Diagnostic Validity of the Comprehensive Attention Test in Patients With Attention-Deficit/Hyperactivity Disorder

  • Objectives: This study verified the diagnostic validity of the Comprehensive Attention Test (CAT) for attention-deficit/hyperactivity disorder (ADHD). Methods: A total of 336 participants were recruited in this study, including 168 patients with ADHD and 168 control group participants who were one-to-one matched for sex and age. We measured selective attention (visual and auditory), sustained attention, interference-selective attention, divided attention, and working memory in the ADHD and control groups using the CAT. The sensitivity, specificity, positive predictive value, and negative predictive value of the CAT were calculated. Results: In this study, it was found that the sensitivity and specificity of the CAT were 0.879 and 0.846 in children, 0.855 and 0.838 in adolescents, and 0.800 and 0.733 in adults, respectively. Conclusion: These results indicate that the CAT has a high diagnostic validity for ADHD from childhood to adulthood,

CHARACTERISTICS OF ATTENTION DEFICIT OF ADHD ON COMPUTERIZED NEUROCOGNITIVE FUNCTION TESTS (전산화 신경인지기능검사를 이용한 주의력결핍/과잉운동 장애의 주의력결핍특성에 관한 연구)

  • Characteristics of attention deficit of attention-deficit hyperactivity disorder(ADHD) were investigated by administering six computerized attention tests of Vienna Test System and four neuropsychological tests to children aged 6-12, with ADHD(n=21) and age-matched normal control children(n=22). The findings indicated that ADHD children show lower level of vigilance, more decline of performance in vigilance task on time, and impaired preparedness to response. They also have selective attention deficit on monitoring tasks, but did not have sustained attention deficit compared with normal control children. On the tasks wich overload their information processing capacity, ADHD children show more impulsive response pattern than normal control children. The performance of ADHD was worse than control on the neuropsychological tests sensitive to frontal lobe dysfunction. The presense 'These' attention deficits supports the theory that the defect of ADHD is due to the dysfunction of more than one brain region, including brain stem reticular formation and frontal lobe.

  • Objectives: This study aimed to investigate the correlation between the Comprehensive Attention Test, Korean-Wechsler Intelligence Scale for Children-Fourth Edition, and Attention-Deficit/Hyperactivity Disorder (ADHD) Rating Scale-IV scores in children and adolescents with ADHD. Methods: Fifty-five children and adolescents diagnosed with ADHD and not taking psychiatric medications were included in this retrospective study. A correlation analysis was performed. Results: Although simple visual and auditory selective attention have diagnostic value in traditional continuous performance tests, this study revealed that inhibition-sustained attention and interference-selective attention are also effective in evaluating ADHD. Furthermore, the correlation between the attention and intelligence test scores varied depending on the use of visual or auditory stimuli. Conclusion: The findings of this study contribute to clarifying our understanding of the cognitive characteristics of children and adolescents with ADHD and can be used in future research.

Effectiveness of Behavior Group Counseling Utilizing the Games of Children with Attention Deficit Hyperactivity Disorder (게임을 이용한 행동적 집단상담이 ADHD 아동의 행동변화에 미치는 효과)

  • The effectiveness of behavior group counseling using the games of children with Attention Deficit Hyperactivity Disorder(ADHD) was studied with 24 primary school children (12 each in the experimental and in the control groups). The subjects were selected on the basis of the Abbreviated Conners Teacher Rating Scale-Revised. The counseling program consisted of 12 sessions over a 6-week period. Effectiveness of the training was evaluated with the Conners Teacher Rating Scale-Revised, the ADHD teacher's rating scale, and the Korea-Child Behavior Checklist. Findings were that the group receiving counseling exhibited improved attention, social relationships, and self control. They also exhibited decreased carelessness-passivity, hyperactivity, hostility and fewer behavior problems.

A Case Report of Noonan Syndrome with Mental Retardation and Attention-Deficit Hyperactivity Disorder (정신지체와 주의력결핍 과잉행동장애를 보이는 Noonan 증후군 1예)

  • Noonan syndrome is characterized by short stature, typical facial dysmorphology, and congenital heart defects. The main facial features of Noonan syndrome are hypertelorism with down-slanting palpebral fissures, ptosis, and low-set posteriorly-rotated ears with a thickened helix. The cardiovascular defects most commonly associated with this condition are pulmonary stenosis and hypertrophic cardiomyopathy. Other associated features are webbed neck, chest deformity, mild intellectual deficit, cryptorchidism, poor feeding in infancy, bleeding tendency, and lymphatic dysplasias. The patient is a 10-year-old boy. He had experienced repeated febrile convulsions. He had typical facial features, a short stature, chest deformity, cryptorchidism, vesicoureteral reflux, and mental retardation. His language and motor development were delayed. When he went to school, it was difficult for him to pay attention, follow directions, and organize tasks. He also displayed behavior such as squirming, leaving his seat in class, and running around inappropriately. Clinical observation is important for the diagnosis, so we report a patient who was diagnosed with Noonan syndrome, mental retardation, and attention-deficit hyperactivity disorder.

  • Objective : This study is to investigate the method for assesment and diagnosis of ADHD, especially focusing on behavior rating scales. Methods : We searched the recent date of the publication and paper in ADHD. Results : For Assesment and Diagnosis of ADHD, various method such as interview with parents, child and teacher, behavior observation, behavior rating scales and neuropsychological test are used. The structured interview consists of the restrictive questions and response, and then have diagnostic algorithm, consequently can be used by untrained clinicians. Of the structured interview, standardization of K-SADS in Korean version is finished. Behavior rating scales, the form of parent, teacher and self-report questionnaires, are used as diagnosis and treatment evaluation of ADHD. Behavior rating scales consist of both ADHD-specific scales and broad-band scales designed to screen for various symptoms (including ADHD symptoms). ADHD-specific scales are useful in differential diagnosis, discrimination of subtype, treatment evaluation, However, broad-band scales are useful in preliminary examination. The neuropsychological tests can evaluate attention deficit and effect of attention deficit on cognitive function and academic performance. The neuropsychological tests also used in diagnosis and treatment evaluation of ADHD. Conclusion : For Assesment and Diagnosis of ADHD, various method are used, especially behavior rating scales are both useful and simple tool for diagnosis and treatment evaluation.

Psychometric Properties of the Korean Translation of the Attention-Deficit/Hyperactivity Disorder Stigma Questionnaire

  • Objectives: This study evaluated the psychometric properties of the Korean version of the attention-deficit/hyperactivity disorder (ADHD) Stigma Questionnaire (ASQ) and the effect of the source of information about mental health on ADHD stigma. Methods: The Korean translation of the ASQ was prepared, and 673 participants, 20-64 years of age, completed the questionnaire using an online panel survey in South Korea. The participants also completed questionnaires detailing sociodemographic variables and the source of their mental health knowledge. Cronbach's alpha coefficient was used to explore the internal consistency of the ASQ. Factor analysis using Varimax rotation was conducted to investigate the structure of the ASQ. Results: The 26-item ASQ demonstrated excellent internal consistency (Cronbach's alpha=0.940). Factor analysis supported a three-factor structure, including Concerns with Public Attitudes, Negative Self-Image, and Disclosure Concerns. There were no significant differences in the total ASQ scores according to sociodemographic characteristics. Participants who reported the internet as their major source of information about mental health showed higher ASQ scores compared to those who used other sources for mental health information. Conclusion: The Korean translation of the ASQ has acceptable psychometric properties among Korean adults. Inaccurate information from the internet could increase the stigma toward ADHD.

  • The incidence of the Attention Deficit Hyperactivity Disorder secondary to the traumatic brain injury, such as traffic accidents, is increasing;the variety of the treatment modality is also increasing. This case was studied to see if OROS Methylphenidate(Concerta), which is one of the most commonly used medication in Attention Deficit Hyperactivity Disorder patients, not only improves the patient's attention, but also their impulsivity, hyperactivity and aggression. According to the case result, the medication showed an improvement of the impulsivity, aggression, and attention in the secondary Attention Deficit Hyperactivity Disorder patients after the traumatic brain injury.

The Association between Attention Deficit/Hyperactivity Disorder Symptoms and the Number of Suicide Attempts among Male Young Adults with Unipolar and Bipolar Depression

  • Objectives: Mood disorder is highly comorbid with attention deficit/hyperactivity disorder (ADHD), and comorbid ADHD symptoms are associated with suicide risk. The aim of this study was to examine the association between comorbid ADHD symptoms and the number of suicide attempts among patients with unipolar and bipolar depression. Methods: In this cross-sectional study, 100 patients with either unipolar or bipolar depression constituted the final sample. Pearson's correlation and multiple regression analyses were conducted to examine the association between comorbid ADHD symptoms and suicide risk. Results: Among patients with bipolar depression, the number of suicide attempts was significantly correlated with ADHD symptoms (r=0.324, p<0.01). ADHD symptoms significantly predicted the number of suicide attempts (β=0.249, p<0.05). Conclusion: Our findings underscore the importance of screening and assessing ADHD symptoms in male young adults with depressive disorders.

  • Attention deficit hyperactivity disorder (ADHD) is a neurobehavioral developmental disorder characterized by a persistent pattern of inattention and/or hyperactivity, as well as forgetfulness, poor impulse control or impulsivity, and distractibility. The recommended evaluation includes documenting the type and severity of ADHD symptoms, verifying the presence of normal vision and hearing, screening for comorbid psychological conditions, reviewing the child's developmental history and school performance, and applying objective measures of cognitive function. Prevailing opinion characterizes ADHD as a disorder of executive function attributable to abnormal dopamine transmission in the frontal lobes and frontostriatal circuitry. A clearly defined etiology remains unknown, but studies suggest a strong genetic link. The aim of treatment is to decrease symptoms, enhance functionality, and improve well-being for the child and his or her close contacts. Stimulants remain the pharmacological agents of first choice for the management of ADHD, and psychosocial, behavioral and educational strategies that enhance specific behaviors may improve educational and social functioning in children with ADHD.
