Development of Enzymatic Recombinase Amplification Assays for the Rapid Visual Detection of HPV16/18

  • Human papillomavirus (HPV) types 16 and 18 are the major causes of cervical lesions and are associated with 71% of cervical cancer cases globally. However, public health infrastructures to support cervical cancer screening may be unavailable to women in low-resource areas. Therefore, sensitive, convenient, and cost-efficient diagnostic methods are required for the detection of HPV16/18. Here, we designed two novel methods, real-time ERA and ERA-LFD, based on enzymatic recombinase amplification (ERA) for quick point-of-care identification of the HPV E6/E7 genes. The entire detection process could be completed within 25 min at a constant low temperature (35-43℃), and the results of the combined methods could be present as the amplification curves or the bands presented on dipsticks and directly interpreted with the naked eye. The ERA assays evaluated using standard plasmids carrying the E6/E7 genes and clinical samples exhibited excellent specificity, as no cross-reaction with other common HPV types was observed. The detection limits of our ERA assays were 100 and 101 copies/µl for HPV16 and 18 respectively, which were comparable to those of the real-time PCR assay. Assessment of the clinical performance of the ERA assays using 114 cervical tissue samples demonstrated that they are highly consistent with real-time PCR, the gold standard for HPV detection. This study demonstrated that ERA-based assays possess excellent sensitivity, specificity, and repeatability for HPV16 and HPV18 detection with great potential to become robust diagnostic tools in local hospitals and field studies.

Adjunctive recombinant human parathyroid hormone agents for the treatment of medication-related osteonecrosis of the jaw: a report of three cases

  • Teriparatide has been effective in treating people diagnosed with medication-related osteonecrosis of the jaw (MRONJ). However, its efficacy is not well established to be accepted as a standard of care. The objective of this paper was to investigate the efficacy of recombinant human parathyroid hormone for the treatment of MRONJ. We report three cases of MRONJ patients with osteoporosis as the primary disease who were treated with a teriparatide agent along with other adjunctive measures. Each patient was administered a teriparatide injection subcutaneously for 16 weeks, 36 weeks, or 60 weeks. Surgical intervention including partial resection, sequestrectomy, decortication, and saucerization took place during the teriparatide administration. Complete lesion resolution was identified clinically and radiographically in all three patients. In patients diagnosed with MRONJ, teriparatide therapy is an efficacious and safe therapeutic option to improve healing of bone lesions. These findings demonstrate that teriparatide in combination with another therapy, especially bone morphogenetic protein, platelet-rich fibrin, or antibiotic therapy, can be an effective protocol for MRONJ.

Rotation Control of Shoulder Joint During Shoulder Internal Rotation: A Comparative Study of Individuals With and Without Restricted Range of Motion

  • Background: Limitations of shoulder range of motion (ROM), particularly shoulder internal rotation (SIR), are commonly associated with musculoskeletal disorders in both the general population and athletes. The limitation can result in connective tissue lesions such as superior labrum tears and symptoms such as rotator cuff tears and shoulder impingement syndrome. Maintaining the center of rotation of the glenohumeral joint during SIR can be challenging due to the compensatory scapulothoracic movement and anterior displacement of the humeral head. Therefore, observing the path of the instantaneous center of rotation (PICR) using the olecranon as a marker during SIR may provide valuable insights into understanding the dynamics of the shoulder joint. Objects: The aim of the study was to compare the displacement of the olecranon to measure the rotation control of the humeral head during SIR in individuals with and without restricted SIR ROM. Methods: Twenty-four participants with and without restricted SIR ROM participated in this study. The displacement of olecranon was measured during the shoulder internal rotation control test (SIRCT) using a Kinovea (ver. 0.8.15, Kinovea), the 2-dimensional marker tracking analysis system. An independent t-test was used to compare the horizontal and vertical displacement of the olecranon marker between individuals with and without restricted SIR ROM. The statistical significance was set at p < 0.05. Results: Vertical displacement of the olecranon was significantly greater in the restricted SIR group than in the control group (p < 0.05). However, no significant difference was observed in the horizontal displacement of the olecranon (p > 0.05). Conclusion: The findings of this study indicated that individuals with restricted SIR ROM had significantly greater vertical displacement of the olecranon. The results suggest that the limitation of SIR ROM may lead to difficulty in rotation control of the humeral head.

개 아토피 피부염에서 3종 프로바이오틱스 복합제의 임상 효능 평가 (Clinical efficacy of L. plantarum, L. reuteri, and Ped. acidilactici probiotic combination in canine atopic dermatitis)

  • Canine atopic dermatitis (CAD) is an inflammatory and pruritic skin disease with a genetic predisposition, characterized by allergic sensitivity. It is known for its distinctive clinical features, including a high recurrence rate and chronic progression. To manage CAD, medications such as steroids and immunosuppressants are commonly used, but consideration should be given to the potential resistance and side effects associated with long-term use. In order to reduce these risks, various adjunctive factors are currently under consideration. One of these adjunctive agents, probiotics have shown effectiveness in regulating atopic dermatitis by modulating immune responses, as demonstrated in several recent studies. In this study, a substance combining three probiotics-L. plantarum, L. reuteri, and Ped. Acidilactici-was used in patients diagnosed with CAD, and its clinical effects and safety were evaluated. The trial involved four groups: a group receiving conventional treatment for atopic dermatitis (A), a group prescribed low-dose probiotics (B), a group prescribed high-dose probiotics (C), and a group prescribed topical probiotics (D). For assessment, the Canine Atopic Dermatitis Extent and Severity Index (CADESI), Trans-Epidermal Water Loss (TEWL) test, gut microbiome, and serum IgE test were conducted. As a result, the CAD severity index (CADESI-4) significantly decreased in the probiotics groups (B & C). In the serum total IgE test, the groups consuming probiotics showed a significant difference, while the group using topical probiotics (D) did not exhibit a significant change. Also, the TEWL test showed improved scores in the probiotics groups (B & C). Therefore, L. plantarum, L. reuteri, and Ped. Acidilactici probiotic combination could be considered as an effective adjunctive treatment, especially for atopic patients with moderate to severe skin lesions.

  • 발달성 고관절 이형성증은 영아기 비구 형성이상으로 인해 고관절 불안정성이 발생하는 질환으로 정확한 초음파 검사가 이루어져야 한다. 특발성 대퇴골두 무혈성 괴사는 아동기 대퇴골두의 일시적인 혈류 장애로 인해 발생하며 무혈성, 분절화, 재골화, 잔여기 단계를 거치게 된다. 대퇴골두 골단분리증은 청소년기에 체중 부하와 같은 스트레스로 인해 대퇴골두가 골단선을 따라 내측으로 전위되는 질환이다. 일과성 고관절 활액막염과 화농성 관절염은 감별을 위해 관절액 흡인술이 필요할 수 있다. 골수염은 연부 조직 부종과 골 용해를 동반할 수 있다. 하지만, 다발성 병변의 경우 랑게르한스 세포 조직구증, 전이성 신경모세포종 등을 감별해야 한다. 본 종설에서는 이런 질환에 대한 영상 검사 방법 및 대표적인 소견을 소개하고자 한다.

  • 면역글로불린 G4(immunoglobulin G4; 이하 IgG4) 관련 질환은 드문 전신성 섬유염증 상태로 IgG4 형질 세포가 풍부한 림프구 침윤과 관련된 기관 비대 또는 종창성 병변을 특징으로 한다. 우리는 48세 여자 환자에서 드문 소견인 연조직 종괴로 발현된 왼팔의 피하층을 침범한 IgG4 관련 질환 증례를 보고하고자 한다. 초음파 및 자기공명영상을 촬영하였으며 악성 또는 염증 병변이 의심되는 불규칙하며 침윤성 경계를 갖는 연조직 종괴가 관찰되었다. 우리는 IgG4 관련 질환의 진단 기준, 조직 병리학적 특징, 영상의학적 특징, 및 치료 방법에 대해 논의하고자 한다.

What are the Appropriate Surgery and Postoperative Surveillance for Intraductal Papillary Mucinous Neoplasm?

  • Although many guidelines for pancreatic cystic neoplasms focus on the management of intraductal papillary mucinous neoplasm of the pancreas (IPMN) at the highest oncological risk, there are many issues that surgeons need to consider at the time to plan the surgical procedures based on characteristics of IPMN subtypes, such as multiplicity of branch duct-IPMN (BD-IPMN) and intraductal spreading of main duct-IPMN (MD-IPMN). For multifocal BD-IPMN, partial pancreatectomy would be selected to remove BD-IPMN with predictors of malignancy, while the other lesions without risk factors can be left, although total pancreatectomy might be considered if the patients have a strong family history of pancreatic cancer. Partial pancreatectomy would be also adequate procedure for MD-IPMN if negative surgical margin for high-grade dysplasia or invasive carcinoma were achieved. It has become to be well-known that patients with BD-IPMN are at increased risk for developing not only IPMN-associated pancreatic ductal adenocarcinoma (PDAC) but also PDAC independent from the IPMN. Hence, the detection of a concomitant PDAC is also an important focus for strategies after resection of BD-IPMNs. Our recent analysis of patients after partial pancreatectomy for MD-IPMN with negative surgical margin identified an unexpected recurrence pattern, which we called "monoclonal skip" recurrence. MD-IPMN seems to be disseminated in the pancreatic ductal systems and MD-IPMN with identical genetic background was detected in the remnant pancreas even in a long time after index surgery. We proposed strategies of postoperative surveillance based on characteristics and natural history of each morphological subtype.

Flow arrest during carotid artery stenting with a distal embolic protection device: A single-center experience and clinical implications

  • Objective: We aimed to investigate the incidence of flow arrest during carotid artery stenting (CAS) with filter-type embolic protection device (EPD), identify any predisposing factors for those situations, and contemplate intraprocedural precautionary steps. Methods: CAS was performed in 128 patients with 132 arteries using filter-type EPD. The characteristics of treated patients and arteries were compared between groups with and without flow arrest. Results: The incidence of flow arrest during CAS with filter-type EPD was 17.4%. In flow arrest group, cases of vulnerable plaques (p=0.02) and symptomatic lesions (p=0.01) were significantly more common, and there were more cases of debris captured by EPD in a planar pattern (p<0.01). Vulnerable plaques were significantly more common in the procedures showing a planar pattern than in the cases with other patterns (p<0.01). Flow arrest group showed a significantly higher rate of ischemic complications (p<0.05), although there were no significant periprocedural neurological changes. The planar pattern of captured debris in filter-type EPD was the only significant risk factor for flow arrest (adjusted odds ratio 88.44, 95% confidence interval 15.21-514.45, p<0.05). Conclusions: Flow arrest during CAS with filter-type EPD is not uncommon and associated with increased ischemic complications. Symptomatic stenoses and vulnerable plaque are related to this event. The planar pattern of captured debris on the EPD was the only significant risk factor for the flow arrest. Clinicians must pay attention to the occurrence of flow arrest and react quickly when performing CAS.

Dental Services of the International Summer Camp Event: Experiences from the 25th World Scout Jamboree, South Korea

  • This study aims to analyze dental care at the 25th World Scout Jamboree (WSJ) using a predesigned standardized dental chart to provide a reference for the operation of dental emergency clinics at future large-scale youth events. The dental charts of all patients treated at the 25th WSJ emergency dental clinic were examined. Fisher's exact tests were used to analyze the differences between youth scouts and adult leaders. Eighty visits of 71 patients were included in the study. The largest number of patients was from Asia-Pacific (49.3%), followed by patients from Europe (40.8%). In terms of chief complaints, oral ulcers (19.7%), lesions of the lips (18.3%), and orthodontic discomfort (15.5%) were the most prevalent. The analysis showed a significant difference (p = 0.0023) between youth scouts and adult leaders in the distribution of patients in the first half (July 30th - August 3rd) and second half (August 4th - August 8th) of the event. There was a higher incidence of dental conditions associated with hot and humid environments, unlike typical emergency dental settings. Therefore, despite adequate preparation, unexpected dental visits may occur, and it is recommended that operations be conducted in community-based dental hospitals to allow for immediate and efficient responses in international camps.

유방 자기공명영상법을 이용한 수술 전 관상피내암으로 진단된 환자의 침윤성 유방암을 예측하는 정량적 분석법 (A Method to Quantify Breast MRI for Predicting Tumor Invasion in Patients with Preoperative Biopsy- Proven Ductal Carcinoma in Situ (DCIS))

  • 목적: 수술전 관상내피암으로 진단된 환자에서 침윤성 유방암을 예측하는 정량적 파라미터를 알아보고자 한다. 대상과 방법: 2009년 1월부터 2010년 3월까지, 조직생검상 관상피내암으로 진단된 41명 환자의 42 자기공명영상을 분석하였다. 정량적 파라미터로는 초기조영증강정도, 최대조영증강 정도, 초기에 빠르면서 최대 조영증강을 보인 시점, 조영증강정도, 동맥기 조영증강비율, 겉보기확산계수, 병변의 지름과 용적을 분석하였다. 정량적 파라미터와 침윤성과 연관관계를 알기 위해 일변량 분석과 다변량 분석을 시행하였다. 결과: 42개의 병변중 23 병변이 침윤성 유방암, 19 병변이 관상피내암으로 확진되었다. 병변의 지름 (p=0.003; $6.5{\pm}3.2$ cm vs. $3.6{\pm}2.6$ cm, respectively)과 조영증강비율 (p=0.036; $1.1{\pm}0.3$ vs. $0.9{\pm}0.3$, respectively)가 통계학적으로 의미있게 침윤성 유방암에서 높게 나왔다. 그외 초기에 빠르면서 최대 조영증강을 보인 시점, 동맥기 조영증강비율, 겉보기 확산계수, 병변의 용적은 침윤성 유방암을 예측하는데 통계학적 의미는 없었다. 결론: 분석한 여러가지 정량적 파라미터 중 병변의 지름과 조영증강비율이 수술전 관상피내암으로 진단된 환자에서 침윤성 유방암을 예측하는데 도움이 된다.
