A Study on the Cause Analysis of Human Error Accidents by Railway Job

  • Purpose: This study investigates human error accidents in the Korean railway sector, emphasizing the need for systematic management to prevent such incidents, which can have fatal consequences, especially in driving-related jobs. Research design, data and methodology: This paper analyzed data from the Aviation and Railway Accident Investigation Board and the Korea Transportation Safety Authority, examining 240 human error accidents that occurred over the last five years (2018-2022). The analysis focused on accidents in the driving, facility, electric, and control fields. Results: The findings indicate that the majority of human error accidents stem from negligence in confirmation checks, issues with work methods, and oversight in facility maintenance. In the driving field, errors such as signal check neglect and braking failures are prevalent, while in the facility and electric fields, the main issues are maintenance delays and neglect of safety measures. Conclusions: The paper concludes that human error accidents are complex and multifaceted, often resulting from a high workload on engineers and systemic issues within the railway system. Future research should delve into the causal relationships of these accidents and develop targeted prevention strategies through improved work processes, education, and training.

Synthesis of Non-hydrate Iron Oleate for Eco-friendly Production of Monodispersed Iron Oxide Nanoparticles

  • In this work, we describe a novel and simple technique to produce non-hydrate surfactant complexes for the formation of highly crystalline fatty acid modified SPIONs by thermolysis of iron oleate (FeOl) complexes in a non-coordinating solvent. FeOl complexes were prepared by direct coordination of iron ions and carboxylic acid; thus, we could control the stoichiometric composition of the precursor by changing the molar ratio of fatty acid and metal ions. The discrete thermal behaviors and chemical coordination of the intermediate non-hydrated FeOl were studied by thermo-analytic techniques including differential scanning calorimetry, thermal gravimetric analysis, and Fourier transform infrared spectroscopy.

  • Debriefing is a conversational session that revolves around the sharing and examining of information after a specific event has taken place. Debriefing may follow a simulated or actual experience and provides a forum for the learners to reflect on the experience and learn from their mistakes. Originating from the military and aviation industry, it is used on a daily basis to reflect and improve the performance in other high-risk industries. Expert debriefers may facilitate the reflection by asking open-ended questions to probe into the framework of the learners and apply lessons learned to future situations. Debriefing has been proven to improve clinical outcomes such as the return of spontaneous circulation after cardiac arrest and the teaching of teamwork and communication in pediatrics. Incorporating debriefing into clinical practice would facilitate the cultural change necessary to talk more openly about team performance and learn from near misses, errors, and successes that will improve not only clinical outcome but also patient safety.

  • A numerical technique is employed to simulate the flow patterns in the human carotid artery and a phantom of the carotid artery made of acrylic material is used to observe the flow phenomena in the carotid artery. For numerical analysis the idealized geometric shape of the carotid artery is constructed to portray the phantom. Steady momentum equation is solved by the finite element method and the numerical results are compared with the results of MRA and color Doppler images.

Barotrauma-Induced Pneumocephalus Experienced by a High Risk Patient after Commercial Air Travel

  • A 49-year-old female with a history of several neurosurgical and otolaryngologic procedures for occipital meningioma and cerebrospinal fluid leaks was diagnosed with pneumocephalus after a one hour flight on a domestic jet airliner. Despite multiple operations, the air appeared to enter the cranium through a weak portion of the skull base due to the low atmospheric pressure in the cabin. The intracranial air was absorbed with conservative management. The patient was recommended not to fly before a definite diagnostic work up and a sealing procedure for the cerebrospinal fluid leak site had been performed. Recent advances in aviation technology have enabled many people to travel by air, including individuals with medical conditions. Low cabin pressure is not dangerous to healthy individuals; however, practicing consultant neurosurgeons should understand the cabin environment and prepare high risk patients for safe air travel.

불어 축소어의 음소화 연구 (A Study on Phonemicization in French Abbreviation)

  • The abbreviation (specially, an acronym) are used more nowadays. However we are using them carelessly unknowing that there are some reducing patterns. In this paper, we will first analyse the right oralization and the phonemicization of abbreviation on the basis of the group types. Then, we will propose necessary and sufficient conditions to recognize how to read or pronounce the acronyms and in which way, when they are converted from the text to the speech. We have limited the use of acronym to the graphem-phoneme relations, and the diversity of the usage to minimized, and therefore we could define clearly the characteristics of the phonetic chains. In conclusion, we could find that there are more phonemicization in producing acronyms with these phonetic chains characteristics, and these phonetic based acronyms are increasingly used in the field of aviation and medicine.

  • Computer simulation and magnetic resonance angiograms(MRAs) are used to understand for flow patterns in the carotid arterial bifurcation. Steady momentum equation is solved by the finite volume method. A Phantom of the carotid artery made of bioacrylic material is used for MRA observation. Flow Patterns are observed by using MRA for flow in the phantom of an automatic closed-type circulatory system filled with sugar 4 w% solution. For numerical analysis the idealized geometric shape of the carotid artery is constructed to portray the phantom. Results of numerical analysis are compared with those of MRA. The flow patterns of the phantom on MRA are almost identical to those of the computer simulation.

  • With the operation of the KC-330 MRTT (Multi Role Tanker Transport), which had been fielded in 2019, the ROKAF (Republic of Korea Air Force) has given fighter pilots a new mission of air refueling. As a result, fighter pilots are more likely to be exposed to risks they have never faced before, and it is necessary to look at the risk factors associated with human factors in air refueling missions. Therefore, in this study, an analysis using the HFACS (Human Factors Analysis and Classification System) model was performed for fighter pilots with air refueling qualifications. This study tried to prevent hazard in advance by discriminating the risk factors according to the human factors related to the fighter pilot during the air refueling mission.

  • In the era of the 4th industrial revolution, the aviation industry is also growing remarkably with the development of artificial intelligence and networks, so it is necessary to study a new concept of crew resource management (CRM), which is required in the process of operating state-of-the-art equipment. The automation system, which has been treated only as a tool, is changing its role as a decision-making agent with the development of artificial intelligence, and it is necessary to set clear standards for the role and responsibility in the safety-critical field. We present a new perspective on the automation system in the CRM program through the understanding of the autonomous system. In the future, autonomous system will develop as an agent for human pilots to cooperate, and accordingly, changes in role division and reorganization of regulations are required.

Blood Pressure Simulator using An Optimal Controller with Disturbance Observer

  • The various blood pressure simulators have been proposed to evaluate and improve the performance of the automatic sphygmomanometer. These have some problems such as the deviation of the actual blood pressure waveform, limitation in the blood pressure condition of the simulator, or difficulty in displaying the blood flow. An improved simulator using disturbance observer is proposed to supplement the current problems of the blood pressure simulator. The proposed simulator has an artificial arm model capable of feeding appropriate fluids that can generate the blood pressure waveform to evaluate the automatic sphygmomanometer. A controller was designed and thereafter, simulation was performed to control the output signal with respect to the reference input in the fluid dynamic model using the proposed proportional control valve. To minimize the external fluctuation of pressure applied to the artificial arm, a disturbance observer was designed on the plant. A hybrid controller combined with a proportional controller and feed-forward controller was fabricated after applying a disturbance observer to the control plant. Comparison of the simulations between the conventional proportional controller and the proposed hybrid controller indicated that even though the former showed good control performance without disturbance, it was affected by the disturbance signal induced by the cuff. The latter exhibited an excellent performance under both situations.
