주의력결핍 과잉행동장애에서 정신자극제를 이용한 약물치료 (Stimulants Medication of Attention-Deficit Hyperactivity Disorder)

  • Attention-deficit hyperactivity disorder (ADHD) is characterized by inattention, hyperactivity, impulsiveness and problems in other higher cognitive processes such as executive function deficits. Currently, there are many treatment modalities, of which pharmacotherapy is the most strongly supported by scientific and clinical evidence. Stimulants, which are first choice in the pharmacological treatment of ADHD, block dopamine reuptake by binding the dopamine transporter and so increasing the concentration of dopamine in synaptic clefts. Stimulants are effective in improving core ADHD symptoms, as well as the nonspecific symptoms, such as aggressiveness and oppositional behavior. Frequently reported short-term adverse effects are decreased appetite, sleep disturbance, headache, dizziness and irritability. Although questions have been raised about the long-term side effects of stimulants, including growth suppression, cardiovascular events, and abuse potential, there is no clear evidence to support these concerns.

Psychometric Properties of the Korean Translation of the Attention-Deficit/Hyperactivity Disorder Stigma Questionnaire

  • Objectives: This study evaluated the psychometric properties of the Korean version of the attention-deficit/hyperactivity disorder (ADHD) Stigma Questionnaire (ASQ) and the effect of the source of information about mental health on ADHD stigma. Methods: The Korean translation of the ASQ was prepared, and 673 participants, 20-64 years of age, completed the questionnaire using an online panel survey in South Korea. The participants also completed questionnaires detailing sociodemographic variables and the source of their mental health knowledge. Cronbach's alpha coefficient was used to explore the internal consistency of the ASQ. Factor analysis using Varimax rotation was conducted to investigate the structure of the ASQ. Results: The 26-item ASQ demonstrated excellent internal consistency (Cronbach's alpha=0.940). Factor analysis supported a three-factor structure, including Concerns with Public Attitudes, Negative Self-Image, and Disclosure Concerns. There were no significant differences in the total ASQ scores according to sociodemographic characteristics. Participants who reported the internet as their major source of information about mental health showed higher ASQ scores compared to those who used other sources for mental health information. Conclusion: The Korean translation of the ASQ has acceptable psychometric properties among Korean adults. Inaccurate information from the internet could increase the stigma toward ADHD.

  • 교통사고 등에 의한 외상성 뇌손상에서 이차적으로 주의력결핍 과잉행동장애가 나타나는 경우가 점차 증가하고 있으며, 이에따른 치료방법도 점차 다양화되고 있는 추세이다. 주의력결핍 과잉행동장애에서 많이 사용되어지는 OROS Methylphenidate를 환자에게 투여하여 인지적인 개선뿐만아니라 충동성, 산만함, 공격성 등의 증상에 효과가 있는지의 여부를 살펴보고자 하였다. 약물투여 전에 비하여 약물투여 후에 환자는 충동성, 공격성, 부주의의 개선효과를 나타내었다.

  • Attention deficit hyperactivity disorder (ADHD) is a common childhood psychiatric disorder. Recently, it has been suggested that brain-derived neurotropic factor (BDNF) may play a role in the pathogenesis of ADHD. Our aim of this review is to understand the physiological functions of BDNF and its potential relationship with ADHD and therapeutic approaches of ADHD. Searches were conducted in Pubmed and Research Information Service System (RISS). In this review, we summarized important literatures for the physiological functions of BDNF in neurodevelopment, change of serum BDNF level in ADHD, association of BDNF polymorphism and ADHD and potential association of treatment of ADHD with serum BDNF level. Further studies are required to more clearly understand the source and the role of BDNF in ADHD and to develop BDNF based-ADHD treatement.

주의력 결핍 과잉활동장애의 행동 특성 - 과잉활동성 유무에 따른 임상적 특성의 비교 - (BEHAVIORAL CHARACTERISTICS IN ATTENTION DEFICIT HYPERACTIVITY DISORDER - COMPARISONS OF ATTENTION DEFICIT DISORDER WITH AND WITHOUT HYPERACTIVITY -)

  • 저자들은 Attention Deficit Hyperactivity Disorder(ADHD) 환자에서 과잉활동증을 동반한 집단과 과잉활동증을 동반하지 않은 집단간에 행동 및 정서적 특성을 비교하고, 각 증상척도들 간의 임상적 관계를 알아보기 위하여 본 연구를 시행하였다. ADHD 환자 36명을 Werry-Weiss-Peters Activity Rating Scale(WWPARS) 평가기준(20점)에 따라 Attention Deficit Disorder with Hyperactivity(ADD+H) 집단 19명과 Attention Deficit Disorder without Hyperactivity(ADD-H) 집단 17명으로 분류하여 Conners Rating Scale(CRS), Home Situations Questionnaire(HSQ), School Situations Questionnair(SSQ), Child Behavior Check-list(CBCL) 등을 시행한 결과 다음과 같은 결과를 얻었다. 1) 두 집단간에 인구학적 특성은 차이가 없었다. 2) ADD+H 집단의 CRS 행동증상척도는 점수는 ADD-H 집단에 비하여 유의하게 높았으며, HSQ와 SSQ를 통하여 가정과 학교에서의 행동문제 상황을 비교한 결과, ADD+H 집단의 행동문제 상황의 유형이나 개수 및 증상의 심도가 ADD-H 집단에 비하여 유의하게 높았다. 3) 각 집단의 CBCL 8개 소척도 점수를 비교하였을 때, ADD+H 집단의 비행척도와 공격성 척도가 유의하게 높았다. 4) ADHD의 행동증상과 CBCL의 행동정서척도 간의 상관관계를 조사한 결과, CRS, WWWPARS의 행동증상척도와 CBCL의 공격성척도, 비행척도, 정서불안정척도 간에는 유의한 양성 상관관계가 있었으며, 특히 공격성척도와 과잉활동증 간에는 매우 유의한 상관성이 있음을 알 수 있었다. 본 연구결과로 미루어 ADHD는 과잉활동증의 유무에 따라 그 임상적 특징이 차이가 있음을 시사하며, 향후 ADHD의 여러 임상적 유형과 약물치료효과 간의 관련성에 대한 연구 및 ADHD 환자에서 행동 특성과 인지기능이 통합된 연구의 필요성이 요구된다.

The Relationship of Clinical Symptoms with Social Cognition in Children Diagnosed with Attention Deficit Hyperactivity Disorder, Specific Learning Disorder or Autism Spectrum Disorder

  • Objective One of the areas of social cognition is Theory of Mind (ToM) is defined as the capacity to interpret, infer and explain mental states underlying the behavior of others. When social cognition studies on neurodevelopmental disorders are examined, it can be seen that this skill has not been studied sufficiently in children with Specific Learning Disorder (SLD). Methods In this study, social cognition skills in children diagnosed with attention deficit hyperactivity disorder (ADHD), SLD or Autism Spectrum Disorder (ASD) evaluated before puberty and compared with controls. To evaluate the ToM skills, the first and second-order false belief tasks, the Hinting Task, the Faux Pas Test and the Reading the Mind in the Eyes Task were used. Results We found that children with neurodevelopmental disorders as ADHD, ASD, and SLD had ToM deficits independent of intelligence and language development. There was a significant correlation between social cognition deficits and problems experienced in many areas such as social communication and interaction, attention, behavior, and learning. Conclusion Social cognition is an important area of impairment in SLD and there is a strong relationship between clinical symptoms and impaired functionality.

과잉운동 품행장애의 내용 타당도에 관한 연구 (A STUDY OF THE CONTENT VALIDITY IN HYPERKINETIC CONDUCT DISORDER)

  • ICD-10의 진단 기준에 새로이 포함된 과잉운동 품행장애(hyperkinetic conduct disorder)의 내용 타당도를 검증하기 위한 연구를 시행하였다. 이를 위한 구체적 방법으로, 주의력결핍 과잉 운동장애 환자군에서 품행장애가 공존한다는 기존의 연구 결과가 있으므로, 반대로 품행장애 환자군에서 주의력결핍 과잉운동장애가 공존하는 정도를 측정하였다. 대상군은 품행장애로 진단된 29명을 환자군으로 삼았고, 정상 대조군으로 41명을 비교하였으며, 각각에게는 Conners의 부모평가 척도의 DSM-III-R에 의한 부모 평가 척도를 시행하였고 결과는 다음과 같다. 1) 품행장애 환자군에서 대조군에 비하여 Conners의 부모 평가 척도와 DSM-III-R에 의한 부모 평가 척도에서 주의력 결핍 과잉운동장애의 정도가 의미 있게 높은 것으로 나타났다. 2) 품행장애 환자군에서 대조군에 비하여 주의력결핍 과잉운동장애의 공존율이 의미 있게 높았다. 이상의 결과로 미루어 품행장애 환자군에서 주의력결핍 과잉운동장애의 공존이 의미있게 높다는 것을 알 수 있었고, 따라서 과잉운동 품행장애의 내용 타당도가 인정되었다.

주의력결핍 과잉행동장애 치료에 대한 문헌적 고찰 -최근 중의 잡지를 중심으로- (The Literature Study for Attention Deficit Hyperactivity Disorder(ADHD) in Traditional Chinese Medical Journals)

  • Objective : The aim of this study was to investigate treatment of Attention Deficit Hyperactivity Disorder(ADHD) in current Traditional Chinese Medical Journals. Methods : From January 2000 to December 2005, we searched Traditional Chinese Medical Journals. Results : 21 articles were selected and classified into two large groups, case report and comparison study. Totally 1137 cases, complete cures are 276, significant improvements and some improvement 1028, complete cure rate 24.27%, total improvement rate 90.41%. Conclusion : Oriental medical treatments for ADHD were investigated to be effective in most trials. But there remains a debate about outcome criteria. Further studies are needed. Treatment methods were herbal medicine, acupuncture, auricular acupuncture, family consult.

The Extent of Ordinary Classrooms Teachers' Knowledge and Perception of Attention Deficit Hyperactivity Disorder (ADHD)

  • The present study aimed to investigate the extent to which ordinary classrooms teachers are informed are familiar with Attention Deficit Hyperactivity Disorder 'ADHD'. The researcher adopted the descriptive approach. The sample of the study consisted of (89) male and female teachers who teach students from grade (1) to grade (6) at public primary schools in Northern Mazar Directorate of Education in Jordan. A questionnaire was employed as the primary tool for collecting the data. The findings of the current study indicated an average level of knowledge of ordinary classroom teachers about ADHD. The findings also indicated that there were no statistically significant differences in the extent of ordinary classroom teachers' knowledge of ADHD due to the variables (gender, qualification, professional experience, grade level being taught).
