  • Patients with short bowel syndrome (SBS) have a high risk of developing parenteral nutrition (PN)-associated complications. Therefore, diet or enteral nutrition and PN should be modified to limit such complications. N balance analysis is a method of calculating the amount of protein required to achieve N equilibrium in the body based on intake and excretion. It is important to reduce dependence on PN and achieve the recommended range of N balance 2-4 g with an appropriate diet. We report a recent experience with nutrition modification using N balance analysis and suggest it as a useful method to reduce dependence on PN in nutrition management of SBS patients and in continuing active intestinal rehabilitation.

  • The purpose of this study was to assess the effect of visual block (eyes open or closed), mental task type, and participation in an exercise program on static balance in the elderly. The subjects were 34 elderly (>65 years old) residents of a social welfare institute in Gyeonggi-do. We measured the following variables. Berg balance scale, mini mental state examination, balance performance monitor (sway area, path, and maximal sway velocity), age, weight, height and whether the subject participated in an exercise program. Scores for the Berg balance scale and mini mental state examination were evaluated to assess static balance ability either alone (single task paradigm) or while performing a mental task (dual task paradigm). Static balance variables that were measured included sway area, path, and maximal sway velocity. Each test was repeated three times. Multiple regressions analysis was used to examine the effect of each variable on static balance ability. For the dual task paradigm, static balance was affected by whether the subject participated in an exercise program. The Berg balance scale score for subjects with their eyes open was affected by whether they participated in an exercise program, while this variable in addition to the height and weight of subjects were determining variables in subjects with their eyes closed. As a result, whether subjects participated in an exercise program affected their static balance irrespective of whether their eyes open or closed. Therefore, regular exercise is recommended for elderly people and further research is needed to examine the relationship between static and dynamic balance while performing mental tasks such as cognition and attention.

  • PURPOSE: The aim of the present study was to determine whether high frequency repetitive transcranial magnetic stimulation (rTMS) can improve balance ability in acute stage stroke patients. METHODS: The study was conducted on 30 subjects diagnosed with hemiparesis caused by stroke. The experimental group consisted of 15 patients that underwent rTMS for 15 mins and the control group consisted of 15 patients that underwent sham rTMS (for 15 minutes). A 70-mm figure 8 coil and a Magstim Rapid stimulator was used in both groups. Patients in the experimental group received 10 Hz rTMS applied to the hotspot in the lesioned hemisphere in 10-second trains with 50-second intervals between trains, for 15 minutes (total 2,000 pulses). Both groups received conventional physical therapy for 30 minutes a day, 5 days a week, for 4 weeks. Static balance ability analysis was performed using the Gaitview system to measure pressure rate, postural sway, and total pressure, and dynamic balance ability analysis was performed to measure pressure variables using a balance system. RESULTS: A significant difference was observed in post-training gains for pressure rate, total pressure in static balance, and overall stability index in dynamic balance between the experimental group and the control group (p<.05). CONCLUSION: The results of this study indicate that high frequency rTMS may be beneficial for improving static and dynamic balance recovery in acute stroke patients.

  • Purpose : This study focuses on the influence of bridge exercise combined with whole body vibration on muscle activity and balance. Methods : 30 stroke patients were recruited for subjects. The subjects were divided to bridge exercise combined stable surface (Group I), bridge exercise combined unstable surface (Group II), and bridge exercise combined bridge exercise (Group III), of which 10 subjects were randomly allocated. The subjects were given 30 minutes of neurologic physical therapy which included gait training and muscular strength training, and additionally given 30 minutes of bridge exercise combined stable surface, bridge exercise combined unstable surface, and bridge exercise combined whole body vibration for each group, five times a week, for 8 weeks. Their muscle activity and balance were analyzed before the intervention. After 8 weeks of the intervention, the mentioned parameters were measured once more for between-group analysis. Results : Comparative analysis of the muscle activity and balance between the groups showed statistically significant difference, and post-hoc analysis showed the Group III had greater changes in muscle activity and balance than Group Iand Group II. Conclusion : Such results revealed that bridge exercise combined with whole body vibration is effective in muscle activity and balance. Based on the current study, more effective program is to be proposed for elite athletes as well as stroke patients. Based on the current study, studies that incorporates various frequencies of vibration is required for development of effective whole body vibration exercise program.

  • For the in-line three cylinder engine whose crankshaft has a phase of 120 degrees, the total sum of unbalanced inertia forces occurring in each cylinder will be counterbalanced among three cylinders. However, parts of inertia forces generated at the No.1 and No.3 cylinders will cause a primary moment about the No.2 cylinder. In order to eliminate this out-of-balance moment, a single balance shaft has been attached to the cylinder block so that the engine durability and riding comfort may be further improved. Accordingly, the forced vibration analysis of the in-line three cylinder engine must be implemented to meet the required targets at an early design stage. In this paper, a method to reduce noise and vibration in the 800cc, in-line three cylinder LPG engine is suggested using the multibody dynamic simulation. The static and dynamic balances of the in-line three cylinder engine are investigated analytically. The multibody dynamic model of the in-line three cylinder engine is developed where the inertia properties of connecting rod, crankshaft, and balance shaft are extracted from their FE-models. The combustion pressure within the No.1 cylinder in three significant operating conditions(1500rpm-full load, 4000rpm-full load and 7000rpm-no load)is measured from the actual tests to excite the engine. The vibration velocities at three engine mounts with and without balance shaft are evaluated through the forced vibration analysis. Obviously, it is shown that the vibration of the in-line three cylinder engine with balance shaft is reduced to the acceptable level .

  • This study aimed to identify differences in the level of work-life balance as well as the effect of independent variables, including family (sharing family work, support for working), organization (culture, support services and systems), and community variables (accessibility to and amount of work-life balance programs), on work-life balance in South Korea, Japan, and the U.K. For these purposes, data were collected from 311, 324, and 322 married, working women (from 30 to 50 years of age) from Korea, Japan, and the U.K., respectively. It was consistently shown that U.K. employees scored higher in work-life balance than Korean and Japanese employees. Compared with Japan and the U.K., Korean participants were significantly lower in terms of work-leisure balance and work-self-development balance. The regression analysis revealed that 'sharing family work with partner' was commonly important and a major factor in all three countries. A 'supportive organizational culture' predicted work-life balance for Korean and Japanese participants, while work-life balance programs had a powerful effect on work-life balance only for U.K. participants. In the case of community variables, there were no significant effects for U.K. participants with regard to work-life balance. In contrast, 'the amount of work-life balance programs offered' was shown to affect the work-life balance of Korean working women, while 'accessibility to the programs' was significantly influential in Japan. We interpret these results according to social, economic, political, and psychological factors.

  • The purposes of this study were to determine the effect of different degrees of severity of diabetic neuropathy on balance function, and to evaluate dynamic balance and functional performance in diabetes patients. Twenty-four subjects with diabetes mellitus were divided into three groups according to results of sensory nerve conduction study. All subjects were evaluated for dynamic balance which was measured using computerized dynamic posturography, and functional performance which was measured using the Berg balance scale. One-way analysis of variance was used to determine whether there were any statistically differences of dynamic balance function and functional performance among the three groups. The Spearrnan's rank correlation was used to determine statistical significance between dynamic balance and age. The results were as follows: 1. Dynamic balance measured using computerized dynamic posturography was significantly lower in the no response group than in the normal amplitude group (p<0.05). 2. Functional performance tested by the Berg balance scale was not statistically different among the three groups (p>0.05). 3. an inverse relationship was found between dynamic balance measured using computerized dynamic posturography and age (r=-0.68, p<0.05). These results suggest that patients with severe diabetic neuropathy have loss of dynamic balance function. Therefore, patients with severe diabetic neuropathy need to have their balance evaluated and receive appropriate education.
