본 연구의 목적은 고혈압노인의 식사시간 별 혈압변화와 식후저혈압(PPH: Postprandial hypotension) 발생을 파악하기 위해 아침, 점심, 저녁 세 끼 식사 전후의 혈압을 측정하였다. 노인 복지시설에 거주하는 고혈압 노인 187명으로부터 서면동의를 받은 후 시행되었다. 혈압측정 도구는 휴대용 혈압모니터(ABPM)를 사용하여 아침, 점심, 저녁의 식전 30분부터 식후 2시간까지 15분 간격으로 측정하였다. 수집된 자료는 SPSS Win 14.0 program으로 빈도와 백분율, 평균과 표준편차, ANOVA와 Scheffé 사후검정을 이용하여 분석하였으며, 연구결과는 다음과 같다. • 수축기혈압과 이완기혈압의 하강 정도는 아침 식후가 점심이나 저녁 식후 보다 유의하게 더 많았다. • PPH 발생률은 아침 식후에는 73.4%, 점심 식후에는 54.2%, 저녁에는 50.2%이었다. • PPH가 가장 많이 발생한 시점은 아침과 점심 저녁 모두 식사 시작 후 90분이었다. • 식후 혈압을 8회 측정한 것을 기준했을 때, 평균 PPH 발생 횟수는 아침 식후가 평균 3.0±2.7회로 점심 평균 2.1±2.5회와 저녁 평균 1.9±2.4회 보다 더 많았다. 고혈압노인의 간호에서는 점심이나 저녁 식사에서보다는 아침식사 후에 PPH 발생이 더 많다는 점을 고려하여야 할 것이다. 또한 식사 시작 후 1시간에서 2시간 동안은 PPH로 인한 건강문제가 발생하지 않도록 주의 관찰과 간호관리가 이루어져야 한다. 아울러 고혈압노인의 PPH를 조절할 수 있는 간호중재를 개발하기 위한 추후연구가 활성화되어야 한다.
Objectives : This study was performed to evaluate the effects of moxibustion on cerebral blood flow, peripheral blood flow and blood pressure. Methods : We designed a randomized, cross-over study that included 20 healthy, male volunteers who were randomly assigned to one of two groups (the moxibustion group or the control group). The initial trial was followed by a washout period of one week before the groups were switched and the trial repeated. The cerebral blood flow and peripheral blood flow of the volunteers in both groups were measured by mean velocity (Vm), peak systolic velocity (Vs), pulsatility index (PI), perfusion units (PU) and mean blood pressure (BP) before the intervention (baseline), 30 minutes after the intervention, and again 60 minutes after the intervention. Results : 1. The Vm increased significantly in the moxibustion group. The Vs also constantly increased in moxibustion group, but there was no significant difference. 2. The PI decreased significantly in the control group at 0-30 minutes and in the moxibustion group at 30-60 minutes. 3. The PU constantly decreased in moxibustion group. In control group, the PU decreased at 30 minutes and increased slightly at 60 minutes. 4. The mean blood pressure did not show any significant difference between the two groups. Conclusions : This study shows that moxibustion at CV12 increased the cerebral blood flow. Based on this result, we can suggest that moxibustion at CV12 might be helpful in improving the functional recovery of stroke patients. However, further research needs to be conducted on a larger numbers of subjects in order to confirm these effects.
Objectives : To estimate the prevalence of hypertension, rates of awareness, treatment modalities, and control of hypertension, and also to identify factors in rural Korea that are related to these aspects of hypertension. Methods : A cross-sectional survey, including blood pressure(BP) measurements and interviews, investigating the variables in the health belief model was peformed from August to November, 1999 on 1,426 (79.4%) study subjects out of 1,797 registered residents over 30 years old in Ibansong-Myun, Chinju City. A second survey was peformed from January to March, 2000 on 376 (80.0%) subjects out of 470 hypertensive (or suspected hypertensive) subjects found in the first survey. Two BP readings were taken in each survey using a standard mercury sphygmomanometer. Hypertension was defined as either a BP reading $\geq$ 140/90 mmHg in both surveys, or as subjects on oral hypertensive medication. Results : Estimated hypertension prevalence was 24.9% for men, and 30.4% for women. Rates of hypertension awareness, treatment (the regular use of oral hypertensives), and control (reduction of BP to <140/90 mmHg) were 52.5%, 34.4%, and 12.9%, respectively. The factors related to lower hypertension awareness in the logistic regression analysis were male gender, farming occupation, and higher perceived barrier to medical treatment (those for whom visiting health professionals is a burden). Conclusions : To improve the low awareness, treatment, and control of hypertension, a more active and out-reaching hypertension control program, including routine BP measurements for every visitor to primary care facilities, is needed in rural Korea.
Angiotensin receptor blockers, such as telmisartan, are considered effective in the treatment of hypertension and proteinuria due to chronic kidney disease (CKD) in cats. It selectively blocks the $AT_1$ receptor and does not affect the $AT_2$ receptor, thus effectively blocking the activity of the renin angiotensin aldosterone system. This study aims to compare over time the changes in various indicators, including systemic hypertension and proteinuria, before and after the administration of telmisartan in cats with CKD. Decrease in blood pressure (BP) (p<0.001) and urine protein to creatinine (UP/C) ratio (p<0.001) were found to be statistically significant over time after the administration of telmisartan. BP and the UP/C ratio were $160{\pm} 22.2$ and $0.50{\pm}0.647$ before telmisartan administration (Day 0), $150{\pm}21.0$ and $0.27{\pm}0.487$ on the 30th day (Day 30), $150{\pm}17.0$ and $0.25{\pm}0.376$ on the 60th day (Day 60), and $140{\pm}17.8$ and $0.15{\pm}0.233$ on the 90th day (Day 90) after administration, respectively. BP and UP/C were statistically significantly lower in cats with CKD over time at each time point from Day 0 to Day 90 at 30 day intervals. Especially after 90 days of telmisartan administration, the improvement of BP and UP/C were estimated to be about 20 mmHg and 0.35, respectively. In conclusion, the oral administration of telmisartan to cats with CKD is effective in improving BP and proteinuria, which has a positive effect on long-term survival in cats with CKD.
Objective: The purpose of this study was to investigate the effect reducing blood pressure (BP) using Xuefuzhuyu Decoction (XFZYD) in adults with essential hypertension (EH). Method: Search engine, such as PubMed, EMBASE, Cochrane library, Web of Science, J-STAGE, CiNii, CNKI, OASIS, NDSL, KISS, RISS, and DBpia, were used. The search period was from the beginning of the search engine to June 30, 2018 and there were no limits regarding languages. The selection and extraction of literatures were performed independently by two authors. Meta-analysis was done on the total effective rate (TER), Systolic BP (SBP) and Diastolic BP (DBP). Cochrane's risk of bias (ROB) was used as the methodological quality assessment scale. Results: Twenty studies were finally selected. We observed that a combination treatment using XFZYD and an antihypertensive drug (AHD) was 5.1 times more effective in lowering BP than using AHD alone on TER. The mean differences in SBP and DBP were -10.65 mmHg (95% Confidential Interval (CI) -13.55 mmHg, -7.74 mmHg, P<0.00001), -5.92 mmHg (95% CI -7.14, -4.38, P<0.00001), respectively. Conclusion: A combination treatment using XFZYD and AHD may be more effective in reducing BP than using AHD alone. Because of the poor methodological quality of the studies conducted thus far, high-quality clinical trials will be required in the future.
Cheonghunhwadam-tang have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CHT Gamypang(CTG) was CHT adding Aurantii Fructus(AF). The effects of CTG on the regional cerebral blood f1ow(rCBF) and mean arterial blood pressure(MABP) is not known. The mechanical Study of CTG on the cerebral hemodynamics is not known too. Therefore, purpose of this Study was to investigate effects of CTG on the rCBF and MABP, mechanism of CTG on the cerebral hemodynamics in rats. The changes of rCBF and BP was determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; CTG extract was significantly decreased rCBF in a dose-dependent, but the change of MABP was not shown. Pretreatment with propranolol(3mg/kg, i.v.) was significantly increased CTG(10 mg/kg, i.v.) induced decrease of rCBF. Pretreatment with indomethacin(3mg/kg, i.v.) and methylene blue(10㎍/kg, i.v.) were increased CTG induced decrease of rCBF too. This results suggest that the mechanism of CTG is mediated by adrenergic β - receptor, guanylate cyclase and cyclooxygenase.
Purpose: To examine effects of application of bronchodilators after endotracheal suction on peak air way pressure (PAP), lung compliance (Cdyn), oxygen saturation ($SpO_2$), heart rate (HR), blood pressure (BP), and respiration rate (RR) in cardiac surgery patients. Method: Data were collected from October 2007 to March 2008. Participants were patients who were treated with bronchodilators through mechanical ventilators in the SICU. At each suctioning, bronchodilators were applied immediately, and at 5, 10, and 15 minutes after suctioning. Besides PAP, Cdyn, $SpO_2$, HR, BP, RR, data on the frequency of side effects were also collected at 15, 30, 60 minutes after nebulization. Data were analyzed using the SAS program. Results: For application of bronchodilator right after suctioning and after, 5, 10 and 15 minutes, PAP and Cdyn did not show any significant differences although PAP was persistently decreased for 15 to 30 minutes and Cdyn increased at 15 minutes and decreased gradually after nebulization. Besides $SpO_2$, HR, BP, and RR, frequency of side effects did not show any significant difference. Conclusion: Bronchodilators can be applied at any time. However, the positive effects of bronchodilation right after suctioning on PAP and Cdyn, as also shown in previous reports, indicate it is a more efficient clinical process to maintain an adequate airway.
목 적 : 주기성 사지운동(periodic limb movement syndrome, PLMS)은 수면 중 팔다리를 비롯한 신체 일부가 불수의적이고 주기적으로 움직이는 현상을 말한다. 여러 연구에 의하면 PLMS는 고혈압의 위험인자로 잘 알려져 있고 각각의 PLMS 이후의 혈압이 20 mm Hg까지 증가한다는 연구도 있다. 본 연구에서는 PLMS가 수면 전후 혈압의 변화에 미치는 영향을 알아보고자 하였다. 방 법 : 야간 수면다원검사상 수면무호흡증이 없고(respiratory distress index < 5), 렘수면행동장애, 기면병 등의 수면장애가 없는 358명(남자 176명, 여자 182명)의 18세 이상 성인을 대상으로 하였다. 이들의 수면다원검사 기록과 수면다원검사 전 저녁 수축기, 이완기 혈압, 수면다원검사 후 아침 수축기, 이완기 혈압, body mass index, 음주, 흡연, 혈압강하제 복용 유무, 벡우울척도(Beck Depression Index), 엡워스졸음척도(Epworth Sleepiness Scale), 피츠버그수면질지수(Pittsburgh Sleep Quality Index)를 포함하는 임상 기록들을 수집하였다. 주기성 사지운동지수(Periodic Leg Movement Index) 15를 기준으로 15 이하인 군과 15 초과인 군으로 나누어 비교하였다. 두 군의 임상적 변수 비교는 t-test와 카이제곱 검정을 사용하였으며 repeated measure ANOVA를 사용하여 각 군에서의 검사 전후 혈압 변화 및 두 군의 혈압 변화 양상을 비교하였다. 결 과 : 대상자 전체에서 PLMS가 시간당 15초과한 군이 PLMS 15이하인 군에 비하여 수면 전후 수축기 혈압이 덜 감소하였다. 이러한 결과는 성별과 연령을 보정하였을 때에도 유의하게 나타났으나 BMI, 음주, 흡연, 혈압강하제 사용, sleep efficiency 변수들을 보정하였을 때에는 통계적으로 유의하지 않았다(0.098). 여성에서 두 군간 연령 이외의 다른 변수들은 유의한 차이를 보이지 않았다($49.0{\pm}14.6$세, $57.1{\pm}14.8$세, p = 0.001). 여성의 경우, 두 군 모두 검사 전, 후 수축기 혈압은 유의한 감소를 보였다(p < 0.001). PLMI 15 이하군의 수축기 혈압이 9.7 mm Hg 감소한 것에 비해 PLMI 15 초과군의 수축기 혈압은 2.9 mm Hg 정도만이 감소하여 수면 전, 후 수축기 혈압 변화 양상이 유의하게 차이가 났다($135.2{\pm}20.3mm\;Hg{\rightarrow}125.5{\pm}18.3mm\;Hg$, $133.5{\pm}17.9mm\;Hg{\rightarrow}130.6{\pm}14.8mm\;Hg$, p = 0.010) 이러한 결과는 연령, BMI, 음주, 흡연, 혈압강하제 복용 유무, sleep efficiency 변수를 보정하여도 유의하게 나타났다(p = 0.024). 남성의 경우 두 군 모두에서 수축기 혈압이 수면 전, 후 유의하게 감소하였지만 PLMS에 의한 교호작용(interaction effect)은 나타나지 않았다. 결 론 : PLMS는 수면 중의 정상 혈압 강하 패턴의 감소와 유의한 연관성이 있었으며 혈압과 관련된 임상 변수를 보정하였을 때에는 여성에서만 유의한 연관성을 보였다. 여성에서 PLMS와 관련하여 발생할 수 있는 고혈압 및 심혈관 질환의 임상적 의미를 확인할 수 있었다.
This study evaluated the thermal physiological and psychological responses elicited when wearing cold protective jackets with aerogel fillings in two cold environments, one without air velocities and one with air velocities (2.3 m·s-1), at an air temperature of 10℃. The participants were five healthy young males. Measures were taken of physiological parameters, blood pressure (BP), heart rate (HR), core temperature, oxygen uptake (Vo2), and microclimate (temperature and humidity). The psychological parameters evaluated were thermal and wetness sensation. No differences were observed in systolic blood pressure, heart rate, and oxygen intake between the conditions. At tympanic temperature, a significant difference was observed between the conditions during exercise (p<.05); . A significant difference was observed in the microclimate temperature of the clothing according to the airflow, and temperature changes in the chest and back revealed different patterns. Significant differences were observed in thermal sensation (whole body (p<.05), chest (p<.05), back (p<.01)) between airflow conditions. The results therefore indicate that cold protective jackets with an aerogel filling are suitable for people operating in low-temperature and airflow environments.

Background: Stellate ganglion block (SGB) is known to increase blood flow to the innervations area of the stellate ganglion. Near infrared spectroscopy reflects an increased blood volume and allows continuous, non-invasive, and bedside monitoring of regional cerebral oxygen saturation ($rSO_2$). We investigated the influence of SGB on bilateral cerebral oxygenation using a near infrared spectroscopy. Methods: SGB was performed on 30 patients with 1% lidocaine 10 ml using a paratracheal technique at the C6 level and confirmed by the presence of Horner's syndrome. The blood pressure (BP), heart rate (HR) and rSO2 were measured before SGB and 5, 10, 15 and 20 minutes after SGB. Tympanic temperature of each ear was measured prior to SGB and 20 minutes after SGB. Results: The increments of the $rSO_2$ on the block side from the baseline were statistically significant at 5, 10, 15 and 20 minutes. The $rSO_2$ on the non-block side compared with the baseline, however, decreased at 15 and 20 minutes. The difference between the block and the non-block sides was significant at 15 and 20 minutes. The BP at 10, 15 and 20 minutes was increased and the HR was increased at 10 and 15 minutes. Conclusions: We observed an increment of the $rSO_2$ on the block side from the baseline; however, the $rSO_2$ on the non-block side decreased.
