Declining auditory performance with advanced age is a well known and common problem that is becoming more Prevalent due to the increasing number of elderly people in our society. We introduce a novel ARCISM (Audiogram Result Computer Input. Save & Management software) data management program which enables the assessment of the risk of past history of hearing impairment on the basis of known risk factors. This study investigates the reference levels of pure tone hearing threshold of normal aging in Korean and the difference between fenders. The subjects were carefully selected by questionnaire and absence of any history of otologic infection, noise exposure. and ototoxic drug among 1603 subjects. who visited Health Promotion Center in Daegu Fatima Hospital for one year. The results show (1) hearing sensitivity declines with age. (2) higher frequency shows steeper slope of hearing declines than lower frequency. and (3) there were more hearing loss at 4 and 8 kHz in men than in women. Due to the ARCISM program. it was possible to manage huge data of hearing results and to obtain the reference level of a9e-related hearing declines. Furthermore, we expect that the results of this study can be the fundamental data for hearing rehabilitation for the elderly and for developing suitable hearing aids for Korean.

Background and Objectives With increasing frequency of car accidents, patients of dizziness caused by car accidents are also increasing. Various types of dizziness or vertigo can occur from car accidents depending on different injury mechanisms. Since accurate diagnosis is important for providing proper treatments, we evaluated clinical characteristics related to vestibular function of patients with dizziness caused car accidents. Subjects and Method In this retrospective case review study that runs from January 2011 to March 2013, a total of 82 patients with dizziness following car accident were enrolled consecutively. We analyzed the final diagnosis of dizziness according to different mechanisms of injury during car accident through clinical record review. Patients who developed dizziness within one month of car accident were included, excluding those who had temporal bone fracture and previous history of dizziness. Results Of the different types observed, 36.6% was head injury, 24.4% whiplash injury, 3.7% complex injury, 2.4% others and the rest was unknown. In the final diagnosis, the different types included 36.6% benign paroxysmal positional vertigo (BPPV), 23.2% unclassifiable dizziness, 18.3% cervical vertigo, 7.3% labyrinthine concussion, 3.7% BPPV with labyrinthine concussion and the rest was others. Of the different types of dizziness symptoms, 58.5% was headache, 45.1% was audiologic symptoms, and others included earfullness, tinnitus and hearing disturbance. Tinitogram and pure tone audiogram results show that 2.9% (27 people) of patients have tinnitus and 7.3% (6 people) have hearing disturbance. Conclusion An accurate diagnosis and timely management would be very important in forming a proper approach for post traumatic vertigo patients.

Objectives: To obtain audiologic basic data to diagnose the noise induced hearing loss of workers in fisheries. Methods: The charts of the referred fishermen with noise induced hearing loss from November 2022 to February 2023 at a general hospital were retrospectively reviewed. Pure tone audiometry, speech audiometry, auditory brainstem response test and auditory steady state response test were conducted. Results: All of them were men over 60 years of age, and the average duration of exposure to noise was 38.9 ± 10.8 years, and the average symptom duration of hearing loss was 13.4 ± 4.3 years. Although the hearing thresholds in the high frequencies were higher than thresholds in the low frequencies, the audiogram showed a down-sloping pattern without rebound at 8 kHz. 10.5% of the cases had thresholds greater than 75 dB in high frequencies, but 57.9% had thresholds greater than 40 dB in low frequencies. Other hearing test results of fishermen were similar to those of general noise-induced hearing loss. Conclusions: Although the fishermen were exposed to noise for a long time, they recognized hearing loss late. The hearing threshold in lower frequencies of the fishermen was higher than expected. Further studies will be needed to analyze the audiologic characteristics of noise-induced hearing loss of the fishermen after confirming noise exposure by conducting a survey on the working environment, such as the noise level and working hours.
Purpose: Alport SD., the most common herectitary rephriris, is a renal disease with rapid progression. Deafness, ocular abnormalities and a specific EM finding may be associated in addition to a family history. We have aralyged retrospectively. Methods: We observed 12 children with Alport syndrome who were diagnosed at Dept. of pediatrics in Kyunghee Univ., College of Medicine, from Apr. 1991 until Jun. 1999. We used four criteria for diagnosis: renal disease, family history, deafness or eye abnormalities, and a specific finding in electron microscopy Results: 2 of 12 patients had all features of the four diagnostic criteria. We could not trace an exact family history in 3 patients, and 6 patients did not exhibit deafness or eye abnormality. One could not have renal biopsy because offer chronic renal failure. Other three criteria were observed in her. The ratio of male to female observed was 1:2 respectively and the mean age of initial renal symptom was 5.6 years. 9 of 12 patients had a family history of renal disease. In the audiogram and ocular examination for 11 of 12 cases, sensorineural hearing loss was observed in 6 and ocular abnormality in 2 cases. In electron microscopic finding, irregular thickness of the capillary basement membranes with lamination of lamina densa and foot process obliteration was noted in 9 of 11 and thin basement membrane with splitting and foot process obliteration was noted in the other 2. The mean period of follow-up was 3 6/12 years. And one patient developed the chronic renal failure until now and had kidney transplantation. Conclusion: For the diagnosis of Alport syndrome, the following four diagnostic criteria are very important : renal disease, family history, deafness or eye abnormalities, and a specific finding on electron microscopy. We expect that more patients can be detected through the analysis of these characteristics.
The study was conducted from May to September in 1994 to investigate applicability of the Hearing Handicap Inventory for the Elderly-Screening version(HHIE-S) in parallel with the pure-tone audiometer to the initial screening test of noise-induced hearing loss(NIHL) in some noise-exposed workers. Subjects were selected by systemic sampling that took every 10th person from 6, 700 workers taking the annual occupational health examination by the department of Health Maintenance of Dongsan Hospital Keimyung University in Taegu. The authors administered the pure-tone audiometric test and self-reported questionnaire of HHIE-S including items of sociodemographic and job-related variables concurrently. The final subjects analysed were 1,019(488 males and 531 females) excluding fourteen persons who had many missing values in their questionnaires. The reliability coefficients of HHIE-S scale by Cronbach's alpha were 0.84. In the univariate analysis of hearing handicap measured by the HHIE-S, work duration, military service and the hearing threshold loss at 1KHz and 4KHz by the initial audiometer were significant in males while age, work duration and hearing threshold loss at 1KHz and 4KHz were significant in females. In the stepwise linear regression analysis, hearing threshold loss at 1KHz and 4KHz, was the only selected variable explaining the hearing handicap in males and hearing threshold loss at 1KHz and 4KHz, age, and work duration were selected in females. In ROC curves for HHIE-S scores against NIHL as gold standard which was defined by the follow-up audiogram as more than 30dB of the average of 0.5/1/2KHz and 50dB at 4KHz, the optimal cutoff for the parallel HHIE-S appeared to be 8. The results suggest that HHIE-S appeared to have some reliability and validity in this data and might be used in screening NIHL in parallel with pure-tone audiometer in noise-exposed workers.
The degree of hearing impairment of chronic otitis media will indicate the nature and severity of middle ear pathology especially condition of ossicular chain, size of ear drum perforation and location of granulation tissue in the middle ear cavity. The subjects were 189 ears of tympanoplasty for chronic otitis media and divided into four groups as follows: Normal ossicular chain with only ear drum perforation (group I), normal ossicular chain with granulation tissue only around the ossicles regardless of any other region (group II), ossicular ankylosis or fixation of handle of malleus to promontory with or without granulation tissue around the ossicle (group III) and ossicular interruption by partial or complete destruction(groupf IV). The results were concluded as follows: 1) The average hearing threshold of chronic otitis media was 44.6 dB and hearing threshold was closely related to the condition of ossicular chain. Hearing threshold became greater in order of normal ossicular chain, ankylosis and interruption. 2) The average hearing threshold of ossicular interrupted group was 49.1 dB and it was greater in the cases of total destruction than that of partial destruction. 3) The hearing loss in the cases of normal ossicular chain with only tympanic membrane perforation was within 45 dB and this level was presumed to represent normal ossicular function. The degree of hearing loss was in proportion to the size of ear drum perforation and when over 45 dB, other middle ear pathology was suggested. 4) In the cases of small ear drum perforation with normal ossicular function, the hearing threshold was within 30 dB. 5) In the type of audiogram, flat type was 30.2% and ascending type 35.4%. Descending type was more frequent in the cases of normal ossicular mobility with granulation tissue around the ossicle and flat type was observed frequently in the cases of ossicular ankylosis. 6) Carhart's notch was seen in 14 cases (7.4%) and observed mainly in ossicular ankylosis. 7) There was no relation between hearing threshold and histopathological type of granulation tissue in chronic otitis media. However the degree of hearing impairment was related to the location of granulation tissue in the middle ear cavity. 8) Authors recognized the granulation tissue compensated the function of interrupted ossicular chain.
Monaural and binaural hearing aid performance under quiet and noisy conditions were compared in regard to (1) the degree of hearing impairment, (2) the symmetry of pure tone audiogram, (3) the automatic gain control of the hearing aid. (4) hearing impairement with recruitment and, word discrimination ability. Performance using binaural hearing aids was consistently superior to that using monaural hearing aids. The results were as follows. 1. Speech detection thresholds were enhanced by a mean of 4.25dB when tested with danavox 747 PP stereo type hearing aid and by a mean of 4.12 dB when tested hearing aids connected seperately to the right and left ears. 2. Binaurally tested speech reception thresholds were superior to monaurally tested thresholds by a mean of 3.56dB when tested in quiet and by a mean of 5.56dB when tested in noise. 3. Binaurally tested word discrimination scores were also superior by a mean of 17.09% in quiet and by a mean 19.63% in noise. 4. Both SRT and word discrimination scores were performed best by subjects with moderately-severe impairement. The performance by one mildly impaired subject was the poorest of all performances. The levels of performance order were; moderately-severe loss, severe loss. moderate loss and mild loss. 5. The data obtained using AGC aids when compaired with that of linear amplification show that when AGC aids were worn in both ears. the results were very poor but when one AGC aid was worn in one ear and linear amplification in the other. the results were good. 6. The advantages of binaural hearing aids were obvious even in cases 1) with great diferences in hearing thresholds between right and left ears, 2) when the subject was unable to discriminate words without vision and. 3) when the subject had extreme recruitme t phenomenon.
Even through the methods of improving hearing are widely practiced in Korea, with its 30 years of history, there are many different opinions regarding the selection of surgical technique and the materials to be used in tympanoplasty. Also there are varying standards of postoperative evaluations. Therefore, we have done research to evaluate hearing improvement which in one of the objective of the middle ear surgery. The research was conducted for one year, January to December 1980. This research compared the level of hearing improvement after tympanoplasty, according to materials used in tympanoplasty and collumelization. Following are the data we have obtained; 1) Total number of cases we have reviewed were 306. Out of this, 35% of the cases were tympanoplasty type 1, 12.7% of the cases were collumelization with mastoidectomy, and 11.9% of the cases received collumelization without mastoidectomy. 2) We have conducted audiometry on 41.1% of the tympanoplasty type 1, 64.1% of the collumelization with mastoidectomy, and 45.7% of the collumelization without mastoidectomy. 3) We have observed above 11 dB hearing improvement in 70% of the tympanoplasty type 1, 36% of the collumelization with mastoidectomy, and 44% of the collumelization without mastoidectomy. 4) Over 11 dB decrease of air-bone gap in 61% of the tympanoplasty type 1,32% of the collumelization with mastoidectomy, and 63% of the collumelization without mastoidectomy. 5) If we look at the hearing improvement according to the materials used in the tympanoplasty, there was above 11 dB improvement of the air conduction in 63% with cartilage and 54% with fascia. In air-bone gap, 56% with cartilage and 52% with fascia. 6) If we look at the hearing improvement according to the materials used in the collumelization, there was above 11 dB improvement in 50% with cartilage, 14% with homograft, 55% with autograft. In air-bone gap, 56%, 21%, and 55% respectively.
