사회인지이론에 근거한 비대면 진료서비스 수용의 영향 요인 (A Study of Factors Influencing the Acceptance of non-Face-to-Face Treatment Based on Social Cognitive Theory)

  • Purpose: This study aimed to elucidate the intention to use non-face-to-face treatment, which was temporarily allowed during the coronavirus disease 2019 pandemic. Based on the social cognitive theory, individual behavioral changes occur through the dynamic interaction of individual, environmental, and behavioral factors. Thus, we investigated the impact of personal, environmental, and behavioral factors on the acceptance of non-face-to-face treatment. Methods: A Web survey was conducted using Korea Research Panel between December 26 and 29, 2022, to examine the conceptual framework. The survey targeted adults aged 19 and older, regardless of whether they had used non-face-to-face treatment. A total of 502 responses were collected. Further, a three-step hierarchical regression analysis was conducted using SPSS Windows software version 25.0. Results: The study showed that 131 out of 502 respondents had experience using non-face-to-face treatment, while 371 did not. The factors that influenced the intention to accept non-face-to-face treatment included the general characteristics of the participants (women, underlying disease), personal factors (usefulness, cost savings, knowledge), and environmental factors (social norms, trust, perceived risk). The model demonstrated an explanatory power of 65%. Conclusion: The results of this study directly show that intention is linked to behavior through the interaction between personal and environmental factors. Further research is needed to explore additional factors influencing the intention to accept non-face-to-face treatment, enabling its effective use in preventing and treating various diseases, including infectious diseases.

  • Prevalence of pediatric obesity has increased worldwide in the last 20 years. Obese children suffer not only physical complications but also mental health problems such as depression, attention deficit hyperactivity disorder (ADHD), and eating disorders, as well as psychosocial impairments, such as school adjustment problems, bullying, and low self-esteem. Recently, there have been some studies on the association of mental health problems and pediatric obesity. In the treatment of pediatric obesity, many previous studies suggest multidisciplinary treatment. However, cognitive behavioral therapy (CBT) has attracted attention because obese children are accompanied by body image distortion, emotion dysregulation, and difficulties in stimulus control. This review is a narrative summary of the recent studies on mental health problems and CBT in pediatric obesity. The relationship between depression/anxiety and pediatric obesity is still inconsistent but recent studies have revealed a bidirectional relation between depression and obesity. Additionally, some studies suggest that obese children may have eating disorder symptoms, like loss of control eating, and require therapeutic intervention for pediatric obesity treatment. Furthermore, impulsivity and inattention of ADHD symptom is thought to increase the risk of obesity. It has also been suggested that CBT can be very effective for mental health problems such as depression, impulsivity, and body image distortion, that may coexist with pediatric obesity, and use of multimedia and application can be useful in CBT.

Influence of clozapine on neurodevelopmental protein expression and behavioral patterns in animal model of psychiatric disorder induced by low-level of lead

  • Exposure to lead during pregnancy is a risk factor for the development of psychiatric disorders in the offspring. In this study, we investigated whether exposure to low levels of lead acetate (0.2%) in drinking water during pregnancy and lactation causes behavioral impairment and affects the expression of proteins associated with neurodevelopment. Lead exposure altered several parameters in rat offspring compared with those unexposed in open-field, social interaction, and pre-pulse inhibition tests. These parameters were restored to normal levels after clozapine treatment. Western blot and immunohistochemical analyses of the hippocampus revealed that several neurodevelopmental proteins were downregulated in lead-exposed rats. The expression was normalized after clozapine treatment (5 mg/kg/day, postnatal day 35-56). These findings demonstrate that downregulation of several proteins in lead-exposed rats affected subsequent behavioral changes. Our results suggest that lead exposure in early life may induce psychiatric disorders and treatment with antipsychotics such as clozapine may reduce their incidence.

알츠하이머 치매 의심 환자의 행동심리증상에 대한 복합한의치료 1례: 증례 보고 (Application of Complex Korean Medicine Therapy to a Patient with Behavioral and Psychological Symptoms of Dementia from Suspected Alzheimer's Disease: Case Report)

  • Objectives: The purpose of this study was to report the clinical effectiveness of the complex Korean medicine therapy on a patient with BPSD (Behavioral and Psychological Symptoms of Dementia) due to suspected Alzheimer's disease. Methods: We experienced an age 73 female who complained of a depressive mood, insomnia, and accompanying symptoms such as abnormal behavior. During treatment, the patient received acupuncture and herbal medicine. The clinical effects were evaluated through KGDS (Korean Form of Geriatric Depression Scale) and subjective statements of the patient. Results: After the complex Korean medicine therapy, the treatments ameliorated overall symptoms that the patient complained about. The patient was more positive and slept better after the treatment. No specificities or adverse effects were reported during treatment. Conclusions: This study suggests the possibility of treating patients with BPSD with complex Korean medicine therapy.

Behavioral Sensitization and M1 Muscarinic Acetylcholine Receptor mRNA Expression in Methamphetamine-Administered Mice

  • Repeated administration of psychostimulants such as amphetamines increases locomotor activity in rodents. These drugs, including methamphetamine, enhance dopaminergic neurotransmission and result in hyper-locomotion and behavioral sensitization. It is well known that the existence of a complex balance between the cholinergic and dopaminergic systems in the central nervous system. Thus, behavioral sensitization by methamphetamine may be related to the expression of the M1 muscarinic acetylcholine receptors gene. The present study investigated the changes of M1R mRNA in hyperlocomotor activity and behavioral sensitization by methamphetamine (2 mg/kg) in mice. Our results showed that M1R mRNA expression was increased in the frontal cortex and the hippocampus region (the CA2 region) in the acute methamphetamine administered group compared to the saline administered group. In the chronic group, M1R mRNA expression was increased in the frontal cortex ill1d the hippocampus regions (CA2 and DG regions) in melt1amphetamine administered group compared to saline control group. These results indicate that acute or chronic treatment of mathamphetamine leads to the region-specific changes in mRNA expression levels of M1R. Therefore, Therefore, the present result suggests that M1R may play a role in modulating of methamphetamine-induced behavioral sensitization in mice.

만성 통증장애의 정신과적 치료 (Psychiatric Treatment of Chronic Pain Disorder)

  • 만성 통증은 신체적, 심리적, 사회환경적 요인들이 복합적으로 작용한 결과로서 나타나기 때문에 생물정신사회적 관점에서 접근해야 하며 치료는 다각적이고 포괄적인 체제하에서 상호 긴밀한 협력을 필요로 한다. 치료의 목표는 즉각적인 통증의 해소보다는 재활에 초점을 맞추어 환자의 활동과 기능이 증진됨에 따라 부수적으로 통증이 완화될 수 있도록 해야 할 것이다. 정신과적 영역에서 만성 통증의 치료는 정신의학적 평가로부터 시작하는데 여기에는 스트레스와 연관된 통증의 정신생리적 기전과 이상 질병행동과 관련된 조작적 조건화 기전, 그리고 증상으로서 통증을 동반하거나 만성 통증에 의해서 유발되는 정신과적 질환들에 대한 평가가 포함된다. 정신과적 치료를 약물치료와 심리적 치료로 나눌 때 약물치료에는 마약성 진통제, 비스테로이드성 소염제, 향정신성약물, 항경련제 들이 포함되는데 그 중에서도 항우울제가 가장 중요한 역할을 한다. 심리적 치료로는 역동적 정신치료와 행동치료적 방법들이 있고 행동치료적 접근법에는 자가조절기법과 조작적 조건화기법 등이 포함되는데, 환자가 갖는 대인관계의 어려움, 신체적 손상, 이상 질병행동 등을 고려하여 적절한 방법을 적용하게 된다.

Conservative Treatment with Occlusal Appliance for Temporomandibular Disorder Patients with Rheumatoid Arthritis

  • Purpose: This study is designed to analyse etiology and bone pattern at the first visit using cone-beam computed tomography (CBCT) and to evaluate the treatment outcome of conservative treatment in temporomandibular disorder (TMD) patients with rheumatoid arthritis (RA). Methods: One hundred condyles in 50 subjects with RA were chosen among the patients who presented to the Department of Oral Medicine of Pusan National University Dental Hospital, diagnosed as TMD. Condylar bone changes were classified by normal, erosive bony change, proliferative bony change and combined group (erosive bony change+proliferative bony change). They were treated conservatively with physical therapy, medication, behavioral therapy and/or occlusal stabilizing splint therapy. After 3 months on average, patients were re-evaluated with regards to subjective symptoms and the clinical findings were investigated. Results: TMD patients with RA have behavioral contributing factors such as parafunctional habit. The results that analyse bone pattern at the first visit using CBCT proliferative bony changes group (32.6%) were more common than erosive bony changes group (15.2%). In comparison between unilateral and bilateral bony change in temporomandibular joint, the ratio showed no significant differences. After 3 months of conservative treatments, pain, noise, limitation of motion (LOM) were markedly improved regardless of occlusal splint therapy. However only LOM was significantly improved through occlusal splint therapy during 3 months. Conclusions: TMD patients with RA had similar behavioral contributing factors and characteristics of CBCT images shown in general TMD patients and also similar response to conservative treatment so it is difficult to differentiate. Therefore when TMD patients show symptoms corresponding to clinical diagnostic criteria of RA at the first visit, serological testing should be conducted and through this, early diagnosis and treatment of RA should be initiated.

Behavioral responses to cow and calf separation: separation at 1 and 100 days after birth

  • Objective: The aim was to compare the behavioral response to full separation of cows and calves maintained together for 100 days or 24 h. Methods: Twelve Holstein-Friesian cow-calf pairs were enrolled into either treatment or industry groups (n = 6 cow-calf pairs/group). The treatment cows and calves were maintained on pasture together for 106±8.6 d and temporarily separated twice a day for milking. The Industry cows and their calves, were separated within 24 h postpartum. Triaxial accelerometer neck-mounted sensors were fitted to cows 3 weeks before separation to measure hourly rumination and activity. Before separation, cow and calf behavior was observed by scan sampling for 15 min. During the separation process, frequency of vocalizations and turn arounds were recorded. At separation, cows were moved to an observation pen where behavior was recorded for 3 d. A CCTV camera was used to record video footage of cows within the observation pens and behavior was documented from the videos in 15 min intervals across the 3 d. Results: Before separation, industry calves were more likely to be near their mother than Treatment calves. During the separation process, vocalization and turn around behavior was similar between groups. After full separation, treatment cows vocalized three times more than industry cows. However, the frequency of time spent close to barrier, standing, lying, walking, and eating were similar between industry and treatment cows. Treatment cows had greater rumination duration, and were more active, than industry cows. Conclusion: These findings suggest a similar behavioral response to full calf separation and greater occurrence of vocalizations, from cows maintained in a long-term, pasture-based, cow-calf rearing system when ompared to cows separated within 24 h. However, further work is required to assess the impact of full separation on calf behavior.

비만치료에 있어서 환자특성에 따른 인지행동요법과 행동수정요법의 효과 비교: 체계적 문헌고찰 및 메타분석 (Comparison of the Effects of Cognitive Behavioral Therapy and Behavioral Treatment on Obesity Treatment by Patient Subtypes: A Systematic Review and Meta-analysis)

  • Objectives: The present study aimed to compare the impacts of cognitive behavioral therapy (CBT) and behavioral treatment (BT) on weight loss and psychological outcomes among patients with three different subtypes of obesity: simple obesity, obesity with binge eating disorder, and obesity with depression. Methods: Embase, PubMed, the Cochrane Central Register of Controlled Trials, Research Information Sharing Service, and Korean Studies Information Service System were systematically searched for randomized controlled trials conducted on or before May 2020, that used CBT to treat obesity. Methodological quality was assessed using Cochrane's risk of bias tool 2 and publication bias was evaluated through the funnel plot using the trim and fill method, Egger's test, and Begg and Mazumdar rank correlation test. A meta-analysis was conducted using a random-effects model and the standardized mean difference with 95% confidence interval (CI) was used to determine effect size. Results: Twenty-one randomized controlled trials with a total of 22 intervention arms and 2,590 patients were included. Our study results revealed that the effects of CBT, compared with BT, on weight loss distinctly differed across all patient subgroups. In the simple obesity group, CBT was more effective than BT (Hedges' g=0.138, CI=0.012~0.264); however, in the obesity with binge eating disorder group, BT was more effective than CBT (Hedges' g=-0.228, CI=-0.418~-0.038); in the obesity with depression group, the effect of CBT was not statistically different from that of BT (Hedges' g=0.276, CI=-0.307~0.859). Further studies with larger sample sizes are required to confirm the outcomes observed in this study. Conclusions: Our results indicated that the effects of CBT on obesity treatment vary based on patient subtype. Therefore, our findings suggest that CBT or BT should be selectively recommended as a treatment strategy for different obesity subtypes.

Parent-Implemented Behavioral Interventions for Challenging Behavior of Young Children with Developmental Disabilities: A Review of Effective Approaches

  • Addressing challenging behavior in young children with special needs is an important issue. This study analyzed the results of literature on parent-implemented behavioral interventions designed to manage the challenging behavior of children with developmental disabilities under 8 years of age. Seventeen relevant studies published in peer-reviewed English journals during the last 15 years were selected for review. The literature review showed that adequately trained parents can serve as a competent intervention agent, thereby making significant contributions to mitigating children's behavior problems. The reviewed studies also demonstrated that parents could play a variety of important roles such as informants and trainers in behavioral interventions. However, the conclusions on parent-implemented behavioral intervention may be tentative rather than conclusive due to previous research limitations including the lack of treatment integrity and social validity data. In addition, this review discussed several implications for practitioners, indicated the limitations of the reviewed studies, and presented suggestions for future research.
