아동기 외상경험과 성인기 양육태도 및 신경증과의 관계 (The Relationship of Childhood Adversity Withadulthood Parenting Attitude and Neuroticism)

  • Objective : The objective of this study was to examine the association between childhood adversity and parenting attitude and neuroticism in adulthood. Methods : Forty nine women were recruited from community and completed Early Trauma Inventory-Short Form (ETI-SF). We compared scores on the Maternal Behavior Research Instrument (MBRI), Neuroticism of NEO Personality Inventory (NEO-PI), and Beck Depression Inventory (BDI) between women with and without childhood adversity. Results : Compared to women without childhood adversity, women with childhood adversity showed more rejecting parenting attitude and higher Depression and Self-Consciousness scores on Neuroticism domain of NEO-PI. Total ETI-SF scores and emotional abuse scores were positively correlated with rejecting attitude scores on the MBRI and BDI scores. Physical abuse scores were positively correlated with rejecting attitude scores. There were positive correlations between rejecting attitude scores on the MBRI, Neuroticism scores on the NEO-PI, and BDI scores. Conclusion : Our results suggest that childhood adversity may have a negative impact on parenting attitude and emotional state in adulthood. For girls who experienced traumatic event, early intervention is needed to prevent the development of neurotic temperament and rejecting parenting in adulthood.

한국 청소년의 우울증상과 부모자녀관계 (The Parent-Child Relationship and Depressive Symptoms in Korean Adolescents)

  • Objectives : The objective of this study was to compare the parent-child relationship of adolescents with depression and those without depression, by using Parent-Child Relationship Instrument (PCRI), Family Relationship Scale (FRS), and Parenting Attitude Test-Youth (PAT-Y). We also investigated the association between the parent-child relationship and depressive symptoms. Methods : Twenty-five adolescents with depression (age $15.0{\pm}2.1years$, 8 boys) and 24 adolescents without depression (age $13.7{\pm}1.3years$, 16 boys) completed the Adolescent-General Behavior Inventory (A-GBI), Adolescent-Mood Disorder Questionnaire (A-MDQ), Beck Depression Inventory (BDI), PCRI, and PAT-Y. The parents of subjects completed the Child Behavior Checklist, Parent-General Behavior Inventory 10-item Mania Scale (P-GBI-10M), Parent-Mood Disorder Questionnaire, and FRS. Independent t-tests, analysis of covariance, and partial correlation analysis were used. Results : The Intimacy (p=.002) and Respect (p=.029) scores of the PCRI were significantly higher in adolescents without depression compared to those with depression. The Intimacy scores of the PCRI showed negative correlation with the BDI and P-GBI-10M and the Strictness scores of the PCRI showed positive correlation with the A-GBI, A-MDQ, and BDI. The Inconsistency, Punishment, and Excessive Expectation scores of the PAT-Y showed positive correlation with the A-GBI and BDI scores. Conclusion : Our results suggest a possible association of the parent-children relationship and parenting attitude with adolescents' depressive symptoms.

A Case of Korean Medicine Treatment for a Patient with Somatic Symptom Disorder Hospitalized after a Traffic Accident

  • Objectives: The purpose of this study was to report the effect of Korean medicine treatment on a patient with somatic symptom disorder hospitalized after a traffic accident. Methods: The subject for this study was a patient diagnosed with somatic symptom disorder that worsened after a traffic accident. During hospitalization, herbal medicine, acupuncture, psychotherapy, and other treatments were performed. Clinical effect was evaluated using Heart Rate Variability (HRV), BDI (Beck Depression Inventory)-II, BAI (Beck Anxiety Inventory), Global assessment (G/A), and Numerical rating scale (NRS). Results: After 36 days of treatment, BDI-II score was reduced from 17 to 5. BAI score was reduced from 23 to 8. Overall somatic symptoms and pain showed decreases. Conclusions: Korean medicine treatment might be effective for somatic symptom disorder that is worsened after a traffic accident.

경항부 통증과 심리적 상태와의 상관성에 관한 연구 (The relationship between neck pain and psychological state in female office workers.)

  • Objectives : The purpose of this study was to investigate the relationship between pain and psychological state in female office workers with neck pain. Methods : Experimental group of 31 healthy subjects complained of neck and arm discomfort related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Outcomes were assessed by meridian-electromyography(MEMG), Neck Disability Index(NDI), Visual Analog Scale(VAS), Beck Depression Inventory(BDI), Stress Reaction Inventory(SRI), Holmes & Rahe Social Readjustment Rating Scale(SRRS). Results : The NDI score and BDI score had a correlationship(r=0.48), and the NDI score and SRI score also had a correlationship(r=0.48), significantly. The NDI score and contraction power of upper trapezius by MEMG had a correlationship significantly, but with the other muscles the NDI score didn't have correlationships. The NDI score and the SRI score were significantly higher in depression group(BDI score ${\circ}{\surd}$14). The contraction power of trapezius by MEMG was significantly lower in depression group. Conclusions : The results suggest that mental stress can be a major risk factor of neck pain in female office workers.

당뇨병 환자에서 우울증상 및 불안증상과 혈당조절과의 연관성 (The Relationship of Anxiety Symptoms and Depressive Symptoms to Glycemic Control in Diabetic Patients)

  • 연구목적 : 당뇨병 환자들은 질환 자체에 대한 부담감과 혈당 관리를 위한 약물 및 식이요법과 운동 요법 등의 유지에 대한 부담감이 스트레스로 작용하고 있으며, 이러한 심리적 스트레스가 혈당조절에 악영향을 줄 수 있다. 당뇨병 환자들은 일반 인구에 비해 우울증 및 불안장애의 유병율이 높으며, 이러한 우울증과 불안장애의 증상은 또 다시 혈당조절에 부정적인 영향을 주게 된다. 이에 본 연구는 우울과 불안증상이 혈당조절에 어떤 영향을 미치는가를 알아보고자 하였다. 방법 : 단국대학교병원 내분비내과에서 외래통원치료 중인 30세 이상의 당뇨병 환자 65명을 대상으로 하여, 정맥 혈액채취로 당화혈색소(Hemoglobin A1c;이하 HbA1c) 수치를 얻었으며, 자가보고식 설문지인 Beck의 우울척도(Beck's Depression Inventory;이하 BDI) 및 Beck의 불안척도(Beck's Anxiety Inventory;이하 BAI)를 시행하여 통계분석을 하였다. 결과 : 65명의 당뇨병 환자 중 경한 우울증상을 보인 환자는 27명(41.5%), 중등도 이상의 우울증상을 보인 환자는 6명으로(9.23%) 일반 인구에 비해 높은 빈도를 보였다. HbA1c와 BDI간에는 유의한 상관관계를 보이지 않았으나 HbA1c와 BAI간에 유의한 상관관계를 보였으며(R=0.567, P<0.001), 다중회귀분석에서 BAI는 혈당조절에 영향을 미치고 있었으나($\beta=0.551$, T=4.912, P<0.001) BDI와 체질량지수, 당뇨병 합병증의 수 및 유병기간은 HbA1c에 유의한 영향을 미치지 않았다. 결론 : 본 연구에서는 일반 인구에 비해 우울증상을 보이는 당뇨병 환자의 빈도가 높았으며, 불안증상을 보이는 당뇨병 환자의 빈도는 높지 않았다. 당뇨병 환자에서 우울증상과 혈당조절간의 유의한 연관성을 보이지 않았으나 불안증상과 혈당조절 간에는 유의한 연관성을 보였다. 결론적으로 우울증상은 혈당조절에 별 영향을 미치지 않으나 불안증상은 혈당조절에 영향을 미치는 것으로 나타났다.

급성 벨마비 환자의 자가보고형 불안 및 우울평가 (Neuropsychologic Analysis of Acute Bell's Palsy)

  • Background: Bell's palsy produces a complex problem that involves not only facial motor weakness, but also psychiatric issues. However, the relationship between facial neuromotor system impairment and psychological adjustment has not been well understood. Methods: We have performed psychological evaluations in patients with acute unilateral Bell's palsy within 2 weeks after onset. Thirty patients with Bell's palsy (10 men, 20 women) were included, who were diagnosed by neurologic examination, electrophysiologic study and/or brain MRI. We measured facial motor scale of impairment (House-Brackmann, HB scale) and psychosocial adjustment [Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI)] at the time of initial presentation and 1 month after diagnosis. Results: The age of the enrolled patients ranged from 16 to 80 years. The mean grade of initial and follow up HB scale were 3.87 (SD: 0.63, range 2~5) and 1.77 (SD: 1.10, range 1~5). The mean score of initial and follow up BAI, BDI were 11.93 (range; 0 to 47, SD: 9.65, very low anxiety), 14.73 (range; 0 to 41, SD: 9.21 minimal depression) and 7.5 (range; 0 to 36, SD: 8.58, very low anxiety), 9.33 (range; 0 to 30, SD: 8.19 minimal depression). There was positive correlation between improvement of HB scale and improvement of BAI and BDI score. Conclusions: Bell's palsy is associated with the psychological problems such as depression and anxiety, and the improvement of motor symptom is associated with the improvement of these psychological problems.

초기성인기와 중년기 우울장애 환자에서 영성이 희망에 미치는 영향의 차이 (Difference of the Effects of Spirituality on the Hope in Early Adults and Middle Aged Patients with Depression)

  • Objectives : The aim of this study was to investigate the relationship between depressive symptom, spiritual well-being and state hope in early adults and middle aged patients with depression. Methods : A total of 180 outpatient were surveyed with Beck Depression Inventory (BDI), Functional Assessment of Chronic Illness Therapy-Spiritual Well-being Scale (FACIT-Sp) and The State Hope Scale (SHS). Mean scores of BDI, FACIT-Sp and SHS were compared between two groups of different age. Correlation analysis and multiple linear regression analyses were performed to identify the effect of BDI and FACIT on SHS according to age. Results : Mean scores of SHS and FACIT were higher significantly in middle aged group than early adults group, while BDI was not different between two groups. And early adults and middle aged patient's state hope showed a significantly negative correlation with spiritual well-being. In regression analysis, the effect of BDI, FACIT on SHS were significant in both groups and the regression model explained 42% of the variance of state hope in early adults group and 26% in middle aged group, respectively. Conclusion : These findings suggest that in depressive symptom and low state hope relationship, spirituality is important protective factor in both aged adult outpatients with depression.

Effects of Forest Healing Program on Depression, Stress and Cortisol Changes of Cancer Patients

  • Patients diagnosed with cancer face mental problems such as alienation, isolation, anxiety about death and fear, recovering from psychological difficulties. In this study, a forest healing program was provided for cancer patients to recover from psychological stress, depression, social isolation and self-esteem caused by cancer and changes in salivary cortisol through psychological and emotional recovery were measured. From September 19 to November 28, 2017, a forest healing program composed of a total of 10 sessions, two hours per session was provided for 12 cancer patients in the Forest of Taegyo located in Yongin. Psychological tests were performed with Social Adaptation Self-evaluation Scale (SASS), Korean-version Perceived Stress Scale (PSS) and Beck Depression Inventory (BDI) and the collected data were analyzed with the SPSS 18.0. The salivary cortisol level was measured along with the psychological tests and were analyzed by a specialized testing agency. The results of the analysis showed that the pre- and post-assessment score of SASS was 29.17 and 25.92, respectively, and that the pre- and post-assessment score of PSS was 30.50 and 23.92, respectively. The pre- and post-assessment score of BDI was 41.00 and 34.83, respectively, which showed significant differences. In addition, the pre- and post-assessment level of saliva cortisol was 3.13 and 1.68, respectively, showing a significant decrease. In short, the forest healing program was found to be effective in reducing physiological changes caused by social isolation and stress due to the emotional and psychological difficulties that the subjects who were diagnosed with cancer and were recovering from it have. In the future, it will be necessary to develop and implement a forest healing program by conducting a forest healing requirement survey on cancer patients.

  • Objectives: To report the efficacy of using EFT and Korean medicine treatment for ASD induced by a traffic accident. Methods: We treated ASD symptoms with Korean medicine (acupuncture, cupping, interferential current therapy, herbal acupuncture therapy). To evaluate results of this treatment, Korean version of PTSD Checklist-5 (PCL-5-K), Beck Depression Inventory (BDI), Insomnia Severity Index (ISI), and Numeric rating scale (NRS) at first visit, 5 weeks, and 12 weeks were used. Results: Korean version of PTSD Checklist-5 (PCL-5-K), Beck Depression Inventory (BDI), Insomnia Severity Index (ISI), and Numeric rating scale (NRS) showed improvements after treatments. Conclusions: This study suggests that Korean medicine and EFT treatment might be effective for ASD.

전업주부의 양육 부담과 우울감이 수면의 질에 미치는 영향: 가계수입의 조절된 매개효과 검증 (The Influence of the Burden of Nurturing and Depression on Sleep Quality in Female Full-Time Homemakers : The Moderated Mediating Effect of Monthly Income)

  • Objectives: This study investigated sleep quality in female full-time homemakers and evaluated the relationship of sleep disturbance with psychological and socio-environmental factors. Methods: This cross-sectional study adopted a structured survey and sequential recruitment method for randomized participation of community-dwelling full-time female homemakers. Sleep quality and mental health were measured using the Korean version of the Pittsburgh Sleep Quality Index (K-PSQI), Korean version of the Beck Depression Inventory-II (K-BDI-II), Korean version of the Beck Anxiety Inventory (K-BAI), and Korean version of the Beck Hopelessness Scale (K-BHS). The willingness-to-pay (WTP) method was selected to measure the self-evaluated monetary value of household service work. The relationship among the main relevant factors was statistically analyzed through a mediation model. Results: A total of 166 participants were analyzed and classified having poor versus good sleep quality (poor : 24.1%, n = 40 ; good : 75.9%, n = 126 ; cut-off point = 9 on the K-PSQI). Significant between-group differences were observed in mental health status (K-BDI-II, p < 0.001 ; K-BAI, p < 0.001 ; K-BHS, p = 0.003). The moderated mediation model was verified, indicating that depression may mediate the association between nurturing burden and sleep disturbance. The path from nurturing burden to depression may be moderated by average monthly household income. Conclusion: A relatively high portion of full-time female homemakers may suffer from sleep disturbance and interactions between psychological and socio-environmental factors might determine sleep quality, suggesting the need for public health policies targeting improvement of sleep quality and mental health among full-time homemakers.
