소아개심술 후 시행한 순환보조장치의 임상적 고찰 (Postcardiotomy Mechanical Circulatory Support in Congenital Heart Diseases)

  • Background: To review the experience that used both ventricular assist device(VAD) and extracorporeal membrane oxygenation(ECMO) for children with congential heart disease requiring postcardiotomy mechanical circulatory support. Material and Method: Between March 1993 and May 1995, we applied mechanical assist device using centrifugal pump to the 16 patients who failed to be weaned from cardiopulmonary bypass(n=15) or had been in cardiogenic shock in intensive care unit(n=1). The diagnosis were all congenital heart diseases and the ages of patients ranged from 20 days to 10 years (mean age=2.5$\pm$3.5 years). Result: The methods of mechanical circulatory support were LVAD(n=13), BVAD (n=1), and ECMO(n=2). The mean assist times were 54.0$\pm$23.7 hours. Post-assist complications were in orders: bleeding, acute renal failure, ventricular failure, respiratory failure, infection, and neurologic complication. It was possible for 9 patients(56.3%) to be weaned from assist device and 5 patients(31.3%) were discharged from hospital. There was no statistical significant between hospital discharged group and undischarged group by age, body weight, cardiopulmonary bypass time, and assist time. Conclusion: The ventricular assist device is an effective modality in salvaging the patient who failed to be weaned from cardiopulmonary bypass, but multiple factors must be considered for improving the results of mechanical circulatory support ; such as patient selection, optimal time of starting the assist device, and prevention and management of the complications.

  • Right heart failure is a relatively common complication after left ventricular assist device (LVAD) implantation. Severe right heart failure can be managed by temporary right ventricular assist device (RVAD) implantation. However, trans-sternal RVAD insertion requires a subsequent third sternotomy for cannula removal. Herein, we present a case of RVAD insertion via a left anterior mini-thoracotomy after LVAD implantation in a patient with alcohol-induced cardiomyopathy.

무릎 체중부하 촬영 보조장치 개발 (Development of Knee Weight-bearing Radiography Assist Device)

  • The knee arthritis patients are generally performed standing position radiography. However, patients with pain caused by degenerative diseases or injuries in the knee have difficulty maintaining a standing position. Therefore, this study aims to develop a knee joint standing weight bearing projection assist device to solve these problems when patients undergo knee radiography due to various diseases. The design of the knee joint weight-bearing radiography assist device is carried out with 3D design and drawing production to secure basic data, electric support and frame manufacture. A fixed device for maintaining the patient's knee standing weight-bearing projection, an electric digital height device, a digital protractor, and a safety device were designed. The arm support is made of PVC with a cushion to relieve the impact and make it easier to remove debris. The digital electric device can be moved up and down according to the patient's height, and a remote control is attached. The safety device is made to be adjustable in size by attaching metal hooks on both sides of the frame and to shield the scrotum. A digital protractor was attached to the side frame to set the desired knee angle. When a self-made assist device was used to perform a knee joint standing weight bearing projection, it helped maintain arm support and lower extremities position. In addition, the height could be adjusted using an assist device during standing projections, which helped the patient maintain the position.

Fabrication of a Breathing Assist Device for Saxophone Players with Breathing Problems

  • The aim of this study was to establish a breathing assist system for saxophone players with breathing problems. Although the saxophone is a popular wind instrument with a reed in its mouthpiece, it can be difficult for people with breathing problems to play this instrument, as it requires adequate breath support for deep and even long breaths. To solve this problem, the authors propose a breathing assist device, which functions like a pneumatic master-slave amplifier, for saxophone players with breathing problems. First, the proposed device is fabricated. Second, the effectiveness of the breathing assist device as a master-slave amplifier is confirmed through experiments. Third, the dynamic characteristics of the device are tested up to 10 Hz, and they demonstrate that the device responds well for up to approximately 5 Hz.

뇌졸중 환자용 동력보조형 상지재활훈련기의 설계 (Design of Upper-limb Rehabilitation Device with Power-assist Function for Stroke Survivals)

  • 본 논문은 뇌졸중에 의한 상지 편마비 환자의 재활훈련을 위한 동력보조형 상지재활훈련기의 설계를 제안하였다. 설계된 상지재활훈련기는 3자유도를 가지고 있으며, 검지와 손목은 독립적으로, 그리고 나머지 손가락은 하나의 모듈로 굽힘과 폄 동작이 가능하다. 손목 기구는 복동식 공암실린더를, 손가락 기구에는 두 개의 전동식 선형구동기를 적용하여 동작을 보조하도록 하였다. 설계를 기반으로 프로토타입 상지재활훈련기를 제작하였으며, 0.8mm이내의 오차로 정상상태에 수렴하도록 위치제어기를 구현하였다. 실험의 결과로부터 동력보조용 상지훈련기가 실현가능함을 보였다.

Rosenberg View 검사를 위한 보조기구의 유용성 평가 (Evaluation of the Usefulness of Assist Device for Rosenberg View Test)

  • Rosenberg Method 특성상 환자는 일정한 시간 동안 자세 유지가 필요하고 무릎 골곡 각도에 따라 관절공간이 다양한 형태로 관찰이 되어 검사에 어려움이 발생하고 있다. 이러한 문제점을 해결하고자 보조기구를 자체 제작하여 보조기구의 유용성을 평가하기 위해 화질을 평가하였다. 하지 Phantom을 이용한 보조기구의 유·무에 대한 SNR과 CNR 그리고 보조기구 자체에 대한 각도의 비교에서 모두 통계적으로 유의하지 않았다(p < 0.05). 보조기구 유·무로 오른쪽과 왼쪽의 내측과 가장자리 사이 거리를 측정한 결과, 보조기구 사용 무(96.00±40.6 mm), 보조기구 유( 134.86±17.68 mm), 통계적으로 유의하게 나타났다(p < 0.05). 대퇴골과 경골에 대한 aLDFA의 관계를 알아보고자 보조기구 유·무를 통해 오른쪽과 왼쪽의 aLDFA을 측정하였고, 오른쪽 aLDFA 보조기구 무 (74.63°±4.87), 보조기구 유(78.91°±3.12), 통계적으로 유의하게 나타났고(p < 0.05), 왼쪽 aLDFA 보조기구 무 (76.39°±4.62), 보조기구 유(79.64°±3.65), 통계적으로 유의하게 나타났다(p < 0.05). 그러나 보조기구 유·무로 오른쪽과 왼쪽의 몸쪽 정강종아리관절과 외측과의 겹친 부위의 거리를 측정한 결과, 오른쪽과 왼쪽 모두 통계적으로 유의한 차이가 없었다(p < 0.05). 결론적으로 자체 제작한 보조기구를 사용하여 무릎과 무릎 사이의 간격이 일정한 진단적으로 가치 있는 영상을 획득할 수 있으며, 대퇴골과 경골의 aLDFA의 관계를 통해 각도가 작을수록 내측과가 JSW에 겹침을 알 수 있었고, 보조기구를 사용으로 aLDFA의 Normal Range 값에 유사한 값을 도출되어 유의성을 확인하였다. 그러나 몸쪽 정강종아리관절과 외측과의 겹친 부위의 거리 평가로 내회전, 외회전에 주의할 필요성이 있는 것으로 판단된다.

적응 학습방식의 신경망을 이용한 좌심실보조장치의 모델링 (Adaptively Trained Artificial Neural Network Identification of Left Ventricular Assist Device)

  • This paper presents a Neural Network Identification(NNI) method for modeling of highly complicated nonlinear and time varing human system with a pneumatically driven mock circulatory system of Left Ventricular Assist Device(LVAD). This system consists of electronic circuits and pneumatic driving circuits. The initiation of systole and the pumping duration can be determined by the computer program. The line pressure from a pressure transducer inserted in the pneumatic line was recorded System modeling is completed using the adaptively trained backpropagation learning algorithms with input variables, heart rate(HR), systole-diastole rate(SDR), which can vary state of system. Output parameters are preload, afterload which indicate the systemic dynamic characteristics. Consequently, the neural network shows good approximation of nonlinearity, and characteristics of left Ventricular Assist Device. Our results show that the neural network leads to a significant improvement in the modeling of highly nonlinear Left Ventricular Assist Device.

혈관 내 폐 보조장치에서의 산소전달속도 예측에 관한 연구 (A Study of Prediction of Gas Transfer rate in Intravascular Lung Assist Device)

  • 본 연구의 목적은 정맥 내 폐 보조장치 설계에 있어서 기체전달을 예측하기 위하여 혈관 내 폐 보조장치를 정맥 내에 삽입하기 전, 여러 가지 설계조건에서 실험적 모델링을 통하여 예측식을 찾고자 시도하였다. 실험결과 중공사 충진율의 함수에 따라 기체전달을 예측할 수 있었다. 실험에 의하여 얻어진 기체전달은 예측식에 의한 기체전달과 유사하여, 식에 대한 신뢰성을 얻을 수 있었다. 그러므로, 중공사의 충진율의 함수에 따라 기체전달을 예측할 수 있으며, 혈관 내 폐 보조장치의 기체전달을 예측하는데 매우 유용하였다.

The First Pediatric Heart Transplantation Bridged by a Durable Left Ventricular Assist Device in Korea

  • Treatment options for children with end-stage heart failure are limited. We report the first case of a successful pediatric heart transplantation bridged with a durable left ventricular assist device in Korea. A 10-month-old female infant with dilated cardiomyopathy and left ventricular non-compaction was listed for heart transplantation. During the waiting period, the patient's status deteriorated. Therefore, we decided to provide support with a durable left ventricular assist device as a bridge to transplantation. The patient was successfully bridged to heart transplantation with effective support and without any major adverse events.

Non-Surgical Resolution of Inflow Cannula Obstruction of a Left Ventricular Assist Device: A Case Report

  • A 55-year-old woman who had received an implantable left ventricular assist device 3 months earlier presented with dyspnea and a low-flow alarm of the device. Computed tomography and log-file analysis of the device system suggested inflow cannula obstruction. Since the patient had cardiogenic shock due to pump failure, venoarterial extracorporeal membrane oxygenation (ECMO) was initiated. With ECMO, surgical exchange of the pump was considered. However, the obstruction spontaneously resolved without surgical intervention. It turned out that an obstructive thrombus was washed out by rebooting the pump. Moreover, the thrombus was embolized in the patient's left subclavian artery. The patient underwent heart transplantation 4 months after the pump obstruction accident and continued to do well.
