Left Ventricular Assist Device Implantation via Dual Left Thoracotomy in an Adult Patient with Congenitally Corrected Transposition of the Great Arteries

  • A 59-year-old man presented for possible durable ventricular assist device (VAD) implantation. He had previously been diagnosed with congenitally corrected transposition of the great arteries, a ventricular septal defect, an atrial septal defect, pulmonary valve stenosis, and aortic valve regurgitation. In the previous 22 years, he had undergone palliative cardiac surgery 3 times. VAD implantation as a bridge to transplantation was planned. Owing to severe adhesions, mesocardia, a left ascending aorta, and moderate aortic regurgitation, we performed VAD implantation and aortic valve closure via a dual left thoracotomy and partial sternotomy.

  • A Ventricular assist device (VAD) is one of the most efficient treatments to raise the survivability of the end stage heart failure patient. However, some of LVAD patients have died for the failures and improper control of LVAD. To detect critical dangers in LVAD, the monitoring methods of LVAD outflow have been requested, because it can be affected by patient's hemodynamic states and abnormal conditions of LVAD. In the case of an external pulsatile LVAD, the air movement through the air line can be used to estimate LVAD outflow. In this study, the air movement in the air-line of the extracorporeal pulsatile LVAD was measured with a differential pressure sensor between different points. The precise estimation of air movement could be achieved by additional measurement of air pressure. In a series of in-vitro experiments, the LVAD outflow were changed according to the afterload of LVAD and the differential pressure of LVAD didn't have close correlation with the LVAD outflow that were measured with an ultrasonic flowmeter at the same time. However, new precise estimation with the data from differential pressure and one point pressure in the air-line showed higher correlations with LVAD outflow.

  • A 17 year-old high school girl was admitted for anterior chest pain. Pulmonary edema and circulatory collapse progressed in spite of the medical treatment and intra-aortic balloon pump. Left ventricular assist device(LVAD, Bio-Pump, Medtronic Bio-Med, USA) was instituted under the impression of acute fulminant myocarditis. The inlet cannula was inserted in the left atrium(LA) via left submammary anterior thoracotomy. Biopsy was taken from left atrial appendage. The outlet cannula inserted to the left femoral artery using PTFE cuff. After 158 hours of extracorporeal circulation, LVAD was able to be weaned successfully with nearly normalized LV motion on echocardiogram, Coxsakievirus was identified with immunochemistry and serum neutralization test. She was discharged without any heart failure symptoms after 23 days of hopitalization.

  • A ventricular assist device (VAD) is used for bridge to heart transplantation and heart diseases. Knowing the status of a pneumatic pulsatile VAD when implanting it into the body is important: when the velocity of blood flow through the VAD is slow, a thrombus may occur, and thrombosis can be fatal to a patient. In order to determine the state of a VAD, various sensors need to be implanted. Because this introduces the risk of infection and difficulties with sensor management, we developed a method for estimating the state of a VAD indirectly via the pressure in an air tube that can be measured in vitro. We compared the measured values to in vitro experimental results. The estimated and measured values showed some errors, but the accuracy can be improved by refining the estimation process to minimize the risk of infection.

Driveline Infections Incidence According to Dressing Methods for Patients with Left Ventricular Assist Device Implantation (좌심실보조장치 수술 환자의 드레싱 방법에 따른 드라이브라인 감염 실태)

  • Purpose: Driveline infection (DLI) is one of the major adverse events of Left Ventricular Assist Device (LVAD). The purpose of this study was to explore the incidence of DLI according to the driveline dressing methods. Methods: This study was a retrospective cohort study that investigated the medical records of 75 patients who implanted LVAD from January 2015 to December 2020 at a hospital in Seoul, Korea. Traditionally, sandwich dressing method was applied until October 2019, after which newly winded dressing method was adopted for driveline dressing to LVAD patients. The outcome variables were compared between sandwich dressing method applied group (n=41) and winded dressing method applied group (n=34). The follow-up period for DLI was 1 year. Results: When compared participants' characteristics, there was no difference between the two groups, except the type of LVAD device. The incidence of DLI was 17.1% in sandwich dressing group, while no infection was found in winded dressing group (p=.011). Conclusion: Although there were difference in the LVAD devices, it is considered that winded dressing contributed to the reduction of DLI. Further research on standardized dressing methods was required for DLI prevention in Korea.

  • There is some equipment that helps user to exercise and to walk. But almost all equipments require some physical strength of their muscles. So we developed a system that could assist walking action of the people with lower-limb disability. The system called as walking stand adopted the balancing mechanism which assures the stable walking, and the 4 link-based mechanism that had 2 degrees of freedom on each leg. The walking stand uses four motors and has two sets of the special link-structure to simulate the human walking mechanism. With our system, even serious disabled with lower-limb disability may enjoy walking rehabilitation. And by adjusting the power, it can be used as the walking assistant mechanism instead of conventional wheelchairs. Experiments showed that our walking stand is applicable to the rehabilitation and also to the mobile device in our daily life for those people who do not have enough physical ability to walk by themselves.

  • As the damage caused by disasters increases rapidly around the world, it is necessary to develop the technology for equipment to reduce human injury. Therefore in the support of fire safety and 119 rescue and rescue technology research and development project, in the "Development of a power assist device for a fireman to rescue humans(2015 ~ 2018)" for life saving restoration, we are developing equipment that satisfies the lifting performance considering the disaster environment and the disaster response scenario(Amount of load over 100 kg, height of over 1 m, height over middle 60 cm, speed over 0.2 m/s). In this study, we propose a lifting performance and environmental test method to evaluate the usefulness of the power-assisted device and analyze and verify detailed specifications of the device through dynamics analysis of the lifting performance test. This study suggests that the proposed test method can be applied practically to evaluate whether a stable performance of a power-assisted device is achieved.

  • A wearable device to assist fingering function for disabled is developed in this study. This is the second paper to report the progress in development of this assisting device. The device is developed for a patient who suffers from cervical spinal cord injury. In the first paper, it was reported that the patient could successfully pick up several types of objects with his paralyzed fingers by using this device. As a next step, the maneuverability of the device under grasping operation is discussed in this paper. Maneuverability of the system is experimentally evaluated. The dexterity in controlling finger force is compared between the cases that non-disabled examinees operate their finger with inherent abilities and that a disabled examinee operates his finger by using the assisting device.

  • EPS (Electric Power Steering) is important device for improving vehicle's dynamics and static performances. This paper deals with simulator design for C-EPS (Colum type-EPS), development assist and returnability control algorithm. First, C-EPS system model was simply designed because EPS system is complex control system that has many unknown variables. These parameters were simplified through assumptions. Second, C-EPS simulator was designed for development of control algorithm. This simulator has SAS (Steering Angle Sensor), dual torque sensor, dual load cell for measuring rack force, dual linear actuator for generating tire force and Data Acquisition System. Using this simulator, control methods ware tested. Third, control algorithm was designed for torque assist and returnability. Assist torque map and returnability torque map were found by lots of simulation test. These torque maps were tuned for EPS actuator control. The simulation result was compared with non-EPS system result. In this research, the C-EPS simulator was designed for development of control algorithm about torque assistant and returnability. Using this simulator, control algorithm was improved.
