노인의 팔 굽힘/폄 저항 운동 시 하중 증가에 따른 근육의 활성도 및 기여도 분석 (Analysis of Activation and Contribution of Muscles of the Elderly During Arm Flexion and Extension Resistance Exercise with Increased Load)

  • The purpose of this study was to assess activation and contribution of muscles of the elderly according to increased loading during the arm flexion extension resistance exercise. Surface electromyographic signals were acquired from biceps brachii, triceps brachii, deltoid posterior, pectoralis major and latissimmus dorsi to determine the difference of the activation of specific muscles between the elderly and young. Five elderly and five young males with no musculoskeletal disease volunteered for the study. Electromyographic activities in the muscles were measured during resistance exercise and normalized to the maximum EMG activity recorded in the maximal voluntary static contraction (MVC). Against the increased loading during arm flexion/extension resistance exercises, the young uses muscles evenly but the elderly uses one specific muscle frequently. Contribution of triceps brachii during extension and deltoid posterior during flexion was principal in the elderly.

  • Purpose : It was to compare changes in respiratory function (pulmonary function, inspiratory function) after four weeks of inspiratory muscle strengthening training (diaphragmatic breathing with upper arm exercise, Power-Breathe breathing) for 36 healthy people. Methods : Subjects were randomly assigned to diaphragmatic breathing with upper arm exercise (Group I) and Power-breathe breathing (Group II) was conducted by the protocol for four weeks five times per week. As the main measurement method for comparison between groups For pulmonary function, Forced Vital Capacity (FVC) and Forced Expiratory Volume at One second (FEV1) were used, and for inspiratory function, Maximum Inspiratory Capacity (MIC), Maximum Inspiratory Pressure (MIP), and Maximum Inspiratory Flow Rate (MIFR) were used. Results : In changes in pulmonary function between groups, FVC and FEV1 showed no significant difference, and in inspiratory function changes, MIC showed no significant difference, but in MIP and MIFR, Group B significantly improved over Group A. Conclusion : The progressive resistance training using the Power-breath device applied to the inspiratory muscle did not show a significant difference in the increase in the amount of air in the lungs and chest cage compared to the diaphragmatic breathing training accompanied by the upper arm exercise. However, by increasing the air inflow rate and pressure, it showed a more excellent effect on improving respiratory function.

  • Purpose: The purpose of this study was to evaluate the relationship between foot pressure and upper arm PNF exercise conducted with elastic bands while standing. Methods: Eighteen asymptomatic patients consented to participate in the study. Foot pressure was measured in the standing position using the Gaitview system for proprioceptive neuromuscular facilitation stretching (PNF) using a right upper arm pattern. Resistance strength was varied based on the type of elastic band used (red=medium, green=heavy, and blue=extra heavy). Statistical significance of the results was evaluated using a repeated one-way ANOVA, and the Bonferroni method was used for the ad hoc test (SPSS ver. 18. for Windows). Results: A significant difference was observed in fore-foot, rear-foot, and everage pressure after upper arm PNF exercise. However, there was no significant difference according to the type of elastic band. Conclusion: Based on the results of this study, an upper arm PNF exercise with and without resistance may affect foot pressure in the standing position.

Curcumin supplementation attenuates the decrease in endothelial function following eccentric exercise

  • [Purpose] Eccentric exercise induces a decrease in vascular endothelial function. Curcumin, a major component of turmeric, has potent antioxidant and anti-inflammatory properties that are associated with vascular protective effects. The present study examined the effect of acute supplementation of curcumin on eccentric exercise-induced endothelial dysfunction in healthy young men. [Methods] Fourteen healthy sedentary young men (range, 21-29 years) were assigned to either the curcumin (n = 6) or placebo (n = 8) group. All subjects consumed either curcumin or placebo before exercise, and eccentric exercise of the elbow flexors was performed with their nondominant arm. Before and 60 min after exercise, brachial artery flow-mediated dilation (FMD), as an indicator of endothelial function, was measured in the non-exercised arm. [Results] Brachial artery FMD significantly decreased following eccentric exercise (p < 0.05) in the placebo group, but acute supplementation with curcumin before exercise nullified this change. The change in FMD before and after eccentric exercise between the placebo and curcumin groups was significantly different (p < 0.05). [Conclusion] The present study found that acute curcumin supplementation could attenuate the decrease in endothelial function, as measured by FMD, following eccentric exercise in healthy young men.

입원초기에 적용된 상지수동운동이 뇌혈관질환자의 환측 관절가동범위, 근력 및 상지 경직에 미치는 효과 (Effects of Passive Upper Arm Exercise on Range of Motion, Muscle Strength, and Muscle Spasticity in Hemiplegic Patients with Cerebral Vascular Disease)

  • Purpose: The purpose of this study was to investigate the effects of passive upper arm exercise on range of motion, muscle strength, and muscle spasticity in hemiplegic patients with cerebral vascular disease. Methods: A quasi-experimental design with nonequivalent control group was utilized. According to inclusion criteria, 25 patients were assigned to the control group with routine care, followed by 25 to the intervention group with passive exercise for 30 minutes per session, twice a day for 2 weeks. Eighteen patients in the intervention group and 17 in the control group completed the posttest measurement, including range of motion for upper arm joints, manual muscle test, and Modified Ashworth Scale for muscle spasticity. Results: The intervention group had a significantly improved range of motion in the shoulder and wrist joints. No interaction effect was found for the elbow joint. No significant differences were found in muscle strength or muscle spasticity between the groups. Conclusion: Results of the study indicate that passive exercise safely applied for two weeks improves range of motion in joints of the upper arm in these patients. Further study with long-term follow-up is needed to verify the role of passive exercise in preventing muscle spasticity in this population.

암컬 등척성 운동 시 상완이두근에서의 EMG 전극 위치에 따른 근 활성 검증 (Surface EMG Verification according to the Electrode Location in Biceps Brachii during Arm Curl Isometric Exercise)

  • Objective: The purpose of this study is to compare the muscle activity by electrode location in the biceps brachii during the arm curl isometric exercise and to provide the basic data needed to develop the proper electrode location of the biceps brachii based on the study results comparing the muscle activity by the angle of the elbow joint. Method: 17 adult males (Age: 21.50±4.63 yrs, height: 175.29±5.97 cm, weight: 63.79±15.31 kg, upper-arm length: 30.10±1.22 cm) participated in the study. In the arm curls isometric exercise, the experiment was divided into 1st and 2nd steps to compare muscle activity according to electrode location in the biceps brachii and muscle activity according to elbow angle change. In the first experiment, the surface electrode was attached at one-third point on the line from medial acromion to cubital fossa, according to the measurement method indicated by SENIAM. The elbow angle was set to 90°. In the second experiment, according to the proposed method of this study, the electrodes were separated at one finger's width in the left and right direction at one-third point on the line from medial acromion to cubital fossa, attached at the long head and short head. From the long head electrode, in about a width of two fingers in proximal direction, a total of three electrodes were attached at the myotendinal junction of the long head. The elbow angles were set as 70°, 90°, and 110°, and the isometric exercise (100% MVC) for 5 seconds was maintained with keeping the forearm and the rope to be 90° for the first and second experiments. Results: During the arm curl isometric exercise, there was no significant difference in SH and SENIAM proposition location proposed by this researcher. LH was shown to be lower than the muscle activity of the location proposed by SENIAM and there were significant (p<.01) differences. MJ appeared lower than the muscle activity of the location proposed by SENIAM and there were significant (p<.001) differences. The muscle activity by the elbow joint angle of SH in the biceps brachii was shown in large order of 70°<90°<110°, but there was no significant difference. The muscle activity by the elbow joint angle of LH was shown in large order of 90°<70°<110°, but there was no significant difference. The muscle activity by the elbow joint angle of MJ was shown in large order of 110°<90°<70°, but there was no significant difference. Conclusion: During the arm curl isometric exercise of the biceps brachii, it is judged appropriate to attach surface electrodes to the location proposed by SENIAM.

Effect of Resistance Exercise Intensity on the Strength and Lymphedema of Upper Extremity in Patients with Breast Cancer

  • Purpose: The purpose of this study was to determine the effects of high and low resistance exercise using an elastic band on the strength and lymphedema of upper extremity in patients with breast cancer. Methods: Seventeen female patients with breast cancer related lymphedema were randomly allocated to the high (n=9) or low (n=8) resistance exercise group. Both groups participated in the elastic band exercise program three times a week for eight weeks. For the high resistance exercise group (HR), the resistance was gradually increased, while maintaining constant intensity of exercise for the low resistance exercise group (LR). Assessments made include the upper extremity muscle strength and lymphedema before and after training. Results: After the exercise program, the HR showed significantly improved shoulder flexion and elbow flexion strength variation compared to the LR (p<0.05). Upper arm edema rate was significantly decreased in HR (p<0.05), but the difference between the two groups was not significant. Conclusion: These findings suggest that the elastic band exercise helps improve the strength and lymphedema of upper extremity in patients with breast cancer. In particular, high resistance exercise is more effective in improving muscle strength and does not exacerbate lymphedema, rather may improve upper arm edema if it is applied with a low elastic bandage or compression sleeve.

  • 본 논문에서는 단순하고 효율적인 학습제어 로직을 운동기구용 로봇팔 시스템에 적용하였다. 일반적으로 운동시에 운동기구에 적용되는 힘은 사용자마다 다르고 같은 사용자라 하더라도 운동시마다 다를 것이다. 이미 사용자의 신체조건에 따라 입력된 최적의 힘과 실제 가해지는 힘 사이의 오차를 보상하는데 본 논문에서는 학습제어를 적용하여 가해지는 힘의 종류(시변 또는 시불변함수)와 상관없이 빠르게 오차를 제거하고 원하는 운동을 추종함을 시뮬레이션을 통하여 확인할 수 있었다.

  • Purpose: The purpose of this study was to determine the effects of a low intensity exercise program with telephone coaching on the physical fitness and physiological index of elderly women who are receiving home visiting health program. Methods: This study was conducted using a non-equivalent control group pretest-posttest design. The participants were 53 senior citizens. The subjects (15) in experimental group 1 performed only a low-intensity exercise program and the subjects (16) in experimental group 2 performed a low intensity exercise program with telephone coaching for 16 weeks. The low intensity exercise program consisted of muscle strength band exercise, stretching, and hand exercise using balls. Results: There were statistical differences in the right grip strength (p=.030), left grip strength (p=.035), raising behind of right arm (p=.046), raising behind of left arm (p=.045), postprandial blood sugar (p=.021), body mass index (p=.029). Conclusion: The low intensity exercise program with telephone coaching is recommended as a physical fitness and physiological index for elderly women in a home visiting health program.

NREH: 다양한 운동과 데이터 수집이 가능한 가정용 상지재활로봇 (NREH: Upper Extremity Rehabilitation Robot for Various Exercises and Data Collection at Home)

  • In this paper, we introduce an upper extremity rehabilitation robot, NREH (NRC End-effector based Rehabilitation arm at Home). Through NREH, stroke survivors could continuously exercise their upper extremities at home. NREH allows a user to hold the handle of the end-effector of the robot arm. NREH is a end-effector-based robot that moves the arm on a two-dimensional plane, but the tilt angle can be adjusted to mimic a movement similar to that in a three-dimensional space. Depending on the tilting angle, it is possible to perform customized exercises that can adjust the difficulty for each user. The user can sit down facing the robot and perform exercises such as arm reaching. When the user sits 90 degrees sideways, the user can also exercise their arms on a plane parallel to the sagittal plane. NREH was designed to be as simple as possible considering its use at home. By applying error augmentation, the exercise effect can be increased, and assistance force or resistance force can be applied as needed. Using an encoder on two actuators and a force/torque sensor on the end-effector, NREH can continuously collect and analyze the user's movement data.
