편마비 환자의 트레드밀과 체중지지의 트레드밀 훈련이 균형능력 및 보행능력에 미치는 영향 (The Effect of Treadmill and Body Weight Support Treadmill Training on Balance and Gait Ability in Hemiplegia Patients)

  • Purpose: The purpose of this study is to investigate and to verification of changes that the effect of treadmill and body weight support treadmill training on balance and gait ability for sub-acute stroke patients during 4 week. Method: 16 subjects who was diagnosed stroke were divided into 2 groups(8-treadmill training group, 8-body weight support treadmill group) by randomized control trial. Both training programs were consisted with 40 minuted, 5 times a week for 4 weeks and after training programs. Analysis: We analysed effects and changes on balance and gait ability. Analyses were performed using PASW ver. 18.0 and results were reported as mean ${\pm}$ standard deviation (S.D.). To investigate within group comparisons and to verification on effects of exercise, we did paired t test and repeated measured ANOVA test. Significance was set at p<.05. Result: Both training programs showed positive changes in Limit of Stability but significant results which is area of hemiside, area of intact side, area of posterior, total area were reported in body weight support treadmill training group. Changes of Berg Balance Scale was significantly increase and it had significant correlation between groups. Changes of 6 Minutes Walking Test was significant increase in both groups but there was no significant changes on Foot print and Romberg test. Conclusion: After considering all factors, both training programs showed effect on improvement of balance and gait ability in sub-acute stroke patients, but body weight support treadmill training group had better improvement in dynamic balance than treadmill training group. For the increase of balance and gait ability in sub-acute stroke patients, we need to continues study on difference of treadmill and body weight support treadmill and then we will give stroke patients a better satisfaction if we develop and provide a rehabilitation program for improvement of balance and gait ability.

  • Purpose: This study seeks to determine the effect of a lower extremity strengthening exercise that uses proprioceptive neuromuscular facilitation (PNF) on the gait and balance ability of a stroke patient. Methods: In this case study, a lower extremity strengthening exercise that used PNF was performed by the subject for 30 minutes 4 times per week for a 4-week period alongside typical rehabilitation therapy. The lower extremity pattern used flexion-abduction-internal rotation with knee flexion, extension-abduction-internal rotation, bilateral leg extension pattern. The 10 m walk test and 6 minute walk test were used to assess gait ability. The Berg balance scale was used to assess balance ability. Results: After the intervention, the time for the 10 m walk test decreased by 5.72 sec, the distance for the 6 minute walk test decreased by 20 meters, and the score on the Berg balance scale improved by 7 points, which indicates the effectiveness of this therapy for stroke patients. Conclusion: The results of this study indicate that a lower extremity strengthening exercise using PNF can improve the gait ability and balance ability of stroke patients.

가상현실 게임을 이용한 4주간 훈련이 노인의 균형 및 보행 능력과 우울증에 미치는 효과 (The Effects of 4 Weeks Training using Virtual Reality Game on Balance and Gait Ability, Depression of Elderly People)

  • Purpose : This study investigated the effect of 4 weeks training of virtual reality game on balance and gait ability, depression of elderly people. Methods : The subjects of this study were 12 elderly people over 65 age. The subjects were treated by training using virtual reality gam (during 60 minutes, 3 times a week for 4 weeks). The results of the experiment were analyzed using paired T-test along with averages and standard deviations as the statistical methods for data analyses. Results : In comparison of depression, gait and balance ability between pre-test and post-test, there were statistically significant differences. Conclusion : Our study finding indicate that training using virtual reality game could improve balance and gait ability, depression in elderly people. According to our study, training using virtual reality game is judged to be used for elderly people with gait and balance problems, depression to prevent fall down.

Effects of Gaze Stabilization Exercise and Cognitive Training on Balance and Gait in Subacute Stroke Patients: Randomized Controlled Trial

  • PURPOSE: The purpose of this study was to evaluate the effects of simultaneous application of gaze stabilization exercise and cognitive training on the balance and gait ability in subacute stroke patients. METHODS: Thirty-five patients diagnosed with stroke within 3-6 months were randomly assigned, and the experimental group (n = 18) to which both gaze stabilization exercise and cognitive training were applied and the control group (n = 17) to which only gaze stabilization exercise was applied were targeted. It was performed for 30 minutes at a time, three times a week, for a total of 4 weeks. Berg Balance Scale, Timed Up and Go test, 10Meter Walking Test, and Walking symmetry were evaluated. RESULTS: In the comparison of changes between Berg Balance Scale, Time Up and Go test, 10 Meter Walking Test, and Gait symmetry, both experimental and control groups showed significant differences before and after the intervention, and in the evaluation of Gait symmetry, significant differences between groups. CONCLUSION: As a result of this study, when gaze stabilization exercise and cognitive training were allied simultaneously, it was possible to improve the balance and gait ability of subacute stroke patients, and had a more significant effect on gait ability. In considered that training that simultaneously applies gaze stabilization exercise and cognitive training can be presented as a balance and gait rehabilitation for stroke patients on the future.

Effects of Diagonal Pattern Self-Exercise on Trunk Control, Balance, and Gait Ability in Chronic Stroke Patients

  • Background: Weakness of the trunk muscles decreases the trunk control ability of stroke patients, which is significantly related to balance and gait. Objectives: To compare the impact of diagonal pattern self-exercise on an unstable surface and a stable surface for trunk rehabilitation on trunk control, balance, and gait ability in stroke patients. Design: Nonequivalent control group design. Methods: Twenty four participants were randomized into the experimental group (diagonal pattern self-exercise while sitting on an unstable surface, n=12) and the control group (diagonal pattern self-exercise while sitting on a stable surface, n=12). All interventions were conducted for 30 minutes, three times a week for four weeks, and the trunk impairment scale (TIS), berg balance scale (BBS), functional gait assessment (FGA), and G-walk were measured. Results: All groups indicated significant increases in all variables (TIS, BBS, FGA, cadence, speed, stride length) after four weeks. The TIS, BBS, FGA, cadence, gait speed, and stride length group-by-time were significantly different between the two groups. Conclusion: We found that, in stroke patients, diagonal pattern self-exercise on an unstable surface is a more effective method for improving trunk control, balance, and gait ability than diagonal pattern self-exercise on a stable surface.

The Effect of Calf Stiffness on Gait, Foot Pressure and Balance in Adults

  • Purpose: The purpose of this study was to investigate the effects of calf tightness on gait, plantar pressure, and balance in adults. Methods: A total of 60 participants were divided into a normal group of 30 subjects with normal dorsiflexion angle (20-25 degrees) and an experimental group of 30 subjects with limited dorsiflexion angle (0-15 degrees) due to calf tightness. Gait ability and foot pressure of the subjects was measured with a treadmill, and the balance ability was measured by PROKIN system. Results: A significant difference in COP length, loading response, and single limb support was observed between groups. The COP length and single limb support ratio in the normal group was greater than in the experimental group, but the experimental group showed a higher ratio for loading response in the gait ratio. Conclusion: Our results indicated that calf tightness was negative effects on balance and gait ability, so assessment of the muscle tightness should be considered during exercise and treatment.

  • Purpose: The aim of this study was to examine the effects of vibration exercise on balance control ability, gait ability, and depression in the elderly. Methods: A total of 24 elderly subjects were recruited and randomized into an experimental (n = 12) or a control (n = 12) group. The experimental group performed both a vibration and a stabilization exercise, and the control group performed a stabilization exercise alone. Balance ability was measured using the functional reach test (FRT), gait ability was measured using the timed up and go test (TUGT), and depression was measured using the Center for Epidemiologic Studies Depression Scale (CES-D). Results: The experimental and control groups showed significant differences for all pre- and post-experiment FRT, TUGT, and CES-D measurements (p <0.05). In a comparison between the two groups, the experimental group in which vibration exercise was applied showed more significant difference in FRT, TUGT, and CES-D than the control group (p <0.05). Conclusion: This study showed vibration exercise to be effective in balance control ability, gait ability, and depression in the elderly.

Effect of upper extremity coordination exercise during standing on the paretic side on balance, gait ability and activities of daily living in persons with stroke

  • Objective: The purpose of this study was to determine the effect of upper extremity coordination exercise (UECE) during standing on the paretic side on balance, gait ability and activities of daily living (ADL) in persons with stroke. Design: A randomized controlled trial. Methods: A total of 27 patients with hemiplegic diagnosis after stroke were divided into two groups. Fourteen patients were in the study group and 13 patients were in the control group. The study group received conventional physical therapy and UECE during standing on the paretic side. The control group received conventional physical therapy and simple upper extremity exercise (SUEE). Subjects in both groups were given upper extremity training for 30 minutes per day, five times a week for 4 weeks. Initial evaluation was performed before treatment and reevaluated 4 weeks later to compare the changes of balance, gait ability and ADL (Korean version of modified Barthel index, K-MBI). Results: Both groups showed a significant effect for balance, gait ability and ADL (p<0.05). In the Independent t-test, between both groups showed a significant effect for balance and gait ability except ADL (p<0.05). Conclusions: In this paper, we investigated the changes in balance, walking, and ADL through UECE. We found significant changes in the study group and the control group. Results of the present study indicated that UECE during standing on the paretic side for 4 weeks had an effect on balance, gait ability and ADL (K-MBI) in persons with hemiplegia after stroke.

발목관절의 시각되먹임 운동 이후 기능적 전기자극이 만성 뇌졸중 환자의 균형 및 보행에 미치는 영향에 관한 융합적 연구 (A Convergence Study on the effects of Ankle Joint Functional Electrical Stimulation after Visual feed-back Ankle training to Improve on Balance, Gait ability in Patient with Chronic Stroke)

  • 본 연구의 목적은 시각적 되먹임 발목관절 운동 이후 기능적전기자극치료가 만성 뇌졸중 환자의 균형능력 및 보행능력에 대한 영향을 알아보기 위함이다. 선정기준에 따라 만성 뇌졸중 환자를 22명을 대상으로 두 군으로 실시하였다. VFAF군은 시각되먹임 발목관절운동 이후 기능적전기자극치료(n=11), CON군(n=11)은 보존적 물리치료를 시행하였다. 훈련은 1일 60분, 1주 5번, 총 8주간 시행하였다. 중재 전·후에 균형과 보행능력을 검사하였다. 훈련결과 VFAF군이 COP, LOS, BBS, FRT, 10m WT에서 CON군에 비해 통계학적으로 유의한 차이를 보였다. 그러므로 시각적 되먹임 발목관절 운동 이후 기능적전기자극치료의 융합은 뇌졸중 환자의 균형능력 및 보행능력의 향상을 위한 효과적인 중재로 임상에서 활용 될 수 있으며, 향후 졸중 환자를 위한 융합중재개발이 요구된다.

Effect of Uneven Surface Gait Training on Ankle Muscle Activation and Balance in Stroke Patients

  • Purpose: The purpose of this study was to investigate the effects of regular training on the uneven surface that stroke patients encounter in their daily life on their ankle joint muscle activity and balance ability. They were divided into two groups: the gait training group on uneven surfaces and the gait training group on normal surfaces. Methods: In this study, 30 patients diagnosed with stroke and undergoing rehabilitation were selected. 15 people in the uneven surface gait training group and 15 people in the flat gait training group were selected. The muscle activation of the ankle muscles was measured when walking again on a even surface after walking on an uneven surface and on a flat ground. After each gait training, the limit of stability and Romberg test were performed to evaluate the balance ability. Results: As a result of the experimental results before and after walking by group, the tibialis anterior muscle activity of the paralyzed side was significantly decreased in the uneven surface walking group. As a result of measuring balance ability after training, the limit of stability in all directions was significantly increased in the uneven surface gait training group, and the area and length moved significantly decreased in the uneven surface gait training group in the Romberg test as well (p<0.05). Conclusion: After walking on uneven surface, it was confirmed that the muscle activity of the ankle joint decreased in normal flat walking, and thus the efficiency of muscle activity was increased. In addition, it was possible to confirm the improvement of the balance ability of the gait training on the uneven surface, and in conclusion, it could be confirmed that it had an effect on the improvement of the walking ability.
