Recent advances in the diagnosis and management of childhood acute promyelocytic leukemia

  • Since the successful introduction of all-trans-retinoic acid (ATRA) and its combination with anthracycline-containing chemotherapy, the prognosis for acute promyelocytic leukemia (APL) has markedly improved. With ATRA and anthracycline-based-chemotherapy, the complete remission rate is greater than 90%, and the long-term survival rate is 70-89%. Moreover, arsenic trioxide (ATO), which was introduced for APL treatment in 1994, resulted in excellent remission rates in relapsed patients with APL, and more recently, several clinical studies have been designed to explore its role in initial therapy either alone or in combination with ATRA. APL is a rare disease in children and is frequently associated with hyperleukocytosis, which is a marker for higher risk of relapse and an increased incidence of microgranular morphology. The frequency of occurrence of the promyelocytic leu-kemia/retinoic acid receptor-alpha (PML/$RAR{\alpha}$) isoforms bcr 2 and bcr 3 is higher in children than in adults. Although recent clinical studies have reported comparable long-term survival rates in patients with APL, therapy for APL in children is challenging because of the risk of early death and the potential long-term cardiac toxicity resulting from the need to use high doses of anthracyclines. Additional prospective, randomized, large clinical trials are needed to address several issues in pediatric APL and to possibly minimize or eliminate the need for chemotherapy by combining ATRA and ATO. In this review article, we discuss the molecular pathogenesis, diagnostic progress, and most recent therapeutic advances in the treatment of children with APL.

Creep behaviour of flexible adhesives

  • Since flexible adhesives are used more and more in structural applications, designers should have a better understanding of its behaviour under various conditions as ultimate load, fatigue load, long-term load and environmental conditions. This paper focuses on long-term load conditions and its effect on flexible adhesives. The creep properties of both PU (PolyUrethane) and SMP (Silyl Modified Polymers) adhesives used for identical applications are considered. To investigate the creep behaviour tests under various conditions were done. The results of those tests are presented and compared. To evaluate these results an empirical method is proposed and discussed. An example illustrates the potential of this method. It is also shown that with use of a probabilistic calibration technique this method results into a simple rule, which can be used to calculate the creep for practical applications. For the studied adhesives, the creep performance of the SMP adhesive is shown to be of the same level or slightly better than of the two PU adhesives. In addition to this empirical method, the principles of a more complex theoretical based method are introduced. The potential of this method is illustrated and future research activities are drawn.

어업 외 투자효과 및 투입산출분석 : 태안시범바다목장사업을 중심으로 (The Economic Effect of Besides Fisheries Profit and Input-Output Analysis: ocused on the Tae-an Trial Sea Farm Project)

  • This paper is to estimate the economic effect of benefits of the R&D and recreational fishing as well as input-output analysis in the Tae-an Trial Sea Farm Project(TTSFP). We use B/C model to indicate the effects of economic valuation. B/C analyses model consists of Benefit Cost Ratio(BCR), Net Present Value(NPV) and Internal Ration of Return(IRR). Using 5.5% discounting rates and the survey data, the sub-models show economically feasible in the all of analysis and analyzed the results as follows. NPV is 42,147 million won, BCR is 3.29 and IRR is 34.30%. This study attempts to apply input-output(I-O) analysis in connecting the economic effect of TTSFP. I-O model was constructed, focusing on three effects; the production-inducing effect, the value-added-inducing effect and employment-inducing effect. There are positive effects on economic value and job creation in Tae-an and Nation.

Experimental study of bearing capacity of strip footing on sand slope reinforced with tire chips

  • Tire chips and tire chips-soil mixtures can be used as alternative fill material in many civil engineering applications. In this study, the potential benefits of using tire chips as lightweight material to improve the bearing capacity and the settlement behavior of sand slope was investigated experimentally. For this aim, a series of direct shear and model loading tests were conducted. In direct shear tests, the effect of contents of the tire chips on the shear strength parameters of sand was investigated. Different mixing ratios of 0, 5, 10, 15 and 20% by volume were used and the optimum mixing ratio was obtained. Then, laboratory model tests were performed on a model strip footing on sand slope reinforced with randomly distributed tire chips. The loading tests were carried out on sand slope with relative density of 65% and the slope angle of $30^{\circ}C$. In the loading tests the percentage of tire chips to sand was taken as same as in direct shear tests. The results indicated that at the same loading level the settlement of strip footing on sand-tire chips mixture was about 30% less than in the case of pure sand. Addition of tire chips to sand increases BCR (bearing capacity ratio) from 1.17 to 1.88 with respect to tire chips content. The maximum BCR is attained at tire chips content of 10%.

  • In this study, the analysis on the distribution of indoor airflow velocity and temperature by using numerical simulation has carried out to make fundamental data for establishing the optimum design of laboratory animal facilities. From the results, it was found that replacement of cage lacks, air supply and exhaust system, supply air temperature, supply air velocity affect to the optimum design of laboratory animal facilities as a important element.

Hydrogenation Properties of MgH2-CaO Composites Synthesized by Hydrogen-Induced Mechanical Alloying

  • Although magnesium-based alloys are attractive materials for hydrogen storage applications, their activation properties, hydrogenation/dehydrogenation kinetics, thermodynamic equilibrium parameters, and degradation characteristics must be improved for practical applications. Further, magnesium poses several risks, including explosion hazard, environmental pollution, insufficient formability, and industrial damage. To overcome these problems, CaO-added Mg alloys, also called Eco-Mg (environment-conscious Mg) alloys, have been developed. In this study, $Eco-MgH_x$ composites were fabricated from Mg-CaO chips by hydrogen-induced mechanical alloying in a high-pressure atmosphere. The balls-to-chips mass ratio (BCR) was varied between a low and high value. The particles obtained were characterized by X-ray diffraction (XRD), and the absorbed hydrogen was quantified by thermogravimetric analysis. The XRD results revealed that the $MgH_2$ peaks broadened for the high BCR. Further, PSA results revealed particles size were decreased from $52{\mu}m$ to $15{\mu}m$.

Intraoperative Neurophysiology Monitoring for Spinal Dysraphism

  • Spinal dysraphism often causes neurological impairment from direct involvement of lesions or from cord tethering. The conus medullaris and lumbosacral roots are most vulnerable. Surgical intervention such as untethering surgery is indicated to minimize or prevent further neurological deficits. Because untethering surgery itself imposes risk of neural injury, intraoperative neurophysiological monitoring (IONM) is indicated to help surgeons to be guided during surgery and to improve functional outcome. Monitoring of electromyography (EMG), motor evoked potential, and bulbocavernosus reflex (BCR) is essential modalities in IONM for untethering. Sensory evoked potential can be also employed to further interpretation. In specific, free-running EMG and triggered EMG is of most utility to identify lumbosacral roots within the field of surgery and filum terminale or non-functioning cord can be also confirmed by absence of responses at higher intensity of stimulation. The sacral nervous system should be vigilantly monitored as pathophysiology of tethered cord syndrome affects the sacral function most and earliest. BCR monitoring can be readily applicable for sacral monitoring and has been shown to be useful for prediction of postoperative sacral dysfunction. Further research is guaranteed because current IONM methodology in spinal dysraphism is still deficient of quantitative and objective evaluation and fails to directly measure the sacral autonomic nervous system.

비외상성 피질하 뇌혈관질환 환자에서 인지평가도구로서 정신상태소검사(MMSE)와 간이인지평가척도(BCRS)의 임상적 유용성에 대한 연구 (A Study on the Clinical Usefulness of MMSE and BCRS for Cognitive Function Test in Patients with Non-Traumatic Subcortical Cerebrovascular Disease)

  • 연구목적 : 각종 질병에서 임상적으로 인지기능평가 및 선별검사로 유용하게 사용되고 있는 정신상태소검사(MMSE)와 간이인지평가척도(BCRS)가 비외상성 피질하 뇌혈관질환 환자에서 인지기능평가에 유용한가를 평가하기 위하여 본 연구를 시도하였다. 연구대상 및 방법 : 1995년 1월 1일부터 9월 30일까지 경희대 한방병원 및 고려대부속병원 신경과에 입원하거나 통원치료중인 비외상성 피질하 뇌혈관 질환 환자 85명과 정상대조군 195명을 대상으로 하였다. 또한 환자군중 65명을 뇌영상 촬영소견에 따라 좌반구손상(21명), 우반구손상(31명), 양반구손상(13명) 환자군으로 나누었다. 이들에게 인지기능은 벤튼신경심리검사(Benton Neuropsycho-logical Assessment : BNA)로, 일상생활기능면은 노인상태에 대한 보호자 측정척도(Geriatric Evaluation by Relative's Rating Instrument : GERRI) 및 도구적 일상생활 활동척도 (Instrumental Activities of Daily Living Scale : IADLs)를 이용하여 평가하였다. 연구결과 : 1) BNA는 좌우지남력과 운동지속성을 제외하고는 모든 하위항목에서 유의한 차이를 보였으나 MMSE 총점수와 하위척도 모두든 환자군과 정상대조군간에 유의한 차이를 보이지 않았다. BCRS는 최근기억과 자신돌봄항목에서만 두 군간에 유의한 차이를 보였다. 2) 편측성에 따른 세군간의 비교에서 BNA는 촉각형태검사(좌측), 손가락위치검사(좌, 우측)에서 세군간 유의한 차이를 보였으나 MMSE나 BCRS에서는 유의한 차이를 보이지 않았다. 3) 일상생활기능과 MMSE, BCRS와의 상관관계에서 정상대조군에서는 MMSE, BCRS의 총점수 및 하위 척도점수가 노인상태에 대한 보호자 측정척도(GERRI)의 인지기능 하위척도점수와만 유의한 상관관계를 갖는 반면, 환자군에서는 노인상태에 대한 보호자 측정척도(GERRI)의 인지기능척도점수 이외에도 사회적 기능척도점수 및 도구적 일상생활 활동척도(IADLs) 점수와도 유의한 상관을 가져 두 군간에 차이를 보였다. 결론 : MMSE와 BCRS는 비외상 피질하 뇌혈관질환 환자에서 인지기능평가나 편측성 구분에 유용하지 않았다. 그러나 이들 검사는 환자군에서 일상생활기능과 같은 기능수준을 반영하고 있음을 보여 주었다.

유색미의 화학적성분 및 항산화활성 (Chemical Compositions and Antioxidant Activity of the Colored Rice Cultivars)

  • 신품종 갈색미를 이용한 다양한 가공식품을 개발하기 위한 연구의 일환으로 먼저 갈색미의 품질 특성을 평가하기 위해 4가지 쌀 품종(백색미, 갈색메벼, 갈색찰벼 및 흑미)의 화학적성분 및 항산화활성을 측정하고 비교 검토한 결과는 다음과 같다. 새로 육종된 갈색메벼 및 갈색찰벼의 일반성분은 대체적으로 백색미와 흑미의 중간 함량을 나타내는 경향이었다. 지방산 중 palmitic, oleic 및 linoleic acids 함량이 거의 96% 이상 차지하고 있었으며, 백색미, 갈색메벼 및 갈색찰벼는 linoleic acid > oleic acid > palmitic acid 순으로, 반면, 흑미는 oleic acid > linoleic acid > palmitic acid순으로 함량이 낮았다. 백색미의 ${\alpah}$-tocopherol 함량은 51.3mg%(건물 중)이었으나 갈색메벼 및 찰벼는 각각 34.5 및 37.2 mg%, 그리고 흑미는 38.6 mg%로서 백색미보다 유색미의 함량이 낯은 경향을 나타내었다. 다음, 유리아미노산 중 glutamic 및 aspartic acids와 같은 산성아미노산의 함량이 가장 높았으며, 흑미는 백색미보다 그 함량이 훨씬 높은 반면, alanine, proline 및 valine은 백색미보다 낮았으며, GABA 함량은 약간 높았다. 한편, 4가지 쌀 품종중 흑미(2.54 %, 건물중)의 수용성페놀 함량이 가장 높았으며, 그 다음으로 갈색찰벼(1.09%) > 갈색메벼(0.69%) > 백색미(0.49%), 순으로 감소하였다. 그리고 흑미는 가장 강한 $DPPH(IC_{50}=90.04{\mu}g/mL)$, superoxide $(IC_{50}=199.92{\mu}g/mL)$ 및 hydroxyl $(IC_{50}=420.73{\mu}g/mL)$ 라디칼 포착활성을 나타내었으며, 그 다음으로 갈색메벼 > 갈색찰벼 > 백색미, 순으로 감소하는 경향을 나타내었다. 이상의 결과로부터 새로 육종된 신품종 갈색메벼 및 갈색찰벼는 대체적으로 백색미와 흑미의 중간적 화학적 품질 특성 및 항산화활성을 나타내었다. 이러한 결과를 미루어 볼 때 새로 육종된 갈색미는 흑미와 함께 향후 기능성 쌀로 활용할 수 있을 것으로 사료된다.

DECO 필름 제조시 발생하는 PET·OPP 합성 폐필름 재활용의 경제성 분석 연구 (An Economic Analysis Study of Recycling PET·OPP Laminated Film Waste Generated during DECO Film Manufacturing)

  • 폐플라스틱에 대한 국가 간 수출입 제한에 따라, 폐플라스틱의 국내 처리 및 재활용 방안을 모색할 필요성이 커지고 있다. 본 연구에서는 국내 A 기업의 폐필름 발생 현황 및 운영 정보를 참고하여 가구 표면 마감용 sheet 및 edge 제조시 발생하는 PET·OPP 합성 폐필름의 재활용사업에 대한 비용 편익을 분석하였다. 폐플라스틱 처리는 그간 영세업체에 맡겨져 현황 파악이 어렵고 수익성 담보가 되지 않아, 재활용 기술이 있더라도 활용되기에 어려움이 따랐다. 본 연구에서는 폐플라스틱 물질 재활용 방안의 하나로써, 폐필름 발생 사업장에서 PET·OPP 합성 폐필름을 박리하여 폐PET 재활용이 이루어지는 것으로 가정하되, 재활용 비율은 정부가 「전주기 탈 플라스틱 대책」에서 제시한 수치를 활용하여 2%, 10%, 20%, 30% 재활용할 경우 비용편익을 분석하였다. 본 연구에서는 폐PET 재활용 방안으로 물리적 박리 기술이 도입되는 것으로 가정하여 폐PET 재활용 비율에 따른 비용편익분석을 수행하였다. 그 결과 폐PET의 재활용 비율이 30% 이상일 경우에만, 비용 편익비율(Benefit-cost ration, BCR)이 BCR ≥1 에 해당하여 최소비용 편익비율을 충족하는 것으로 나타났으며 이 때, BCR 값은 1.32였다. 향후 정부의 지원금 할당비율과 단가가 상향될 것으로 예상됨에 따라 비용 편익비율은 더욱 높아질 것으로 사료된다. 해당 사례는 유사 업종의 사업장에 폐PET 재활용 및 수익 창출을 위한 시범 Case가 될 것으로 기대된다.
