Clinical Experience of Photodynamic Therapy in Five Patients with Advanced Lung Cancer (진행성 폐암에서 광역동 치료로 호전된 5례에 관한 임상적 경험)

  • Background : A tracheobronchial obstruction in lung cancer is associated with significant morbidity and mortality due to dyspnea, cough, hemoptysis, and recurrent respiratory infection. It is well known that one of standard treatments is photodynamic therapy (PDT) in tracheobronchial obstruction after radiotherapy, chemotherapy, and/or surgery. We reported here the role of PDT in airway obstruction in patients advanced lung cancer. Method : Pre-treatment protocol consisted of clinical, radiologic, and bronchoscopic examination, pulmonary function test, and assessment of Karnofsky performance status. A 2 mg/kg of porfimer sodium was injected intravenously, and then followed by cylindrical and/or interstitial irradiation with 630 nm of laser after 48 hours. The repeated bronchoscopy for debridement of necrotic tissue and re-illumination was performed after 48 hours. Result : Improved airway obstruction and selective tumor necrosis were achieved by photodynamic therapy in all cases. Dyspnea and performance status were improved in three cases. A purulent sputum, fever and hemoptysis were improved in one of five cases. After PDT, all patients showed temporarily aggravation of dyspnea, two of five showed febrile reaction for a few days and nobody presented photosensitivity reaction, hemoptysis and respiratory failure. Conclusion : Our experiences of PDT are effective in palliation of inoperable advanced lung cancer in terms of tracheobronchial obstruction.

Current Status and Direction for Future Development of Evidence-based Nursing in Korea (국내 근거기반간호의 현황과 발전 방향)

  • Purpose: The purpose of this study was to identify the current status of evidence-based research, education, and practice, and to suggest a directions for the future development of evidence-based nursing. Methods: To examine the current status of evidence-based nursing research, experimental research among studies published in the Journal of Korean Academy of Nursing from 2008 to 2010 were analyzed. We suggested a direction for the future development of evidence-based research based on the analysis and literature reviews. We also suggested a direction for the future development of evidence-based education and practice based on literature reviews. Results: The results showed that designs and methods of studies were insufficient to use evidences derived from the studies in terms of evidence-based research, and experimental studies consistent with themes were found to be deficient. In the future, the methods and rationale for the design of experimental studies need to be clearer In addition, a test for intervention effects through repetitive studies and a connection between the intervention effects and a protocol for clinical practice will be required. In terms of evidence-based education, curriculum revision and development of teaching methods including contents related to evidence-based practice and research methodology in undergraduate and graduate programs will be required. Evidence-based practice in Korea was less actively done than with that of foreign countries. Some large hospitals have recently attempted systemic activities to promote evidence-based nursing practice in clinical settings. To activate evidence-based practice, the perception of nurses for evidence-based practice needs to be changed and support for education and administration of the program is required. Also, evidence-based practice guidelines, protocols, and assessment tools need to be developed. Conclusion: Evidence-based nursing in Korea is in the process of evolving. To realize evidence-based nursing, we need to develop nursing science based on evidences of closer communication and cooperation in terms of nursing research, education, and practice.

  • Integrated information to water quantity and quality is essential for planning water resources management as well as operating water-related infrastructures. Because data collection process including monitoring and maintenance is separated in different governmental agencies in Korea, integrating quantity and quality may provide effective and better management implementation. In this study, a number of suggestions regarding integration of water monitoring were concluded in terms of technological, legal and institutional implications. First, it is necessary to discuss national water monitoring plan, national water information management plan, agreement of standard terms of monitoring between ministries, and to revise the law(river law and water quality management law). Present stations for water monitoring should be used for both of quantity and quality monitoring. If station is newly installed or relocated, it is better that one single agency maintain monitoring frequency and data management as well. In addition, a monitoring protocol need to be agreed by each of parties. In order to develop integrated monitoring system, quality assurance of the collected data should be properly maintained. Since many purposes haven been concerned using of data analysis and assessment so far, it may not be easy to integrate water quantity and quality monitoring in a short period. However, the alternatives including enhancing institutional regulations and programs, advanced technology may promote an efficient integrated water monitoring.

Evaluation of Dose and Image Quality of Lens according to Baseline during Brain CT Scan (두부 전산화단층촬영 시 기준선에 따른 수정체 선량과 화질 평가)

  • It is important to minimize the exposure dose during an examination and obtain good quality images at the same time. This study compared the beam harding effect according to the baseline superior orbito meatal line(SOML), orbito meatal line(OML), inferior orbito metal line(OML) and measured the exposure dose of the lens, especially in brain CT examinations, which generally apply to head diease patients. The beam harding effect assessment of each image along the baseline was performed quantitatively using the Image J program, and the exposure dose of the lens was detected by OSLDs and compared. As a result, As a result, when the SOML was used as the reference line, the dose of the lens was decreased by 85.08% at 80 kV and by 79.7% at 80 kV, compared to when IOML was used as the baseline. If the gantry angle at brain CT was parallel scan to SOML, there were no significant differences in the exposure to the lens and between the OML and IOML. Therefore, this study has shown that it is efficient to have a parallel scan on SOML as a protocol during Brain CT examinations.

  • This study was implemented as a part of the experiment to develop two kinds of soil-based Benzo[a]pyrene (BaP) proficiency testing materials (PTMs) for soil analysis. A test was carried out for the check of solubility of the reference material (high purity reagent) using several solvents. Another test was also conducted for the evaluation of homogeneity and stability of two kinds of candidate soil reference materials. The test analysis of BaP in terms of the candidate materials was conducted according to the Standard Soil Analytical Methods by Ministry of Environment. Dissolution of the reference material was shown to vary depending on solvent type and was higher in the order of Dichloromethane > Acetone > Acetone/MeOH (9 : 1) > N-hexane. In addition, the slope on calibration curve for BaP standard solutions was largest on BaP standard solutions prepared with dichloromethane of the tested solvents. Such tendency appeared egually in the commercial BaP standard solution. Therefore, it is thought to be reasonable to use dichloromethane as the solvent in case of the standard stock solution that is used for the measurement of BaP concentration in soil. ISO 13528 and IUPAC protocol were used for verification of homogeneity on the two kinds of soil candidate materials, Both candidate materials were sufficiently homogeneous. Stability assessment of the two candidate materials was made according to ISO Guide 35 and the result showed that both batches did not have any long-term and short term stability issues that might occur during shipping. However, monitoring results of BaP concentration in soil showed that BaP concentration of the two batches measured at 15 days after the sample preparation was reduced by about 24~37% compared with that of the samples measured on 0 day of the sample preparation. Identification was done with several treatments such as irradiation and sterilization etc. The major cause was shown to be irradiation to the samples.

Habitat Type Classification System of Korean National Parks (국립공원 서식지 유형 분류 체계 구축)

  • This study was conducted to develop a habitat type classification system and its map based on the ecological characteristics of species, spatial type, vegetation, topography, and geological conditions preferred by species. To evaluate the relationships between species and their habitats in Korean national parks, we prepared a classification standard table for systematic classification of habitat types. This classification system divides habitats into 6 low-level and 59 mid-level ecological classes based on habitat structure. The mid-level system divided forest ecosystems into 20 subtypes, stream and wetland ecosystems into 8 types, coastal ecosystems into 7 types, arable land into 6 types, development land into 9 types, and 1 type of marine ecosystem. A habitat classification map was drawn utilizing square images, detailed vegetation maps, and forest stand maps, based on the above habitat classification system, and it covered 1,461 plots spanning 21 national parks. The habitat classification system and survey protocol, which consider domestic habitat conditions, should be further developed and applied to habitat assessment, to enhance the utility of this study.

Occupational Therapy for Community Mobility in Stroke Patients : Systematic review (뇌졸중 환자의 지역사회이동을 위한 작업치료 중재: 체계적 고찰)

  • Objective : The purpose of this study was to analyze occupational therapy intervention on the community mobility for stroke patients, and to provide evidence of intervention in the clinical fields. Methods : A systematic review was executed according to the PRISMA checklist. The accessed database was PubMed, EMBASE, Cochrane Library (CENTRAL), ProQuest Dissertations & thesis (PQDT), RISS, and KoreaMed. We included the articles published from 2005 to September 2018. RoBANS checklist was used to evaluate the quality of the articles. Included articles, totally eight, were categorized according to the type of intervention. Results : The study design of the literature was varied from two-group randomized trial, quasi-experimental study, case-control trial, one group pre-post comparison study, and cross-sectional study. In the evidence level, 6 articles were included in level II (75%). The percentage of low risk of bias in each article ranged from 52.5%~87.5%. Four studies (50%) provided intervention based on virtual reality or virtual environment. The three (37.5%) provided intervention based on the protocol, and the other (12.5%) did wheelchair training. All studies reported significant effects of the intervention. Conclusion : This systematic review provided evidences to use proper intervention in the clinical fields. Various type of studies should be conducted to prove the effect of occupational therapy intervention for community mobility.

Evaluation of Indoor Mold Exposure Level in dwelling Using DNA-Based Mold Assessment Method (DNA 기반 곰팡이 평가기법을 활용한 주택의 실내 곰팡이 노출수준 평가)

  • Objective: Allergic diseases such as asthma due to fungal exposure in houses have increased, and proper management is urgent. Mold can grow in the air, floor, walls, and other areas according to environmental conditions, and there are many limitations to the conventional methodology for examining fungal exposure. For this reason, the degree of fungal contamination is being evaluated by ERMI (Environmental Relative Moldiness Index), a quantitative analysis method proposed by the EPA. In this study, we compared ERMI values between water-damaged dwellings and non-damaged ones to evaluate the effectiveness of Korean ERMI values. We also explored the association of ERMI values with the level of airborne mold and characteristics of dwellings. Methods: Floor dust was collected after installing a Dustream collector on the suction port of a vacuum cleaner. The collected samples were filtered to remove only 5 mg of dust, and DNA was extracted using the FastDNA SPIN KIT protocol. Results: The ERMI values were found to be 19.6 (-6.9-58.8) for flooded houses, 7.5 (-29.2-48.3) for leaks/condensation, and 0.8 (-29.2-37.9) for non-damaged dwellings. The airborne concentration of mold for flooded, leakage or condensed, and non-damaged houses were $684CFU/m^3$, $566CFU/m^3$, and $378CFU/m^3$, respectively. The correlation between ERMI values and the levels of airborne mold was low (R = 0.038), but a weakly significant association of the ERMI values with the concentration of particulate matter ($PM_{10}$) was observed as well(R=0.231,P<0.05). Conclusions: Our findings show that the reference value using ERMI can be used to distinguish water-damaged and non-damaged dwellings. It is believed that ERMI values could be a promising tool for assessing long-term fungal exposure in dwellings.

Effect of tDCS on Cognitive Function of Patients With Stroke: A Systematic Review and Meta-Analysis (뇌졸중 후 인지장애에 대한 경두개 직류 자극: 체계적 고찰 및 메타분석)

  • Objective : This study aimed to analyze the effect of transcranial direct current stimulation (tDCS) on cognitive function recovery in patients with stroke. Methods : Data published in Korean and foreign academic journals from 2009 to 2019 were searched using the NDSL, RISS, PubMed, and CINAHL databases. A total of 11 experimental research articles were selected based on the inclusion and exclusion criteria. A qualitative assessment was conducted, and a meta-analysis of nine results from seven of the stuides was performed using the Comprehensive Meta-Analysis 3.0 program. Results : Based on the results of the meta-analysis, the attention and memory effect sizes were 0.725 and 0.796, respectively, which were both considered a "medium effect size". Statistically significant changes were observed in both the areas (p<0.05). Conclusion : The results of this study confirmed that tDCS can be useful in the rehabilitation of patients with stroke with limited cognitive function. In addition, the application methods differed, indicating that a formalized tDCS protocol is required.

Image Quality and Dose Assessment According to Examination Mode during Head CT Examination (두부 CT 검사 시 검사 모드에 따른 화질 및 선량평가)

  • To evaluate the usefulness of Volume Axial Mode by comparing analyzing the exposure dose of the patients and the quality of each images from CT images obtained from high pitch mode using the local phantom or volume axial mode to determine the usefulness of he volume axial mode in diagnosing the head and cervical disease in adults. High Pitch Mode, Helical Mode, and Volume axial Mode as adult phantom were tested according to 70 kVp, 80 kVp, and 100 kVp tube voltages during an adult frontal CT scans. The equipment used was GE's Revolution (GE Healthcare, Wisconsin USA) model and iMED X-ray Phantom. The exposure dose of phantom was compared using the images obtained from each protocol, and the image quality was compared by calculating SNR and CNR by setting ROI on each image. When examined using Volume Axial Mode, the exposure dose of phantom was measured 17.12% lower than Helical Mode, 5.35% lower than High Pitch Mode, and both SNR and CNR were improved. Volume Axial Mode is a useful test that reduces investigation time without table movement using high speed rotary scanner, and in which exposure dose is reduced and image quality is improved by acquiring images in a short time of 0.28 seconds of phantom than using High Pitch Mode and Helical Mode. In addition, the fast testing time of Volume Axial Mode can be seen as the biggest advantage CT scans of emergency patients or patients with physical discomfort.
