한우송아지의 BVD바이러스 감염과 Aspergillus fumigatus의 복합감염 (Systemic Aspergillosis associated with bovine viral diarrhea virus infection in Korean native calves)

  • Systemic aspergillosis were outbroken of 17 Korean native calves at livestock farm in Kyunggi province. Two 3 months old calves were died after having diarrhea, coughing, dyspnea, nasal discharge, and abdominal breathing. These calves were diagnosed of aspergillosis as multifocal to coalescing pyogranulomatous inflammation with intralesional aspergillus infiltration at liver, kidney, spleen, heart, lymph nodes, lungs, intestine and cerebrum. Aspergillus fumigatus was cultured from the affected tissues. And the small intestine was positive against bovine viral diarrhea (BVD) virus on FA test. We considered that the secondary fungal infections killed the calves after BVD virus infection primarily.

Lymphadenopathy Associated with Disseminated Aspergillosis in a Jindo Dog in Korea

  • A 2-year-old outdoor, spayed, female Korean Jindo dog was referred for thoracic and abdominal masses. Diagnostic imaging revealed generalized lymphadenopathy and a significant amount of pleural effusion. The cytological evaluation of the pleural effusion and fine-needle aspiration of the mediastinal lymph nodes confirmed the presence of fungal hyphae. The histopathological examination of the mediastinal lymph nodes stained with Grocott's methenamine silver revealed fungal mycelia, and Aspergillus terreus was identified based on fungal DNA sequencing. Thus, the dog was diagnosed with disseminated canine aspergillosis caused by A. terreus. This case report describes a rare case of disseminated canine aspergillosis caused by A. terreus in a Korean Jindo dog.

상악동내 국균증과 편평상피세포암의 동시발생 증례: 조직학적 연구 및 문헌고찰 (Squamous cell carcinoma occurring with aspergillosis in the maxillary sinus: a case report and histological study)

  • The coexistence of aspergillosis and squamous cell carcinoma (SCC) in the maxillary sinus was very rare. To our knowledge, this is the second report of coexistent SCC and aspergillosis in the maxillary sinus. A 58-year-old man underwent surgery for unilateral maxillary sinus infection with oroantral fistula. In the surgical specimen, SCC and aspergillosis were co-detected with routine and immunohistochemical stainings. Moreover, human papillomavirus 18 (HPV-18) was detected by polymerase chain reaction in the sinus specimen. The patient was re-operated with subtotal maxillectomy and has been followed up for two years without any evidence of recurrence or metastasis. Although it is not understood how aspergillosis could induce carcinoma formation, the chronic inflammation caused by prolonged fungal infection might be carcinogenic. Moreover, HPV-16 and -18 were another causative pathogens of SCC in the head and neck region. We recommend careful examination, including preoperative cytology, in patients with maxillary sinus fungal infections because of the potential for cancer development.

양측성 실명을 동반한 접형동 아스페르길루스증 1 예 (A case of Bilateral Near Blindness Secondary to Isolated Sphenoid Sinus Aspergillosis with Headache)

  • Sphenoid sinus aspergillosis is notorious for its serious complications, such as permanent cranial nerve deficits and possible death. The most common associated symptoms are headache, followed by visual changes, and cranial nerve palsies. Because of an insidious onset, frequently resulting in missed and delayed diagnosis, sphenoid sinus aspergillosis is a potentially lethal medical condition. We report a case of visual loss secondary to isolated sphenoid sinus aspergillosis. A 69-year-old man presented to our hospital with the complaint of headache. The headache started one year previously and was described as severe dull pain localized bilaterally to the temporo-orbital region. The patient took daily NSAIDs for the pain. The neurological examination was normal. The MRI of the brain showed a left sphenoid sinusitis. A transnasal endoscopic superior meatal sphenoidotomy was performed. Aspergillosis was confirmed after a surgical biopsy was obtained. The patient was discharged from hospital without antifungal therapy. One month later, the patient complained of headache and loss of vision bilaterally. The orbital MRI showed a left cavernous sinus and bilateral optic nerve invasion. The loss of visions was permanent. In our case, the diagnosis was delayed; antifungal agents were not administered after surgery and the patient lost his vision as a result. Therefore, early diagnosis and proper treatment are important. Although the treatment of an invasive type of aspergillus has not been established, surgical removal of a nidus and aggressive antifungal therapy are recommended.

중추신경계의 Aspergillosis (Aspergillosis of Central Nervous System)

  • Objectives : Aspergillosis of central nervous system(CNS) is a rare pathologic condition and it has been known to be difficult to diagnose and treat. We analyzed seven cases of central nervous system aspergillosis. The clinical characteristics, and the problems in diagnosis and treatment are discussed with review of previous literatures. Material and Methods : We reviewed the clinical records, radiological findings, and pathologic reports of 7 patients with aspergillosis which involved CNS. Results : Five patients were immunocompetent, and infection was related with previous operation in 4 of them. Two patients were immunocompromised and had no history of operation. Five patients had intracranial lesions and two had spinal lesions. Mean duration from the onset of initial symptom to pathologic diagnosis was 2.4 months. Mean duration from the previous operation to the onset of symptom was 9.3 months, and from the onset of symptom to diagnosis was 2.9 months in the patients who had histories of operation. All of them were treated with surgical procedures and intravenous and oral antifungal agents, resulting in cure in 6 cases. Mean duration of the treatment was 4.9 months. Conclusion : Because aspergillosis of CNS is a rare disease and is difficult to be differentiated from the pyogenic abscess or recurrent tumor, the pathologic diagnosis is very important for adequate treatment. Although the prognosis of aspergillosis of CNS has been known to be poor, adequate surgery for both diagnosis and treatment and antifungal chemotherapy resulted in good outcome.

폐 Aspergillosis 의 외과적 치료 (Surgical treatment of pulmonary aspergillosis)

  • Since tuberculosis was a common pulmonary disease in Korea, Aspergillosis was easily misdiagnosed as tuberculosis and an acute form of Aspergillosis was misinterpreted as pneumonia because of their similarities in the X-ray findings. This investigation is designed to illustrate the clinical features and preoperative diagnosis and surgical role in the management of this disease. In a retrospective review of operative cases from Jan. 1963 through Dec. 1983, 36 cases were analyzed. Peak age incidence lies in the 3rd decade [41.7%]. All cases had a history of treatment with antituberculous drugs under diagnosis of pulmonary tuberculosis and the most common chief complaint was hemoptysis [69.5%]. Only nine cases [25%] showed cavitary lesions with mycetoma and preoperative sputum study for fungus showed low positive valve [42.3%]. Anatomical location of lesion was located mainly upper lobe [66.7%] and most of cases were managed by lobectomy. We experienced 7 cases of complication; they were postoperative empyema, hepatic failure, esophageal varix bleeding. Postoperative pathologic findings showed that 29 cases [80.5%] were combined with tuberculosis 3 cases were combined with bronchiectasis and 4 cases were not combined with other disease. In conclusion, when the patient has a longstanding history of pulmonary tuberculosis and has a hemoptysis, he must be suspected fungus super infection. Resectional surgery is the treatment of choice for symptomatic localized disease and needed resection in asymptomatic patient to prevent possible fatal sequelae in the future.

폐 Aspergillosis 15례 검토 (Pulmonary Aspergillosis: Review of 15 Cases)

  • Fungi of the genus Aspergillus are ubiquitous Saprophytes in nature. Many reports since the first report of human aspergillosis by Slayter[1847], have suggested that this is not a rare condition. Author have reviewed 15 cases of pulmonary aspergillosis obtained in Dept. of Thoracic Surgery, Busan University Hospital from 1966 through 1976, and taken literatural consideration. The results were obtained as follows. 1. Ages ranged from the first to sixth decades, with the majority [73.3%] in the third and fourth decades. The male to female ratio was two to one. Both upper lobes were predominantly involved [80%]. 2. Hemoptysis was the most common symptom and it was appeared in 66.6%. 3. Only 4 cases [26.7%] were take correct diagnosis before operation. Remained cases were difficult to be taken the correct diagnosis preoperatively because of lower discovery rate of aspergillus in application on immunological typical X-ray findings and non-specific symptoms of them. 4. Preoperatively, 8 cases [53.3%] were taken diagnosis as pulmonary tuberculosis. 5. Underlying diseases were present in 11 cases [73.3%] and 6 cases of them were pulmonary tuberculosis.

폐 Aspergollosis의 외과적 치료[제 2보] (Surgical Treatment of Pulmonary Aspergillosis[II])

  • This investigation is designed to illustrate the clinical features '||'&'||' preoperative diagnosis, surgical role in the management of pulmonary aspergilloma, '||'&'||' compare with the previous study. We reviewed 42 cases of surgically treated pulmonary aspergillosis from Jan. 1984 to July 1992. The peak age incidence laid in the 2nd '||'&'||' 3rd decade of 25 cases[59.5%]. The 41 cases[97.6%] had a history of treatment with anti-tuberculous drugs under impression of pulmonary tuberculosis. The most common complaint was hemoptysis in 28 cases[66.7%]. The 21 cases showed so called "Air-meniscus sign" on the preoperative chest X-ray. As a preoperative diagnostic modality, the positivity was 30.0%, 57.8%, 88.5% on the fungus culture of sputum for Aspergillus, Chest CT, serum immunodiffusion test for A. fumigatus, respectively. The 37 cases[88.1%] can be diagnosed as pulmonary aspergillosis pre-operatively by any diagnostic method. The anatomical location of aspergilloma was mainly upper lobe in 17 cases[40.5%] '||'&'||' the majority of cases were managed by lobectomy. The postoperative pathologic findings showed that 18 cases[41.0%] were combined with tuberculosis '||'&'||' 15 cases[34.1%] were not combined with any other disease[Pr imary Aspergillosis]. The 6 cases showed postoperative empyema including 4 cases of bronchopleural fistula, 3 cases showed postoperative bleeding. One case was died postoperatively due to respiratory failure. In conclusion, when the patient who has longstanding history fo pulmonary tuberculosis '||'&'||' hemoptysis, must be suspected fungal super infection. The resectional surgery is the treatment of choice for symptomatic localized disease. And compare with the previous study, preop. chest CT '||'&'||' immunodiffusion test were more commonly available '||'&'||' showed high positivity.

  • 뇌에 발생하는 아스페르질루스증은 최근 들어 급증하고 있다. 이 질병은 흔히 면역이 저하된 환자들에서 호발하며, 다른 원발 장기로부터 중추 신경계로의 혈행성 전파 또는 인접 구조물로부터의 직접 침입이 주요 발생 기전으로 알려져 있다. 본 증례는 특이 과거력 없는 29세 남환에서 발견된 고립된 뇌 병변에 대한 보고이며, 정위적 뇌 조직검사에 의한 병리 결과상 아스페르질루스균이 확인 되었다.
