  • In this paper, the autonomic function analysis system was designed to noninvasively assess the autonomic function of cardiovascular system. Orthostatic stress protocol was designed to stimulate the autonomic nervous system. designed protocol and analysis algorithm were evaluated by experiments for 25 normal subjects and 22 hemiplegia patients. Data were processed by usign the power spectral analysis. Nwe indexes of autonomic function, LF$_{N}$ and HF$_{N}$, were proposed and were compared with LF/HF ratio. New indexes of the sympathetic and parasympathetic activity, respectively. The IST and the DPT are balanced and have positive value for normal subjects during orthostatic stress but not for hempilegia patients. This result suggest that the IST and the DPT are used as new criteria of normal autonomic function during orthostatic stress.ess.

Autonomic dysfunction in patients with orthostatic dizziness

  • Orthostatic dizziness is feeling dizzy or lightheaded when standing up. Hemodynamic orthostatic dizziness can be caused by autonomic dysfunction such as orthostatic hypotension or postural tachycardia syndrome. The interpretation of the autonomic function test results in patients with orthostatic dizziness is crucial for diagnosing and managing the underlying condition. The head-up tilt and Valsalva tests are especially important for evaluating adrenergic function in patients with hemodynamic orthostatic dizziness. However, it is important to note that autonomic function tests do not cover the entire diagnostic process, since their findings need to be considered along with the detailed history and physical examination results of the patient because various differential diagnoses exist for orthostatic dizziness. Ensuring appropriate treatment by interpreting the autonomic function test results can help to determine the improvement of and prevents falls from orthostatic dizziness.

Vasovagal syncope with mild versus moderate autonomic dysfunction: a 13-year single-center experience

  • Background: An adequate large-scale pediatric cohort based on nationwide administrative data is lacking in Korea. Purpose: This study aimed to differentiate patients with VVS by autonomic dysfunction severity using the composite autonomic severity score (CASS) and compare the clinical manifestations and prognosis between patient subgroups. Methods: We retrospectively reviewed the medical records of 66 VVS patients divided into 3 groups by CASS. To compare the differences between these groups, we analyzed VVS type, triggers, prodromal symptoms, management of syncope, and prognosis between patients with mild versus moderate autonomic dysfunction. Results: Of our 66 patients with VVS, 41 had mild autonomic dysfunction (62.1%) and 25 had moderate autonomic dysfunction (37.9%). We found no significant intergroup differences in age, sex, inducible factor (P=0.172), prodromal symptoms, laboratory findings, head-up tilt test, type of syncope, or prognosis (P=0.154). Conclusion: We found no evidence that autonomic dysfunction degree is affected by VVS characteristics, test findings, parameters, or prognosis; therefore, no further evaluations are needed to classify autonomic dysfunction severity.

  • A model of the cardiovascular system coupling cell, hemodynamics and autonomic nervecontrol function is proposed for analyzing heart mechanics. We developed a comprehensive cardiovascular model with multi-physics and multi-scale characteristics that simulates the physiological events from membrane excitation of a cardiac cell to contraction of the human heart and systemic blood circulation and ultimately to autonomic nerve control. Using this model, we delineatedthe cellular mechanism of heart contractility mediated by nerve control function. To verify the integrated method, we simulated a 10% hemorrhage, which involves cardiac cell mechanics, circulatory hemodynamics, and nerve control function. The computed and experimental results were compared. Using this methodology, the state of cardiac contractility, influenced by diverse properties such as the afterload and nerve control systems, is easily assessed in an integrated manner.

Distribution of Autonomic Neurotransmitters in the Human Vocal Fold (인간의 성대조직에서 자율신경 전달물질의 분포)

  • The vocal fold has three major function-phonation, respiration and protection, and is richly innervated. The vocal 1314 its autonomic innervation-adrenergic and cholinergic from superior cervical ganglion and the vagus nerve, respectively. The action of both system account for vasoregulation and glandular activity. In e vocal fold several kin of neuropeptides, including SP, CGRP, VIP, TH, NPY, ENK have been reported at the animal including cat or dog. But information regarding the distribution of autonomic nerve fibers containing neuropeptides in the human vocal fold is lacking. Two neuropeptides are of special interest : 1) vasoactive intestinal polypeptide(VIP)that is known to be contained in the parasympathetic(cholinergic) neuron. 2) tyrosine hydroxylase(TH)is located in the cytoplasm of noradrenergic neuron and is the rate-limiting enzyme in noradrenaline synthesis. To understand specific autonomic function of vocal fold we did immunohistochemical examination of VIP and TH in the human vocal fold.

The Influence of Transcutaneous Electrical Stimulation on Autonomic Function (경피신경전기자극이 자율신경계에 미치는 영향)

  • Purpose:The purpose of this study is to investigate the effect of transcutaneous electrical stimulation on autonomic nervous system using heart rate variability analysis. Methods:31 subjects were evaluated with HRV before and after a single high-frequency TENS for 15minutes treatment. The standard deviation of all the normal RR-interval(SDNN) and lower frequency/high frequency ratio(LF/HF ratio) were recorded with TAS-9. Results:After single TENS treatment, autonomic adaptation as analyzed by SDNN was increased from $42.47\pm12.96$ to $4.43\pm16.76$(p>0.05) and autonomic balance as analyzed by LF/HF ratio was increased from $1.4\pm0.45$ to $1.45\pm0.41$(p>0.05). Conclusion:In this study, SDNN and LF/HF ratio changed in health young students as a result of single TENS treatment. But there is no significant difference between pre-treatment and post-treatment of SDNN and LF/HF ratio.

The Relationship between Cyber Leisure Activity and Autonomic Function in High School Students (청소년의 사이버 여가활동과 자율신경계 기능의 관계)

  • Purpose: The purpose of this study was to explore the relationship between cyber leisure activity and autonomic function in high school students. Methods: The subjects of this study were 242 second graders at high school located in D city. The research design was descriptive correlation study. Data were collected from August 26 to September 17, 2010, using a cyber leisure questionnaire and heart rate variability (SA-6000, Medicore, Korea) for autonomic function. Results: Total time for the cyber leisure activity was $391.3{\pm}262.0$ (min/week). Time for internet usage and computer game was $234.5{\pm}140.1$ (min/week) and $156.8{\pm}164.3$ (min/week), respectively. There were significant positive correlation between total cyber leisure time and sympathetic activity (r=.247, $p$ <.001). Conclusion: Excessive cyber leisure activity could increase sympathetic activities in high school students. Thus, nursing intervention to reduce the total cyber leisure time will be helpful to improve their neurophysiological status.

Effects of Self-stretching and Joint Mobilization to Upper Thoracic Vertebrae in Craniovertebral Angle and Autonomic System Function in Chronic Cervical Pain (만성 목통증에 대한 자가신장과 위등뼈 관절가동술이 머리척추각 및 자율신경계 기능에 미치는 효과)

  • PURPOSE: This study aimed to examine the effects of self-stretching (SS) and joint mobilization (JM) on pain, craniovertebral angle (CVA), autonomic system function in chronic cervical pain patient with forward head posture (FHP). METHODS: A total of 30 male college students were selected as study subjects, and were divided into Group I (general physical therapy; GPT, n=10), Group II (GPT+SS, n=10), Group III (GPT+JM, n=10). All groups were evaluated three times a week for 4 weeks. Pain was measured by visual analogue scale (VAS), CVA was measured using digital goniometer and autonomic system function (heart rate; HR, skin conductivity; SC, LF norm, HF norm, LF/HF ratio) was measured by Biofeedback ProComp Infiniti. After 4 weeks, paired t-test was used to compare the changes within the group and one way ANOVA was used to compare those between the groups. RESULTS: In Group I, VAS was significantly decreased. In Group II and III was a significantly change in all items. In comparison between Group I and II was a difference in all items except HR. In comparison between Group I and III was a difference in all items. In comparison between Group II and III was a difference in VAS, LF norm and LF/HF ratio. CONCLUSION: This study showed that SS and JM can effectively reduce pain and normalize the autonomic system function.

Clinical and autonomic characteristics in patients with postural tachycardia syndrome (기립빈맥증후군 환자의 임상적 및 자율신경 특성)

  • Postural tachycardia syndrome (POTS) is common, although not so well-known variant of cardiovascular autonomic disorder characterized by an excessive heart rate increase on standing. POTS is probably underdiagnosed due to the heterogeneity in both presentation and etiology. This study aimed to evaluate the clinical and autonomic features in patients with POTS. We reviewed the medical records of patients with POTS. Medical records include onset age, sex, presenting symptoms, body mass index (BMI) and prognosis. All patients had an autonomic function and laboratory tests. Ninety-nine patients met the inclusion criteria for POTS (51.5% male; mean±SD age, 20.0±9.7 years; mean±SD, BMI 21.9±3.9). Common presenting symptoms were a brief loss of consciousness, dizziness, blurred vision and headache. Autonomic function tests showed abnormal quantitative sudomotor axon reflex testing in 20 patients of 99 POTS patients. The abnormal post-ganglionic sympathetic sudomotor function is generally considered to reflect a neuropathic form of POTS. In treatments, 83 patients were treated by non-pharmacological management including lifestyle changes and 16 patients required the initiation of pharmacological therapies. Most patients with POTS showed a relatively favorable prognosis. POTS is a chronic disease with a substantial subset of patients recovering within a few years after the initial presentation. Future efforts should focus on better understanding of POTS pathophysiology and designing randomized controlled trials for the selection of more effective therapy.

Evaluation of Autonomic Function in Patients with Hyperhidrosis by Power Spectral Analysis of Heart Rate Variability (심박변이도(Heart Rate Variability) 측정을 통한 다한증(多汗症)환자의 자율신경계(自律神經系)기능 평가)

  • Objectives : Essential hyperhidrosis is a socially and occupationally disabling disorder. There are many suggestions that hyperhidrosis is associated with the autonomic nervous system. This study was designed to investigate the autonomic nerve system the patients with hyperhidrosis. Methods : 348 palmar and plantar hyperhidrosis patients and 20 systemic hyperhidrosis patients in the ambulatory care were investigated. All patients were inspected at the first medical examination, using the PSA of HRV. HRV was measured for 5 minutes after 5 minutes' bed rest. We checked the frequency domain analysis of HRV (i.e. TP, VLF, LF, HF and LF/HF) which were transformed into natural logarithm of patients against the standard measures of the HRV components. Then, we compared the means of patients with means and highest values of in the normal range of Koreans 1) between the patients with palmar and plantar hyperhidrosis and the healthy controls and 2) between the patients with the systemic hyperhidrosis and healthy controls, by the frequency domain analysis of heart rate variability (HRV), respectively. We then compared the results between 1) and 2). Results : The values of natural logarithmic measures of LF/HF ratio in the patients with palmar and plantar hyperhidrosis we examined were significantly higher than the values of healthy controls, but those in the patients with systemic hyperhidrosis were not higher. Conclusion : This study suggests that the autonomic nervous function did not simply increase but that the balance between sympathetic and parasympathetic nervous function did matter in patients with palmar and plantar hyperhidrosis. This balance included that the sympathetic nervous function aggravated comparatively with parasympathetic nervous function. On the other hand, we couldn't find any significance of the patients with the systemic hyperhidrosis as for the autonomic nervous function.
