Effect of Subfractions of Carthamus tinctorius L. Semen on the Lipid Peroxidation and Oxygen Free Radical Scavenging Enzyme Activities in CCl_4$-induced Hepatotoxic Rats (홍화자 분획물의 사염화탄소 유발 간손상 흰쥐에서 지질과산화와 oxygen free radical 제거 효소 활성도에 미치는 영향)

  • Previous studies have shown that methanol extract and its butanol fraction of Carthamus tinctorius L. Semen have the hepatoprotective effect on the CCl4-induced hepatotoxicity. The hepatoprotective effect of the subfractions of butanol fraction has been evaluated by analyzing oxygen free radical scavenging enzyme activities and histopathological examinations. In BS-5 subfraction treated group, the activity of superoxide dismutase has been significantly increased as compared with that of CCl4, treated rats. Antioxidant activity has been evaluated by the examination of the scavenging activity against 1,1-diphenyl-2-picrylhydrazyl radical. BS-5 subfraction has shown strong antioxidant activities. The histopathological examination showed that the treatment of BS-5 subfraction has relieved the ballooning degeneration of hepatocytes which had been generated by CCl4. It appears that the protective effect of BS-5 subfraction would be mediated of the attenuation of lipid peroxidation by acting as a free radical scavenger, which were based on the increase of superoxide dismutase activity.

Bronchoscopic Intervention for Airway Disease (기도질환 환자의 치료기관지경술)

  • Surgical resection and reanastomosis has been the treatment of choice in patients with tracheobronchial stenosis. Recent development of bronchoscopic intervention has been replacing the role of surgery in these patients. After summarizing the upto date data of bronchoscopic intervention, the proper management of tracheobronchial stenosis will be presented. Bronchoscopic intervention would be much effective when performed under rigid bron- choscopy, due to the stable patients' condition and endoscopic view. The usual method of intervention includes ballooning, Nd-YAG laser resection, bougienation, mechanical airway dilatation, stenting and photodynamic therapy. Silicone stents are very effective in patients with tracheobronchial stenosis to maintain airway patency. Bronchoscopic intervention provided immediate symptomatic relief and improved lung function in most of patients. After airway stabilization, stents were removed successfully in 2/3 of the patients at a 12-18 months post-insertion. Less than 5% of patients eventually needs surgical management. Acute complications, including excessive bleeding, pneumothorax, and pneumomediastinum develops in less than 5% of patients but managed without mortality. Stent-related late complications, such as, migration, granuloma formation, mucostasis, and restenosis are relatively high but usually controlled by follow-up bronchoscopy. In conclusion, bronchoscopic intervention, including silicone stenting could be a useful and safe method for treating tracheobronchial stenosis.

An Experimental Investigation of Yarn Tension in Simulated Ring Spinning

  • Yarn tension is a key factor that affects the efficiency of a ring spinning system. In this paper, a specially constructed rig, which can rotate a yam at a high speed without inserting any real twist into the yarn, was used to simulate a ring spinning process. Yarn tension was measured at the guide-eye during the simulated spinning of different yarns at various balloon heights and with varying yarn length in the balloon. The effect of balloon shape, yarn hairiness and thickness, and yam rotating speed, on the measured yarn tension, was examined. The results indicate that the collapse of balloon shape from single loop to double loop, or from double loop to triple etc, lead to sudden reduction in yarn tension. Under otherwise identical conditions, a longer length of yarn in the balloon gives a lower yarn tension at the guide-eye. In addition, thicker yarns and/or more hairy yarns generate a higher tension in the yarn, due to the increased air drag acting on the thicker or more hairy yarns.

  • The $I_c$ degradation behaviors of externally-reinforced Bi-2223 superconducting tapes under pressurized liquid nitrogen were investigated. Tapes with different thickness of reinforcement layers were compared and the results showed that when the bending strain was calculated at the outer surface, the tape with the thicker reinforcement showed a better bending strain tolerance of $I_c$, but when the bending strain was calculated at the outermost filament, the $I_c$ degradation behavior became identical for all tapes. $I_{c0}$ decreased with the increase of applied pressure. After depressurization to atmospheric pressure from 1 MPa, the $I_c$ was completely recovered to its initial values. Ballooning occurred after a thermal cycle.

Marfan syndrome and symptomatic dural ectasia: A case report and literature review

  • Dural ectasia refers to the widening or ballooning of the dural sac surrounding the spinal cord. It can affect any plane of the spinal canal, but occurs primarily in the lumbosacral region. Dural ectasia is present in 63-92% patients who have Marfan syndrome, and is related to Ehlers-Danlos syndrome, neurofibromatosis type I, and ankylosing spondylitis. The most common symptoms are low back pain, headache, weakness, numbness above and below the affected limb, and occasional rectal and genital pain. However, in most patients, dural ectasia is usually asymptomatic. We report the case of a 5-year-old boy who presented with a severe headache who had been diagnosed with Marfan syndrome. During the evaluation, magnetic resonance imaging of the lumbar and sacral spine revealed dural ectasia. To our knowledge, this is the first report on Marfan syndrome with symptomatic dural ectasia in Korea. We concluded that dural ectasia should be suspected in patients diagnosed with Marfan syndrome who have a severe headache.

The Effects of Scutellaria Radix Extract on the Alcohol-Induced Fatty Acid Synthesis of Liver in Rats (알코올로 유도된 흰쥐의 간 지방 형성에 황금 추출물이 미치는 효과)

  • Objectives: Alcoholic fatty liver is a potentially pathologic condition which can progress to steatohepatitis, fibrosis, and cirrhosis. The objective of this study is to investigate the effects of Scutellaria Radix (SR) extract on the alcoholic fatty liver induced by long-term EtOH administration. Results: Male Sprague Dawley rats were used in this study. All animals were randomly divided into Normal group, treated with saline (n=10); EtOH group, treated with ethanol (n=10); EtOH+SR group, treated with ethanol+Scutellaria Radix extract (n=10). For oral administration of ethanol in EtOH and EtOH+SR group, the ethanol was dissolved in distilled water in concentrations of 25% (v/v). Throughout the experiment of 8 week, the rats were allowed free access to water and standard chow. Sample group were administrated by Scutellaria Radix extract daily for 8 weeks. Results: The levels of hepatic marker such as aspartate aminotransferase and alanine aminotransferase were altered. Histopathological changes such as ballooning, fatty and hydropic degeneration were reduced and the expression of tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) was significantly attenuated by Scutellaria Radix extract. Conclusions: These data suggest that Scutellaria Radix extract attenuated the alcoholic simple fatty liver by improving hepatic lipid metabolism via suppression of $TNF-{\alpha}$ protein. Scutellaria Radix could be effective in protecting the liver from alcoholic fatty liver.

A Case of Dural Ectasia with Low Back Pain and Sciatica Treated with Integrative Korean Medical Treatment

  • Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.

Burst criterion for Indian PHWR fuel cladding under simulated loss-of-coolant accident

  • The indigenous nuclear power program of India is based mainly on a series of Pressurised Heavy Water Reactors (PHWRs). A burst correlation for Indian PHWR fuel claddings has been developed and empirical burst parameters are determined. The burst correlation is developed from data available in literature for single-rod transient burst tests performed on Indian PHWR claddings in inert environment. The heating rate and internal overpressure were in the range of 7 K/s-73 K/s and 3 bar-80 bar, respectively, during the burst tests. A burst criterion for inert environment, which assumes that deformation is controlled by steady state creep, has been developed using the empirical burst parameters. The burst criterion has been validated with experimental data reported in literature and the prediction of burst parameters is in a fairly good agreement with the experimental data. The burst criterion model reveals that increasing the heating rate increases the burst temperature. However, at higher heating rates, burst strain is decreased considerably and an early rupture of the claddings without undergoing considerable ballooning is observed. It is also found that the degree of anisotropy has significant influence on the burst temperature and burst strain. With increasing degree of anisotropy, the burst temperature for claddings increases but there is a decrease in the burst strain. The effect of anisotropy in the ${\alpha}$-phase is carried over to ${\alpha}+{\beta}$-phase and its effect on the burst strain in the ${\alpha}+{\beta}$-phase too can be observed.

Rigid Bronchoscopy for Post-tuberculosis Tracheobronchial Stenosis

  • The healing process of tracheobronchial tuberculosis (TB) results in tracheobronchial fibrosis causing airway stenosis in 11% to 42% of patients. In Korea, where pulmonary TB is still prevalent, post-TB tracheobronchial stenosis (PTTS) is one of the main causes of benign airway stenosis causing progressive dyspnea, hypoxemia, and often life-threatening respiratory insufficiency. The development of rigid bronchoscopy replaced surgical management 30 years ago, and nowadays PTTS is mainly managed by bronchoscopic intervention in Korea. Similar to pulmonary TB, tracheobronchial TB is treated with combination of anti-TB medications. The indication of rigid bronchoscopy is more than American Thoracic Society (ATS) grade 3 dyspnea in PTTS patients. First, the narrowed airway is dilated by multiple techniques including ballooning, laser resection, and bougienation under general anesthesia. Then, most of the patients need silicone stenting to maintain the patency of dilated airway; 1.5 to 2 years after indwelling, the stent could be removed, this has shown a 70% success rate. Acute complications without mortality develop in less than 10% of patients. Subgroup analysis showed successful removal of the stent was significantly associated with male sex, young age, good baseline lung function and absence of complete one lobe collapse. In conclusion, rigid bronchoscopy could be applied to PTTS patients with acceptable efficacy and tolerable safety.

Left Ventricular Outflow Tract Obstruction Caused by Accessory Mitral Valve Tissue in a Child -A case report- (소아에서 부승모판막(Accessoy Mitral Valve Tissue)에 의한 좌심실 유출로 협착 - 1예 보고 -)

  • Accessory mitral valve tissue is a very rare congenital cardiac malformation and it is an uncommon cause of left Ventricular outflow tract obstruction(LVOTO). The pathogenetic mechanism of subaortic obstruction is thought to be systolic ballooning of the abnormal valve tissue into LVOT. We are reporting a case of an accessory mitral valve tissue that was associated with LVOTO, and this was completely relieved after trans-aortic surgical excision of the accessory tissue.
