Bilateral Lipoma Arborescens of the Knee in an Adolescent Patient (청소년에서 양측 슬관절에 발생한 수지상 지방종)

  • Lipoma arborescens (LA) is a rare lesion that causes joint effusion and pain. LA usually presents as a unilateral knee lesion, and bilateral lesions are extremely rare. LA among adolescent patients is also extremely rare with the disorder having a much higher incidence among adults. This paper reports a 14-year-old boy who was treated for bilateral knee LA. X-rays revealed the characteristic features of LA in both knees, and arthroscopic synovectomy was performed to resect the lesions. Postoperative recovery progressed smoothly, and the patient was eventually relieved of pain and swelling. LA should be considered when adolescent patients present with bilateral pain and swelling, even though this condition is rare in this age group. Furthermore, surgical synovectomy for a complete resection is an appropriate treatment option.

Arthroscopic Treatment of Synovial Chondromatosis of the Shoulder Joint with Mini-open Procedure for the Lesions of Biceps Tendon Sheath (견관절에 발생한 활액막 연골종증의 관절경적 치료에서 소절개를 이용한 상완 이두건 건초 병변의 병행 치료)

  • Synovial chondromatosis is an uncommon condition, and the involvement of the shoulder joint is rare. A 15 year old female patient presented to author's institution for right shoulder pain. We checked the plain radiographs and MRI. And they showed that a diagnosis of synovial chondromatosis in the shoulder, and they also demonstrated that the disease involved the bicipital tendon sheath as well as glenohumeral joint. We removed all loose bodies with total synovectomy by arthroscopic procedure, and a miniopen procedure for the lesions of biceps tendon sheath. Arthroscopic treatment affords excellent visualization of the shoulder joint with less morbidity. However, with current arthroscopic techniques, it is difficult to manage the synovial chondromatosis of biceps tendon in bicipital groove. The authors suggest that the complete elimination of synovial chondromatosis involving shoulder requires a mini-open procedure for the lesions of biceps tendon sheath in addition to the arthroscopic resection of the affected synovium and loose body removal in the glenohumeral joint.

Pyogenic Arthritis of the Shoulder in Patient with Infective Endocarditis -A Case Report- (감염성 심내막염 환자에서 동반된 화농성 견관절염 - 증례보고 -)

  • Purpose: We report a case of pyogenic arthritis of the shoulder secondary to infective endocarditis. Materials and Methods: A 70 year-old male who had suffered from pyogenic arthritis of the left shoulder secondary to infective endocarditis was treated with artificial valvuloplasty, arthroscopic synovectomy and drainage. Results: Infection was cured and the patient achieved a good functional outcome. Conclusion: Pyogenic arthritis of the shoulder is rarely associated with infective endocarditis. However, if the symptoms are misdiagnosed as musculoskeletal symptoms associated with infective endocarditis, serious complications may arise. As such, musculoskeletal symptoms associated with infective endocarditis should be paid careful attention.

  • Pigmented villonodular synovitis most commonly occurs in the knee joint and rarely extends into the popliteal space, which not only has mimicked Baker's cyst but also has been misdiagnosed as malignant tumor. We report a case of a diffuse pigmented villonodular synovitis of knee joint which is extended into the popliteal space as like a popliteal cyst. We treated this case by arthroscopic total synovectomy and excision of cyst. There was no recurrence during more than one year after operation.

Aseptic Synovitis after Meniscal Repair using Biodegradable Meniscus Arrow - A Case Report - (생체분해성 Meniscus Arrow를 이용한 반월상 연골 봉합술후 발생한 비세균성 활액막염 -증례보고-)

  • This is a report on a aseptic synovitis diagnosed and treated arthroscopically following the meniscal repair using biodegradable Meniscus Arrow$^{(R)}$(Bionix Inc, Malvern, USA). Histological examination revealed chronic nonspecific synovitis and birefringent material. Immunohistochemical tests were positive in lysozyme, ${\alpha}$-1-antitrypsin and ${\alpha}$-1-antichymotrypsin. The patient was received arthroscopic synovectomy, and then pain and swelling of the knee joint was relieved. Range of motion was improved to full range. As far as we know, this is the first case of aseptic synovitis after meniscal repair using biodegradable Meniscus Arrow$^{(R)}$.

  • Most pigmented villonodular synovitis (PVNS) is occurred in knee joint and finger of hand. PVNS is rarely occurred in shoulder joint. In English and French literatures, less than 30 cases were reported. We report 3 PVNS cases with rotator cuff tears, which was treated by arthroscoic extensive synovectomy, debridement and rotator cuff repair. The PVNS with rotator cuff tear in shoulder joint was rarely reported in the Korean literature.

  • Klebsiella pneumoniae causes that liver abscess mostly, also spread to pneumonia, meningitis, urinary tract infections. Septic arthritis caused by K. pneumoniae is a quite rare and has not been reported in Korea. Therefore, the authors report a case of the septic arthritis in the knee joint caused by K. pneumoniae in a patient with type 2 diabetes mellitus and osteoarthritis of the knee that successfully treated by early detection and arthroscopic synovectomy.

  • Purpose: We reported the results and efficiency of arthroscopic treatment in a state of the lateral traction about the degenerative arthris of elbow Materials and Methods: Twenty one elbows with the degenerative arthritis who were followed up for at 12 months were enrolled in this study. Male were 15 cases, female were 6 cases, the mean age was 47 years and right dominant hand was 14 cases. In all cases, during arthroscopic treatment under the traction of 10 pounds, we had done synovectomy, excision of loose body and anteroposterior spur. After operation, immobilization was done in the full extension state, and then continuous passive motion (CPM) was started two day Results: The average preoperative ROM of the elbow joint was $30{\sim}l15$ degree and the average postoperative ROM of the elbow joint was $5{\sim}130$ degree. The increasement of ROM was totally 41 degree in extension 25 degree and flexion 16 degree. The decrement of VAS in pain was from 7.5 into 2.3 and the increasement of the satisfactory function was from 1.8 into 9.0. Complication was in two cases. One was paresthesia of ulnar nerve, but resolved. The other was bullae formation around the elbow joint, but cured. Conclusion: Regarding degenerative arthritis of elbow, arthroscopic treatment showed excellent result in recovery of range of motion and relief of pain. We could obtain good visual field with distraction in lateral position.

Cellulitis and Ankle Joint Synovitis after Acupuncture at BL60: A Case Report (곤륜(BL60) 자침 후 발생한 족부의 봉와직염 및 족관절 윤활막염: 증례 보고)

  • The purpose of this study is to report a case diagnosed with cellulitis and ankle joint synovitis after acupuncture treatment. An 85-years old female suffering from low back pain was managed by acupuncture including BL60. After treatment, unintended ankle pain occurred. The pain was diagnosed with cellulitis and synovitis by MRI. The patient was transferred to the Western medical hospital, and ankle joint arthroscopic synovectomy was performed. It was difficult to find a clear relationship between acupuncture and ankle joint synovitis. But it would be reasonable to assume that cellulitis was associated with acupuncture needling and synovitis was followed. Serious adverse effects, such as septic arthritis, are rare, but need significant attention. For acupuncture treatment on the ankle, especially using BL60, the depth and manipulation should be carefully performed along with sterilization.

Treatment of Infection after Anterior Cruciate Ligament Reconstruction: Graft Retention (전방 십자 인대 재건술 후 감염의 치료: 이식건의 보존)

  • Purpose: To report the results of graft retention treatment for infection cases after anterior cruciate ligament(ACL) reconstruction. Materials and Methods: From a group of 511 consecutive ACL reconstruction patients, we report 5 who sustained septic arthritis. All patients underwent arthroscopic lavage, debridement, synovectomy with graft retention, and treatment with intravenous antibiotics. Results: All patients were evaluated at an average of 34 months after operation. The infection was successfully eradicated. No patient showed instability with KT-2000 arthrometer testing. The clinical outcome was inferior to normal ACL reconstruction patients. Two patients were graded as nearly normal and 3 patients were graded as abnormal with International Knee Documentation Committee evaluation form. Conclusion: There were no recurrences of septic arthritis or bone infection with graft retention treatment for infection patients after ACL reconstruction.
