종합 건강검진 과정에서 나타난 일부 중년기 여성의 건강문제 (Health Problem of the Middle-Aged Women)

  • This study was carried out to offer the basic data for more qualitative lives of the middle-aged women through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the middle-aged women, the differences of main symptoms in each subject, positive ratcs in screening tests, the difference of screening tests with age and the correlation of obesity and blood pressure with screening tests. All subjects were 218 women(40-59years) who had a check-up at a comprehensive health check-up center in K University Hospital in Seoul from July 1, 1998 to August 31. 1998. Questionaires were developed to get subjects, general characteristics. main symptoms, medical examination and diagnosed disease by the researcher referring through the questionaires of comprehensive health check-up center and the medical recorders were used as a tool of study. The researcher analyzed the data with SPSS PC+ ; the distribution of the subjects, general characteristics and main symptoms with percentage : the difference between main symptoms and screening tests with X$^2$-test, ANOVA. The results are as follows. 1) Among the age distribution, most were 40-45 years old and unemployed. 2) The most common complaints were indigestion in digestive system, cough in respiratory system, heart beat in cardiovascular system, general sweat in endocrine system. edema in urinary system, easy bruise in hematologic system, backache in musculoskeletal system, headache in psychoneurologic system, lochia in gynecology, itch in dermatology system, eye ball pain in visual system and tinnitus in auditory system. 3) In main symptoms according to age, the 40-45 age group had a higher rate of complaints in digestive system and gynecology than other age group ; the 46-49 age group in endocrine system. 4) The main symptoms according to diagnosis had no difference statistically. 5) The screening tests which showed high rates of abnormality were mammography(the highest), abdominal ultrasonography, upper gastrointestinal series/gastrofiberscopy, PFT. In all age group the highest rate of abnormality was seen in mammography. 6) In screening tests according to age. the 46-49 age group showed higher positive rates of pap smear, blood sugar test, urine test than any other age group ; the 50-55 age group showerd higher positive rates of obesity, BP, chest X-ray, abdominal ultrasonography, ESR, SGOT, CRP and urine micro. 7) In correlation of obesity with screening test, the obesity group showed higher positive rates of blood pressure, chest X-ray, abdominal ultrasonography, HCT and CRP than the normal weight group. 8) In correlation of blood pressure with screening test, the hypertension group showed higher positive rate of obesity, chest X-ray, abdominal ultrasonography, pap smear and blood sugar than the normal blood pressure group. In conculusion, the middle-aged women have various physical symptoms and affected by age. The obesity and BP have an important effect on the health of the middle aged women. Therefore, this study is considered significant as data for qualitative lives of the middle-aged women playing an important part of family health care by catching of their health problem complaints for prevention of disease and promotion of health.

청훈화담탕가미방이 국소뇌혈유량 및 평균혈압에 미치는 작용기전 연구(IV) (The Mechanism Study of Cheonghunhwadam-tang Gamypang on the Regional Cerebral Blood Flow and Mean Arterial Blood Pressure in Rats(IV))

  • Cheonghunhwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CTG was CHT adding Aurantii FructusㆍGastrodae Rhizoma. CTG was significantly increased regional cerebral blood f1ow(rCBF) in a dose-dependent, and CTG was decreased mean arterial blood pressure(MABP) compared with normal MABP(100%) in a low dosage, but was increased MABP in a dose-dependent, was aliked with normal MABP in a high-dosage. Therefore, purpose of this Study was to investigate experimental mechanism of CHTAG on the cerebral hemodynamics(rCBF, MABP) in rats. The changes of rCBF and MABP were determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; Pretreatment with indomethacin(3㎎/㎏, i.v.) was significantly inhibited CTG induced increase of rCBF and pretreated with propranolol(3㎎/㎏, i.v.) was inhibited CTG induced increase of rCBF, but pretreatment with methylene blue(10㎍/㎏, i.v) was increased CTG induced increase of rCBF. Pretreatment with indomethacin was decreased CTG induced increase of MABP, but pretreatment with propranolol and methylene blue were increased CTG induced increase of MABP. This results suggest that the mechanism of CTG is mediated by cyclooxygenase.

성인 직장 남성의 생활습관과 체질량지수, 혈압 및 혈중지질농도의 상관성 (Relationships among Lifestyle, BMI, BP, and Serum Lipid Profiles in Working Men)

  • Purpose: The purpose of this study was to examine lifestyle, BMI, BP, and lipids profiles in male subjects and to explore the relationships among variables. Methods: A total of 148 male subjects were recruited from one life insurance company from December 1, 2005 to February 28, 2006. Data collection methods were structured questionnaire, anthropometry and serum analysis. The relationships among lifestyle, BMI, BP, and serum lipid profiles were assessed by descriptive analysis, t-test, ANOVA, and partial Pearson's correlation coefficient of variables after controlling for age, educational level, and economic status. Results: The mean BMI of the participants was relatively high as 25.38(range: 18.38 - 32.83). The differences of serum lipid profiles according to age, educational level, and economic status were significant. 'Use of caffeine and drugs'(r = -.187, p < .05) and 'consciousness of safety'( r= -.200, p < .05) was negatively related to BMI. Higher score of 'type of personality' domain was correlated with lower systolic BP(r = -.221, p < .01) and lower diastolic BP(r = -.195, p < .05) and was positively correlated with HDL(r = .191, p < .05). Conclusion: 'Use of caffeine and drugs', 'consciousness of safety' and 'type of personality' of lifestyle as well as 'dietary habit' and 'exercise' played a key role in circulatory disease.

한국 소아 청소년 정상 혈압 참고치 (Normative blood pressure references for Korean children and adolescents)

  • 목 적 : 소아 청소년 건강관리에서 혈압 측정의 중요성이 점차 강조되고 있는 시점에서 한국의 소아 청소년들을 위한 정상 혈압 참고 치를 만들고자 하였다. 방 법 : 2005년에 새로운 신체성장 표준치를 만들기 위해 시행한 전국 소아 청소년들의 신체계측사업에 혈압 계측사업을 포함시켜 7-20세에 해당하는 소아 청소년 57,433명(남: 29,443명, 여: 27,990명)에서 신장, 체중 및 혈압 계측치를 얻었다. 혈압은 자동 진동혈압계인 Dinamap Procare 200(GE Inc., Milwaukee, Wi, USA)를 이용하여 측정하였고 앉은 자세에서 안정한 후 우측 팔에서 5분 간격으로 2번 측정하였다. 두 측정치의 평균치를 수축기 혈압과 이완기 혈압으로 기록하여 원 자료로 하였다. 체중이 +3SD 이상인 과체중아들 2,373명의 혈압은 제외하고 분석 하였다. 혈압 백분위수 표는 성별, 연령별, 신장 백분위수별로 만들어 졌으며 통계기법은 2004 Task Force in NHLBI (USA)이 사용한 통계기법을 원용한 고정 변형 LMS 방법을 사용하였다. 결 과 : 성별, 연령별(7-18세), 신장 백분위수별(5th, 10th, 25th, 50th, 75th, 90th, 95th) 혈압 참고치가 완성되었으며 신장 백분위수에 대한 자료는 질병관리본부 홈페이지인 http://www.cdc.go.kr/webcdc/에서 확인 할 수 있다. 고혈압의 기준치는 성별, 연령별, 신장대비에서 95 백분위수 이상으로 하고, 전고혈압 기준치는 90-95 백분위수 또는 수축기 및 이완기 혈압 130/80 mmHg 이상으로 정하였다. 결 론 : 이번에 마련된 우리나라 소아 청소년 정상 혈압 참고치는 처음이기 때문에 사용기준 및 고혈압의 기준은 앞으로 지속적인 논의와 연구가 필요할 것으로 생각된다. 또한 본 혈압 표는 자동 진동혈압계로 마련된 것이기 때문에 수동 수은 혈압계를 이용한 혈압 치를 평가하는 기준표로 사용하는 것은 주의해야 할 것이다.

Changes in body weight, blood pressure and selected metabolic biomarkers with an energy-restricted diet including twice daily sweet snacks and once daily sugar-free beverage

  • BACKGROUND/OBJECTIVES: The type of sweet snack incorporated into an energy-restricted diet (ERD) may produce differential effects on metabolic improvements associated with body weight (BW) loss. This study compared effects of incorporating either twice daily energy-controlled dark chocolate snacks plus once daily sugar-free cocoa beverage (DC) to non-chocolate snacks plus sugar-free non-cocoa beverage (NC) into an ERD on BW loss and metabolic outcomes. MATERIALS/METHODS: In an 18-week randomized comparative trial, 60 overweight/obese premenopausal women were assigned to DC (n = 30) or NC group (n = 30). Dietary intake was measured at baseline and week 18, and BW, anthropometrics, blood pressure (BP) and serum glucose, insulin and lipid concentrations were measured at baseline, and weeks 6, 12 and 18. Data were analyzed using repeated measures ANOVA. RESULTS: Using intention-to-treat analysis, women in DC and NC groups reduced energy intake (both P < 0.001) and lost $4.4{\pm}0.6kg$ and $5.0{\pm}0.9kg$ (both P < 0.001), respectively. Both groups lowered systolic and diastolic BP [DC = 2.7 (P < 0.05), 2.7 (P < 0.01); NC = 3.4 (P < 0.01), 4.2 (P < 0.01) mmHg, respectively]. Glucose and insulin concentrations decreased by 0.72 mmol/L (P < 0.001) and 13.20 pmol/L (P < 0.01) in DC group and by 0.83 mmol/L (P < 0.001) and 13.20 pmol/L (P < 0.01), respectively, in NC group. Total cholesterol increased in NC group (P < 0.05), with no significant lipid changes in DC group. There were no significant differences in biomarker outcomes between groups. CONCLUSIONS: Overweight/obese premenopausal women following an 18-week ERD that included either DC or NC sweet snack and sugar-free beverage lost equivalent amounts of BW and improved BP measurements and glucose and insulin concentrations.

Relationship between the regulation of blood pressure and in vivo noradrenergic neural activities in the locus coeruleus of young Spontaneously Hypertensive Rats

  • The purpose of the present study was to determine whether in vivo noradrenergic neural activity in the locus coeruleus is related to the development of hypertension. Two groups of animals were prepared, 1) young spontaneously hypertensive rats (SHR) and 2) age-matched normotensive wistar kyoto rats (WKY). At il weeks of age, the release of norepinephrine (NE) and 3,4-dihydroxyphenylglycol (DOPEG) from locus coeruleus of young SHR and WKY as an index of neural activity were determined by in vivo microdialysis along with blood pressure (BP) at three conditions : 1) normal; 2) elevated BP by systemic injection of phenylephrine and 3) alpha-1 adrenoceptor stimulated by perfusion of phenylephrine into the locus coeruleus through microdialysis probe. Basal releases of NE and DOPEG from the iocus coeruleus were 0.415+/-0.089pg/20min, 1.311+/-0.293 pg/20min in SHR and 0.204+/-0.078 pg/20min, 1.492+/-0.365 pg/20min in WKY respectively. Basal release of NE from the locus coeruleus of SHR was significantly greater than that of WKY. Phenylephrine systemic injection caused elevation of BP in both SHR and WKY in a dose related manner. Following phenyephrine injection, the releases of NE and DOPEG from the locus coeruleus of SHR were significantly decreased, whereas there were no significant changes in the releases of NE and DOPEG In young WKY. Alpha-1 adrenoceptor stimulation in the locus coeruleus by perfused phenylephrine through microdialysis probe caused pressor responses in both SHR and WKY, but the magnitude of pressor response in SHR was larger compared with that in WKY. The result from the present study suggests that noradrenergic neural activity in locus coeruleus may contribute as one of triggering factors for the expression of hypertension in young SHR.

Positive impact of integrated amrita meditation technique on heart rate, respiratory rate and IgA on young healthy adults

  • The objective of the current study was to find out the effect of Integrated Amrita Meditation Technique (IAM) on blood pressure (BP), heart rate (HR), respiratory rate (RR) and IgA. One hundred and fifty subjects were randomized into three groups IAM, Progressive Muscle Relaxation (PMR) and Control. Baseline values were collected before giving the training for all the subjects and the IAM and PMR groups were given training in the respective techniques. BP, HR, RR and IgA were recorded manually at 0 h, 48 h, 2 months and 8 months after the first visit. HR was found to be reduced in the IAM group 48 h onwards and the fall sustained till 8 months (p < 0.05). IAM group showed significant drop when compared to the PMR group and control group in all the subsequent visits (p < 0.05). RR decreased significantly in the IAM group in the third and fourth visits (p < 0.05). RR of IAM showed significant decrease when compared to PMR and control from the third visit onwards. IgA showed significant increase in comparison with PMR and control in the third and fourth visits. BP did not show any difference in any of the visits. There was subject dropout from randomization to completion of the study, in all the three groups. The significant decrease in HR and RR and increase in IgA in the IAM group when compared to the PMR and control group shows the efficacy of the technique in reducing the physiological stress indicators for up to 8 months.

고혈압 침치료에 관한 최근 연구 동향 고찰 - Pubmed와 국내 논문 중심으로 - (A Review of Recent Acupuncture Treatment for Hypertension - PubMed and Domestic Studies)

  • Objectives: The purpose of this study was to summarize current research trends and results regarding acupuncture treatment for hypertension. Methods: Articles published from 2009 to 2019 were searched on domestic databases (OASIS, NDSL, RISS) and PubMed using the keywords 'Hypertension', 'Hypertensive', 'Acupuncture', 'EA' (electroacupuncture), 'Autonomic nervous system', 'ANS', 'CVD' (cardiovascular disease), '고혈압', '침', '전침', and '자율신경계'. The studies were analyzed according to the year, type of study (domestic or foreign articles, nationality of the first author), acupuncture type for human and animal studies, BP (blood pressure) measurement, and acupuncture intervention. Results: In total, 21 articles were selected. There were 8 randomized controlled trials (RCTs), 3 non-randomized controlled trials (nRCTs), 2 before-after studies, 1 case series, and 7 animal studies. Manual acupuncture (11 articles) was most frequently used, and ST36 (足三里) was most frequently used in human studies. In 18 of 21 articles, acupuncture was reported as effective in lowering systolic blood pressure (SBP) or diastolic blood pressure (DBP) in hypertensive patients. Conclusions: Acupuncture treatment for hypertension is deemed useful. Further studies are necessary for application of acupuncture therapy in clinical practice.

일 농촌지역 성인여성의 건강관련행위와 혈압 위험요인에 관한 연구 (A Study on Health Behaviors and the Risk Factors of Blood Pressure of Adult Women in a Rural Area)

  • Purpose: This study was conducted to evaluate the health behaviors and to find out risk factors of blood pressure of adult women in a rural area. Method: The convenient sample consisted of 159 adult women who lived in G-gun. The data was collected using a self-report questionnaire for health behaviors and mercury type sphygmomanometer for BP, between Jun I and August 15, 2003. Health behaviors measured smoking, alcohol, salt, lipid, stress, exercise, coffee, BMI and medication. To accomplish the goal of study, descriptive statistics, t-test, $x^2$-test, ANOVA and multiple regression analysis were. performed with SPSS 10.0. Results: The average age of subjects was 49.2(SD7.34)years old. The average SBP and DBP of subjects were 126.22mmHg(SDl6.73) and 8 1.25mmHg(SDl 0.31). There were significant differences in smoking(p=.000), cigarette consumption(p=.001), smoking duration(p=.000), BMI(p=.033), medication (p=.001), family history(p=.000) between normotensive and hypertensive. The main risk factors on SBP were medication, age, BMI, family history and smoking duration by 35.7% of the total variance these variables explained SBP. The main risk factors on DBP were BMI, education and medication by 17.60% of the total variance these variables explained DBP. Conclusion: These results suggest that health professional have to emphasize prevention of obesity, lasting medication and no smoking for prevention and management of hypertension in community health promotion program.

Changes in Cerebral Hemodynamics and Sympathetic System During a Combination of Subway Noise with Mental Activity

  • Subway environments such as crowd, passenger's gab, or subway-generated mechanical noise may become a potential stressor. The present study was sought to determine whether subway noise with or without mental activity affects cerebral hemodynamics and sympathetic system. Fifty-four healthy volunteers were divided group I which underwent subway noise (n=24) and group II which underwent a combined mental activity (mental arithmetic) with subway noise (n=30). Sympathetic factors such as heart rate (HR), blood pressure (BP) and heart rate-systolic pressure product (RPP), and mean blood flow velocity in the middle cerebral artery (MCAV) were measured before (baseline), during and after the noise-exposure. Systolic and diastolic blood pressure, HR and RPP significantly increased in group II (P<0.05) but not in group I during the noise-exposure. Peak-MCAV, diastolic-MCAV and mean-MCAV in the both groups were elevated during the noise-exposure (P<0.05) and the increased ratios in group II were greater than those in group I. These results suggest that a combined mental activity with subway noise may be a stressor which affects cerebral hemodynamics and sympathetic system.
