Background Various focal heating devices are popular in Korea under the cultural influence of the traditional ondol under-floor method of home heating. These devices can cause severe burn-like injuries resulting from device malfunction or extended with low heat contact. In addition to injuries under these high heat contact, burns can be occurred by low heat exposure with prolonged periods despite the devices are properly functioning. In order to develop strategies to reduce the duration of periods of illness due to low-temperature burns, we analyzed and compared treatment methods and therapeutic periods for this type of injury. Methods This retrospective study included 43 patients burned under low heat conditions. Patients were divided into an operative group and a conservative group. The patients in the operative group underwent at least one surgical excision, and were further subdivided into early and late visit groups. The conservative group was treated only with dressings. We compared the treatment periods between the operative group and the conservative group, and also compared the preparation periods and treatment periods between the two operative groups. Results The average treatment period was significantly shorter in the operative group (P=0.02). In the early visit operative group, both wound preparation and treatment were briefer than in the late visit group. Conclusions We recommend that early proper burn care and early surgical intervention, including appropriate excision, are feasible ways to reduce the treatment period of lowtemperature burn patients.

Background Excessive use of corticosteroids therapy along with gross immunocompromised conditions in the novel coronavirus disease 2019 (COVID-19) pandemic has raised the risks of contracting opportunistic fungal infections. Here, we describe our experience with the implementation of a surgical protocol to treat and reconstruct rhino-orbital-cerebral mucormycosis. Methods A retrospective review of our prospectively maintained database was conducted on consecutive patients diagnosed with mucormycosis undergoing immediate reconstruction utilizing our "Mucormycosis Management Protocol." All patients included in this study underwent reconstruction after recovering from COVID-19. Wide local excision was performed in all cases removing all suspected and edematous tissue. Reconstruction was done primarily after clear margins were achieved on clinical assessment under a cover of injectable liposomal amphotericin B. Results Fourteen patients were included. The average age was 43.6 years and follow-up was 24.3 days. Thirteen patients had been admitted for inpatient care of COVID-19. Steroid therapy was implemented for 2 weeks in 11 patients and for 3 weeks in 3 patients. Eight patients (57.1%) had a maxillectomy and mucosal lining resection with/without skin excision, and six patients (42.8%) underwent maxillectomy and wide tissue excision (maxillectomy and partial zygomatic resection, orbital exenteration, orbital floor resection, nose debridement, or skull base debridement). Anterolateral thigh (ALT) flaps were used to cover defects in all patients. All flaps survived. No major or minor complications occurred. No recurrence of mucormycosis was noted. Conclusion The approach presented in this study indicates that immediate reconstruction is safe and reliable in cases when appropriate tissue resection is accomplished. Further studies are required to verify the external validity of these findings.
Background: In the present study, the age- and sex-adjusted Constant score (CS) in a normal Indian population was calculated and any differences with other population cohorts assessed. Methods: The study participants were patients who visited the outpatient department for problems other than shoulder and healthy volunteers from the local population. Patients without shoulder pain/discomfort during activity were included in the study. Subjects with any problem that might affect shoulder function (e.g., cervical, thoracic spine, rib cage deformity, inflammatory arthritis) were excluded. Constant scoring of all participants was performed by trained senior residents under the supervision of the senior faculty. Shoulder range of movement and strength were measured following recommendations given by the research and Development Committee of the European Society for Shoulder and Elbow Surgery (2008). A fixed spring balance was used for strength measurement; one end was fixed on the floor and the other end tied with a strap to the wrist of the participant, arm in 90° abduction in scapular plane with palm facing down. Results: Among the 248 subjects (496 shoulders), the average age was 37 years (range, 18-78 years), 65.7% were males (326 shoulders) and 34.3% females (170 shoulders). The mean CS was 84.6±2.9 (males, 86.1±3.0; females, 81.8±2.9). CS decreased significantly after 50 years of age in males and 40 years of age in females (p<0.05). The mean CS was lower than in previous studies for both males and females. Heavy occupation workers had higher mean CS (p<0.05). A linear standardized equation was estimated for calculating the adjusted CS for any age. Conclusions: Mean CS and its change with age differed from previous studies among various population cohorts.

In this study, operator habitability was numerically evaluated in the event of a fire at the main control bench board (MCB) in a reference main control room (MCR). It was investigated if evacuation variables including hot gas layer temperature (HGLT), heat flux (HF), and optical density (OD) at 1.8 m from the MCR floor exceed the reference evacuation criteria provided in NUREG/CR-6850. For a fire model validation, the simulation results of the reference MCR were compared with existing experimental results on the same reference MCR. In the simulation, various input parameters were applied to the MCB panel fire scenario: MCR height, peak heat release rate (HRR) of a panel, number of panels where fire propagation occurs, fire propagation time, door open/close conditions, and mechanical ventilation operation. A specialized-average HRR (SAHRR) concept was newly devised to comprehensively investigate how the various input parameters affect the operator's habitability. Peak values of the evacuation variables normalized by evacuation criteria of NUREG/CR-6850 were well-correlated as the power function of the SAHRR for the various input parameters. In addition, the evacuation time map was newly utilized to investigate how the evacuation time for different SAHRR was affected by changing the various input parameters. In the previous studies, it was found that the OD is the most dominant variable to determine the MCR evacuation time. In this study, however, the evacuation time map showed that the HF is the most dominant factor at the condition of without-mechanical ventilation for the MCR with a partially-open false ceiling, but the OD is the most dominant factor for all the other conditions. Therefore, the method using the SAHRR and the evacuation time map was very useful to effectively and comprehensively evaluate the operator habitability for the various input parameters in the event of MCB fires for the reference MCR.

In this paper, we described a received signal model in terms of the starting point of FFT window and derive a post-detection SINR for the receiver with MMSE filter and the corresponding filter coefficients in order to mitigate the effects of interferences caused by time and frequency selective fading channels and time difference of arrival (TDoA) in OFDMA-based Mesh Networks. In addition, we proposed an MMSE Bidirectional Successive Detection (BSD) technique which can suppress the effects of interferences among multi-nodes without any redundant FFT operations. It was shown by simulation that the proposed interference suppression technique has not an error floor at higher average SNR than 30dB in terms of 64QAM uncoded BER contrary to the conventional techniques.

The purpose of this paper is to suggest the strategical lifting postures able to alleviate imbalanced EMG amplitude leading to an increase in low back muscle fatigue while lifting asymmetric load dynamically. Eleven male subjects are required to lift symmetrically an external load with 15.8kg and load center of gravity (LCG) deviated 10cm to the right from the floor to the waist height at the speed of about 25cm/sec. The EMG amplitudes on bilateral low back muscles (Longissimus, Iliocostalis, and Multifidus) are recorded during 2sec and analyzed. Independent variables are trunk postures (No bending vs. Bending to the LCG) and feet placements (Parallel vs. Right foot in front of the other vs. Right foot behind the other). Dependent variables are EMG amplitude average on six muscles and the EMG amplitude difference between right and left muscle group. Results indicate the phenomenon showing an amplitude increase in the left muscle group is equal to an decrease in the right one is observed in dynamic as well as static lifts, bending the trunk to the LCG increases amplitude discrepancy more than no trunk bending, and the amplitude discrepancy in one foot ipsilateral to LCG in front of the other foot is lowest among other foot postures. As bilateral EMG amplitude discrepancy increases total low back muscle fatigue, the strategical combination of no trunk bending and one foot close to LCG in front of the other is recommended for preventing elevated incidence of low back pain (LBP).
The fee-for-service system is used as the main payment system for health care providers in Korea. It has been argued that it can't reflect differences in the medical practice costs across regions because the fee schedule is calculated based on the average cost. So, some researchers and providers have disputed that there is need for adopting geographic practice cost index (GPCI) used in the United States for the Medicare program for the elderly to the fee-for-service payment system. This study performed to identify whether the difference in the practice costs among regions exists or not and to examine the feasibility of applying GPCI to Korea payment system. For this purpose, we calculated modified-GPCI and examined considerations to introduce GPCI in Korea. First we identified available data to calculate GPCI. Second, we made applicable GPCI equations to Korea payment system and computed it based on four types of regions (metropolitan, urban, suburban, and rural). We also categorize the regions based on the availability of the medical resources and the capability of utilizing them. As a result, we found that there wasn't any significant difference in the GPCI by regional types in general, but the indices of rural areas (0.91-0.98) was relatively low compared to the indices of other regions (0.96-1.07). Considering the need to use GPCI floor, the pros and cons of using GPCI, and the concern of the regional imbalance of resources, the introduction of GPCI needs to be carefully considered.

Numerical analysis of shear walls governed by flexural behavior is conducted for the seismic control performance of proposed friction dampers installed at the center of coupling beams. Control effectiveness of shear walls connected by beams with the proposed dampers are compared for single shear wall with same flexural rigidity. Average responses of the shear walls with the dampers are found with seven scaled-downed earthquakes based on KEC 2005 design spectrum. Slip load is the most important design parameter. It is designed to be 5, 10, 20, 30, 60, 90% of total vertical shear force at damper location to prevent damper slip in specific stories. Nonlinear time-history analysis is conducted by using SeismoStruct analysis program. Seismic control performance of the dampers is evaluated for base shear, energy dissipation, curvature and top-floor displacement. Results show that the dampers are the most effective in reducing the responses when their total slip load is 30% of total vertical shear force.
Litterfall, decomposition, and dynamics of n and p in decomposing litter were investigated for 2 years in red pine ( pinus densiflora) and chinese thuja( thuja orientalis)stands in the limestone area. Average litterfall in red pine and chinese thuja stands were 4, 535kgDM ha-1 yr-1 and 5, 010 kgDM ha-1 yr-1, respectively. Seasonal litterfall in red pine and chinese thuja stands showed peaks in November. concentrations of N and P in the needle litter were lowest in the winter when the greast litterfall occurred, and highest in the summer when the least litterfall occurred. However, those in chinese thuja scale leaf litter showed litter seasonal variation.amount of N and P returned to the forest floor through litterfall were 29.02kgN ha-1 yr-1 , 2.81 kg P ha-1 yr-1 for red pine stand, and 31.06 kg N ha-1 yr-1 , 2.86kgP ha-1 yr-1 for chinese thuja stand, respectively. After 21 mounts elapsed, needle and chinese thuja scale leaf litterbags lost 34.8% and 32.5% of the initial weight, respectively. N concentrations in the docomposing needle and chines thuja scale leaf litter decreased by 19% and 30%, respectively, after 1 month elapsed, and then gradually increased to exceed the initial concentration after 9 months elapsed and then gradually increased to exceed the initial concentration after 9 months elapsed in both of them, P in needle and chines thuja scale leaf litter decreased by 54% and 57% of the initial concentration, respectively, after 1 month elapsed. unlike N, P concentration in the decomposing litter did not exceed that of initial ones. Neyt immobilization period of N and P in decomposing litter did not occur over the study period.

This study details the actual wearing conditions and favored design of ready-made pants for obese women, particularly for those in their twenties and thirties. The actual problems that customers feel and the preferred design of pants were suggested based on the survey and from the standpoint of consumers. The findings of the survey are as follows. The actual wearing conditions in the survey, 80.5% like wearing pants. The main reason to prefer pants is that pants are more convenient for social or professional work activities. The main reason to avoid wearing pants is that the pants (in general) do not mask an obese body shape. The most favored was Denim/Elastic and the second most favored was Cotton/Elastic from the survey. On average, elastic materials are preferred over non-elastic materials. The reason was the comfort in wearing and convenience in social or professional activities that could mask obesity. In terms of the style, the most favored was blue jeans and the second favored was semi-formal. A straight-silhouette is the most favored as it was able to help mask obesity. The most favored position for the waistline is 4 cm lower than the natural waistline. The most favored pant length is where the pants slightly touch the floor when they stand with shoes on. For obese women the pants development that covers the weak points of the body shape giving, which is comfortable and active are desired.
