The Clinical study of Su-Gi therapy's Effects on Bell's palsy by observing of Y-system (Y-system으로 관찰한 Bell's palsy에 미치는 수기요법의 영향에 관한 임상적 연구)

  • Objects : The purpose of this study is to investigate the effect of Su-Gi therapy for Bell's palsy by using Y-system(Yanagihara's unweighted grading system). Methods : We investigated 25 patients with Bell's palsy who had visited in the M, H Korean medicine hospital in Gyeonggi Province from December 27th, 2010 to April 8th, 2015. The Su-Gi therapy was done by 1 times daily. And each patients had been treated more than 25 days. We evaluated the change of them by using Y-system. Results : 1. We investigated 25 patients with Bell's palsy. 80% of the patients were females(20 patients), 20% of the patients were male(5 patients). The average age of patients was 47±15.15. The average period of Adm. treatment was 64% of the patients(16 patients) have left facial palsy, 36% of the patients(9 patients) have right facial palsy. 2. The mean Y-system score before treatment was 17.80±6.2, and the mean score after 25th days treatment was 33.68±4.0. Changes in the mean Y-system Score for each case according to the treatment days was increased significantly. 3. The mean Y-system score after 4th days treatment had increased by an 1.24±2.7 as compared to before treatment, the score after 10th days treatment was 8.7±4.7, the score after 14th days treatment was 11.84±5.8, the score after 20th days treatment is 14.72±6.7, and the score after 25th days treatment was 15.88±6.9. Every score was significantly increased. Conclusions: 1. Su-Gi therapy can be defined that is mainly using the hands to touch or movement of the human body skin, meridians and acupuncture points, muscles, joints and so on. And that is the treatment to communicate and harmonize to the meridians and acupuncture points, and to prevent of diseases, and to keep health. 2. All names of An-Gyo, An-Ma, Jum-Hyul, Chu-Na, massage, and so on should be referred to as Su-Gi therapy. And that individual names are to be classified and separated as the type or method of Su-Gi therapy. 3. The results of the treatment of Hwidam's Su-Gi therapy for Bell's palsy by using Y-system were significant.

Clinical Studies on Obesity and Right-left of Patients with Bell's palsy (구안와사(口眼喎斜)의 비수(肥瘦)와 좌우(左右)에 관한 임상적 고찰)

  • This study was desiged to investigate the Obesity and Right-left(region) of Patients with Bell's palsy. We measured the sex, age, BMI and pulse diagnosis of 149 patients who were diagnosed as Bell's palsy. The results were as follows : In distribution of sex, the ratio of male was 52.35%(78 cases), female 47.65%(71 cases). The distribution of age revealed that 40s was the most in 50 cases(33.6%). The distribution of region in facial palsy was left 73 cases, right 76 cases(1:1.04). In distribution of region in facial palsy patients with obesity, the ratio of left was 32.86%(49 cases), right 34.23%(51 cases). But facial palsy patients with obesity was the most in 100 cases(67.11%), low weght was 3 cases(2.01%). In distribution of pulse diagnosis in facial palsy patients with obesity, the ratio of huh-mac(虛脈) was 63.64%(42 case), sil-mac(實脈) 36.36%(24 cases). The huh-mac(虛脈) was simlliar to gi-huh(氣虛). So we found that the facial palsy patients with obesity was more gi-huh(氣虛) than with low weght. In distribution of region in facial palsy patients with obesity-huh-mac(虛脈), the ratio of left was 41.38%(12 cases), right 58.62%(17 cases).

Clinical Studies on the General Features and the Obesity-Skinniness of Patients with Bell's Palsy (구안괘사(口眼喎斜)환자의 일반적 특성 및 비수(肥瘦)에 따른 임상적 고찰)

  • Objective : This study was aimed to investigate the general features and differences between obesity and skinniness of patients with Bell's palsy. Methods : We measured the sex, age. BMI. pulse diagnosis and HBGS (House-Brackmann Grading System) of 234 patients who were diagnosed with Bell's palsy. Results and Conclusions : The results with statistical significance were as follows (1) The distribution of age revealed that 40s was the most at 30.8 %: (2) The improvement period in facial palsy patients with sub-paralysis was shorter than whole-paralysis. And in one part the more we treated, the shorter the improvement period was: (3) In distribution of fat rate in facial palsy patients, obesity was the most at 61.37%, low weight 15.88%. So we found that the fatter the patients was. the higher the onset rate was: (4) In distribution of pulse diagnosis in facial palsy patients with obesity. the ratio of Xu mai (虛脈) was 67.06%. Shi mai (實脈) 32.94%. The Xu mai was similar to Qi xu (氣虛). So we found that the facial palsy patients with obesity were more Qi xu than with low weight. In distribution of pulse diagnosis in facial palsy patients with skinniness, the ratio of Chi mai (遲脈) was none. Shuo mai (數脈) was most: (5) In distribution of region in facial palsy patients with obesity-Xu mai. the ratio of left was 45.10%, right 54.90%, but this result was not statistically significant.

The Clinical Analysis of Recurrent Bell's Palsy (재발 벨마비의 임상 분석)

  • Background: Idiopathic facial nerve palsy, or Bell's palsy (BP), is a common and important disease. Recurrent Bell's palsy has been known as a rare entity with only a few cases in the literature. Methods: A total of 111 consecutive patients with acute BP patients were enrolled at Daegu Catholic University Hospital from July 2005 to March 2007. We classified the patients into two groups - single BP and recurrent BP - and compared them by demographic data, clinical features, MRI findings and prognosis. The degree of BP was graded according to the House and Brackmann facial nerve grading system. Results: Recurrent BP was observed in 10 (9%) patients. The number of recurrence was varied from 2 to 5. The mean age of first attack in recurrent BP was $35.70{\pm}23.65$ years old and was earlier than that of the single BP ($50.94{\pm}16.21$ year). The larger proportion of the single BP had an abnormal enhancement of affected facial nerve (91.3%) than the recurrent BP (50%). The recurrent BP showed worse prognosis than the single BP. The associated conditions, etiology, and clinical features were similar between two groups. Conclusions: In comparison with single BP, recurrent BP showed earlier onset of first BP attack, less frequent abnormal enhancement of facial nerve on MRI, and worse prognosis.

  • The clinical data and thermographic imaging were analyzed on the 38 cases of Bell's palsy who were treated admission in the Oriental Medicine Hospital of Wonkwang University from January 2002 to May 2003. 38 patients with Bell's palsy were within one week after the onset of the paralysis, and thermal type in the DITI were hypo or hyper generally. Nerve conduction test(ENOG and EMG) examined in two weeks after onset. We studied interaction effect between thermal type and paralysis grade on admission day. We studied each main effect ; paralysis grade on admission day - nerve conduction test, nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, paralysis grade on admission day - paralysis grade after 4 weeks, sasang constitution - nerve conduction test. The following results were obtained that interaction effect between thermal type and paralysis grade on admission day showed no significance, each main effect ; paralysis grade on admission day - nerve conduction test, paralysis grade on admission day - paralysis grade after 4 weeks, showed significance, each main effect ; nerve conduction test - thermal type, thermal type - paralysis grade after 4 weeks, sasang constitution - nerve conduction test, showed no significance.

Treatment of Bell's Palsy by Stellate Ganglion Block (안면신경마비에 대한 성상교감신경절 차단요법)

  • Bell's palsy is not a serious disease and recovery is spontaneous in more half of the patients, but this recovery is long delayed in many instances and the cosmetic effect of an altered facial appearance is psychologically distressing. The etiology is unknown, but Kettle's ischemic hypothesis has been widely accepted. The aim of treatment is to reduce edema and improve circulation to the facial nerve. Stellate ganglion block (SGB) resulted in abolishing cerebral vascular spasm and in increasing cerebral blood flow. Thus, stellate ganglion block is effective in treatment of Bell's palsy. From 1978 to Oct. 1988, we have treated 222 patients (20 patients were recurred cases) with facial palsy by SGB and analysed the effect of SGB. We noticed that SGB was very effective in early repetitive treatment (90% recovery). This study is reported and the literature is reviewed.

Incidence, clinical features and prognosis of Bell's palsy in children (소아에서 벨 마비의 발생빈도, 임상적 특징 및 예후에 대한 연구)

  • Purpose : Bell's palsy is defined as an idiopathic facial nerve paralysis of sudden onset. In spite of intensive clinical and experimental investigation, there is still uncertainty in the incidence, etiology, and preferred mode of treatment in children. The objective of this study was to analyze clinical outcome and prognosis of children with Bell' palsy. Methods : We analyzed 61 cases of Bell's palsy diagnosed at the Department of Wonkwang University Hospital from January 1998 to July 2006. The inclusion criteria were any children with acute isolated unilateral lower motor neuron type of facial nerve palsy. The clinical findings and investigations were reviewed including age, sex, affected site, seasonal incidence and result of steroid treatment. Chi-square and Fisher's exact test was used to compare clinical outcome between duration of complete recovery and age. Results : There was no difference in incidence according to sex or age. Incidence was higher in summer and winter. There was no difference in complete recovery rate and duration between steroid treated group and control group. In the group of children younger than 6 years, duration of complete recovery was shorter than older children. Conclusion : We found increasing the incidence of Bell's palsy in summer and winter. Children younger than 6 years had shorter duration in complete recovery.

Clinical comparison studies on 30 cases of Bell's palsy patients with posterior ear pain by Sa-Am acupuncture Sojangjeonggyeok(小腸正格) & General acupuncture (이후통(耳後痛)을 동반한 Bell's palsy 환자 30례에 대한 소장정격(小腸正格)과 체침(體鍼)의 임상적 비교연구)

  • Objective : The study was designed to evaluate the effects of treatment of Bell's palsy patients with posterior ear pain by the Sa-Am acupuncture. Methods : From December 5th 2003 to May 22th 2004, the clinical comparison studies were carried out 30 cases of Bell's palsy patients with posterior ear pain treated by Sa-am acupuncture Sojangjeonggyeok(Group 1) & General acupuncture(Group 2), who had been treated in Dept. of acupuncture and moxibustion, Oriental Medical Hospital, Dae-Jeon University. Results : The result obtained as follows; 1. There was no significant difference between the two groups in the degree of improvement by H-B grade(p>0.05). 2. Group 1 is more effective than Group 2 in the VAS score after 5, 10 days of each treatment about mastoid pain($p{\leq}0.05$). Conclusion : In this study, although Sa-am acupuncture Sojangjeonggyeok was effective treatment of the posterior ear pain, there was no significant difference between the Group 1 and Group 2 in the degree of improvement of facial palsy. So further research is needed continuously.

Comparison studies on 20 cases of Bell's palsy patients by acupunture and Rainbow therapy & acupunture (특발성 안면신경 마비 환자 20례에 대한 체침과 체침및 Rainbow therapy병행치료의 비교연구)

  • Objective : We suggested the clinical effect of Rainbow therapy on Bell's palsy. Methods : 20 Bell's palsy patients were divided into two groups. One group(A group) was treated by acupunture and the other group(B group) was treated by acupunture and Rainbow therapy. The effect of these treatments was evaluated by Yanagihara's unweighted grading system and House-Brackmann grading system. Results and Conclusions : In Yanagihara's unweighted grading system After 1 week and 2weeks treatment, group B marked more higher than group A in treatment outcome. We discovered that it is significant differences between two groups. After 3 weeks treatment, group B marked more higher than group A in treatment outcome but it is not significant differences between two groups. In House-Brackmann's facial nerve grading system, After 1 week treatment, group B marked more higher than group A in treatment outcome. We discovered that it is significant differences between two groups. After 2 weeks and 3 weeks treatment, group A marked more higher than group A in treatment outcome but it is not significant differences between two groups.

A Clinical Study of Oriental-Western Medicine Treatment on Facial Nerve Paralysis (말초성 안면신경마비의 한양방 협진치료에 관한 임상적 연구)

  • Objectives : Facial palsy is not uncommon disease and most patients with facial palsy are peripheral type, as in Bell' s palsy. In western medicine, oral steroid is the mainstay of the treatment. Recently, oriental-western treatment became one of the alternative modality for the treatment of the facial palsy. However, the treatment result and the degree of patient' s satisfaction were not evaluated. In this study, we tried to characterize the clinical characteristics, short-term recovery rate and degree of patient' s satisfaction after oriental-western medicine treatment on facial palsy of peripheral type. Methods : Between May 2008 and December 2008, we examined 16 patients who presented with facial palsy of peripheral type in Dongguk University Medical Center. Clinical characteristics and recovery rate was analyzed by retrospective chart review. The degree of patient' s satisfaction was measured by 5 point scale. Results : The causes of facial palsy were Bell' s palsy (87.5%) and the Ramsay-Hunt syndrome (12.5%). The highest age groups of facial palsy were 6th and 7th decades. Most frequent accompanying symptom was postauricular pain. After oriental-western medicine treatment, 10 patients (62.5%) showed recovery of facial palsy better than House-Brackmann grade 2. 11 patients (68.8%) were satisfied with the oriental-western medicine treatment. Conclusions : Considering the degree of patient' s satisfaction and treatment result, we believe that oriental-western treatment could be safe and reliable protocol for the treatment of facial nerve palsy of peripheral type.
