Determination of Ethyl Carbamate in Maesil Extract and Estimated Daily Intake (매실농축액의 에틸카바메이트 분석과 노출량 평가)

  • The analytical method for ethyl carbamate (EC) in maesil (Prunus mume) extract was developed with deuterium-labeled ethyl carbamate as an internal standard. Samples were neutralized with an addition of 1 N sodium hydroxide solution, followed by a solid phase extraction with a Chem Elut cartridge. A standard curve exhibited a good linearity with correlation coefficient of 0.9991. The limit of detection (LOD) and limit of quantification (LOQ) were 2.91 ng/g and 8.83 ng/g, respectively. The recovery rate of EC ranged from 91.40% to 120.90%. The precision never exceeded 12.57% (intra-day) and 11.03% (inter-day). Samples were comprised of 24 home-made and 7 commercially-available maesil extracts. Eight home-made samples contained EC at levels between 3.39 and 75.76 ng/g. Three commercially-available samples had EC at levels between 11.67 and 20.16 ng/g. Average daily intakes of EC from maesil extracts for consumers were 0.23 g/kg of body weight. Based on a benchmark dose confidence limit ($BMDL_{10}$) of 0.25 mg/kg of body weight/day, the margin of exposure (MOE) of EC in maesil extract for consumers was 94,150, which is not of concern. Considering that a daily intake of maesil extract has been increasing, further studies on the formation of EC in maesil extract is needed.

Whole brain radiotherapy using four-field box technique with tilting baseplate for parotid gland sparing

  • Purpose: The aim of this study is to evaluate the efficacy and feasibility of four-field box whole brain radiotherapy (FB-WBRT) with tilting baseplate by comparing bilateral WBRT (B-WBRT). Methods and Materials: Between March 2016 and September 2018, 20 patients with brain metastases underwent WBRT using the four-field box technique. WBRT is performed with a dose of 30 Gy in 10 fractions daily. Two computed tomography simulations per person were performed. One was in the traditional supine position for B-WBRT and the other by applying the tilting acrylic supine baseplate to elevate the head by 40° for FB-WBRT. The B-WBRT used the field-in-field technique, which is the most commonly used method in our institution. The FB-WBRT comprised anterior, posterior, and bilateral beams. A wedge was applied in anterior and posterior fields to compensate for skull convexity. Results: The average of Dmean of both parotid glands was 10.2 Gy (range, 3.8 to 17.8 Gy) in B-WBRT and 5.4 Gy (range, 2.0 to 11.7 Gy) in FB-WBRT (p < 0.05). Compared to B-WBRT, FB-WBRT reduced the mean dose of the right and left parotid glands from 10.1 Gy to 4.9 Gy and from 10.4 Gy to 5.8 Gy, respectively (p < 0.05). Further, V5, V10, V15, V20, and V25 for the parotid gland decreased significantly in FB-WBRT (p < 0.05). The Dmax and Dmean of lens decreased according to the dose-volume histogram. Conclusion: Compared to B-WBRT, FB-WBRT with a tilting baseplate is a simple and effective method that takes feature of noncoplanar beam to protect the parotid gland.

Pharmacokinetic profiles of levofloxacin after intravenous, intramuscular and subcutaneous administration to rabbits (Oryctolagus cuniculus)

  • Levofloxacin pharmacokinetic profiles were evaluated in 6 healthy female rabbits after intravenous (I/V), intramuscular (I/M), or subcutaneous (S/C) administration routes at a single dose of 5 mg/kg in a 3 × 3 cross-over study. Plasma levofloxacin concentrations were detected using a validated Ultra Performance Liquid Chromatography method with a fluorescence detector. Levofloxacin was quantifiable up to 10 h post-drug administration. Mean AUC0-last values of 9.03 ± 2.66, 9.07 ± 1.80, and 9.28 ± 1.56 mg/h*L were obtained via I/V, I/M, and S/C, respectively. Plasma clearance was 0.6 mL/g*h after I/V administration. Peak plasma concentrations using the I/M and S/C routes were 3.33 ± 0.39 and 2.91 ± 0.56 ㎍/mL. Bioavailability values, after extravascular administration were complete, - 105% ± 27% (I/M) and 118% ± 40% (S/C). Average extraction ratio of levofloxacin after I/V administration was 7%. Additionally, levofloxacin administration effects on tear production and osmolarity were evaluated. Tear osmolarity decreased within 48 h post-drug administration. All 3 levofloxacin administration routes produced similar pharmacokinetic profiles. The studied dose is unlikely to be effective in rabbits; however, it was calculated that a daily dose of 29 mg/kg appears effective for I/V administration for pathogens with MIC < 0.5 ㎍/mL.

Daily localization of partial breast irradiation patients with three-dimensional ultrasound imaging

  • Purpose: Accurate localization of the lumpectomy cavity during accelerated partial breast radiation (APBR) is essential for daily setup to ensure the prescribed dose encompasses the target and avoids unnecessary irradiation to surrounding normal tissues. Three-dimensional ultrasound (3D-US) allows direct visualization of the lumpectomy cavity without additional radiation exposure. The purpose of this study was to evaluate the feasibility of 3D-US in daily target localization for APBR. Materials and methods: Forty-seven patients with stage I breast cancer who underwent breast conserving surgery were treated with a 2-week course of APBR. Patients with visible lumpectomy cavities on high quality 3D-US images were included in this analysis. Prior to each treatment, X-ray and 3D-US images were acquired and compared to images from simulation to confirm accurate position and determine shifts. Volume change of the lumpectomy cavity was determined daily with 3D-US. Results: A total of 118 images of each modality from 12 eligible patients were analyzed. The average change in cavity volume was 7.8% (range, -24.1% to 14.4%) on 3D-US from simulation to the end-of-treatment. Based on 3D-US, significantly larger shifts were necessary compared to portal films in all three dimensions: anterior/posterior (p = 7E-11), left/right (p = 0.002), and superior/inferior (p = 0.004). Conclusion: Given that the lumpectomy cavity is not directly visible via X-ray images, accurate positioning may not be fully achieved by X-ray images. Therefore, when the lumpectomy cavity is visible on US, 3D-US can be considered as an alternative to X-ray imaging during daily positioning for selected patients treated with APBR, thus avoiding additional exposure to ionizing radiation.

Analysis of PAHs (polycyclic aromatic hydrocarbons) in Ground Coffee Using GC-tandem Mass Spectrometry and Estimation of Daily Dose (GC-tandem mass spectrometry를 이용한 분쇄원두커피 중 PAHs(polycyclic aromatic hydrocarbons) 분석법 연구 및 인체노출량 평가)

  • The purpose of this study was to develop an analytical method for determining 15 polycyclic aromatic hydrocarbons (PAHs) of EU priority using gas chromatography (GC)-tandem mass spectrometry (MS). The PAHs in ground coffee were analyzed after being extracted using methods such as saponification-liquid-liquid extraction, Soxhlet extraction, and solid-liquid extraction. The solid-liquid extraction method showed the greatest repeatability and most efficient reduction of the matrix effect. GC-tandem MS for the quantification of the 15 PAHs showed better resolution and lower limit of detections (LODs) than GC-MS-selected ion monitoring (SIM) and high performance liquid chromatography with fluorescence detector. LODs of this method for the ground coffee types were 0.002-0.1 ${\mu}g/kg$ and limit of quantifications (LOQs) were 0.006-0.2 ${\mu}g/kg$ The recoveries ranged from 52.6 to 93.3%. Forty-six commercial types of ground coffee were analyzed to determine their PAHs contamination levels. PAHs concentration ranged from ND to 5.988 ${\mu}g/kg$. This study was conducted with toxicity equivalence factors, the U.S. EPA recommendation to identify dietary risks for PAHs in different types of coffee. The estimated average daily dose of PAHs was $5.24{\times}10^{-8}$ mg/kg body weight/day.

Role of fractionated radiotherapy in patients with hemangioma of the cavernous sinus

  • Purpose: We performed this retrospective study to investigate the outcomes of patients with hemangioma of the cavernous sinus after fractionated radiotherapy. Materials and Methods: We analyzed 10 patients with hemangioma of the cavernous sinus who were treated with conventional radiotherapy between January 2000 and December 2016. The median patient age was 54 years (range, 31-65 years), and 8 patients (80.0%) were female. The mean hemangioma volume was $34.1cm^3$ (range, $6.8-83.2cm^3$), and fractionated radiation was administered to a total dose of 50-54 Gy with a daily dose of 2 Gy. Results: The median follow-up period was 6.8 years (range, 2.2-8.8 years). At last follow-up, the volume of the tumor had decreased in all patients. The average tumor volume reduction rate from the initial volume was 72.9% (range, 18.9-95.3%). All 10 of the cranial neuropathies observed before radiation therapy had improved, with complete symptomatic remission in 9 cases (90%) and partial remission in 1 case (10%). No new acute neurologic impairments were reported after radiotherapy. One probable compressive optic neuropathy was observed at 1 year after radiotherapy. Conclusion: Fractionated radiotherapy achieves both symptomatic and radiologic improvements. It is a well-tolerated treatment modality for hemangiomas of the cavernous sinus.

Effect of zinc on the immune response and production performance of broilers: a meta-analysis

  • Objective: This study performed a meta-analysis of published trials to determine the effects of zinc on the immune response and production performance of broilers. Methods: A database was built from published literature regarding the addition of zinc forms or doses and their relation to the immune response and production performance of broilers. Different doses or forms of zinc were identified in the database. The recorded parameters were related to the immune response and production performance. The database contained a total of 323 data points from 41 studies that met the criteria. Then, the data were processed for a meta-analysis using a mixed model methodology. The doses or different forms of zinc were considered fixed effects, different studies were treated as random effects, and p-values were used as the model statistics. Results: An increase in zinc dose increased (p<0.05) pancreas metallothionein (MT) and zinc concentrations in the plasma, tibia and meat, all in quadratic patterns, but linearly decreased (p<0.05) the heterophil/lymphocyte (H/L) ratio. Regarding the different zinc forms, both inorganic and organic zinc increased (p<0.05) the zinc concentrations in the plasma and tibia, the calcium and phosphorus contents in the tibia, and the antioxidant activity of superoxide dismutase in meat as compared to control. An increase in zinc dose increased average daily gain (ADG) and decreased feed conversion ratio (FCR) following a quadratic pattern (p<0.05). Inorganic and organic zinc decreased (p<0.05) FCR and H/L ratio than that of control, but these two forms were similar for these parameters. Conclusion: Zinc addition has a positive impact on immunity and broiler production. Zinc can suppress stress and inhibit the occurrence of lipid peroxidation in broilers, and it can also improve ADG, FCR, and the quality of broiler carcasses.

Polycarbonate, in turn, is used in a wide array of plastic products, with new applications continuously being developed. Also it has been used to produce epoxy resins and polycarbonate plastics for food container. This study was carried out to investigate the effects of bisphenol A on lactation period to dams and F1, Sprague-Dawley females were mated with on 2:1 ratio basis. Various doses of bisphenol A (0, 2, 20, 200, and 2,000 $\mu\textrm{g}$/kg) were daily administered to females for 21 days after parturition. Dams and offsprings were sacrificed at the time of weaning. The results were as follows, 2000 $\mu\textrm{g}$/kg of bisphenol A decreased the dams' body weight at post-partum 18 days and also 200 and 2000 $\mu\textrm{g}$/kg of bisphenol A decreased the body weight of neonates at the days of post-partum 21 days. Bisphenol A increased the relative weights of liver and spleen in male offsprings, depending on the doses. But female offsprings showed high relative organ weights of ovaries, and low relative organ weights of uterine in a some dose-response manners. High dose of bisphenol A induced low viability of neonates exposed during lactation period. The dams treated with bisphenol A showed prematured estrous stage. Bisphenol A was recovered about 21.2% average in serum of dams, and also in offsprings'. The results indicate that the bisphenol A induces estrous cycle during lactation period in dams, also reaches to the of offspring through breast milk. Thus bisphenol A exopsed to dams and neonates via lactation induces some estrogenic and toxic effects.

Risk Assessment for Non-Cancer Effects of Volatile Organic Compounds in Children's Products (어린이용품에 함유된 휘발성유기화합물의 비발암 위해성평가)

  • Background: This study was conducted to assess health risks in regard to exposure by children to volatile organic compounds (VOCs) in children's products. Methods: Ten VOCs were measured by head-space gas chromatography in children's products, including toys, oil pastels, sign pens, furniture, ball pools, and playmats. We estimated the average daily dose (ADD) via inhalation during the use of these children's products and calculated hazard quotient (HQ) by dividing ADD by reference dose of VOCs. Results: Among the measured VOCs, five compounds were identified in children's products: benzene, ethylbenzene, styrene, toluene, and xylene. The detection rates of VOCs in toys, ball pools, furniture, playmats, sign pens, and oil pastels were 85%, 100%, 100%, 30%, 100%, and 60%, respectively. The maximum levels of VOCs were 0.18 mg benzene/kg in toys, 5.92 mg toluene/kg in playmats, 10.37 mg ethylbenzene/kg in ball pools, 24.85 mg xylene/kg in toys, and 118.29 mg styrene/kg in ball pools. From exposure levels of VOCs in the children's products HQs were calculated within a range of $5.71{\times}10^{-10}$ to $4.77{\times}10^{-4}$. The HQ of xylene was the highest for children aged 0-6 playing on the playmats. However, the HQ via inhalation exposure to VOCs in individual products did not exceed 1.00. Conclusion: Based on the results, it was concluded that the use of these children's products do not pose health risks to children.
