The Barangay Integrated Development Approach for Nutrition Improvement of the Rural Poor, BIDANI(a Nutrition-in-Development Network Program) (지역 종합개발계획 접근에 의한 빈농 영양개선사업 -영양ㆍ개발 네트워크 프로그램-)

  • BIDANI is the action-research program. BIDANI aims to be truly a people's program embodying their own activities and aspiration. BIDANI sees an integrated development approach at the community level with participatory services embodied in a Barangay Integrated Development Plan(BIDP) designed by the people themselves. Community situational analysis is conducted by the people to identify the priority problems and potential resources in the barangay. Participatory planning, using the “bottom up” apporach, is exercised to formulate a BIDP. Proper motivation and advocacy encourage barangay people's participation. Accessibility and efficiency in the use of various services and resources of government and private agencies increase. Family groups who are at high-risk to malnutrition become aware of the importance of nutrition through their participation in development program activities. Integration of political and socio-economic concerns at the lowest level is operationalized. Implementation and sustainability of the program on a wider scale from a model project to a model program is facilitated through institutionalization at the municipal/city level with the mayor as the project director. “Top to bottom” planning through a City/Municipal Integrated Development Program(C/MIDP) interacts with “bottom up” planning at the barangay level. The establishment of a local Training School for Barangay Development(TSBD) in each municipality and city for continuing education of indigenous village workers and barangay people is a vital component for success and viability. The role of non-political entities such as academic institutions and non-government organizations, as catalytic agents of development, is stressed.

Analysis for Trends and Causes of the Decline in Korean Students' Positive Experiences about Science (우리나라 학생의 과학긍정경험 추이 및 하락 원인 분석)

  • This study analyzed the trends and causes of the decline in Korean students' positive experiences about science (PES). To do this, 4th to 10th grade students were sampled by grade at general elementary, middle, and high schools in Seoul, and then a questionnaire was administered to ask the students about their PES and the causes for their decline. The results of one-way ANOVA for Test for Indicators of Positive Experiences about Science (TIPES) revealed that there were no statistically significant differences according to grade and school level in the overall mean of TIPES scores. However, the results were slightly different for each sub-component. That is, in 'science academic emotion,' the mean of elementary school students was statistically significantly higher than that of middle school students. In addition, the mean of 4th graders was significantly higher than the mean of middle school 1st graders, middle school 3rd graders, and high school 1st graders, respectively. The mean of high school students was statistically significantly higher than that of middle school students in 'science-related career aspiration.' In the 'science-related self-concept', 'science learning motivation,' and 'science-related attitude,' the differences in scores according to grade and school level were not statistically significant. The main causes of the decline in each sub-components of PES were somewhat different depending on the school level. Based on these results, the ways to improve students' PES were sought according to grade and school level.

A Case of Neck Soft Tissue Infection in Patient with Nasopharyngeal Cancer; Misdiagnosed as Local Progression during External Radiotherapy (비인두암 환자에서 방사선 치료중 암의 국소진행으로 오인된 경부 연조직 감염 1례)

  • A 38-year-old man, who was diagnosed nasopharyngeal carcinoma (stage T4N2Mx) had rapidly growing lower neck mass (at level IV area) with local inflammation sign during concurrent chemoradiotherapy. After we performed Color Doppler sonography and fine needle aspiration biopsy of the neck mass, we differentiated neck soft tissue infection from tumor extension. Size of the mass decreased after antibiotics therapy. It was difficult to differentiate neck infection from tumor progression during treatment in naspharyngeal carcinoma.

Nonlinear Interaction between Consonant and Vowel Features in Korean Syllable Perception (한국어 단음절에서 자음과 모음 자질의 비선형적 지각)

  • This study investigated the interaction between consonants and vowels in Korean syllable perception using a speeded classification task (Garner, 1978). Experiment 1 examined whether listeners analytically perceive the component phonemes in CV monosyllables when classification is based on the component phonemes (a consonant or a vowel) and observed a significant redundancy gain and a Garner interference effect. These results imply that the perception of the component phonemes in a CV syllable is not linear. Experiment 2 examined the further relation between consonants and vowels at a subphonemic level comparing classification times based on glottal features (aspiration and lax), on place of articulation features (labial and coronal), and on vowel features (front and back). Across all feature classifications, there were significant but asymmetric interference effects. Glottal feature.based classification showed the least amount of interference effect, while vowel feature.based classification showed moderate interference, and place of articulation feature-based classification showed the most interference. These results show that glottal features are more independent to vowels, but place features are more dependent to vowels in syllable perception. To examine the three-way interaction among glottal, place of articulation, and vowel features, Experiment 3 featured a modified Garner task. The outcome of this experiment indicated that glottal consonant features are independent to both the place of articulation and vowel features, but the place of articulation features are dependent to glottal and vowel features. These results were interpreted to show that speech perception is not abstract and discrete, but nonlinear, and that the perception of features corresponds to the hierarchical organization of articulatory features which is suggested in nonlinear phonology (Clements, 1991; Browman and Goldstein, 1989).

Asymmetrical diffuse Interstitial Lung Lesions with Multiple Lymphadenopathy (다발성 임파절 이상증을 동반한 비대칭적 폐 간질성 병변)

  • We have experienced a case of sarcoidosis appearing asymmetrical diffuse interstitial lung lesions with multiple lymphadenopathy. This patient was a 57 year-old female who had been in good health until 2 months ago. At that time she noted the onset of an exertional dyspnea and weakness. Pulmonary function tests showed moderate obstructive pattern with mild decreased DLco. In the chest CT, multiple lymphadenopathy with small nodular lesions are scattered, and the impression was a metastatic lymph nodes with lymphangitic carcinomatosis. In bronchofiberscopy, we noted luminal narrowing by extrinsic compression in the right middle and lower lobe bronchi. And microscopic examination of by bronchofiberscopic biopsy showed chronic inflammation. Thus we performed subcarinal and tight supraclaviclar lymph nodes aspiration biopsy cytology, and that revealed class 0 and class 1, respectively. Finally, we performed an excisional biopsy for the right scalene lymph node, which revealed the specimen as a noncaseating granuloma. The angiotensin convecting enzyme level was overt two folds compared to normal value. And the patient had negative PPD skin test and hyperglobulinemia. After 18 weeks treatment with prednisone, the signs and symptoms which the patient clad suffered from, disappeared.

A Study of Exploration- Oriented Mathematical Modeling: (탐구지향 수학적 모델링에 관한 연구)

  • Modern society's technological and economical changes require high-level education that involve critical thinking, problem solving, and communication with others. Thus, today's perspective of mathematics and mathematics learning recognizes a potential symbolic relationship between concrete and abstract mathematics. If the problems engage students' interests and aspiration, mathematical problems can serve as a source of their motivation. In addition, if the problems stimulate students'thinking, mathematical problems can also serve as a source of meaning and understanding. From these perspectives, the purpose of my study is to prove that mathematical modeling tasks can provide opportunities for students to attach meanings to mathematical calculations and procedures, and to manipulate symbols so that they may draw out the meanings out of the conclusion to which the symbolic manipulations lead. The review of related literature regarding mathematical modeling and model are performed as a theoretical study. I especially concentrated on the study results of Freudenthal, Fischbein, Lesh, Disessea, Blum, and Niss's model systems. We also investigate the emphasis of mathematising, the classified method of mathematical modeling, and the cognitive nature of mathematical model. And We investigate the purposes of model construction and the instructive meaning of mathematical modeling. In conclusion, we have presented the methods that promote students' effective model construction ability. First, the teaching and the learning begins with problems that reflect reality. Second, if students face problems that have too much or not enough information, they will construct useful models in the process of justifying important conjecture by attempting diverse models. Lastly, the teachers must understand the modeling cycle of the students and evaluate the effectiveness of the models that the students have constructed from their classroom observations, case study, and interaction between the learner and the teacher.

ST714-SCCmec type IV CA-MRSA isolated from a Child with Recurrent Skin and Soft Tissue Infections in South Korea: A Case Report (ST714-SCCmec type IV CA-MRSA에 의한 피부 연부조직 감염증으로 내원한 소아 증례)

  • Skin and soft tissue infections (SSTIs) caused by community-associated (CA)-methicillin-resistant Staphylococcus aureus (MRSA) have become a worldwide concern. An otherwise healthy 16-month-old Korean girl was admitted because of skin abscess on the left chest wall with a history of recurrent SSTIs since the age of 6 months. Immunologic evaluation including serum immunoglobulin level and nitroblue-tetrazolium (NBT) test were normal. Pus and nasal swab cultures revealed CA-MRSA ST714-SCCmec type IV with the Panton-Valentine leukocidin (PVL) genes, which was initially reported in the Netherlands in 2006 and has not been previously reported in Korea. The skin abscesses were successfully treated by needle aspiration and the use of antibiotics. In addition, nasal mupirocin was applied as a decolonization method. No more episodes of SSTI were observed over a follow-up period of 10 months.

Therapeutic Feasibility Study and Clinical Trial of Intrahepatic $I^{-131}-Lipiodol$ on Patients with Hepatocellular Carcinoma ($Lipiodol-I^{-131}$의 경간동맥 주입에 의한 간암치료의 가능성에 관한 연구)

  • An iodized oil such as Ethiodol or Lipiodol was selectively retained in the tumor vessels of the large hepatomas as well as in the small daughter hepatomas for long periods following the intra-arterial hepatic injection of such contrast material. The specific aim of the study is to deliver a high internal radiation dose to hepatocellular carcinoma (HCC) in an attempt to control the disease. We were able to replace a small fraction of the stable iodine (I-127) of the 37% iodine in Lipiodol by the $I^{-131}$ with 100% exchange efficiency. $I^{-131}$ labeled Lipiodol was injected through the super-selected tumor feeding artery under superselection or into the proper hepatic arterial level of patients who have malignant hepatomas confirmed by aspiration cytology serum AFP and various imaging modalities. Clinical traial was performed on 43 cases during recent 6 months and follow-up observation was carried out. No severe complications or other adverse reactions were encountered until nowdays. $I^{-131}-Lipiodol$ was stable in vivo and no significant activity was noted in the thyroid, stomach, blood and urine after the injection. Only small fraction of radioisotope activity was noticed in the both side of lungs. Tumor to normal liver radio was very high. Therefore, $I^{-131}-Lipiodol$ (or P-32-Lipiodol) will be effective delivering high internal radiation dose to the tumor while delivering small radiation doses to normal tissues. Labeling, tumor dose calculation and preliminary findings will be presented.

  • Background: The Bethesda System for Reporting Thyroid Cytopathology is one of the main classification systems for thyroid nodules. It expects that 7% of all fine needle aspiration biopsies will be reported as atypia or follicular lesions of undetermined significance, and 5-15% of these undetermined nodules are malignant. Our study is a retrospective analysis of variables that may be associated with outcome in patients with indeterminate thyroid nodules. Materials and Methods: Patients who underwent thyroidectomy in our institution between 2010 and 2014 were retrieved from the institutional records database. Patient demographics and medical histories were recorded. All ultrasonography reports were examined for nodule features and biochemical blood levels, hormone levels and complete blood counts were recorded. Results: A total of 103 patient cytopathology reports were regarded as belonging to the undetermined category. Some 35% of patients had malignant nodules. Median preoperative red cell distribution width (RDW) level was 13.6 inthe benign group, while it was 14.3 in patients with malignancy, demonstrating a significant correlation (p=0.003). Only calcification presence was significantly different between benign and malignant groups on ultrasonography (p=0.034). Conclusions: Ultrasonography is one of the primary tools for this matter. RDW levels may become another promising tool to predict malignancy.

MRI Finding of a Case of Relapsed Acute Mixed Type Leukemia with Bone Marrow Necrosis (재발한 급성 혼합형 백혈병에 동반된 골수괴사 1례의 MRI 소견)

  • Bone marrow necrosis is a rare complication of a variety of diseases affecting the marrow. The cause and incidence are unknown, and reports of treatment response are rare. We describe a case of relapsed acute mixed type leukemia with bone marrow necrosis. The patient was a 10 year old female diagnosed with acute mixed type leukemia four years ago. She had been on second remission state for 1 year, presented with severe back pain, tenderness in lower extremities, low-grade fever and general weakness. Her level of serum lactic dehydrogenase on admission was increased. Bone marrow aspiration from both posterior iliac crest showed marrow necrosis. Subsequent examination showed the same feature. Hip MRI showed heterogenous low signal intensity in both iliac bone on T-1 weighted image and heterogenous high signal intensity on T-2 wieghted image. Remission induction therapy was started but she expired on 59th hospital day due to the complication of sepsis.
