계단 보행 시 불안정성 신발 착용에 따른 슬관절 부하에 대한 연구 (A Study on Changes in Knee Joint Loading during Stair Gait with Unstable Shoes)

  • Purpose: The purpose of this study is to compare kinematics and kinetics on the knee joint between stair gait with unstable shoes and barefoot in healthy adult women. Methods: Seventeen healthy adult women were recruited for this study. The subjects performed stair ascent and descent with unstable shoes and barefoot. The experiment was repeated three times for each stair gait with unstable shoes and barefoot. Measurement and analysis of the movements of the knee joint were performed using a three-dimensional analysis system. Results: Statistically significant differences in the knee muscle force of semimembranosus, biceps femoris-long head, biceps femoris-short head and sartorius, patellar ligament, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair ascent. Statistically significant differences in the knee muscle force of sartorius, rectus femoris, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair descent. Statistically significant differences in the knee flexor moment of semitendinosus, biceps femoris-long head, biceps femoris-short head, sartorius, rectus femoris, vastus intermedialis, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair ascent. Conclusion: Therefore, wearing unstable shoes during stair gait in daily life is considered to influence knee joint kinematics and kinetics due to the unstable shoes, and thus suggest the possibility that reducing the risks of pain, and knee osteoarthritis, stabilizing the knee joint caused by changes in the loading of the knee joint.

  • Objective: The purpose of this study were to investigate the standing balance, dynamic activity in hemiplegic patients according to the types of ankle-foot orthosis(AFO) and to determine the most effective type of AFO for gait training. Method: A prospective study was performed for 16 patients with hemiplegia who was able to walk independently. Static balance and dynamic activity were compared in two condition : 1) barefoot and SPAFO, 2) barefoot and HPAFO. Static balance and dynamic activity characteristics were evaluated by Active Balance while they were standing with in two condition AFO and barefoot. Results: There were significant difference in standing balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in dynamic activity balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in gait speed between barefoot and wearing SPAFO and HPAFO(p<0.05). Conclusion: This study showed that wearing SPAFO and HPAFO gave fair amount of improvement to balance and gait ability of hemiplegic patients.

Foot Scan 측정을 통한 노년 여성의 보행 분석 (Gait Analysis on the Elderly Women with Foot Scan)

  • This study compares the gait characteristics of elderly women during barefoot walking and walking with sneakers. We measured foot angles, max foot pressure, peak plantar pressure of each plantar region, velocity of Center of Pressure(COP), and axis shifting of COP with an RS-scan system. Elderly women's foot angles were narrower when walking with sneakers than when barefoot walking. We found that the subtalar joint angle (representing ankle joint flexibility) affected walking stability. Regarding the peak plantar pressure of each foot region, pressures were high in the medial regions and the pressures greatly varied depending on the region measured during barefoot walking. The COP moved significantly faster when walking with sneakers than barefoot walking and suggests that elderly women walked faster in sneakers. Axis shifting of the COP decreased during walking with sneakers and indicated that gait balance improved when walking with sneakers. The findings of the present study can be utilized as foundational data for elderly women's gait characteristics as well as data for the production of functional footwear. Future research that focuses on various types of shoes, age groups, and gender are recommended for the development of more functional footwear for stable gaits.

신발의 형태가 노인의 보행에 미치는 영향 (The Effect of Footwear on Gait in Older Adults)

  • Purpose : The purpose of this study was to evaluate the effect of footwear on gait in older adults. Methods : The footwear consists of loafer with fixation and mule and slipper without fixation. Twenty one female older adults voluntarily participated in this study. Each participant walked on the gait system GAITRite and measured temporal-spatial gait parameters. And the participants wore loafer, mule, slipper randomly. We measured stance time, swing time, heel-to-heel BOS, double support time, velocity, cadence, FAP as temporal-spatial gait parameters. Results : Three gait parameters showed significant difference statistically among 7 gait parameters. The stance time increased as loafer, mule, barefoot, slipper orderly. And there was a significant difference statistically (p<.05). The swing time increased as slipper, barefoot, mule, loafer orderly. And there was a significant difference statistically(p<.05). And the heel-to-heel BOS increased as barefoot, loafer, mule, slipper orderly. And there was a significant difference statistically(p<.05) according to footwear type. Conclusion : The footwear type with high stability like loafer is considered better than footwear type with low stability like mule and slipper for the elderly.

Effect of Kinesiology Taping for Ankle Instability in Stroke Patients

  • Background: Ankle instability usually occurs after stroke, and contributes to unsafe walking and associated risk of falling in the affected patients. Objective: To investigate the effects of kinesiology taping (KT) on gait and balance ability (center of pressure, CoP) in patients with ankle instability after stroke. Design: One group, pre-post design. Methods: A total of 11 patients with ankle instability after stroke were enrolled. In all subjects, the gait and balance ability were assessed under 2 conditions: KT and barefoot. Gait and balance ability was assessed using GAITRite system and FDM-S platform. Results: Comparison between KT and barefoot condition, KT condition was significantly higher in velocity, cadence, step length, and stride length than barefoot condition (P<.05). KT condition was significantly lower in CoP path length and sway speed than barefoot condition (P<.05). Conclusion: KT indicated potential as a helpful method for walking and balancing ability in patients with ankle instability after stroke. Therefore, this study recommends KT as an option applicable to the stroke with ankle instability.

20세 이상 성인의 구두 굽 높이에 따른 균형과 보행형태의 변화 (Changes in Balance and Gait Patterns with Different Heel Heights Among Women in Their 20's)

  • Purpose : The purpose of this study is to examine the effect of different heel heights on gait deviation and balance. Method : Participants were 16 women majoring in physical therapy in Kyungbuk college, located in Yeong-ju, North Kyungsang Province. Balance and gait patterns were measured by Good Balance and GAIT Rite. Result : Statistically significant differences were found in swing right measured by Gait-rite, and also in Analyze End, Ant-post, Med-Lat, and gait ability score(p < .05) regarding dynamic balance ability. Conclusion : There was no statistically significant difference in gait ability between those who walked barefoot and highheeled. However, balance ability was different "between them". This shows that heel height can lead to decrease in gait ability. Further research should include more participants and use a wide range of heel heights.

성인 여성의 맨발 보행과 운동화 착용 보행 시 주기 비교 (Comparison of Barefoot and Shod Gait Cycle for Adult Women)

  • 본 연구는 맨발 보행과 운동화 착용 보행에서 보행주기의 차이를 비교하여 보행역학에 따른 신발 개발에 기초자료를 제공하고자 한다. 발의 변형과 이상이 없는 정상 성인 여성 30명을 대상으로 보행 주기를 측정하였다. 먼저 운동화를 착용하고 보행하여 주기를 측정한 후, 맨발로 보행하여 주기를 측정하여 데이터를 얻었다. 이후 두 데이터를 대응표본 T-test를 이용하여 비교하였다. 실험 결과 맨발 보행에서 입각기 좌측(p<.001), 우측(p<.005), 체중부하기 좌측(p<.009), 우측(p<.002), 전유각기 좌측(p<.002), 우측(p<.011), 양하지 지지기(p<.004)가 증가하였고, 중간 입각기 좌측(p<.016), 우측(p<.001), 유각기 좌측(p<.001)이 감소하였다. 이는 맨발 보행이 다양한 발의 감각의 입력을 증가시켜 보행안정성이 높은 보행이 가능해 졌다고 보여 지며, 향후 보행 주기에 의거하여 맨발보행과 가까운 신발 개량이 필요하다고 사료된다. 향후 신발의 개량을 위해 신발 종류에 따른 보행주기 연구가 필요할 것이다.

A Study on Changes in Lower Limb Joint Angles during Stair Walking with High Heel

  • Purpose: The purpose of this study is to compare kinematics on lower limbs between stair walking with high heel and barefoot in healthy adult women. Methods: 18 healthy adult women were recruited in this study. The subjects performed stair ascent and descent with high heels and barefoot. The experiment was conducted in random order and repeated three times for each stair walking with high heels and barefoot. The movements of lower limb joints were measured and analyzed using a three-dimensional analysis system. Results: The ankle, knee, and hip flexion angles on the sagittal plane exhibited statistically significant differences between stair ascent and descent with high heels and barefoot. The pelvic forward tilt angles showed statistically significant differences only during stair ascent. The ankle inversion, hip abduction and pelvic lateral tilt angles on the frontal plane showed statistically significant differences between stair walking with high heels and barefoot. On the transverse plane, the hip rotation angles showed statistically significant differences between the high-heeled and barefoot gait during stair ascent and descent. However, the pelvic rotation angles showed no statistically significant differences. Conclusion: Therefore, wearing high-heeled shoes during stair walking in daily life is considered to influence lower limb kinematics due to the high heel, and thus poses the risks of pain, and low stability and joint damage caused by changes in the movement of lower limb joints.

패브릭 발목 보조기가 경직성 뇌성마비 아동의 시공간적 보행 변수에 미치는 즉각적인 효과 (Immediate Effect of Fabric Ankle-Foot Orthosis on Spatiotemporal Gait Parameters in Children With Spastic Cerebral Palsy)

  • The purpose of this study was to investigate the immediate effect of fabric ankle-foot orthosis on spatiotemporal gait parameters, compared to a barefoot condition in children with spastic cerebral palsy. Eleven children with spastic cerebral palsy participated in this study. Spatiotemporal gait parameters were measured with the GAITRite system. Fabric ankle-foot orthosis significantly improved Timed Up and Go test time and gait velocity. There was no significant difference in cadence. The step time significantly improved in both the more and less affected foot compared to the barefoot condition. The step length of the affected foot also significantly improved, but there was no significant difference in the step length of the less affected foot. There was significant improvement in the stride length of both the affected and less affected foot, but no significant difference in single stance or double stance. The fabric ankle-foot orthosis could improve stability, and selective control of the joint and promote better walking in children with cerebral palsy. Consequently, the fabric ankle-foot orthosis might be an alternative assistive device for neurological populations as a primary role instead of the typical ankle-foot orthosis.

발목관절 보조 도구에 따른 만성 뇌졸중 편마비 장애인의 보행 비교 (Comparison of gait ability according to types of assistive device for ankle joint of chronic hemiplegic stroke survivors)

  • Background: The purpose of this study was to compare the differences in gait and mobility according to the types of assistive device for ankle joint including ankle foot orthosis (AFO), non-elastic tape, elastic tape, and high ankle shoes in chronic hemiplegic stroke survivors. Design: A cross-over design. Methods: Twelve hemiplegic stroke survivors participated in this study, and they walked under 5 different conditions including bare feet, wearing a AFO, wearing a non-elastic tape, wearing a elastic tape, and wearing a high ankle shoes. During the participants walked, the spatio-temporal gait analysis and mobility examinations were performed. For the spatio-temporal gait analysis (gait velocity and cadence, step length, stride length, and single and double leg support time) and mobility examinations, the gait mat, TUG and TUDS were used. Results: As s results, on the AFO, non-elastic tape, elastic tape, and high ankle shoes, there were significantly differences in the all spatio-temporal gait parameters, TUG, and TUDS compared to barefoot (p<0.05). In particular, all spatio-temporal gait parameters, TUG, TUDS were significantly improved with AFO compared to barefoot. TUG was significantly improved with AFO compared to non-elastic tape, TUG and TUDS were significantly improved with AFO compared to elastic tape, gait velocity was significantly improved with non-elastic tape compared to high ankle shoes, gait velocity and TUG were significantly improved with elastic tape compared to high ankle shoes, and TUDS was significantly improved with non-elastic tape compared to elastic tape. Conclusion: The AFO, non-elastic tape, elastic tape, and high ankle shoes showed a positive effect on gait and mobility compared to barefoot, and among them, wearing AFO was most effective for improving gait and mobility of chronic hemiplegic stroke survivors.
