Effects of Interactive Metronome Intervention on Behavior Symptoms, Timing, and Motor Function of Children With ADHD (상호작용식 메트로놈 중재가 주의력결핍과잉행동장애의 행동증상, 타이밍, 및 운동기능에 미치는 효과)

  • Objective : The purpose of this study is to investigate the effect of modified Interactive Metronome (IM) program which is applicable to clinical practice based on the IM protocol on the behavioral symptoms, timing and motor function of children with Attention Deficit Hyperactivity Disorder (ADHD). Method : This study used one-group pretest-posttest research design. 13 ADHD children aged 7-12 years in Busan were participated in this study. The participants were underwent 24 sessions of 30 minutes intervention, 3 times a week. Evaluations were performed before- and after the intervention. Measurements used in this study were Korean-ADHD Rating Scale for behavior symptom, Long Form Assessment (LFA) for the timing, and second version of Bruininks-Oseretsky Test of Motor Proficiency (BOT-2) for the motor function. Results : There was a significant improvement in hyperactivity / impulsivity among the behavioral symptoms, and there was a statistically significant improvement in timing, hand coordination, and body coordination. Conclusion : Modified IM program for clinical application has significant effect on improving behavioral symptoms, timing and motor function of children with ADHD.

Analysis of Working Memory for Attention Deficit Hyperactivity Disorder (ADHD) Children using fMRI (주의력결핍 과잉행동성장애(ADHD) 아동의 작업기억 과제 수행 시 fMRI 분석)

  • Attention deficit hyperactivity disorder (ADHD) students' intellctual defects, learning problems, and poor academic achievements seem to be due to significantly lower intelligence compared to the normal students, but rather the characteristic of inability to pay attention at a given time can be seen as the more attributing reason. In this study, a comparison between the ADHD students and the normal students will be performed using a fMRI analysis in order to differentiate the brain function between the two groups during a working memory task performance and to assess the difference in the activated regions of the brain. Clinical survey examinations and fMRI measurements were performed for a group of 26 elementary students from the Incheon area. The stimulus of fMRI was a working memory. Cartography statistically analyzed parameters and the Statistical Package of Social Sciences using single-sample t-test, two-sample t-test, were analyzed by multiple regression analysis, the statistical significance level was p<0.05 in, respectively. The disproportionate developments could be seen in the ADHD students group such as the frontal cortex, parietal cortex, thalamus, and caudate nucleus, among others. In addition, as some students felt the increase in the difficulty of working memory task performance, the orbitofrontal cortex and the hippocampus were activated, which seems to be the result of an effort for looking for an answer. More types of ADHD students needs to be secured as research subjects, and more stimulations for fMRI experiments should be considered as it would be useful in the overall evaluation of brain function.

MMPI Characteristics of Parents of Children with ADHD (주의력결핍 과잉행동장애 부모에서 MMPI 특성)

  • Objectives: The current study investigated the personality characteristics of parents of children with attention-deficit hyperactivity disorder (ADHD) using the Minnesota Multiphasic Personality Inventory (MMPI), which is commonly used in clinical medicine. Methods: Participants were 117 parents of children with ADHD (96 boys and 21 girls) and 77 parents of comparison children (50 boys and 27 girls), who completed the Korean version of the MMPI. Results: The MMPI scores of the fathers of ADHD children were significantly higher on the Psychopathic Deviate (Pd), Masculinity-Femininity (Mf), Paranoia (Pa), and Psychasthenia (Pt) scales than the comparison group's were. In addition, the mothers of ADHD children had higher MMPI scores on the traits of Hypochondriasis (Hs), Psychopathic Deviate (Pd), and Schizophrenia (Sc) than the comparisong roup had, but were not significantly higher. Conclusion: The fathers of ADHD children might be antisocial, irresolute, passive, paranoid, and anxious. In addition, mothers of ADHD children might have hypochondriacal, antisocial, and/or psychological confusional traits, but these were not be significantly high. These results suggest that the psychopathology of parents of ADHD children might correlate with their children's ADHD.

  • The purpose of this study was to examine the relationship among children's ADHD symptoms, parenting stress and behavior regarding nurturing. The data were provided by 59 mothers of children with ADHD who is aged between 4 and 12. Mothers of children with ADHD were recruited through 3 departments of child psychiatry. The Korean version of Parenting Stress Index-Short Form, the Korean version of revised Maternal Behavior research Instrument(MBRI) and ADHD Rating Scale(K-ARS) was used to assess parenting stress, behavior regarding nurturing and ADHD symptoms respectively. Significant correlation was found between parental distress and warmth, rejection behavior. Parental distress is significant influence on behavior regarding nurturing through multiple regression analysis. Significant correlation was found between ADHD symptom and rejection behavior. ADHD symptom is significant influence on behavior regarding nurturing through multiple regression analysis. Behavior regarding nurturing is related to ADHD symptom, parental distress. Therefore, it is needed for not only treatment related to ADHD symptom but also interventions for behavior regarding nurturing for mothers of children with ADHD.

  • The author studied EEG differences among normal, children, children with emotional behavioral problems determined by Rutter's questionnaire and children with attention-deficit hyperactivity disorder according to DSM-III-R. The results are as follows ; There were no differences in incidence and pattern of abnormal EEG between normal control children and children with emotional behavioral problems. But children with attention-deficit hyperactivity disorder tend to show lower average amplitude, and less wave percentage time of alpha wave in occipital area than normal did, furdermore wave percentage time of alpha wave appeared more in left occipital area than in right occipital area, These results demonstrated that emotional behavioral problems in elementary school children may not be related to EEG abnormality, Where as children with attention deficit hyperactivity disorder showed underdeveloped alpha waves as compared with normal control children.

Revisiting History of Treatment of Attention Deficit Hyperactivity Disorder : Pharmacologic Approach (주의력결핍 과잉행동장애 치료의 역사적 재조명 : 약물치료적 접근)

  • Besides from medical data, the patients who were previously called as attention disorder, mental instability, moral imbeciles, or moral defectives, can also be identified by exploring literatures and historical figures. In the past, as we can notice from the titles, they were recognized as a 'moral defect group'. And rather than treating them, separation from the society was the main solution. After the endemic encephalitis from 1917 to late 1920s, however, many survivors suffered from behavioral problems similar to those of the previous 'moral defect group' and studies on the relationship between brain damage and behavior problems were started henceforth. After being known as the 'minimal brain dysfunction', it was developed into the current attention-deficit/hyperactivity disorder. While the disease concept changed and developed over time, after numerous trials and errors, treatment medication starting from central nervous system stimulants such as amphetamine and methylphenidate is used for treatment in children and adult patients with ADHD, and most recently non-stimulants such as atomoxetine has become the one of the first line treatment options. Although we went through a thorough verification process of the safety and efficacy of the medication by contemplating the historical development process, we believe that adjustment is needed for remaining concerns on medication abuse and slight differences in disease paradigm and therapeutic philosophy depending on cultures.

  • Attention-deficit hyperactivity disorder (ADHD) is one of the most common childhood psychiatric conditions. In 2007, the Korean Academy of child and Adolescent Psychiatry developed Korean ADHD practice parameter. Advances in the scientific evidence of ADHD caused practice parameter to be modified and updated. The present guidelines developed by ADHD translational research center summarize current literature for the treatment of ADHD in children and adults. This parameter includes the clinical evaluation for ADHD, comorbid conditions associated with ADHD, clinical feature and course, research on the etiology of the disorder, and psychopharmacological and non-pharmacological treatments for ADHD.

Effect of the Ayres Sensory Integration Intervention on the Motor Skills and Occupation Participation of Preschool Children with Attention-Deficit/Hyperactivity Disorder (Ayres의 감각통합중재가 학령전기 주의력결핍 과잉행동장애(ADHD) 성향 아동의 운동기능 및 작업참여에 미치는 영향)

  • Objective : This study aimed to investigate the impact of Ayres' sensory integration (ASI) intervention on motor skills and occupational participation of preschool children with attention-deficit/hyperactivity disorder (ADHD). Method : Children with ADHD aged between 4 and 6 years who met the inclusion and exclusion criteria were randomly recruited through screening tests. The subjects were divided into an experimental group (10 subjects) and a control group (8 subjects). The instruments used were the Bruininks-Oseretsky test of motor proficiency-2 (BOT-2), Pediatric Evaluation of Disability Inventory (PEDI), and Goal Attainment Scale (GAS) to evaluate occupational participation. The subjects in the experimental group underwent individual sensory integration therapy according to the ASI principles for 40 minutes twice a week in a total of 16 sessions over eight weeks. The control group did not receive the ASI intervention. Data analysis was performed using the Mann-Whitney U test, chi-squared test, Wilcoxon signed-rank test, and Cohen's d test in SPSS 20.0. Results : The ASI experimental group had significantly higher scores in total motor composite, manual coordination, body coordination, strength, and agility in motor function than the control group (p<.05). The two groups did not differ significantly in terms of occupational participation (PEDI), but GAS scores for individual target activities were significantly higher in the experimental group than in the control group (p<.05). Conclusion : This study shows that the ASI intervention has positive effects on motor skills and occupation participation among preschool children with ADHD.

Compared Effectiveness of 10-Session Social Skill Training for Korean Early Elementary School Children in Two Groups Diagnosed as Pure ADHD and ADHD with Comorbidity (초등학교 저학년 대상 ADHD 단독군, 동반 질환을 가진 ADHD군에서의 10회기 사회기술 훈련의 효과 비교)

  • Objectives : Children with attention-deficit hyperactivity disorder (ADHD) have problems in social interactions. We compared the effect of 10-session social skill training (SST) among two groups, children with pure ADHD, and those with ADHD with comorbidity. Methods : Consecutive 10-session SST was conducted for 34 children from 2006 to 2012. There were 22 children with pure ADHD (male 20, female 2), and 12 children suffering from ADHD with comorbidity (male 11, female 1). All children took medication as prescribed by their doctors before the start of SST. The Child Behavior Checklist (CBCL), the Korean Personality Inventory for Children (K-PIC), the Conner's Rating Scale, the ADHD Rating Scale, and the Home Situation Questionnaire were completed by mothers before and after the SST. All children completed the Child Depression Inventory, the Stat-Trait Anxiety Inventory for Children, the Self-Concept Scale and the ADHD Diagnostic System before and after the SST. Results : Only children with pure ADHD showed improvement in anxiety and self-concept in scales rated by children. In the CBCL rated by parents, the pure ADHD group and the ADHD with comorbidity showed improvement in both externalizing and internalizing subscales. In the K-PIC rated by parents, the pure ADHD group showed improvement in most outcomes and ADHD with comorbidity showed positive change in verbal development. Conclusion : These results suggest that SST has significant positive effects on both the pure ADHD and ADHD with comorbidity group. Further research is needed in order to target diverse comorbidity groups with ADHD to improve the effectiveness of the SST.

A Study on the Relationships between Convergence Art Education and Therapy Children with disabilities -Focusing on the ADHD children education- (융합 예술 교육과 장애 아동 치료효과간의 관련성에 관한 연구 -ADHD 아동 교육 사례를 중심으로-)

  • ADHD (Attention Deficit Hyperactivity Disorder) appears most in childhood and shows attention deficit, hyperactivity and impulsive activity. If these symptoms are left untreated, they consistently remain as difficulties throughout the childhood and in some cases they will last until adolescence and adulthood. Various researches on music therapy as well as psychological therapy and Korean classical music appreciation have currently been conducted. However, a variety of programs have introduced in the name of treatment but the improvement results through the application of programs and teaching methods, in reality, have hardly been a specific case. Thus, music educators as well as this researcher should make the efforts to form holistic characters and have interest in countless children with ADHD. Accordingly, this researcher, who has taught piano lessons, has performed a wide variety of convergence art teaching methods by applying methods of Dalcroze, Kodaly, and Orff to children with ADHD, misanthropy or lack of affection. As a result, symptoms have lessened and been treated for ADHD children with attention deficit. On the basis of this experience, there have appeared more educational effects by applying these to other children. This researcher is certain that this study will a foundation of music therapy education for children with each kind of syndrome.
