Iodine Quantification on Spectral Detector-Based Dual-Energy CT Enterography: Correlation with Crohn's Disease Activity Index and External Validation

  • Objective: To correlate CT parameters on detector-based dual-energy CT enterography (DECTE) with Crohn's disease activity index (CDAI) and externally validate quantitative CT parameters. Materials and Methods: Thirty-nine patients with CD were retrospectively enrolled. Two radiologists reviewed DECTE images by consensus for qualitative and quantitative CT features. CT attenuation and iodine concentration for the diseased bowel were also measured. Univariate statistical tests were used to evaluate whether there was a significant difference in CTE features between remission and active groups, on the basis of the CDAI score. Pearson's correlation test and multiple linear regression analyses were used to assess the correlation between quantitative CT parameters and CDAI. For external validation, an additional 33 consecutive patients were recruited. The correlation and concordance rate were calculated between real and estimated CDAI. Results: There were significant differences between remission and active groups in the bowel enhancement pattern, subjective degree of enhancement, mesenteric fat infiltration, comb sign, and obstruction (p < 0.05). Significant correlations were found between CDAI and quantitative CT parameters, including number of lesions (correlation coefficient, r = 0.573), bowel wall thickness (r = 0.477), iodine concentration (r = 0.744), and relative degree of enhancement (r = 0.541; p < 0.05). Iodine concentration remained the sole independent variable associated with CDAI in multivariate analysis (p = 0.001). The linear regression equation for CDAI (y) and iodine concentration (x) was y = 53.549x + 55.111. For validation patients, a significant correlation (r = 0.925; p < 0.001) and high concordance rate (87.9%, 29/33) were observed between real and estimated CDAIs. Conclusion: Iodine concentration, measured on detector-based DECTE, represents a convenient and reproducible biomarker to monitor disease activity in CD.

Analysis on the Dynamic Behavior according to Suspension Structure of the Urban Railway Vehicle (전동차 현가구조에 따른 동적거동특성 분석)

  • Urban railroad vehicles carry many passengers and are the core of an urban railroad transportation system. Therefore, the dynamic performance of the vehicle must be ensured. Dynamic behaviors such as the vibration and ride comfort of railway vehicles are affected by the structure of the suspension system. We analyzed the dynamic behavior of a railway vehicle according to the suspension system of an urban railway vehicle, which is mainly operated in Korea. For two types of vehicles with different suspension structures, the vibration of the vehicles on railway tracks was measured, and dynamic behavior characteristics such as vibration, ride, and vibration reduction rate were analyzed. The result of the test shows that the vibration performance of the body is superior to that of B-bogie in the lateral direction and that of A-bogie in the vertical direction. Overall, the ride quality of the A-bogie car is superior to that of B-bogie. When analyzing the vibration attenuation rate of primary suspension system, the vibration attenuation performance of B-bogie with coil spring was superior to that of A-bogie with a conical rubber spring. The secondary suspension system has better vibration attenuation performance for A-bogie with air springs compared to coil springs.

Impact of Model-Based Iterative Reconstruction on the Correlation between Computed Tomography Quantification of a Low Lung Attenuation Area and Airway Measurements and Pulmonary Function Test Results in Normal Subjects

  • Objective: To compare correlations between pulmonary function test (PFT) results and different reconstruction algorithms and to suggest the optimal reconstruction protocol for computed tomography (CT) quantification of low lung attenuation areas and airways in healthy individuals. Materials and Methods: A total of 259 subjects with normal PFT and chest CT results were included. CT scans were reconstructed using filtered back projection, hybrid-iterative reconstruction, and model-based IR (MIR). For quantitative analysis, the emphysema index (EI) and wall area percentage (WA%) were determined. Subgroup analysis according to smoking history was also performed. Results: The EIs of all the reconstruction algorithms correlated significantly with the forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) (all p < 0.001). The EI of MIR showed the strongest correlation with FEV1/FVC (r = -0.437). WA% showed a significant correlation with FEV1 in all the reconstruction algorithms (all p < 0.05) correlated significantly with FEV1/FVC for MIR only (p < 0.001). The WA% of MIR showed the strongest correlations with FEV1 (r = -0.205) and FEV1/FVC (r = -0.250). In subgroup analysis, the EI of MIR had the strongest correlation with PFT in both eversmoker and never-smoker subgroups, although there was no significant difference in the EI between the reconstruction algorithms. WA% of MIR showed a significantly thinner airway thickness than the other algorithms ($49.7{\pm}7.6$ in ever-smokers and $49.5{\pm}7.5$ in never-smokers, all p < 0.001), and also showed the strongest correlation with PFT in both ever-smoker and never-smoker subgroups. Conclusion: CT quantification of low lung attenuation areas and airways by means of MIR showed the strongest correlation with PFT results among the algorithms used, in normal subjects.

Change of PET Image According to CT Exposure Conditions (CT 촬영 조건에 따른 PET 영상의 변화)

  • PET-CT improves performance and reduces the time by combining PET and CT of spatial resolution, and uses CT scan for attenuation correction. This study analyzed PET image evaluation. The condition of the tube voltage and current of CT will be changed using. Uniformity phantom and resolution phantom were injected with 37 MBq $^{18}F$ (fluorine ; 511 keV, half life - 109.7 min), respectively. PET-CT (Biograph, siemens, US) was used to perform emission scan (30 min) and penetration scan. And then the collected image data were reconstructed in OSEM-3D. The same ROI was set on the image data with a analyzer (Vinci 2.54, Germany) and profile was used to analyze and compare spatial resolution and image quality through FWHM and SI. Analyzing profile with pre-defined ROI in each phantom, PET image was not influenced by the change of tube voltage or exposure dose. However, CT image was influenced by tube voltage, but not by exposure dose. When tube voltage was fixed and exposure dose changed, exposure dose changed too, increasing dose value. When exposure dose was fixed at 150 mA and tube voltage was varied, the result was 10.56, 24.6 and 35.61 mGy in each variables (in resolution phantom). In this study, attenuation image showed no significant difference when exposure dose was changed. However, when exposure dose increased, the amount of dose that patient absorbed increased too, which indicates that CT exposure dose should be decreased to minimum to lower the exposure dose that patient absorbs. Therefore future study needs to discuss the conditions that could minimize exposure dose that gets absorbed by patient during PET-CT scan.

The Peak Systolic Pressure-End Systolic Volume Index Relation in Patients with Coronary Artery Disease Using Radionuclide Gated Blood Pool Scintigraphy (관상동맥질환에서 심장풀스캔을 이용한 최고수축기혈압-수측기말용적곡선의 분석)

  • For measurement of ventricular performance, ejection fraction (EF) has gained wide acceptance. But EF is influenced not only by changes in muscle function but also by changes in cardiac loading conditions. In case of valvular heart disease which is variable in loading conditions, EF cannot be reliable as an index of myocardial contractility. The end systolic pressure (ESP)-end systolic volume (ESV) relation, howver, is known to represent myocardial contractility, independent of changes in loading conditions. Similar results can be obtained by using peak-systolic pressure (PSP) instead of ESP. To evaluate the utility of the peak systolic pressure-end systolic volume index (PSP-ESVI) relation as an index of myocardial function, we measured $PSP&ESVI$ in 19 partents with coronary artery disease before $(PSP_1\;&\;ESVI_1)$ and after $(PSP_2\;&\;ESVI_2)$ sublingual administration of nitroglycerin. PSP was measured with standard mercury sphygmomanometer during gated blood pool scintigraphic study. ESVI was measured by count derived method after attenuation correction. $PSP_2\;&\;ESVI_2$ measurement was started when the fall of PSP was greater than 5 mmHg after 7-14 minutes post-administration of nitroglycerin. Mean values $({\pm}S.D.)$ of $PSP_1\;&\;ESVI_1$ was $124.9({\pm}20.7)mmHg\;&\;59.4({\pm}39.9)ml/M^2$. Mean values $({\pm}S.D)$ of $PSP_2\;&\;ESVI_2$, was $113.2({\pm}19.9)mmHg\;&\;37.5({\pm}26.1)ml/M^2$. There was a significant difference between mean values of $PSP_1\;&\;PSP_2$, (p<0.01), and mean values of $ESVI_1\;&\;ESVI_2$, (p<0.01). $PSP_1-PSP_2/ESV_1-ESVI_2,\;PSP_1/ESVI_1$ and EF were in the range of 0.14-5.19 mmHg/ml/$M^2$, 0.67-7.68 mmHg/ml/$M^2$ and 10.8%-74.5% respectively. $PSP_1-PSP_2/ESVI_1-ESVI_2$, and EF showed exponential correlation (r=0.85, P<0.01). The correlation coefficient between $PSP_1/ESVI_1$ and EF was 0.73(p<0.01). With the above results, we suggest that $PSP_1-PSP_2/ESVI_1-ESVI_2$, and $PSP_1/ESVI_1$, can be used as an index of myocardial function.

Nutritional Status and Bone Mineral Density of Elderly Women in Asan

  • Nutrition factors play an important role in the pathogenesis of osteoporosis. The purpose of this study is to investigate the relationship between nutritional status and bone mineral density of elderly women. Three hundred thirty five elderly women (over 65 years) in Asan were divided into three age groups (group 1, less than 70 y ; group 2, from 70 to 75 y ; group 3, 75 y or more). Total alkaline phosphatase and serum calcium (Ca) were analyzed using spectrophotometric procedure. Data for food and nutrient intakes were obtained by a 24-hour recall method. Bone density was measured by broadband ultrasound attenuation (BUA) using QUS-2. Age differences were tested with the X 2 test for categorical variables and with ANOVA and Tukey's test for continuous variables. Correlation was conducted to test the association between bone density and nutrient intake. The subjects in age groups 1,2 and 3 were $36.7\%$, $32.8\%$ and $30.4\%$, respectively. Height and body weight of the subjects were significantly decreased with age. Average bone density of the subjects in group 3 was lower than the other age groups. Osteoporosis determined by t-score is $17.9\%$ for group 1, $24.5\%$ for group 2 and $55.9\%$ for group 3 (p < 0.001). The serum Ca level of the subjects in group 2 was significantly lower than that of group 1 although mean values in all age groups are within the normal range. Dietary Ca intake, nutrient adequacy ratio (NAR) and index of nutrient quality (INQ) were decreased with age. Bone density was negatively correlated with age (p < 0.001), while body mass index (p < 0.01) was positively related with bone density. Although partial correlation did not reveal the significant correlation of BMD and dietary calcium after controlling for age, since calcium intake was very poor compared to sodium and phosphorous intakes, recommendation of more calcium intake for elderly women especially those over 75 years must be continuously emphasized.

Monitoring of Microbial Diversity and Activity During Bioremediation of Crude Oil-Contaminated Soil with Different Treatments

  • The present study compared the microbial diversity and activity during the application of various bioremediation processes to crude oil-contaminated soil. Five different treatments, including natural attenuation (NA), biostimulation (BS), biosurfactant addition (BE), bioaugmentation (BA), and a combined treatment (CT) of biostimulation, biosurfactant addition, and bioaugmentation, were used to analyze the degradation rate and microbial communities. After 120 days, the level of remaining hydrocarbons after all the treatments was similar, however, the highest rate (k) of total petroleum hydrocarbon (TPH) degradation was observed with the CT treatment (P<0.05). The total bacterial counts increased during the first 2 weeks with all the treatments, and then remained stable. The bacterial communities and alkane monooxygenase gene fragment, alkB, were compared by denaturing gradient gel electrophoresis (DGGE). The DGGE analyses of the BA and CT treatments, which included Nocardia sp. H17-1, revealed a simple dominant population structure, compared with the other treatments. The Shannon-Weaver diversity index (H') and Simpson dominance index (D), calculated from the DGGE profiles using 16S rDNA, showed considerable qualitative differences in the community structure before and after the bioremediation treatment as well as between treatment conditions.

The Inhibitory Effects of Liriopis Tuber on the Bleomycin-induced lung fibrosis in C57BL/6J mice (맥문동(麥門冬)이 C57BL/6J 생쥐의 Bleomycin 폐섬유화(肺纖維化)에 미치는 영향(影響))

  • Objectives : The aim was to identify the inhibitory effects of Liriopis Tuber on bleomycin-induced lung fibrosis by analysing the changes of imflammatory cell cytokines and SHI(Semiquantitative Histological Index), Materials and Methods : In this study fibrosis prone C57BL/6J mice were used. Control group was treated with blomycin(0.06mg/0.1 ml) by IT(intratracheal) instillation which is a popular method of inducing lung fibrosis and sample group took Liriopis Tuber water extract(38.0mg/10g body weight) orally for 14 days after IT instillation of blomycin. We measured the total and differential count of WBC, $IFN-{\gamma}$ & IL-4 in mice BALF and SHI(Semiquantitative Histological Index) from lung tissues of mice. BALF and lung tissues of mice were taken 14 days after IT instillation of blomycin. Results : In sample group total WBC count, proportion of neutrophil, SHI and IL-4 significantly(p<0.05) decreased, proportion of macrophage significantly(p<0.05) increased and proportion of lymphocyte, $IFN-{\gamma}$ did not decrease significantly. Conclusions : This study suggests that Liriopis Tuber has an inhibitory effects of pulmonary fibrosis by attenuation of inflammation and Th2 immune response. To determine whether this herbal medicine contribute to cure and prophylaxis of pulmonary fibrosis, further studies on the role of $IFN-{\gamma}$ relating to fibrosis are required.

Proposal of CT Simulator Quality Assurance Items (전산화단층 모의치료장치의 정도관리 항목 제안)

  • A quality assurance of computed tomography(CT) have done seven items that were water attenuation coefficient, noise, homogeneity, spatial resolution, contrast resolution, slice thickness, artifact using by standard phantom. But there is no quality assurance items and methods for CT simulator at domestic institutions yet. Therefore the study aimed to access the CT dose index(CTDI), table tilting, image distortion, laser accuracy, table movement accuracy and CT seven items for CT simulator quality assurance. The CTDI at the center of the head phantom was 0.81 for 80 kVp, 1.55 for 100 kVp, 2.50 for 120 mm, 0.22 for 80 kVp at the center of the body phantom, 0.469 for 100 kVp, and 0.81 for 120 kVp. The table tilting was within the tolerance range of ±1.0° or less. Image distortion had 1 mm distortion in the left and right images based on the center, and the laser accuracy was measured within ±2 mm tolerance. The purpose of this study is to improve the quality assurance items suitable for the current situation in Korea in order to protect the normal tissues during the radiation treatment process and manage the CT simulator that is implemented to find the location of the tumor more clearly. In order to improve the accuracy of the CT simulator when looking at the results, the error range of each item should be small. It is hoped that the quality assurance items of the CT simulator will be improved by suggesting the quality assurance direction of the CT simulator in this study, and the results of radiation therapy will also improve.

Association Between Physical Activity and Quantitative Ultrasound in a Rural Population (신체활동과 골초음파지표와의 연관성)

  • Osteoporosis is characterized by low bone mass and the microarchitectural deterioration of bone tissue with a consequent increase in bone fragility and susceptibility to fracture. It has been suggested that speed of sound (SOS) and broadband ultrasound attenuation (BUA) of quantitative ultrasound sonography (QUS) may provide information about not only bone density but also the microarchitecture and elastic properties of bone. Physical inactivity reduced mechanical usage and it made process to the bone changes. This study aimed to association between the physical activity and the QUS parameters in 1305 (593 men, 712 women) aged 20 years over in a rural population. Two QUS parameters, BUA (p=.23) and SOS(p=.73) were measured at the right calcaneus of postmenopausal women, no significant associations were observed between sports index and SOS and BUA. These results suggest that work, non-sports leisure physical activity (p<.01) have a significant influence on QUS parameters in a rural population. Physical activity are meaningful predictor of QUS parameters of the calcaneus in a rural population.
