Clinical Convergence Angle of Prepared Tooth for full Veneer Crowns (전부 피개관의 치아 형성 시 축면 경사각에 대한 조사)

  • The convergence angle of a prepared tooth is a very important factor in the retention and resistance of a crown restoration. But various intraoral environments and clinician's techniques make it difficult to obtain the ideal inclination. Therefore, in this study, clinical convergence angle of a prepared tooth was investigated. The data was collected from the patient models of prosthodontic residents and the patient models of general practitioners. The images of mesiodistal and buccolingual surfaces were taken with a digital camera to evaluate the convergence angle on 'ImageJ' program. The images were classified according to the criteria (1. Clinician group, 2. Position in the dental arch, 3. The purpose of abutment preparation)and then analyzed. The mean convergence angle of a prepared tooth for Korean clinicians was $15.02^{\circ}$ (${\pm}10.13^{\circ}$). 1. It was significant in the convergence angle between the general practitioner group and the prosthodontic resident group(p<0.05). 2. It was significant between the mesiodistal and buccolingual surface in the the prosthodontic resident group(p<0.05). 3. For the general practitioner group, it was significant when anteriors and premolars were compared with molars(p<0.05). For the prosthodontic resident group, it was significant when anteriors and premolars were compared with molars (p<0.05). 4. When divided into upper and lower arches, for the general practitioner group, it showed significant difference in the buccolingual aspect(p<0.05). Also in the prosthodontic resident group, it showed significant difference in the buccolingual aspect(p<0.05). 5. Dividing left and right sides of the arches, there was no significant difference in the general practitioner group and the prosthodontic resident group(p>0.05). 6. In the general practitioner group, it was significant in the mesiodistal axial convergence angle of single crown abutment and 3 unit bridge abutment(p<0.05). In the prosthodontic resident group, it was significant in the mesiodistal and overall axial convergence angle of single crown abutment and 3 unit bridge abutment(p<0.05). Clinical convergence angle of prepared tooth in Korea was included in agreement with other studies investigating convergence angle that ranged from 10 to 22 degrees, achieved in clinical practice.

TEMPOROSPATIAL PATTERNS OF PROGRAMMED CELL DEATH DURING EARLY DEVELOPMENT OF THE MOUSE EMBRYOS (생쥐 배자발생초기의 세포자기사 발현 양상에 관한 연구)

  • The pattern of programmed cell death(PCD) has been examined during the early developmental period of development in mouse embryos, from embryonic day 4.5(E4.5) to E11.5 Embryos from Balb/c breedings were harvested at various embryonic stages between E4.5 and El1.5. Cell death was analysed by in situ terminal deoxynucleotidyl transferase mediated dUTP nick end labeling(TUNEL) staining in tissue sections and whole embryos. At the blastocyst stage(E4.5), a very few apoptotic cells were found in the inner cell mass of the blastocyst. In the early egg cylinder stage(35.0-5.5), a few apoptotic cells were detected in the embryonic ectoderm, the embryonic endoerm and the proamniotic cavity. In the advanced egg cylinder stage(E5.5-6.5), TUNEL-posifive cells were observed in the extra-embryonic ectoderm and extra-embryonic endoderm as well as in the embryonic ectoderm, embryonic visceral endoderm and proamniotic cavity. In the streak stage(E6.75-7.75), many TUNEL-positive cells were found in the ectoplacental cone. In contrast, only very few apoptotic cells were found in the chorion and extra-embryonic endoderm in extra-embryonic regions. In intra-embryonic region, a few apoptotic cells were randomly found in the embryonic ectoderm, mesoderm and visceral endoderm. At the early somitogenesis stage(E8.0-8.5), most apoptotic cells were observed in the most cranial portion of neural fold (neural ectoderm and adjacent ectoderm). At the mid somitogenesis stage(39.0-9.5), the otic placode first showed TUNEL-positive at this stage. Small number of TUNEL-positive cells were also first seen around optic placode and branchial arches. Three streams of TUNEL-positive cells were clearly seen in the cranial region at 59.5-9.75. At E10.5, apoptotic cells were localized in the developing eye, the junctional portion of medial nasal, lateral nasal and maxillary processes, the lateral portion of branchial arches, the junction of bilateral mandibular processes, and apical ectodermal ridges of limb buds. At E11.5, apoptotic cells were noticeably decreased in most area, except the developing limbs and several somites in the tail region. In this study, the global temporospatial pattern of PCD throughout early development of mouse embryos was discussed. It may provide the basis for further studies on its role in the morphogenesis of the embryo.

  • It is saving material cost and construction cost by replacing conventional building materials, and It has advantages for aesthetic value. In the Europe, the United States, Japan and other country research about BIPV is actively being carried out and marketability is also being infinity expanding. Arch type PV systems efficiency characteristics is different depending on PV array's directly connection, parallel connection and arches angle, but is a lack of analysis on this nowadays. When the arch type PV system design up, they consider about aesthetic value and they didn't consider about generation efficiency. In this paper, we try to improve the efficiency through optimization of arch type PV system and estimation of the efficiency parameters of the arch type PV system, such as latitude, longitude, temperature, insolation, arch angle and each kind loss from system organization. For improving Arched PV system efficiency, proposed multiple control inverter system, and using simulation tool of Arched PV system "Solar pro", flat-plate type and many arch type PV system configuration the driving characteristics were compared and analyzed.

  • Background: The polycystic ovary syndrome (PCOS), characterized by hyperandrogenism and chronic anovulation, is a common endocrine disorder in women. PCOS, which is associated with polycystic ovaries, hirsutism, obesity and insulin resistance, is a leading cause of female infertility. In this condition there is an imbalance in female sex hormones. All the sequelae symptoms of PCOS gradually lead to cancer in the course of time. It is heterogeneous disorder of unknown etiology so it is essential to find the exact cause. Materials and Methods: In this study both invasive and non-invasive techniques were employed to establish the etiology. Diagnosis was based on Rotterdam criteria (hyperandrogenism, ovulatory dysfunction, PCOM) and multiparameters using buccal samples and dermatoglypic analysis and cytogenetic study for 10 cases and four age and sex matched controls. Results: In clinical analysis we have observed the mean value of total testosterone level was 23.6nmol/L, total hirsutism score was from 12-24, facial acne was found in in 70% patients with 7-12 subcapsular follicular cysts, each measuring 2-8 mm in diameter. In dermatoglypic analysis we observed increases in mean value ($45.9^{\circ}$) of ATD angle when compared with control group and also found increased frequency (38%) of Ulnar loops on both fingers (UU), (18%) whorls on the right finger and Ulnar loop on left finger (WU) and (16%) arches on right and left fingers (AA) were observed in PCOS patients when compared with control subjects. Features which could be applied as markers for PCOS patients are the presence of Ulnar loops in middle and little fingers of right and left hand. The buccal micronucleus cytome assay in exfoliated buccal cells, we found decrease in frequency of micronuclei and significant increases in frequency of karyolysed nuclei in polycystic ovarian syndrome patients. Chromosome aberration analysis revealed a significant increase in frequency of chromosome aberrations (CAs) in PCOS patients when compared with controls. Conclusions: From this present work it can be concluded that non-invasive technique like dermatoglypics analysis and buccal micronucleus cytome assays with exfoliated buccal cell can also be effective biomarkers for PCOS, along with increased CAs in lymphocytes as a sign of genetic instability. There is a hypothesis that micronuclei and chromosomal aberrations could have a predictive value for cancer. From this present work it can be concluded to some extent that non-invasive technique like dermatoglypics and buccal cell analysis can also be effective for diagnosis.

DENTAL CARIES TREATMENT FOR A PATIENT WITH ANGELMAN SYNDROME : A CASE REPORT (엔젤만 증후군(Angelman syndrome) 환자의 치아 우식 치료 : 증례 보고)

  • Angelman syndrome is a rare disorder caused by deletion or inactivation of genes on the maternally inherited chromosome 15. This neurodevelopmental disorder is characterized by developmental and intellectual delay, speech impairment, sleep disturbance, seizures, motor dysfunction, and frequent laughing or smiling. Orofacial characteristics include a prominent mandible, large mouth, prominent cheeks, a tendency to rest the tongue between the dental arches, excessive drooling, and excessive chewing behavior. Patients with this syndrome usually require general anesthesia even in a simple operation, because of risk of perioperative seizure during dental procedure. This is a case report about dental treatment of a 3-year-old female patient with Angelman syndrome under general anesthesia. This case suggests that the dental treatment under general anesthesia can be considered a safe component for the uncooperative, delayed developmental patients with underlying disease. Also, periodic dental exam appointment should be made to provide the patients with preventive treatments and to make them remain familiar with the dental environment.

Anterior Spacing and Crowding in the Primary Dentition in Hwaseong City : A Preliminary Study (화성시 거주 유치열기 어린이의 전치부 치간 공극과 총생에 대한 예비연구)

  • The aim of this study was to assess the spacing and crowding of the anterior teeth in the primary dentition in Hwaseong city. Photographs of the anterior segment of 237 children satisfied the criteria. The presence of primate spaces and developmental spaces was assessed by the proximal contacts. Physiologic spacing was observed in 47.3% and 38.0% of the cases in the upper and lower arches, respectively. 43.5% showed the presence of two-segment contact or crowded dentition. Physiologic spacing was observed more in boys than in girls. In the maxilla, primate space was more frequent than developmental space; however, in the mandible, the difference was low. In the maxilla, the space between the central incisor and the lateral incisor was more frequent than the space between both central incisors. In contrast, in the mandible, the space between both central incisors was more common than the space between the central and lateral incisors or between the lateral incisors and canine. The present study describes the tendency for anterior spacing and crowding in the primary dentition. Further longitudinal studies with a larger sample are needed. Dentists should consider these concepts of spacing or contact/crowding when performing full coronal restorations of primary anterior teeth.

Direct detection of cariogenic streptococci in metal brackets in vivo using polymerase chain reaction (교정용 메탈 브라켓에서 자가중합효소연쇄반응을 통한 치아우식증 원인균의 탈출)

  • Streptococcus mutans and Streptococcus sobrinus are major etiological agents in enamel demineralization around orthodontic appliances. This study was designed to examine the prevalence of these streptococci on orthodontic brackets in vivo using polymerase chain reaction. Four incisor brackets in the upper and lower arches were removed and collected from 80 patients at the time of debonding. The genomic DMA of adhered bacteria was extracted and each dextranase gene of S. mutans and S. sobrinus was amplified using the specific oligonucleotide primers. The results showed that the maxillary incisor brackets were colonized by both cariogenic streptococci to a somewhat higher degree than that taken from the mandible. The prevalence of S. mutans was $50.0\%$ on the maxillary incisor brackets and $33.8\%$ on the mandibular incisor brackets, and that of S. sobrinus was $17.5\%$ and $15.0\%$, respectively. Both species were detected on the maxillary incisor brackets of 7 patients $(8.8\%)$ and the mandibular incisor brackets of 5 patients $(6.3\%)$. These results suggest that cariogenic streptococci can adhere to the incisor brackets and may be resident species on the incisor brackets.

MANAGEMENT OF DIASTEMA AFTER REMOVAL OF MESIODENS (정중 과잉치 발거 이후 정중 이개의 폐쇄)

  • Early detection and prudent management of mesiodens or supernumerary tooth should be considered essential in reducing disturbance in the eruption and position of the adjacent permanent incisor. While it is true that the presence of diastema may be regarded as normal at the early mixed dentition stage, the early detection and removal of the mesiodens is a prerequisite to facilitate spontaneous alignment or subsequent approximation of the permanent central incisors. In many cases, diastema due to mesiodens can be physiologically corrected spontaneously after the extraction of mesiodens. The best choice of treatment of diastema may be observation. Orthodontic intervention is required only spontaneous closing of diastema does not occur within observation period. In orthodontic intervention, careful treatment plan should be established. Clinician gives considerations to angulation of central and lateral incisor, proximity of lateral incisor, developmental stage and position of canine, pattern and extent of anterior crowding. Orthodontic movement should be done slowly with light force. In addition, periodic radiographic observation are needed to monitor the root development and root resorption. Case 1, 2 and 3 showed physiologic closures after the extraction of mesiodens. In these cases, acceptable alignment of central and lateral incisors was obtained. In case 4, orthodontic correction for diastema was performed successfully after the extraction of mesiodens. After the orthodontic closure of the diastema, it was decided that a retainer was not needed, because the dentition was under a dynamic stage in exchanging teeth and also developing arches.

A STUDY ON THE NORMAL DENTAL ARCH FORM OF KOREAN ADULT (한국인 성인의 정상 치열궁 형태에 관한 연구)

  • Many geometric curves are presented as representative form of normal dental arches by many authors; circle, ellipse, parabola or catenary curve. Among them those except circle seems difficult to be adopted as a guide in ideal arch form construction and practically many orthodontists chose circle as a standard. Author preferred circle of Bonwill's theory in study of anterior teeth alignment of Korean adults. Eighty three dental models which possess proper occlusion and good arch form were selected and copies of their occlusal surfaces obtained by Ricopy machine. The use of Ricopy machine made it possible to draw arch form exactly. Mesiodistal widths of six anterior teeth were measured and they were added to combined mesiodistal width of six anterior teeth. Circle, that include the points of two cuspal tips of canines and one incisal edge of central incisor were drawn. Distances of lateral incisors that are deviated from arc of this circle were measured and classified into four grades by degree and three groups by kind of teeth deviated. By counting the number of samples involved degree of fit of the circle to arch contour of Korean adult was described. Then, size of radius of circle, intercanine width and intermolar width were measured and evaluated their ratios to combined mesiodistal width of six anterior teeth. In normal occlusion of Korean adult anterior teeth seems to be arranged on an arc of circle the radius of which is similar to combined mesiodistal width of six anterior teeth. Intercanine width and intermolar width have rather constant ratios to combined width of six anterior teeth.

Malunion of the Jaw Fractures Complicated Following the Primary Managements (악골절 치료후 부정유합에 관한 임상적 연구)

  • PURPOSE : This is to review the complicated jaw fractures that had been referred for revision of the unsatisfactory results, and to provide proper managements for the easily complicated jaw fractures. MATERIALS & METHODS : Twenty-nine patients who had been revised due to malunion or complicated fractures of facial bones for last 3 years were reviewed. The main problems required for revision, type of fractures complicated, the primary managements to be reclaimed, the specialties to be involved, the management to be reclaimed, time elapsed to seek reoperation, type of revision surgeries, residual complication were analysed with medical records, radiographs and final examinations. RESULTS: The major complaints were malocclusion(79.3%), facial disfigurement(41.3%), TMJ problems (13.7%), neurologic problems(10.3%), non-union(10.3%), and infection(6.8%). Unsatisfactory results were occurred most frequently after improper management of the multiple fractures of the mandible (62.2%), combined fractures of maxilla and mandible (20.6%), fracture of zygomatico-maxillary complex and midpalate (17.2%). The complications to be corrected were widened or collapsed dental arches (79.3%), improperly reduced condyles (41.3%), painful TMJ (34.4%), limited jaw excursion (31.0%), over-reduction of zygoma (13.7%), and nonunion with infection(13.7%). and dysesthesia (10.3%). The primary managements were nendereet by plastic surgeons in 82.7%(24/29) and by oral surgeons in 7.6%(2/29). Main causes of malunion are inadequate ORIF in 76%, unawareness & delay in 17%, and delayed due to systemic cares in 17%. 76% of 29 patients had been in state of intermaxillary fixation for over 4 weeks. Revision were done by means of "refracture and ORIF"in 48.2%(14/29), orthognathic osteotomies with bone grafts in 55.1%(16/29), and camouflage countering & alloplastic implantations in 37.9%(11/29), TMJ surgeries in 17.2%, micro-neurosurgeries in 11.6%. Residual complications were limited mouth opening in 24.1% (7/29), paresthesia in 13.7%, resorption of reduced condyle in 10.3%. CONCLUSIONS : Failure of initial treatment of jaw fractures is due to improper diagnosis and inadequate treatment with lack of sufficient knowledge of stomatognathic system. It is crucial to judge jaw fracture and patients accurately, moreover, the best way of treatments has to be selected. Consideration of these factors in treatment could minimize the complication of jaw fractures.
