The Effects of Auricular Acupressure Therapy on Middle-aged and Old Adults' Dry Eye Syndrome (이압요법이 중·노년의 안구건조증에 미치는 효과)

  • Purpose: This study aimed to examine the effect of auricular acupressure therapy on reducing the ocular symptoms and signs for dry eye syndrome. Methods: The participants who were aged ≥ 40 years old and met the inclusion criteria of the ocular surface disease index score ≥ 13 and a tear film break-up time ≤ 10 seconds were enrolled into the two groups: experimental group (n=29) and control group (n=27). Experimental group received auricular acupressure therapy for 8 weeks. Seed stickers were applied to the eye, liver and tubercle point of each ear once a week. Data were collected at pre and 4 and 8 weeks after the treatment and analysed the efficacy of intervention by repeated measures ANOVA. Results: There were significant differences in the ocular surface disease index, standard patient evaluation of eye dryness and a tear film break-up time in both eyes at 8 weeks after the treatment between the two groups. Conclusion: The findings indicate that the auricular acupressure therapy may be helpful for relieving symptoms of dry eye as a nursing intervention. In addition, it could also be utilized as a self-care practice using proper education and training.

The Optimal Period of the Pedicles Implantation for the Patent Vasculature in the Prefabricated Periosteofascial Flap through the Vascular Pedicles Transfer (혈관조직의 전위를 이용한 선조작 골건막피판의 제작 시 안정된 혈관화를 위한 적절한 혈관경 이식기간)

  • This study was designed to investigate the optimal period of pedicles implantation in the prefabricated periosteofascial flap using a vascular tissue transfer. Flap prefabrication was prepared with a transposition of the central pedicles of right auricle on the calvarium of the New Zealand white rabbit. Thirty flaps were divided into five groups of six flaps, including control group (group I) of the conventional periosteofascial flap based on the right lateral border of parietal bone. The prefabricated flap was elevated as a $2{\times}2cm$ sized island flap and reposed in place in 1, 2, 3, and 4 weeks after the pedicles transfer in groups II, III, IV, and V, respectively. Five days after flap repositioning, the flap viability and vascularity were evaluated with microangiography and histological study quantitatively. The flap survival was increased in accordance with the implanted period of the pedicle. New vessels developed around the implanted pedicle in the 2nd week, and overall vascularization of the flap was accomplished in the 3rd week. The flap with 4 weeks of implantation period, however, showed the same survival rate as the control group. In conclusion, prefabricated periosteo- fascial flap can be created with a vascular tissue transfer, and the optimal duration of the pedicle implantation is more than 4 weeks to obtain adequate flap survival.

  • A new anatomical head model was implemented based on the MR and CT images of the head of a volunteer whose head shape is close to the domestic standard. In order to compare SARs (specific absorption rates) of heads with different shapes, we calculated SARs in the two anatomical head models. The one is the new model and the other is that of the black race and was made at National Library Medicine in USA. The head model and a phone model were arranged in the computational space to be the touch or cheek position of CENELEC (European Committee for Electrotechnical Standardisation) and FCC guidelines. From the obtained results, we can see that the smaller head produces the higher whole head-averaged SAR. However, it seems that the localized SAR averaged over 1 g or 10 g is more dependent on the shape of the auricle rather than that of the head size.

Tragus formation during concha-type microtia repair using a chondrocutaneous island flap

  • Formation of an ideally-shaped tragus remains one of the most challenging issues during staged tragus reconstruction in microtia patients. The authors describe a new method used to treat a unique case of concha-type microtia in which the 10-year-old male patient had only a portion of pre-existing cartilage at the tragus site. An anomalous skin lump was also present. During the initial stages of the reconstruction, the two-stage Nagata method was used for surgical correction of the microtia. An autologous rib cartilage graft was used to form the ear framework. A temporoparietal fascia flap was also constructed. Remnant skin tags and anomalous cartilage that accompany microtia are usually removed during microtia repair. However, the cartilage and skin lump were preserved during the reconstruction. The skin lump was later used to form a vascularized chondrocutaneous island flap that supplemented the portion of cartilage during tragus formation. The result was a new tragus that was satisfactorily improved in both size and shape. Patients with concha-type microtia may benefit from the use of this new method for tragus formation.

Two-stage Ear Reconstruction with Canaloplasty in Congenital Microtia (외이도성형술을 병행한 선천작은귀증의 두단계 재건)

  • The current authors performed two-stage ear reconstruction of microtia using autogenous costal cartilage combined with canaloplasty of the acoustic meatus in a team approach. In the first stage, lobule transposition, fabrication of the cartilage framework, and implantation of the framework were peformed. In the second stage, elevation of the auricle, cartilage graft for posterior auricular sulcus, coverage with the mastoid fascia flap and skin graft, and concha excavation were performed. The canaloplasty was combined simultaneously in patients with radiologic and audiometric evidence of cochlear function in the second stage. A total of 36 consecutive patients with congenital microtia were treated from 1998 to 2003. Among them, 27 patients(male: 18, female: 9) ranging from 7 to 43 years old were combined with canaloplasty. The follow-up period was one year to 5 years. Thirteen patients exhibited improved hearing over 30 dB PTA(pure tone average), 9 patients below 30 dB, and 5 patients with no improvement. Complications related to the canaloplasty were chronic drainages of the auditory meatus and meatal stenosis. Lobule type deformity combined with the canaloplasty showed higher complications than concha type. Therefore, in the lobule type, meticulous manipulation is necessary to reduce complications after the canaloplasty.

  • Ramsay-Hunt syndrome is a related but more serious disorder caused by herpes zoster viral infection of the geniculate ganglion. It is characterized by unilateral painful vesicular rash of the uvula, palate, auricle, ear canal, and postauricular area, but it can extend into the facial tissues as well. Paralysis of the facial nerve is often seen, and there can be disequilibrium and hearing problems also. We experienced continuous cervical epidural block (CCEB) with intermittent stellate ganglion block is effective in Ramsay-Hunt syndrome. CCEB should be considered to the treatment of choice in Ramsay-Hunt syndrome.

  • Stimulation of the auricle is known to be effective in reducing obesity. The aim of the present study is to investigate whether stimulating a specific auricular acupuncture point is effective on suppression of appetite. Cholesystokinin (CCK) is known to induce a powerful feeding response after central administration and particularly abundant in the cerebral cortex. In food-deprived rats exhibiting a strong drive for feeding, decreased CCK release was markedly detected in the cerebral cortex of the brain. Needling the fasted rats on the specific auricular region increased the CCK level of the cerebral cortex. In conclusion, stimulating the specific auricular points increases CCK-expression in the cerebral cortex of the fasted rats and may be useful for controlling obesity.

The best time to pratice is early in the morning or before go to bed, but a litter difference by the writer's opinion, so there is not consistency. 5. Clinical Experience of Basosquamous Cell Carcinoma (기저편평세포암의 치험례)

  • Purpose: Basosquamous carcinoma is a rare malignancy, with features of both basal cell carcinoma and squamous cell carcinoma. It is considered as aggressive tumor with a high risk of recurrence and metastases. Authors report a case of basosquamous cell carcinoma. Methods: A 72 year-old man, who had an erythematous ulcer on his left auricle, described a slow growing lesion, starting at the posterior surface of the superior helix with a steady increase in size during the past 10 years. At operation, auricular cartilage was grossly invaded by the tumor and was, therefore, amputated with tumor-free margins. Results: Histopathologic examination was revealed a basosquamous cell carcinoma. On positron emission tomography/computed tomography (PET/CT) and neck CT were negative for signs of further nodal involvement or metastases to other organs. At follow-up 6 months later, his wounds were noted to be well healed, with no evidence of local recurrence or identifiable metastases. Conclusion: Because basosquamous cell carcinoma has a significant potential to recur and metastasize, surgical excision for this type of carcinoma should be more extensive than that performed for conventional basal cell carcinoma or squamous cell carcinoma. In addition, regional lymph nodes should be monitored and close follow-up should be carried out.

Identification of SNPs Related to 19 Phenotypic Traits Using Genome-wide Association Study (GWAS) Approach in Korean Wheat Mini-core Collection

  • Based on the simple sequence repeat (SSR) marker, a Korean wheat core collection were established with 616 wheat accessions. Among them, the SNP genotyping for the entire genome was performed using DNA chip array to clarify the whole genome SNP profiles. Consequently, a total of 35,143 SNPs were found and we re-established a mini-core collection with 247 accessions. Population diversity and phylogenetic analysis revealed genetic diversity and relationships from the mini core set. In addition, genome-wide association study (GWAS) was performed on 19 phenotypic traits; ear type, awn length, culm length, ear length, awn color, seed coat color, culm color, ear color, loading, leaf length, leaf width, seeding stand, cold damage, weight, auricle, plant type, heading stage, maturation period, upright habit, and degree of flag leaf. The GWAS was performed using the fixed and random model circulating probability unification (FarmCPU), which identified 14 to 258 SNP loci related to 19 phenotypic traits. Our study indicates that this Korean wheat mini-core collection is a set of germplasm useful for basic and applied research with the aim of understanding and exploiting the genetic diversity of Korean wheat varieties.
