A case of Hallermann-Streiff syndrome with aphakia (무수정체안을 동반한 Hallermann-Streiff 증후군 1예)

  • Hallermann-Streiff syndrome is a rare disease. Approximately 150 cases have been reported, including 6 cases in Korea. The authors experienced a case of Hallermann-Streiff syndrome in a 6-year-old female with aphakia. The syndrome is characterized by a bird-like face, dental abnormalities, hypotrichosis, atrophy of the skin, bilateral microphthalmia, and proportionate dwarfism. A brief review of the literature was conducted.

The correction principle of Aphakia using the Intraocular Lens (안내렌즈를 이용안 무수정체안의 교정원리)

  • In this paper, we studied the principle that correct the emmetropia to the aphakia using intraocular lens. At present, in an ophthalmic clinic, I.O.L which is using for correction of the vision clinically has presented how to prescribe more accurately by using optical method. To correct the Aphakia for the emmetropia, we considered that the basic cause of Ametropia was the refraction and the axial length. The correction principle is made equal to the Back Focal Length(BFL) and the vitreous length from lens to retina. For the confirmation of the correction principle, we used the Gullstrand number I eye model in emmetropia. For the myopia and hyperopia, we used the clinical data and replaced crystalline lens with intraocular lens.

The Study on Accommodation for eyes with pseudo-crystalline lenses (인공수정체안의 조절반응에 관한 연구)

  • Physiological accommodation in aphakia has been reported 0.50D. Also, accommodation appears in pseudo-crystalline lens. However accommodation was not clear how to works in the eyes. Therefore, the purpose of this study is that accommodation has been affected to changes of axial length and corneal refractive power when the accommodative stimulus was taken 10 diopters to person who has pseudo-crystalline lens. To determine of the accommodative response of pseudo-phakic patients who had no side effects over 2 months, we have measured the axial length, corneal refractive power in 48 patients(96 eyes). When the accommodative stimulus was taken about 10 diopters, the axial length increased by $0.05{\pm}0.06mm$ and corneal refractive power increased $0.04{\pm}0.24$ diopter. The changes of axial length in the myopia was increased by $0.19{\pm}0.18D$, emmetropia was increased by $0.12{\pm}0.16D$ and hyperopia was $0.10{\pm}0.19D$. The results of measurement revealed that the mean value was increased $0.03{\pm}0.13D$ in the myopia, and increased $0.21{\pm}0.13D$ in the emmetropia, and decreased $0.02{\pm}0.34D$ in the hyperopia.

Trabeculectomy Using Mitomycin C in Aphakic and Pseudophakic Eyes (무수정체안과 인공수정체안에 대한 Mitomycin C를 이용한 일차 섬유주절제술의 효과)

  • Background : The safety and efficacy of trabeculectomy with Mitomycin C (MMC) for surgical treatment in aphakic and pseudophaic eyes were retrospectively evaluated. Materials and Methods : The authors reviewed 51 eyes of 45 patients who had been followed up for at least 6 months after trabeculectomy using MMC for aphakic and pseudophakic eyes. The success rate and complications were analyzed. The success criteria included intraocular pressures of 21 mmHg or less with or without glaucoma medications and no loss of light perception. Surgical failure was defined as a postoperative loss of light perception in patients with preoperative vision better than light perception, additional glaucoma surgery, or phthisis bulbi in patients with preoperative vision of no light perception. Results : The average follow up period was 27.7 months and the intraocular pressure was controlled under 21 mmHg in 36 eyes of 51 (70.6%) after the procedure with or without medication for glaucoma. Using the Kaplan-Meier survival analysis, the cumulative success rate at the 3-, 6-, 12-, 24- and 36-month intervals were 98.0%, 94.1%, 91.9%, 83.4% and 75.5%, respectively. The complications observed were hyphema (4 eyes), serous choroidal detachment (4 eyes), hypotony (3 eyes), and endophthalmitis (1 eye). Conclusion : Trabeculectomy using Mitomycin C for the treatment of aphakic and pseudophaic eyes was safe and effective.
