Clinical Features of Clonorchiasis (간흡충증의 임상 양상)

  • Background: $Clonorchis$ $sinensis$ is an important human parasite that is usually found in the biliary tract. It is widely distributed in East Asia in Taiwan, Japan, China and Korea. About 2 million people are estimated to be infected with $C.$ $sinensis$ in Korea. This study was conducted to evaluate the state of infestation and clinical aspects of $C.$ $sinensis$. Material and Methods: We evaluate 1,063 patients with clonorchiasis retrospectively at Yeungnam University Hospital and Health Promotion Center from January 2001 to December 2003. All patients were diagnosed by a positive stool test for $C.$ $sinensis$ eggs. Clinical features, laboratory data and imaging studies were evaluated. Results: The ratio between affected males and females was 3.6 : 1. The highest infection rate was noted in the 40-60 age groups regardless of sex. The infection rate of $C.$ $sinensis$ among the residents of urban areas was higher than in rural areas. There were 813 (76.5%) patients who were asymptomatic. Common symptoms included right upper abdominal pain in 7.6% and epigastric pain in 7.2%. For the laboratory studies, serum ALP and GGT levels were within normal limits in 85.2% and 56.0% respectively. Peripheral eosinophilia was noted in 66.1% of cases. Of the 83 cases undergoing ERCP, 27 cases (32.5%) showed peripheral dilatation of the intrahepatic duct. Conclusion: This study suggests that $C.$ $sinensis$ infection is still endemic even in rural areas of Korea and that asymptomatic infections are common. Further work should be focused on the early diagnosis and treatment of patients with subclinical infection.

Existence of antibodies against bovine enterovirus in humans and various animals in Korea (사람 및 동물에 대한 소 엔테로바이러스 항체 분포 조사)

  • Bovine enteroviruses (BEVs) were separated into two groups, BEV-1 and BEV-2. BEVs, found in cattle worldwide, usually cause asymptomatic infections and are excreted in the feces of infected animals. Antibodies against BEV have been found in different species including human, cattle, sheep, goats, dogs, horses and monkeys in the world. This study aimed to investigate prevalence of the neutralizing antibodies for BEVs in human and animals in Korea. Antibodies against BEV-1 in humans, cattle, pigs, goats, horses and dogs were shown to be 46.8%, 48.3%, 70.6%, 11.5%, 11.5% and 6.3% respectively. Also, antibodies against BEV-2 were shown to be 98.7%, 68.1%, 89.2%, 59.4%, 9.4% and 96.9% respectively. We found that the neutralizing antibodies against these viruses are common in Korea. The prevalences of antibodies against BEV-1 were lower than those against BEV-2 in humans and in all animals except horses. These results showed that the BEV is considered endemic in cattle in many regions in Korea.

Prevalence of Antibody for Hepatitis A Virus among Hospital Workers

  • Hepatitis A virus (HAV) is an epidemiologically important virus with a worldwide distribution. It causes acute hepatitis in humans. HAV infection is often subclinical or asymptomatic in children; however, symptomatic acute infections become more common with age. In this study, we investigated the prevalence of anti-HAV among hospital workers in Jeonbuk province. 447 (127 males, 320 females) persons were included in the study from January to June, 2011. Anti-HAV (total and IgM) in the sample serum was measured by VIDAS Hepatitis A (bioMerieux, France), and IgM positive samples were tested in terms of their serum alanine aminotransferase (ALT) levels by a HITACHI 7600-010 automatic analyzer (HITACHI, Japan). The overall prevalence of total anti-HAV was 45.9% (205/447), and rate for males (60.6%) was higher than that for females (40%). According to the age group, the prevalence rates of total anti-HAV were 20% (1/5) in those under 20 years old, 11.3% (18/160) in those 20-29 years, 49.7% (71/143) in those 30-39 years, 86.5% (83/96) in those 40-49 years, 92.9% (26/28) in those 50-59 years, and 40% (6/15) in those over 60 years old. The total prevalence of anti-HAV IgM was 0.8% (4/447), and rate for males (1.6%) was higher than that for females (0.6%). Through this study, we determined the prevalence of anti-HAV among hospital workers in Jeonbuk province. The prevalence of the HAV antibody overall was low in hospital workers under 40 years old and in those over 60 years old. Therefore, an improvement in sanitary conditions and hygiene and vaccinations against HAV in this population are recommended.

  • More than half of the cryptococcal infections occur in acquired immune deficiency (AIDS) patients, and more than half of the non-AIDS patients with cryptococcosis are immunocompromised. Most immunocompromised patients have meningoencephalitis at the time of diagnosis. Without the appropriate therapy, this form of the infection is invariably fatal. Death can occur any time from 2 weeks to several years after the onset of symptoms. Pulmonary cryptococcosis in immunocompromised patients is usually asymptomatic, but coughing, chest pain, fever, or hemoptysis may occur in immunocompetent patients. Pulmonary cryptococcosis symptoms in immunocompetent patients tend to improve without treatment. Here, we describe the various pulmonary manifestations of cryptococcal pneumoniae in three immunocompetent patients.

  • The Unilateral absence of a pulmonary artery (UAPA) is an uncommon congenital anomaly. Approximately 160 cases have been reported in the literature since Frantzel's first report in 1968. Most of the patients with UAPA are asymptomatic but some patients may suffer from recurrent respiratory infections, hemoptysis, or pulmonary hypertension. The diagnosis could be suspected from the chest roentgenogram and lung scan, and definitely confirmed by pulmonary angiography. We experienced a case of UAPA in a 39-year-old male with the recurrent hemoptysis. Chest X-ray revealed that the left lung volume was moderately decreased and the heart and mediastinum were displaced to the left side. Lung perfusion scan showed that the left lung was not perfused. Pulmonary angiography revealed the absence of the left main pulmonary artery. Aortic arch and descending aorta on aortogram were right sided. Blood supply to the left lung was originated from numerous systemic collaterals from intercostal and brachiocephalic origin. No other intrinsic or internal abnormalities of the cardiac chambers were noted.

  • Since 2012, outbreaks of the Middle East respiratory coronavirus (MERS-CoV) have been reported, including the Republic of Korea. To date, most of the people infected with the virus are adults. Herein we describe the clinical characteristics of cases of MERS-CoV infection among children. As of October 29, 2015, MERS-CoV has caused 34 pediatric infections, which accounts for 2.1% of all cases. The median age was 13 years (range 9 months to 17 years) and where gender has been reported (n=33), 57.6% cases were male. About half of the patients were asymptomatic and the majority of the symptomatic patients had respiratory symptoms. In general, the clinical outcome in children was favorable. Among the four patients who died of progressive pneumonia, three had documented comorbidities. MERS-CoV infection in children has a lower incidence and mortality compared to adults.

  • Background: Chlamydophila felis, formerly known as Chlamydia psittaci var. felis, is frequently associated with ocular, respiratory, and occasionally reproduction tract infections. Even though the infection is sometimes asymptomatic, it potentially results in a latent immunosuppressive infection. Objective: This study aimed to identify occurrences of feline chlamydophilosis, rarely reported in cats in Indonesia. Methods: The observation was conducted in three cats with clinical signs of Cp. felis infection, particularly relapsing conjunctivitis. The cats' histories were recorded based on owners' information. Conjunctival swabs were sampled for cytology examination and molecular assay detection. A phylogenetic tree was generated using MEGA-X software to reveal group clustering. A post-mortem examination was performed on the cat that died during an examination. Results: Cp. felis was detected in both cytological examination and polymerase chain reaction assay. The phylogenetic tree demonstrated that the Cp. felis isolated in this study clustered with several other isolates from the other countries. Cp. felis can be isolated from cats with different clinical manifestations and levels of severity. The chronic fatal infection demonstrated interstitial broncho-pneumonia under histopathological examination. Conclusions: Molecular assay of Cp. felis is always recommended to obtain a definitive diagnosis of feline chlamydophilosis since the disease can have various clinical manifestations. Even though it may be subclinical and is often not fatal, an infected cat may be a carrier that could spread the pathogen in the surrounding environment. Serious disease management is suggested to avoid high costs associated with regularly relapsing disease.

Antiviral Treatment Reveals a Cooperative Pathogenicity of Baculovirus and Iflavirus in Spodoptera exigua, a Lepidopteran Insect

  • The beet armyworm, Spodoptera exigua, is a serious insect pest infesting various vegetable crops. Two infectious insect viruses, baculovirus and iflavirus, are known to induce epizootics in S. exigua populations. Indeed, some laboratory colonies have appeared to be covertly infected by these viruses. Diagnostic PCR tests detected two different viruses: Spodoptera exigua multiple nucleopolyhedrosis virus (SeMNPV) and iflaviruses (SeIfV1 and SeIfV2). Viral extract from dead larvae of S. exigua could infect Sf9 cells and produce occlusion bodies (OBs). Feeding OBs to asymptomatic larvae of S. exigua caused significant viral disease. Interestingly, both SeIfV1 and SeIfV2 increased their titers at late larval stages. Sterilization of laid eggs with 1% sodium hypochloride significantly reduced SeMNPV titers and increased larval survival rate. Double-stranded RNA (dsRNA) specific to SeIfV1 or SeIfV2 significantly reduced viral titers and increased larval survival rate. To continuously feed dsRNA, a recombinant Escherichia coli HT115 expressing SeIfV1-dsRNA was constructed with an L4440 expression vector. Adding this recombinant E. coli to the artificial diet significantly reduced the SeIfV1 titer and increased larval survival. These results indicate that laboratory colony collapse of S. exigua is induced by multiple viral infections. In addition, either suppression of SeMNPV or SeIfV infection significantly increased larval survival, suggesting a cooperative pathogenicity between baculovirus and iflavirus against S. exigua.

Comparison of canine vector-borne diseases in rural dogs based on the prevention status

  • Canine vector-borne diseases (CVBDs) are transmitted by different groups of hematophagous arthropod vectors that are distributed worldwide and can cause significant health problems for dogs. The aim of this study was to investigate and compare the prevalence of selected CVBD pathogens in rural outdoor dogs based on prevention status. Between June 2017 and February 2019, blood samples were collected from 343 clinically healthy rural dogs composing two different groups: systematically managed dogs (SMD; n=92) and personally managed dogs (PMD; n=251). Vaccination and preventive medications were applied strictly following the programmed schedule for the SMD group; in contrast, in the PMD group, they were applied only when requested by the dog owners. Serological and molecular assessments showed that significantly more dogs in the PMD group were infected with B. gibsoni (P<0.001) and D. immitis (P=0.001) than those in the SMD group. These findings suggest that the regular use of preventive medications and environmental controlling efforts contribute to reducing the prevalence of CVBD pathogen infections. In addition, dogs infected with certain kinds of CVBD pathogens could remain asymptomatic, suggesting that continuous monitoring and periodic preventive treatment should be conducted even for clinically healthy dogs.

Comparative genome characterization of Leptospira interrogans from mild and severe leptospirosis patients

  • Leptospirosis is a zoonotic disease caused by spirochetes from the genus Leptospira. In Thailand, Leptospira interrogans is a major cause of leptospirosis. Leptospirosis patients present with a wide range of clinical manifestations from asymptomatic, mild infections to severe illness involving organ failure. For better understanding the difference between Leptospira isolates causing mild and severe leptospirosis, illumina sequencing was used to sequence genomic DNA in both serotypes. DNA of Leptospira isolated from two patients, one with mild and another with severe symptoms, were included in this study. The paired-end reads were removed adapters and trimmed with Q30 score using Trimmomatic. Trimmed reads were constructed to contigs and scaffolds using SPAdes. Cross-contamination of scaffolds was evaluated by ContEst16s. Prokka tool for bacterial annotation was used to annotate sequences from both Leptospira isolates. Predicted amino acid sequences from Prokka were searched in EggNOG and David gene ontology database to characterize gene ontology. In addition, Leptospira from mild and severe patients, that passed the criteria e-value < 10e-5 from blastP against virulence factor database, were used to analyze with Venn diagram. From this study, we found 13 and 12 genes that were unique in the isolates from mild and severe patients, respectively. The 12 genes in the severe isolate might be virulence factor genes that affect disease severity. However, these genes should be validated in further study.
