Peritoneal Dialysis in Dogs: 20 cases (2006-2008) (개에서 복막투석 적용: 20 증례 (2006-2008))

  • Peritoneal dialysis (PD) is a clinical technique that therapeutically removes toxic solutes from body fluids and normalizes endogenous solutes whose aberrant concentrations disrupt normal physiology. This study retrospectively evaluated clinical outcomes and complications of PD in 20 dogs with renal failure. Blood works (total count of red blood cells (RBC), packed cell volume (PCV), the serum biochemical, and electrolyte values related to renal insufficiency) and complications associated with peritoneal dialysis, and clinical outcomes were recorded before and after PD. Additionally, creatinine reduction ratio (CRR) and urea nitrogen reduction ratio (URR) were calculated for evaluating the efficacy of PD. PD resulted in a significant (p < 0.05) reduction in blood urea nitrogen (BUN) concentration in 19 dogs, while a significant (p < 0.05) reduction in creatinine concentration in 17 dogs. The complications of PD were hypoalbuminemia (12/20, 60%), anemia (10/20, 50%), subcutaneous dialysate leakage (9/20, 45%), bacterial peritonitis (6/20, 30%), dialysate retention (5/20, 25%) and limb edema (4/20, 20%). This study demonstrated that PD was effective in reducing the magnitude of azotemia in dogs with renal failure especially in acute phase, although the complication rate was high but manageable.

The Effects Of Geumguesingitang And Dohongsamultang On Rats With Nephrosis Induced By Puromycin Aminonucleoside (금궤신기탕 및 도홍사물탕(桃紅四物湯)이 Puromycin Aminonucleoside로 유발(誘發)된 백서(白鼠)의 신증(腎症)에 미치는 영향(影響))

  • The effects of Geumguesingitang and Dohongsamultang on rats with nephrosis induced by a single tail-intravenous injection of puromycin aminonucleoside(PAN), 2.5mg/l00g of body weight were evaluated in the present study. The effects of Geumguesingitang and Dohongsamultang on PAN nephrosis were evaluated by measuring (1)the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, blood urea nitrogen(BUN) and uric acid in the serum, (2)the concentrations of protein, creatinine, glucose, occult blood and volume of the 24 hours urine and (3)the volume of intake water. The results are summerized as follows; 1. In the Control group as compared to the Normal. the amount of protein of 24 hours urine was significantly increased, the concentrations of albumin and total protein were significantly decreased. Total lipid, cholesterol and triglyceride in the serum were significantly increased. The concentrations of creatinine, BUN, uric acid in the serum, the amount of glucose and occult blood of 24 hours urine were also increased significantly. 2. In the Geumguesingitang group as compared to the Control, the increase in the amount of urinary protein during 24 hours induced by PAN was significantly suppressed, and the concentrations of total protein and albumin in the serum were significantly increased. The concentrations of total lipid, cholesterol and triglyceride in the serum were significantly inhibited. The decrease of the concentrations of creatinine and uric acid in the serum were also observed significantly. 3. In the Dohongsamultang group as compared to the Control, the increase of the amount of protein and glucose of the 24 hours urine induced by PAN were significantly inhibited, and the concentrations of total protein and albumin in the serum were increased significantly. The concentrations of total lipid, cholesterol and triglyceride in the serum were decreased significantly. The decrease of the concentrations of creatinine and uric acid in the serum were observed significantly. It can be inferred that Geumguesingitang has effects on improving proteinuria, hypoproteinemia effectively. It has an effect on hyperlipidemia significantly relieved. And relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure. It can be inferred that Dohongsamultang improves hyperlipidemia effectively. It has effects on proteinuria, hypoproteinemia in nephrotic syndrome significantly relieved. And relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure.

Description of Clinicopathologic Changes during the Development and Clinical Resolution of Experimentally induced Canine Nephrotic Syndrome (실험적으로 유발한 개 신증후군의 진행 및 치유과정에서의 임상병리학적 변화)

  • The purpose of this study is to evaluate urine protein-to-creatinine ratio as a parameter for early detection of nephrotic syndrome and as a parameter for monitoring effectiveness in early course of treatment. Nine healthy dogs were sensitized by intravenous injection with 1 $\mu$g of endotoxin and 5 mg of native bovine serum albumin. After 1 week, 120 mg of cationized bovine serum albumin was injected intravenously 5 times a week. Among nine dogs, five dogs were confirmed as having developed glomerulonephritis and nephrotic syndrome by increase of urine protein-to-creatinine ratio(>1.0), hypoalbuminemia (<1.5 g/dl), hypercholesterolemia (> 240 mg/dl) and azotemia (BUN>40 mg/dl). During the induction of glomerulonephritis and the progression to nephrotic syndrome, the increase of urine protein-to-creatinine ratio was firstly detected. 1 to 4 weeks later, hypoalbuminemia, hypercholesterolemia, and azotemia were detected. Prednisolone (2.2 mg/kg, bid) was administered orally to the dogs with induced nephrotic syndrome. In early stage of treatment, the increase of serum albumin and decrease of serum cholesterol were detected. 1 to 4 weeks later, decrease of urine protein-to-creatinine ratio was detected. It was concluded that urine protein-to-creatinine ratio is a useful parameter for early detection of nephrotic syndrome, and serum albumin and cholesterol are useful parameters for the monitoring in early course of treatment in nephrotic syndrome.

  • Diabetic nephropathy is defined that renal glomeruluses are damaged by diabetes melitus and proteins that are not excreted to urine normally are exhausted, or do not excrete waste matter to urine, so-called chronic renal failure state. Clinical stage is explained divided 5. If reached 4 or 5 stage, overt proteinuria is occurred to clinical nephropathy, and hypoalbuminemia, azotemia, hypertension, and edema become serious. Concept about edema is differing comprehension about mechanism of a disease, and treatment methodaccording to Sasang Constitution in Sasang Constitutional Medicine. I treated edemtous Soyangin(少陽人) patient with Dojeokgangkitang(導赤降氣湯) who diagnosed as diabetic nephropathy and wrongly treated in Soeumin. I have got good results, so I do case study.

  • Rhabdomyolysis is a syndrome resulting from skeletal muscle injury with release of muscle cell contents into the plasma. It has been reported as a cause of acute renal failure(ARF), and often associated with alcohol abuse, muscle compression, infections, and generalized seizure. Rhabdomyolysis-induced ARF is rare in children. We experienced a case of rhabdomyolysis-induced ARF in a 12-year-old boy who presented with azotemia and oliguria secondary to convulsion. After the control of convulsion by antiepileptic drugs, the daily urine output gradually increased and systemic features recovered with appropriate hydration and alkalinization.   • A 2-year-old, female, Maltese dog (3.2 kg of body weight) was referred to the Veterinary Medical Teaching Hospital, Chungnam National University with vomiting, anorexia and depression. Twelve hours prior to the onset of clinical signs, the dog ingested some grape skins (about 60 g). Physical examination at the time of presentation showed peripheral edema and mucous pallor. Blood and blood chemical analysis revealed anemia and moderate azotemia with elevated blood urea nitrogen (107.2 mg/dl), creatinine (6.3 mg/dl) and hyperphosphatemia (11.3 mg/dl). Echogenicity of renal cortex were observred by ultrasonography. The dog was diagnosed as acute renal failure occurred by grape skin toxicosis. The dog was treated with supportive care such as fluid therapy, diuretics (furosemide) and phosphorus binder (almagate). A normal condition of blood and blood chemical findings and clinical signs was observed at five days after treatment, and prognosis is good to date.

  • A 7-year-old, intact male Pomeranian(weighing 2.2 kg), was presented with clinical signs of prolonged anorexia, polydypsia/polyuria, severe azotemia, proteinuria and heart murmur. Diagnostic studies found chronic kidney disease, severe hypertension and hypertrophic cardiomyopathy. The dog was treated with 2-day peritoneal dialysis, blood transfusion, anti-hypertensive therapy with amlodipine and conservative therapies direct to chronic renal failure. This is a rare case of hypertensive cardiomyopathy complicated with chronic kidney disease in dogs.

  • Two dogs were presented with melena, vomiting and depression after accidental swallowing of candy form of Strepsils (flurbiprofen), which is one of non-steroidal anti-inflammatory drugs used in human medicine for controlling a sore throat. These dogs had common signs of anemia induced by gastrointestinal ulceration and hemorrhage with azotemia and leukocytosis. The dogs were treated with blood transfusion, fluid therapy, proton-pump inhibitor, antiemetics, mucus protectant and antibiotic. Although most of clinical signs of two dogs were resolved, azotemic problem with evidence of renal injury have remained.

A Case of Microscopic Polyangiitis Presenting As Diffuse Alveolar Hemorrhage (미만성 폐포출혈 양상을 나타낸 현미경적 다발성 혈관염 1례)

  • A diffuse alveolar hemorrhage is a rare manifestation in microscopic polyangiitis. Recently we experienced a case of diffuse alveolar hemorrhage associated with microscopic polyangiitis, which was diagnosed with the typical clinical manifestations, ANCA and a renal biopsy. A 71 year old female was admitted complaining of coughing and dyspnea. A chest X-ray, HRCT and BAL revealed a diffuse alveolar hemorrhage. A diffuse alveolar hemorrhage was noted during a bronchoscopy. She also had proteinuria, microscopic hematuria and mild azotemia. The renal biopsy showed necrotic glomerulonephritis without immune complex deposits or granuloma. Under the diagnosis of microscopic polyangiitis, she was treated with steroid pulse therapy, and prednisolone with cyclophosphamide subsequently. She showed marked improvement in the clinical manifestations.

Radiographic Features of Traumatically Caused Retroflexion of Urinary Bladder in Two Dogs

  • The urinary bladder retroflexion, flipped over backwards into the pelvic canal, by trauma has been rarely reported in dogs. This paper describes clinical and radiological features of urinary bladder retroflexion in two dogs with a history of traffic accident. The main clinical signs were hind limb lameness, severe pain and dysuria. Radiography and ultrasonography were performed to evaluate the patient's damages. No remarkable findings were observed except pelvic fracture on survey radiograph. On cystourethrography, caudal displacement and retroflexion of the bladders were identified. Cystopexy was performed and the bladders were repositioned. After surgery one dog with severe azotemia and bilateral hydroureteronephrosis was deteriorated noticeably and euthanized by request of the owner, while the other dog was recovered no recurrence of the problem by 12 months post-surgery. It should be considered that contrast study and serial assessment in urinary tract be important to demonstrate the evidence of bladder retroflexion and evaluate the prognosis in dogs with severe trauma.
