Purpose: This study was conducted to assess the effects of robot-assisted gait training with visual feedback on gait, balance, and balance confidence in patients with chronic stroke. Methods: Thirty subjects with chronic stroke were randomly assigned to two groups: the experimental group (n=15) and the control group (n=15). The experimental group performed robot-assisted gait training for 30 minutes and the control group performed gait training with assisted devices training for 30 minutes after both groups performed conventional physical therapy for 30 minutes. Both groups performed the therapeutic interventions for 5 days per week, for a period of 4 weeks. For assessment of the 10 m walking test (10 MWT), Figure of 8 on the walk test (F8WT), Timed-Up and Go test (TUG), and Berg Balance Scale (BBS) were used to test the gait and balance, and the Korean version of the Activities-specific Balance Confidence Scale was used to test the balance confidence. Results: The experimental group showed significant improvement in the 10 MWT and the K-ABC (p<0.05), and the control group showed significant improvement in the BBS and the TUG (p<0.05). In four measurements, there were significant differences between the two groups (p<0.05), and the control group showed significant improvement in the F8WT at pre and post intervention (p<0.05). Conclusion: This study demonstrated that Robot-assisted gait training with visual feedback is an effective intervention for improving straight gait abilities and balance confidence, while the control group showed some improvement in curve gait and balance. Thus, we suggest both Robot-assisted gait training with visual feedback and gait training with assisted devices training exercise as a therapeutic intervention in chronic stroke rehabilitation.
Purpose: This study examined the effects of action-observation training using the Y-Balance on the balance ability of young adults. Methods: Thirty-four healthy adults were randomized into an action-observation group (n=17) or a control group (n=17). All subjects performed the Y-Balance test before and after watching the video. The action observation group watched a video of someone performing a Y-Balance test, and the control group watched a video of scenery unrelated to the training. The subjects were measured through a Y-Balance test for both the length of the legs extended in three directions and the Y-balance composite score. Results: A significant difference in the Y-balance composite score was observed between the two groups. A part of the direction of the extended leg in the action observation group was increased significantly (posteromedial direction of the right leg, posterolateral direction of the right leg, posteromedial direction of left leg) compared to the control group. Conclusion: These results suggest that action observation training only could help improve balance.
Since Romberg's test in 1953, the analysis of postural sway during upright stance has been widely used as a tool for evaluating balance and disorders of the postural control system. This review describes the methods that have been used to evaluste the static and dynamic performance of the postural control system. Various identification methods of postural control system based on standing balance are discussed and measures of postural sway are described. The application areas of standign balance research, with an emphasis on postural control evaluations, are also briefly described. This review can be used to gain an understanding of the dynamics of human standing balance.
Hemiplegic patients usually present with difficulty maintaining balance. Balance retraining is the major component of rehabilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of prorpioceptive exercise program on the improvement of balance in the patients with chronic hemiplegia. Thirty subjects (mean age $57.0{\pm}9.8$) were recruited and the subjects were divided into a proprioceptive group and a visual group. The subjects for the proprioceptive group were participated in the proprioceptive exercise program for 4 weeks, and the visual group were treated visual feedback training using Balance Master. At 4 week follow-up test, Berg Balance Scale significantly improved 1.1 points (p<.01), Timed Up & Go test improved 4.2 second (p<.01), and weight distribution during sit to stand also improved 5.0% (p<.01). As a result of this study, the proprioceptive control approach improved dynamic balance in the patients with chronic hemiplegia. It is suggested that there was no benefit of visual feedback training like as Balance Master when administrated in combination with other physical therapy interventions, compared with physical therapy alone using proprioceptive control approach to hemiplegia.
The purpose of this study was to identity effects of virtual reality(VR) program related to standing postural control on balance, gait and brain activation patterns in chronic hemiplegic stroke patients. Subjects were assigned randomly to either VR group (n=12) or the control group (n=12) when the study began. Both groups received conventional physical therapy for 2 to 3 times per week. In addition to conventional physical therapies, VR group trained 3 types of virtual reality programs using IREX for standing postural control during 4 weeks (4 times/week, 30 minutes/time). Subjects were assessed for static and dynamic balance parameters using BPM, functional balance using Berg Balance Scale related to movement of paretic lower limb before and after 4 weeks of virtual reality training. The results of this study were as follows. 1. Following VR training, VR group demonstrated the marked improvement on dynamic mean balance, anteroposterior limits of stability (AP angle) and mediolateral limits of stability (ML angle). 2. Following VR training, both groups scored higher on Berg Balance Scale. However, a comparison of mean change revealed differences between groups. In conclusion, these data suggest that the postural control training using VR programs improve dynamic and functional balance performance in chronic hemiplegic stroke patients.

PURPOSE: The purpose of this study was to investigate the effect of sensorimotor training in unstable surface on balance ability of elderly women. METHODS: Total of 40 subject were recruited from two separated institution for elderly women in Changwon and conveniently allocated into experimental and control groups. Twenty women were included into experimental group and another twenty women were into control group. Experimental group performed sensorimotor training in unstable surface and control group were only activities of daily living during eight weeks. To assess static balance ability used One leg stance test (OLST) and to assess dynamic balance ability used timed up and go test (TUGT) and Functional reach test (FRT). RESULTS: Control group did not show any difference in balance ability. However static balance ability in experimental group improved on hard or soft surface only eye open condition. Also dynamic balance ability in experimental group improved TUGT or FRT. CONCLUSION: Sensorimotor training in unstable surface improved balance ability of elderly women.

In this study, the balance beam control subsystem, new type of construction machinery using the mechanism of CMG (control moment gyro), for the attitude control of an unstructured object such as a beam carried by a tower crane, is designed and implemented. The balance beam controller consists of a wheel spinning at high speed and an outer gimbal for controlling the attitude of the wheel. Two motors, one for the wheel and the other for the gimbal, are used. Applying force to the spin axis of the wheel, as an input of the system, leads the torque about the axis because of the gyro effects. This torque is used to control the attitude of the unstructured object in this study. For the stabilizer function, in addition, holding the load at the current position, the attitude of the wheel is freed by cutting the power applied to the gimbal motor of the balance beam controller, which result in the braking force to stop the load by gyro effect. The works presented here include the mechanical system of the balance beam controller, the remote controller, the servo controller and the control software for the system. We also present experimental results to show that the system we proposed is useful as a new construction machinery which can control the attitude of the beam hanging from a tower crane.
A decrease in the ability to maintain static and dynamic balance after stroke could be related to the inability to select reliable sensory information in producing relative motor action needed to maintain postural stability. The purpose of this study was to compare the effects of two different types of surface conditions on the balancing ability of subjects with stroke. Eighteen hemiparetic subjects were assigned to an experimental and control group participating in a six-week rehabilitative therapeutic exercise program focusing on balance and mobility. Exercises were performed 3 to 5 times per week in a stable surface condition by the control group, and in an unstable surface condition by the experimental group. Pre- and post test assessments involved the measurement of the static balance and dynamic balance, respectively by 7-item Berg Balance Scale-3P and by Pro-3 Balance System. Results showed that under the unstable surface condition, static balance in the experimental group showed more improvement than that of the control group.(Statistically, not very significant.) All the aspects of dynamic balance and mediolateral sway(balance) improved significantly than those of the control group. However, there were no significant differences between two groups. Overall, it can be concluded that under the unstable surface condition, the rehabilitative therapeutic exercise programs are effective in improving the dynamic balance of stroke subjects. The results suggest that the adaptation of the unstable surface in the rehabilitative therapeutic exercises could be effective for the patients with hemiplegia in balance. Further studies are needed to confirm the effectiveness of the unstable surface on improving balance and postural stability of hemiplegics.
Purpose: Patients with total knee arthroplasty (TKA) have impaired balance and movement control. Exercise interventions have not targeted these impairments in this population. This study aimed to investigate the effect of complex balance exercises on unstable ground, on the gait, balance, and daily living ability of patients with total knee arthroplasty. Design: Randomized controlled trial. Methods: The participants consisted of 30 patients placed into two groups of 15 each: a experimental group (complex balance exercise) and a control group (physical therapy exercise). Both group exercise was applied for 3 times a week for 30 minutes for four weeks. Force plate for balance ability and Timed up and go (TUG) test were the primary outcome measures. The secondary outcome measures included 10-m walk test (10MWT) and the daily living ability using the Knee Outcome Survey Activities of Daily Living scale (KOS-ADL). Results: The result of this study showed that the experimental group had a significant difference in TUG and 10MWT than the control group, and balance was significantly different in CEA, CPL, and CAV. There was a significant difference in daily living ability between the experimental and control groups. This study confirms that the physical therapy with complex balance exercise on unstable surfaces has positive effects on balance, gait and daily living ability in patients with total knee arthroplasty. Conclusion: As a result of this study, complex balance exercise on unstable surface was more effective in improvement gait, balance and daily living ability in total knee arthroplasty. From this study, physical therapy with complex balance exercises on unstable ground may be proposed as and effective intervention method for improving gait, balance, and daily living ability in patients with early total knee arthroplasty.
The purpose of study wan to compare the effect of balance performance in the elderly by the strengthening exercise. Thirty-one health elderly women aged 60 to 79 years participated in this study. Participants were divided into exercise(21) and control group(10). Exercise participants received strengthening exercise for 45 minutes in three times a week for 6 weeks while control subjects continued their normal activities, Exercise included resisted hip flexion, extension, abduction and adduction, knee flexion and extension, ankle dorsiflexion and plantrarflexion. All subjects were assessmented clinical test of sensory interaction and balance. one leg stance test, Berg balance test. Exercise and control subjects were teated before, midway through, and at the end of the trial. These collected data were analyzed by using oneway and repeated ANOVA, scheffe's test, 1-test and correlation. The results of this study were as fellows. 1. There were statistically significant difference in balance performance clinical teat of sensory interaction, and balance(p<.01), one leg stance test(p<.05), Berg balance test(p<.05) by the strengthening exercise. 2. There was correlation between static balance and dynamic balance(p<.01). 3. There was correlation between static balance and weight(p<.05), dynamic balance and height(p<.01), and weight(p<.01).
