0.05) 결론: KT-2000 Knee Ligament Arthrometed의 결과에 영향을 주는 인자는 슬관절 주변 근육의 이완, 슬관절의 굴곡 각도, 경골의 회전. 전위력의 강도, 적용시점, 그리고 키, 체중등의 신체적 요인 등이 있으나 능숙한 기계사용과 정확한 슬관절 위치에서 검사할 때 전방 십자 인대 파열에 대한 진단에 유용한 기구이다.
Objective: The purpose of this study was to access quality of life and evaluate pre and post surgery management for persons with lower limb amputations. Method: This study was designed as a telephone survey. The subjects were patients who underwent their lower limb amputation between January 1994, and February 2005 at Asan Medical Center in seoul. sixty one of 203 subjects had granted consent and were studied. Results: The major cause of below knee amputations was vascular disease. Traumatic injuries were more common among subjects with above knee amputations. 93.4% of subjects were in use of prosthesis and 70.1 days were required to fit the prosthesis. 68.4% of subjects were able to ambulate with single point cane or without assistive devices. 80.3% of subjects complained phantom pain. Most subjects expressed the unsatisfactory result regarding the functional usage of prosthesis and education from hospital. Conclusion: Pre and post surgery rehabilitation program will benefit to promote better functional status and quality of life for persons with lower limb amputations.
This study analyzed the lower body type of 30's Korean men to develop a slim-fit pants pattern. As the analysis data, direct measurement data of anthropometric measured value in the 6th Size Korea(KATS, 2010) was used as basic data with 481 men in 30's as analysis objects. The result are as follows. First, the result of analyzing factors for the lower body type classification indicated five factors. Second, the result of executing group analysis (with the independent variable of 5 clusters extracted from the factor analysis)classified the following three types. Type 1(36.8%) displayed a medium height of lower body type, small waist and hip, slim and fit body type with a slim shape between the knee and ankle. The shape between the waist and hip had characteristics of a slight curve and short length. Type 2(35.6%) displayed lowest height of a lower body type that was large and thick between the waist and the hip. The drop value of the waist and the hip was small; therefore, the body type was flat with a minimal curve. The underpart type (below the knee) was the thickest and the length was short. Type 3(27.7%) displayed the highest lower body type, a medium level waist size, flat and narrow waist and belly. This body type had a curve with big drop value of the waist and the hip, lower part from the hip to the ankle (including the knee) and a thick calf with along leg.
In this study, new design method of prosthetic foot was suggested which can evaluate the performance of prosthetic foot by implementing amputee's gait simulation using the finite element analysis. The basic shape of ESPF(Energy Storing Prosthetic Foot) was designed which is suitable for the below-knee amputee considering mechanical properties and kinematic properties. And, the performance evaluations were performed using the Taguchi method with orthogonal array L25. As a result, average main effect of factors for the ESPF's performance were calculated and then optimum condition of given shape was selected. Essential particulars for the performance evaluation from the simulation result were the quantity of external work needed in stance phase, the quantity of transferred energy from the ESPF through the knee, and the vertical displacement of knee at toe-off. Reasonable optimum condition was obtained from the using performance index. From this study, it was found that it is necessary for the design of ESPF to consider the geometrical data related to the magnitude of load on elastic material.
