고열, 발진을 동반하는 전신형(systemic) 소아기 류마티스 관절염(juvenile rheumatoid arthritis; JRA) 환아 증례 1례 보고 (A Case Report of Systemic Type Juvenile Rheumatoid Arthritis with Fever and Eruption)

  • Objectives This study is to report a case that has an important meaning as a result of treating Juvenile Rheumatoid Arthritis. We investigated a patient who had to maintain his life with western medicines such as DMARDs, NSAIDs and steroids for a long time. The patient has recovered from all symptoms and his ESR, CRP has been back to normal range with oriental medicine treatment. Methods The patient had fever, especially repeated fever during the afternoon, pain and swelling of joints, generalized skin eruption, anorexia, delayed growth, weight loss, fatigue. So we treated him with herbal medicine and reduced his western medicine. The aim of treatment was recovery from Juvenile Rheumatoid Arthritis after discontinuance of all western medicine. Results The symptoms of systemic type Juvenile Rheumatoid Arthritis was vanished and the patient maintains his condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. His ESR and CRP levels were back to the normal range. After this treatment the patient's height and weight has been increased which showed a significant meaning in growth to the child. Conclusions This study showed that oriental medicine can elevate the Juvenile Rheumatoid Arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.

관절염 환자의 자조관리과정 강사지원자의 지식 및 태도 분석 (Analysis of Knowledge and Attitude of Applicants of Trainers for Self-help Program of Arthritis Patients)

  • The purpose of this study is to analyze the knowledge and attitude of applicants on self-help teacher's program for arthritis patients. Subjects of the study constituted 407 applicants who attended the self-help teacher's program for arthritis patients. Self report questionnaires were used to measure the knowledge about arthritis and the attitude toward self-help education. In data analysis, SPSS PC 9.0 Program was utilized for descriptive statistics and one-way anova analysis. The results were as follows : Average age of the participants were 37, while 62% married, 62% had 4-year or above education, and 32.7% had experience of community health nursing. The mean score of knowledge about arthritis was 17.99/28 ; the mean score of applicants' appraisal on the arthritis patients' ability of self-help was 31.66/40 ; the mean score of applicants' attitude toward self-help education for arthritis patients was 35.64/56. According to regions of education for applicants' self-help program, there were not significant differences in applicants' knowledge and appraisal and attitude on self-help program.

류마티스 관절염 환자가 받는 가족지지 정도에 따른 일상활동과 삶의 질 차이 (The Influence of Family Support on the Activities of Daily Living & Quality of Life among the Patients with Rheumatoid Arthritis)

  • The aim of this study was to investigate the difference in the level of ADL & QOL by family support in the patients with rheumatoid arthritis. The subjects of the study consisted of fifty-six outpatients with rheumatoid arthritis at a university hospital in Taegu. The instrument used in this study were the family support scale developed by Kang(1984), the ADL scale by Katz et al.(1970) and Barthel(1973), the QOL scale developed by Jo(1993). Data was analysed using descriptive statistics, Pearson Correlation, Chi-square, ANOVA with SPSS program. The major findings are as follows : 1. There was not significant differences in the level of ADL by family support in the patients with rheumatoid arthritis(F=.436, p=.649). 2. There was significant differences in the level of QOL by family support in the patients with rheumatoid arthritis(F=3.782, p=.029). This study showed that the higher level of family support contributed to the better level of QOL in the patients with rheumatoid arthritis. It is recommended to promote the level of family support of rheumatoid arthritis patients with low family support QOL.

류마티스관절염과 섬유조직염환자의 증상, 일상생활, 자기효능 및 가족지지 비교 연구 (A Comparative Study on Symptoms, Living Activities, Self-efficacy and Family Support between Rheumatoid Arthritis Patients and Fibromyalgia Patients)

  • This study was conducted to compare symptoms, living activities, self-efficacy and family support between rheumatoid arthritis patients and fibromyalgia patients. The subjects were consisted of 150 rheumatoid arthritis patients and 125 fibromyalgia patients at H Rheumatism Clinic. Data were collected by means of structured interview with questionnaires. The results were as follows: 1. Fibromyalgia patients have more symptoms than rheumatoid arthritis patients have. But the latter has more difficulties concerning living activities. 2. Self-efficacy score of fibromyalgia arthritis patients was higher than that of rheumatoid arthritis patients, but both the scores were at moderate level. 3. The level of family support of rheumatoid arthritis patients was moderate, similar as that of fibromyalgia patients. As self-efficacy and family support are important factors to relieve symptoms of both diseases, their promotion holds a key post.

  • The studies look at the causes, pathology, symptom, diagnosis, treatment of rheumatoid arthritis in the sight of east-western medicine. Following are the results : 1. Rheumatoid arthritis korean-medically belongs to symptom such as Bi(痺), Poung Bi(風痺), Tong Poung(痛風), Youk Jeol Poung(歷節風), Bak Hoe Poung(白虎風), Bak Hoe Youk Jeol Poung(白虎歷節風), etc. 2. The causes of Rheumatoid arthritis are endopathic and exopathic : endopathically it tis related to Chil Jeong(七情) and visceral imbalance, and exopathically it is related to Euk Eum (六淫) and environmental factors. 3. To figure out rheumatoid arthritis, we need to diagnose first the joint symptom and the general body condition, we need about 6 weeks to get the exact decision after the serologic test, the immune and inheritance test, the synovium test. 4. There are four kinds of treatments for rheumatoid arthritis : medicin, acupucture and moxibustion, suction cup and purging away the blood, physical therapy. Cheong Eol So Bi(淸熱消痺) can be used for the acute stage and Jang Bu Seong Shai(臟腑盛衰) is to be considered for the chronic stage. 5. Rheumatoid arthritis should be constantly treated to prevent the patient from the deformity of joint complication.

만성 관절염 환자를 대상으로 한 극복력과 우울의 상관성 조사 (A Correlational Study on the Mastery and Depression in Chronic Arthritis Patients)

  • Purpose: The purpose of this study was to investigate the relationship between the level of mastery and depression in chronic arthritis patients. Method: The subjects for this study were 100 patients registered in S University Hospital, and the period of data collection was from June 20, 2006 to August 30, 2006. Results: The cronbach's ${\alpha}$ of the research instruments were .70-.86. In data analysis, SPSSWIN 12.0 program was used for descriptive statistics. The results were as follows. 1) The range of total mastery scores was from 11 to 28 and the mean score of the depression in chronic arthritis patients was 17.88. 2) The range of total depression scores was from 20 to 72 and the mean score of the depression in chronic arthritis patients was 39.99. 3) The level of mastery was significant correlation with the level of depression in chronic arthritis patients(r=-.466, p<.01). Conclusion: Mastery had significant correlation with depression in patients who have chronic arthritis. Therefore, the strategy of nursing intervention which improve mastery must be developed for patients who have chronic arthritis.

  • Arthritis is a major cause of joint pain, stiffness, and subsequent disability which adversely affects quality of life. Seriously, it can lead to long term social and psychological effects including loss of independence, depression, and anxiety. Arthritis is usually treated with joint replacement surgery or medications. However, the artificial joint is temporary and pharmacological measures have side effects, such as addiction or hypersensitivity. Thus, orthotics has been developed to improve arthritis as a nonpharmacological measure. The increased regional load across compartments of articular cartilage is an important factor in the cause of the arthritis. Mubal$^{(R)}$, a clinical shoe insole, has a sliding function to help people to walk straight and realign the body balance. The slide of Mubal$^{(R)}$ reduces the knee joint loading in patients with arthritis. In addition, pumping function of Mubal$^{(R)}$ can mitigate arthritis by stretching the squashed nerves from lumbar to cervical vertebral and actively circulating blood of pelvic limb. In addition, Mubal$^{(R)}$ could help to stimulate the growth plate. Therefore, Mubal$^{(R)}$ can be used for the child with short stature as well as patients with arthritis.

류마티스 관절염 환자가 지각하는 통증과 피로 (A Study on the Pain, Fatigue Perceived by Rheumatoid Arthritis Patients)

  • Purpose: The purpose of this study was to investigate the relationship between the level of pain and fatigue in rheumatoid arthritis patients. Method: The subjects for this study were 120 patients registered in S University Hospital, and the period of data collection was from June 20, 2005 to October 15, 2005. Results: The research instruments used in this study were the Graphic Rating Scale of Pain and Fatigue, and the cronbach's ${\alpha}$ of Fatigue scale was .90. In data analysis, SPSSWIN 12.0 program was used for descriptive statistics. The results were as follows. 1) The range of total pain scores was from 10 to 100 and the mean score of the pain in rheumatoid arthritis patients was 53.70. 2) The range of total fatigue scores was from 20 to 58 and the mean score of the fatigue in rheumatoid arthritis patients was 36.04. 3) There was a significant difference in pain according to duration of disease(F=3.934, p<.05). 4) There was a significant difference in fatigue according to duration of disease(F=3.442, p<.05). 5) The level of fatigue was significant correlation with the level of pain in rheumatoid arthritis patients(r=.217, p<.01). Conclusion: The level of fatigue was significant correlation with the level of pain in rheumatoid arthritis patients. Therefore nursing interventions decreasing the pain perceived by rheumatoid arthritis patients are needed to decrease there fatigue levels.

$18{\beta}$-Glycyrrhetinic Acid의 항 감염성관절염효과 (Effect of $18{\beta}$-Glycyrrhetinic Acid on Septic Arthritis Caused by Candida albicans)

  • A polymorphic fungus, Candida albicans, causes various forms of infections such as disseminated candidiasis and vaginitis. Recent reports indicate that the fungus is a main etiological agent for the arthritis. In search of new sources for treatment of the fungal arthritis, we examined $18{\beta}$-glycyrrhetinic acid ($18{\beta}$-GA) against C. albicans-caused septic arthritis. The compound is isolated from Glycyrrhizae Radix that is known to have various immunomodulating activities and is one of the most popular herbal medicines. For induction of animal model of a septic arthritis, mice were given an emulsion form of C. albicans cell wall mixed with Complete Freund's Adjuvant (CFA) via footpad-injection. To determine prophylactic and therapeutic effects, the component was given to the animals before or after the induction of the arthritis, respectively. Data showed that intraperitoneal administration of $18{\beta}$-GA resulted in reduction of the inflammation, indicating the component had both prophylactic and therapeutic activities. For investigation of mechanism of the $18{\beta}$-GA, inhibitory effects on NO (nitiric oxide) and on T-lymphocyte proliferation were determined. Results demonstrated that $18{\beta}$-GA suppressed NO production from LPS (lipopolysaccharide)-treated macrophages and also inhibited proliferation of Con A (concanavalin A)activated T-cells. Taken together, $18{\beta}$-GA, a pentacyclic triterpene, has anti-arthritic activity against C. albicans-caused septic arthritis, possibly by blocking NO production and T-cell suppression.

류마티스 관절염 환자의 무기력과 건강행위에 관한 연구 (Helplessness and Health Behavior of Patients with Rheumatoid Arthritis)

  • Purpose: This study was to find out the relationships between helplessness and health behavior of patients with rheumatoid arthritis. Method: The subjects were 293 rheumatoid arthritis patients who visit 2 rheumatology outpatient clinics in G city. Data were collected by questionnaires including Arthritis helplessness index(Nicassio, Wallston, Callahan, Herbert, & Pincus, 1985)and health behavior assessment scale(Jo, Oh & Choe, 2000). The data were analyzed with Pearson correlation coefficient, t-test, ANOVA and Duncan's multiple range test. Results: The majority of the subjects were women and the mean age was 50.4years. The mean duration of illness was 8.9years. There were negative correlations between the degree of helplessness and the degree of health behavior. The degree of helplessness was significantly high in those who were older, female, less educated, out of workforce, and those who had no chances to attend any arthritis-related classes. The degree of health behavior was significantly low in those who were older, high school graduate, in higher economic status, married, and those who had chances to attend arthritis-related classes. Conclusion: To reduce the helplessness of rheumatoid arthritis patients, the nursing intervention which is to increase the health behavior should be developed and provided to the rheumatoid arthritis patients.
