한방 치료로 호전된 대상포진 후 신경통 환자 치험 5례 (Five Cases of Postherpetic Neuralgia Improved by Traditional Korean Medical Treatment)

  • Objectives : The purpose of this study is to report the effect of Traditional Korean Medical treatment on Postherpetic Neuralgia. Methods : Five patients suffering from postherpetic neuralgia were treated with Warm Needling, Bee venom Acupuncture, Cupping Therapy, Sipjeondaebo-tang. To evaluate the results of this treatment, we used Visual Analogue Scale(VAS), Pain Rating Scale(PRS), Quality of Sleep(QSP). Results : After treatment, VAS, PRS, QSP and clinical symptoms of 5 patients were improved remarkably. Conclusions : According to the results, These findings suggest that Traditional Korean Medical treatment can be effective for the treatment of postherpetic neuralgia.

뇌졸중 후 중추성 통증 환자에 대한 봉독약침요법약침 치료 효과에 대한 임상적 연구 (Clinical Study on the Effect of Bee Venom Acupuncture Therapy on the Post-stroke Pain)

  • 2004년 5월부터 2004년 9월까지 동서한방병원에 입원한 환자로 뇌 전산단층조영술이나 자기공명영상에 의해 뇌졸중으로 진단받은 환자들 중 중추성 통증을 가진 20명중 기존의 침구 및 한약 치료, 물리치료를 통해 통증에 있어서 만족할 만 한 개선이 없던 환자들을 대상으로 중풍칠처화(中風七處火) 및 아시혈(阿是穴)을 중심으로 3주에 걸쳐 봉독약침을 시술하고 환자의 주관적 통증을VAS로 객관화하여 비교한 결과 아래와 같은 결론을 얻었다. 1. 대상 환자는 남자 11명(55%), 여자9명(45%)이 고 평균연령은 57.95세였으며, 뇌경색은 12명 (60%), 뇌출혈은 8명(40%)이었다. 2. 발병 후 치료 시작 시점까지 경과 된 기간은 3 개월 이상이 12병(60%)으로 가장 많았고, 15 일 이상 3개월 미만이 7명(35%), 1개월 미만이 7명(5%)명 순으로 나타났다. 3. 통증 부위는 편측 상지가 10명(50%)으로 가장 많았으며, 편측 상하지 7명(35%), 편측 하지 2명(10%), 체간 1명(5%)의 순으로 나타났다. 4. 통증 양상은 복합적으로 나타났고 이를 중복해서 측정한 결과 쑤시는 증상이 14명(70%)으 로 가장 많았으며, 저린감 6명, 욱신거림 5명, 냉감 혹은 열감4명, 이질통 3명의 순으로 나타났다. 5. VAS통증 점수는 치료 전 $7.25{\pm}1.888$에서 1주 치료 후 $6.45{pm}1.932$(P<0.01)에서 2주후에는 $5.45{\pm}1.791$로(P<0.01), 3주 치료 후 에는 $4.45{\pm}1.761$(P<0.01)로 유의성 있게 감소하였다.

위증(痿證)의 침구 치료에 대한 문헌 고찰 (Acupuncture & Moxibustion Bibliographic Studies on the Wei symptom for Clinical Treatment)

  • This study was performed to investigate the acupuncture & moxibustion therapy of the Wei symptom(痿證) in order to find out the clinical possibility. We investigated the acupuncture & moxibustion therapy of Wei symptom through six books published after 1993, searched sites as like 'Oasis', 'Pubmed', 'RISS', 'KISS', 'NDSL' and 'Google scholar'. Acupuncture & moxibustion prescriptions recorded in the orietal medicine literatures that were published before and after 1993 had a similar basis. Since 2000, in the clinical papers on the wei symptom searched from domestic journal, sa-am and dong's acupuncture therapy were mainly applied, but papers were confined in a case study. Since the mid-2000s, the clinical effectiveness of bee venom and electroacupuncture therapy on the wei symptom were uncovered, but the researches about the chracteristics in accordance with acupoints are more required. Since 2010, the mechanism of the bee venom and electroacupuncture on the wei symptom was revealed in animal experiments. On this basis, the systematic clinical studies beyond case paer or pilot study is(are) needed. The development of various treatment implements as like electroacupuncture, pharmacopuncture was required to access the incurable disease like wei syptom, and the efforts of providing evidence through systemic clinical trials are needed.

봉독약침자극이 Catecholamine성 신경세포의 활성변화에 미치는 영향 (Effect of the bee venom aqua-acupuncture on the neuronal activities of catecholaminergic system in brainstem)

  • This study was designed to evaluate the effect of the bee venom(BV) aqua-acupuncture on the neuronal activities of catecholaminergic(tyrosine hydroxylase : TH, dopamine ${\beta}$ hydroxylase : D${\beta}$H) system in the brainstem. After the BV aqua-acupuncture was applied on Chok-Samni(ST36) and the gluteal part(Blank locus) in rats. Also, the number of colocalization between catecholamine containing neurons and Fos immunoreactive neurons were analyzed by using the double immunohistochemical technique. The results of the experiments were summarized as follows : 1. In DR and LC, Chok-Samni group and the Blank locus group showed more significant increase in the number of colocalization between TH containing neurons and Fos immunoreactive neurons than the control group. Furthermore, Chok-Samni group showed more significant increase than the Blank locus group. Also, in Arc, Chok-Samni group showed more significant increase than the Blank locus group and the control group. 2. In LC, Chok-Samni group showed more significant increase in the number of colocalization between D${\beta}$H containing neurons and Fos immunoreactive neurons than the Blank locus group and the control group. Also, in A5, Chok-Samni group and the Blank locus group showed more significant increase than the control group. Chok-Samni group showed more significant increase than the Blank locus group. However, there was no significant change in A7. Consequently, the BV aqua-acupuncture increased more potent the number of Fos immunoreactive neurons and the activity of catecholaminergic neurons. Furthermore, the BV aqua-acupuncture was more effective on Chok-Samni than Blank locus group. These results indicate that the BV aqua-acupuncture is very effective therapy to control pain. The therapeutic effect of BV aqua-acupunture may associated with the endogenous modulatory system such as catecholamine. Those data from the study can be applied to establish the effective treatment of the BV for pain control in the clinical field.

Bee Venom Acupuncture for Shoulder Pain: A Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Objectives: Our review aimed to summarize and evaluate evidence on the effectiveness of bee venom acupuncture (BVA) in the treatment of shoulder pain. Methods: Randomized controlled trials (RCTs) evaluating the effectiveness of BVA on shoulder pain were searched up to October 2019 in 11 electronic databases (Medline, Embase, CENTRAL, CiNii, CNKI, VIP, Wanfang, Kmbase, NDSL, RISS, OASIS). The methodological quality of the included RCTs were evaluated using Cochrane Risk of Bias tool and a meta-analysis was performed. Results: Seven studies were included in the review, and four studies were included in the meta-analysis. Comparing BVA plus conventional therapy (CT) with saline injection plus CT, it showed an effect in favor of BVA plus CT in visual analog scale (VAS) and pain rating scale (PRS) (p = 0.02, p = 0.009, respectively). Comparing BVA plus physiotherapy (PT) with saline injection plus PT, it showed that there was no significant difference in VAS and verbal rating scale (VRS) between the two groups. Conclusion: This systematic review and meta-analysis suggest that BVA could be beneficial as an adjuvant treatment for shoulder pain.

홍화 약침의 견비통에 대한 임상적 연구 (Clinical Study on Effect of Carthmi-Flos Herbal acupuncture theraphy on shoulder pain)

  • Objective : This study is designed to find out the effects of Carthmi-Flos Herbal acupuncture therapy on shoulder pain. Methods : Under the assumption that Bee-venom Therapy may be effective for treatment seborrheic dermatitis, Bee-venom acupuncture was injected subcutaneously into the lesion including Gv24(神庭), Gv23(上星), Gv22, Gv20(白會), B4(供差), B5(五處), B6(承光), S8(頭維), etc. Results : 1. Female patients were 18 cases and these were more 12 cases than male patients. Fifty aged patients were the most(11 cases, 36.7%). 2. Below 1 month duration of disease was the most(14 cases 46.7%). 3. The effects of treatment by condition of patients was follows : After treatment, Grade IV was decreased 26.7%, Grade III was decreased 6.6% and Grade II was increased 23.3%. 4. The effects of treatment by Duration was follow : Below 1 month & 1 month to 6 month were improved highly. 5. In evaluating of satisfaction, exellent was 6 cases, and good was 19 cases. The percentage of above good was 83.3%. Conclusions : According to the results, Carthmi-Flos Herbal acupuncture may be good effects on the decrease of shoulder pain.

항암화학요법 유발 말초신경병증에 대한 봉독 약침 요법의 효과 및 기전에 대한 실험연구 고찰 (Review of Experimental Researches on Bee Venom Pharmacopuncture Therapy for Chemotherapy-induced Peripheral Neuropathy)

  • Chemotherapy-induced peripheral neuropathy (CIPN) is one of the most common dose-limiting side effects of neurotoxic chemotherapeutic agents that lead to decreased quality of life and dose reduction, delay or even cessation of treatment. The purpose of this systematic review is to evaluate the effect and the underlying mechanisms of bee venom (BV) pharmacopuncture therapy for CIPN in animal models. We searched for the available experimental literature using BV for CIPN through the Pubmed databases. Ten experimental studies were finally included in this review. In the oxaliplatin or paclitaxel-induced CIPN animal model, BV significantly relieved pain caused both mechanical and cold stimulation. It was suggested that the effect of BV is mediated by the stimulation effect of spinal α1- and α2-adrenergic receptors as a potential mechanism. In the future, more experimental studies are needed.

Comparative Study on the Effects of Bee Venom Pharmacopuncture According to the Treatment Method for Knee Osteoarthritis

  • Objectives: The purpose of this study is to compare the effects of bee venom pharmacopuncture (BVP) therapy according to the methods used to treat knee osteoarthritis (OA): intra-acupoint combined with intra-articular injection, intra-acupoint injection, and intra-articular injection. Methods: A total of 69 patients were recruited by the Department of Acupuncture & Moxibustion at Dong-Eui Oriental University Hospital from February 1 to July 23, 2012. The patients were assigned to 3 groups: the first group with intra-acupoint combined with intra-articular BVP Injection (the experimental group), the 2nd group with intra-acupoint BVP injection (control group II), and the 3rd group with intra-articular BVP injection (control group II). The participants were assigned in the order in which they were recruited. Treatments were done twice a week, for a total of 9 times. The effectiveness was assessed by using the visual analouge scale (VAS) and the Korea Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Results: All three groups exhibited significant VAS and KWOMAC effects. Moreover, the 4 week follow-up after the final treatment showed a persistence of BVP effects. However, when the groups were compared, no statistically significant differences in VAS and KWOMAC were noted, but when improvement was considered, the results showed that intra-articular injection was more effective than intra-acupoint injection. Especially, intra-acupoint combined with intra-articular injection was the most effective among the three treatments. Conclusions: Combining intra-acupoint with intra-articular injection, depending on the patient's symptoms, may produce better results when conservatively treating knee OA.

Synergistic Effects of Bee Venom and Natural Killer Cells on B16F10 Melanoma Cell Growth Inhibition through IL-4-mediated Apoptosis

  • Objectives : We investigated the synergistic effects of bee venom (BV) and natural killer (NK) cells on B16F10 melanoma cell apoptosis mediated by IL-4. Methods : We performed a cell viability assay to determine whether BV can enhance the inhibitory effect of NK-92MI cells on the growth of B16F10 melanoma cells, and western blot analysis to detect changes in the expression of IL-4, $IL-4R{\alpha}$, and other apoptosis-related proteins. EMSA was performed to observe the activity of STAT6. To confirm that the inhibitory effect of BV and NK cells was mediated by IL-4, the above tests were repeated after IL-4 silencing by siRNA (50 nM). Results : B16F10 melanoma cells co-cultured with NK-92MI cells and simultaneously treated by BV ($5{\mu}g/ml$) showed a higher degree of proliferation inhibition than when treated by BV ($5{\mu}g/ml$) alone or co-cultured with NK-92MI cells alone. Expression of IL-4, $IL-4R{\alpha}$, and that of other pro-apoptotic proteins was also enhanced after co-culture with NK-92MI cells and simultaneous treatment with BV ($5{\mu}g/ml$). Furthermore, the expression of anti-apoptotic bcl-2 decreased, and the activity of STAT6, as well as the expression of STAT6 and p-STAT6 were enhanced. IL-4 silencing siRNA (50 nM) in B16F10 cells, the effects of BV treatment and NK-92MI co-culture were reversed. Conclusion : These results suggest that BV could be an effective alternative therapy for malignant melanoma by enhancing the cytotoxic and apoptotic effect of NK cells through an IL-4-mediated pathway.

인체폐암세포에서 봉독에 의한 prostagladin E2 생성 및 telomerase 활성 저하 (Bee Venom-induced Growth Inhibition of Human Lung Cancer Cells was Associated with Inhibition of Prostagladin E2 Production and Telomerase Activity.)

  • 본 연구에서는 봉독의 처리에 따른 A549 폐암세포의 증식억제에서 $PGE_2$ 생성 및 telomerase 활성의 변화 관련성을 조사하였다. A549 세포의 증식은 봉독 처리에 의하여 유의적으로 감소되었으며, 이는 apoptosis 유발과 연관성이 있음을 알 수 있었다. 봉독 처리 농도의 증가에 따라 COX-2의 발현이 전사 및 번역 수준에서 모두 감소되었으며 이에 따른 $PGE_2$의 생성이 현저하게 감소되었으나, COX-1의 발현에는 큰 변화가 없었다. 또한 봉독 처리에 따라 telomere 조절인자들 중, hTERT, hTR 및 c-myc의 발현이 억제되었으며, telomerase의 활성도 매우 감소되었다. 본 연구의 결과는 $PGE_2$ 생성과 telomerase 활성 저하가 봉독의 항암 작용 표적인자로서 작용될 수 있음을 보여준다.
