배경: 대동맥궁 치환 수술 시 뇌 보호법은 필수불가결하며, 다양한 방법이 시도되고 있다. 이중 순행성 뇌관류법의 이용은 완전 순환 정지시간을 최소화하고 뇌관류를 가능하게 하여 중추신경계 합병증을 줄일 수 있는 방법 중의 하나이다. 본 연구의 목적은 우액와동맥을 통한 순행성 뇌관류를 이용하여 대동맥궁 치환술을 시행한 환자들의 임상 결과를 조사하는 데 있다. 대상 및 방법: 2004년 7월부터 2006년 7월까지 대동맥궁 치환 시 대동맥궁 분지의 전부 혹은 일부를 치환 후 우액와동맥을 통한 순행성 뇌관류를 시행하면서 대동맥궁 치환을 한 11명의 환자를 대상으로 하였다 남자 환자는 9명, 여자 환자는 2명이었다. 수술 당시의 진단은 A형 대동맥 박리(5/11, 45.5%), A형 벽내혈종(1/11, 9%), 대동맥류(5/11, 45.5%)였고, 평균 나이는 $60.3{\pm}12.8$세였다. 우액와동맥으로 삽관하여 심폐기 가동 후 초저체온 상태(평균 최저 비인두 체온 $11.7{\pm}1.0^{\circ}C$)에서 완전순환정지를 시행하였고, 이 기간 중 일부에서 역행성 뇌관류법을 시행하였다. 결과: 완전 순환 정지 시간은 $31.1{\pm}16.9$분이었으며 역행성 뇌관류 시간은 $21{\pm}17.8$분이었다. 뇌관류를 위한 각각의 도관과 대동맥궁 분지를 연결하는 데 걸린 평균시간은 $24.1{\pm}9.3$분이었다. 술 후 사망 환자는(0/11, 0%) 없었고, 영구적 신경학적 합병증 또한 없었다. 결론: 더 많은 수의 환자와 장기적인 성적에 대한 연구가 필요하지만 우액와동맥을 통한 순행성 뇌관류를 이용한 대동맥궁 치환술은 대동맥궁 조작을 최소화하고 색전 및 전반적인 허혈에 의한 뇌합병증을 줄일 수 있는 방법으로 생각된다.
토끼는 심폐바이패스(CPB) 실험동물로서 많은 장점을 가지고 있음에도 불구하고 토끼에서 CPB운용법의 확립은 기술적으로 대단히 어려운 것으로 알려져 있다. 한편 저체온 순환정지법은 심장수술에서 유용하게 사용되고 있으나 뇌 보호상의 문제점이 지적되고 있다. 스테로이드는 일반적으로 뇌부종 치료에 효과가 있는 것으로 알려져 있으나 순환정지시 뇌보호에 미치는 영향에 관해서는 아직 명확하게 규명되지 못하고 있다. 이런 관점에서 본 연구는 첫째 토끼에서 CPB운용법을 확립하고 둘째 이를 바탕으로 순환정지시 스테로이드에 의한 뇌보호 효과를 분석할 목적으로 시행하였다. 흰 토끼 15마리(평균 체중 3,5kg)를 3군의 실험군에 각각 5마리씩 사용하였다. 제 1실험군(대조군)은 순환 정지시 토끼를 수술대와 평행된 자세로 유지하였으며, 제 2실험군에서는 대조군과 다른 실험방법은 동일하나 순환정지시 토끼를 트렌델렌부르그 자세로 유지하였다. 제 3실험군에서는 트렌델렌부르그 자세와 함께 순환정지전 스테로이드(methylprednisolone 30 mg/kg)를 투여하였다. 실험방법은 토끼를 마취시킨후 정중흉골절 개술로 심장을 노출시키고 상행대동맥 및 \ulcorner심방부속지에 각각 3.3mm 동맥캐늘라 및 14 Fr 단일 정맥캐늘라를 삽관하였다. CPB 회로에는 롤러 펌프와 기포형 산화기를 사용하였다. 충전액은 토끼혈액 120-150cc를 포함하여 약 450cc를 사용하였다. 전체 실험시간은 70분으로 심폐바이패스 시작후 10일 동안 관류 및 표면냉각법으로 체온을 20도(직장)까지 감소시킨뒤 40분 동안 순환정지를 시켰다. 순환정지후 관류를 재개하여 20분 동안 재가온으로 체온을 정상화시키면서 심장 박동이 되돌아오는 것을 확인하였다. 관류 유속은 80~ 90mg/min 으로 시작하였고 체온 하강에 따라 유속을 조절하였다. 실험후 토끼를 희생시킨뒤 바로 부검을 시행하여 뇌, 척수, 신장, 십이지장, 폐, 심장, 간장, 비장, 췌장, 위장의 일부를 채취하여 수분함유량을 조사하였다. 각 실험군간의 수분양 비교는 Kruskal-Wallis 비모수 검정법에 의해 분석하였다. CPB 중 관류 유속 변화는 60~l00ml/kg/min 이었다. 동맥압은 대부분 35-55mmhg 사이에서 유지되었다. 재가온후 심장은 전례에서 박동을 재개하였다. 동맥혈가스분석 결과 심한 조직 허혈을 의미하는 정도의 대사 성 산증은 발견되지 않았다. 각 실험군별 조직수분양 측정 결\ulcorner 뇌를 포함한 각 장기들에서 실험군간체 유의 한 차이는 발견되지 않았다. 이러한 실험결과를 통하여 저자들은 (1) 적절한 기법하에서는 토끼에서도 정상 적인 심폐바이패스 운용이 가능하다는 것과, (2) 본 실험 범주에서는 저체온 순환정지시 트렌델렌부르그 자 세에 의한 뇌부종 발현에 대한 스테로이드의 예방효과를 검정할 수없다는 결론을 얻었다.
Purpose: Children born with single ventricle physiology demonstrate poor growth rate and suffer from malnutrition, which lead to increased morbidity and mortality in this population. We assume that an anabolic steroid, oxandrolone, will promote growth in these infants by improving myocardial energy utilization. The purpose of this paper is to study the efficacy of oxandrolone on myocardial energy consumption in these infants. Materials and Methods: We modeled single ventricle physiology in a lamb by prenatally shunting the aorta to the pulmonary artery and then postnatally, we monitored cardiac energy utilization by quantitatively measuring the first order reaction rate constant, $k_f$ of the creatine-kinase reaction in the heart using magnetization transfer $^{31}P$ magnetic resonance spectroscopy, home built $^1H/^{31}P$ transmit/receive double tuned coil, and transmit/receive switch. We also performed cine MRI to study the structure and dynamic function of the myocardium and the left ventricular chamber. The spectroscopy data were processed using home-developed python software, while cine data were analyzed using Argus software. Results: We quantitatively measured both the first order reaction rate constant and ejection fraction in the control, shunted, and the oxandrolone-treated lambs. Both $k_f$ and ejection fraction were found to be more significantly reduced in the shunted lambs compared to the control lambs, and they are increased in oxandrolone-treated lambs. Conclusion: Some improvement was observed in both the first order reaction rate constant and ejection fraction for the lamb treated with oxandrolone in our preliminary study.

The ventricular assist device(VAD) helps to reduce the overload against the patient's native heart(NH). The pulsatile VAD pumps out the ventricular blood to the aorta with pulsatile flow. If the VAD pulsates simultaneously with the NH, the ventricle of the NH could confronts abnormally elevated aortic pressure, and this could deteriorate the ventricle rather than assist to recover it. Thus counterpulsation algorithms to avoid copulsation have been adopted by many VADs, but these methods utilize electrocardiography or arterial pressure signals, which may have difficulties to acquire consistently for a long period. In this study, the copulsation estimation algorithm for the counterpulsation is developed using the VAD outlet pressure signal. The VAD outlet pressure signal is good to maintain for a long time and the sensor part could be integrated to the VAD as a built-in module. From the VAD outlet pressure signal and its pump rate information calculated with Fast Fourier Transform, pulse peaks by the VAD and the NH were extracted and the next copulsation time at which the VAD and the NH would pulsate simultaneously was estimated. This estimation algorithm was implemented by using PC MATLAB software and tested for various pump rate conditions with mock circulation system. For each condition, the copulsation time was estimated successfully. Consequently, the results showed the possibility to use the outlet cannula pressure signal in the copulsation estimation.
저자들은 발열과 끙끙거림을 주소로 내원한 2개월 된 소아에서 빈도가 드문 선천성 식도 중복의 감염, 파열로 인하여 농흉, 유미흉과 종격동염을 초래하고 이로 인하여 대동맥류를 초래한 1례를 경험하였기에 보고하고자 한다. 본 증례의 희귀함과 종격동염, 농흉, 대동맥류와 같은 다양한 합병증의 발생은 식도 중복을 진단하는데 있어서 어려운 점이었다.
NO and cyclooxygenase-2 (COX-2) are contributes to vascular inflammation induced by various stimulation. The mechanism, which explains a linkage between NO and COX-2, could be of importance in promoting pathophysiological conditions of vessel. We investigated the effects of NO donors on the COX-l and COX-2 mRNA/protein expression, as well as the nitrite production in culture medium of vascular smooth muscle cell (VSMC). VSMC was primarily cultured from thoracic aorta of rat. In this experiments, COX-l and COX-2 mRNA/protein expressions were analysed and nitrite productions were investigated using Griess reagent. VSMC did not express COX-2 protein in basal condition (Nonlipopolysaccharide (LPS) stimulated). In LPS-stimulated experiments, after 3 hours of NO donor pretreatment, LPS $10{\mu}g/ml$ was treated for 24 hours. COX-l protein expressions were unchanged by SNP and NOR-3. NOR-3 significantly increased COX-2 mRNA/protein expression under LPS stimulation. In contrast, SNP did not increase COX-2 mRNA/protein expression under LPS stimulation. Nitrite production was higher in NOR-3 treatment than SNP treatment under LPS stimulation. These results suggest that the expression of COX-2 in VSMC is regulated by NOR-3, COX-2 expressions were depending on the types of NO donor and LPS stimulation in VSMC.

Objective : The main causes of spinal cord ischemia are a variety of vascular pathologies causing acute arterial occlusions. We investigated neuro-protective effects of kefir on spinal cord ischemia injury in rats. Methods : Rats were divided into three groups : 1) sham operated control rats; 2) spinal cord ischemia group fed on a standard diet without kefir pretreatment; and 3) spinal cord ischemia group fed on a standard diet plus kefir. Spinal cord ischemia was performed by the infrarenal aorta cross-clamping model. The spinal cord was removed after the procedure. The biochemical and histopathological changes were observed within the samples. Functional assessment was performed for neurological deficit scores. Results : The kefir group was compared with the ischemia group, a significant decrease in malondialdehyde levels was observed (p<0.05). Catalase and superoxide dismutase levels of the kefir group were significantly higher than ischemia group (p<0.05). In histopathological samples, the kefir group is compared with ischemia group, there was a significant decrease in numbers of dead and degenerated neurons (p<0.05). In immunohistochemical staining, hipoxia-inducible factor-$1{\alpha}$ and caspase 3 immunopositive neurons were significantly decreased in kefir group compared with ischemia group (p<0.05). The neurological deficit scores of kefir group were significantly higher than ischemia group at 24 h (p<0.05). Conclusion : Our study revealed that kefir pretreatment in spinal cord ischemia/reperfusion reduced oxidative stress and neuronal degeneration as a neuroprotective agent. Ultrastructural studies are required in order for kefir to be developed as a promising therapeutic agent to be utilized for human spinal cord ischemia in the future.

Connective tissue growth factor (CTGF) is a potent pro-fibrotic factor, which is implicated in fibrosis through extracellular matrix (ECM) induction in diabetic cardiovascular complications. It is an important downstream mediator in the fibrotic action of transforming growth factor ${\beta}$ ($TGF{\beta}$) and is potentially induced by hyperglycemia in human vascular smooth muscle cells (VSMCs). Therefore, the goal of this study is to identify the signaling pathways of CTGF effects on ECM accumulation and cell proliferation in VSMCs under hyperglycemia. We found that high glucose stimulated the levels of CTGF mRNA and protein and followed by VSMC proliferation and ECM components accumulation such as collagen type 1, collagen type 3 and fibronectin. By depleting endogenous CTGF we showed that CTGF is indispensable for the cell proliferation and ECM components accumulation in high glucose-stimulated VSMCs. In addition, pretreatment with the MEK1/2 specific inhibitors, PD98059 or U0126 potently inhibited the CTGF production and ECM components accumulation in high glucose-stimulated VSMCs. Furthermore, knockdown with ERK1/2 MAPK siRNA resulted in significantly down regulated of CTGF production, ECM components accumulation and cell proliferation in high glucose-stimulated VSMCs. Finally, ERK1/2 signaling regulated Egr-1 protein expression and treatment with recombinant CTGF reversed the Egr-1 expression in high glucose-induced VSMCs. It is conceivable that ERK1/2 MAPK signaling pathway plays an important role in regulating CTGF expression and suggests that blockade of CTGF through ERK1/2 MAPK signaling may be beneficial for therapeutic target of diabetic cardiovascular complication such as atherosclerosis.

Objective: To establish diagnostic criteria for pulmonary arterial hypertension (PAH) in children by using parameters obtained through noninvasive cardiac computed tomography (CCT). Materials and Methods: We retrospectively measured parameters from CCT images of children from a single institution in a multiple stepwise process. A total of 208 children with mean age of 10.5 years (range: 4 days-18.9 years) were assessed. The variables were classified into three groups: the great arteries; the ventricular walls; and the bilateral ventricular cavities. The relationship between the parameters obtained from the CCT images and mean pulmonary arterial pressure (mPAP) was tested and adjusted by the children's body size. Reference curves for the pulmonary trunk diameter (PTD) and ratio of diameter of pulmonary trunk to ascending aorta (rPTAo) of children with CCT images of normal hearts, adjusted for height, were plotted. Threshold lines were established on the reference curves. Results: PTD and rPTAo on the CCT images were significantly positively correlated with mPAP (r > 0.85, p < 0.01). Height was the body size parameter most correlated with PTD (r = 0.91, p < 0.01) and rPTAo (r = -0.69, p < 0.01). On the basis of the threshold lines on the reference curves, PTD and rPTAo both showed 88.9% sensitivity for PAH diagnosis, with negative predictive values of 93.3% and 92.9%, respectively. Conclusion: PTD and rPTAo measured from CCT images were significantly correlated with mPAP in children. Reference curves and the formula of PTD and rPTAo adjusted for height could be practical for diagnosing PAH in children.
배경: 하행 흉부대동맥류를 포함한 대동맥 질환을 가진 환자들에서 히생한 대동맥내 스텐트-그라프트 삽입술의 치료효과와 추적성적 및, 대상 환자들의 임상적 특징을 통해 치료방법으로서의 적응과 역할을 알아보았다. 대상 및 방법: 1995년 이후 모두 8명의 환자에서 진단방사선과와 협진으로 스텐트-그라프트 삽입술을 시행하였으며, 시술 전 진단으로는 외상성 대동맥 파열이 3례, 동맥경화증성 대동맥류 3례, 흉부가성대동맥류 파열 1례, 매독성 대동맥염에 의한 흉복무대동맥류가 1례였다. 시술은 혈관조영실에서 혈관조영을 하면서 시행하였고, 전신마취가 3례, 국소마취가 3례였고, 2례에서 경막외 마취하에 시술하였으며, 사용한 스텐트-그라프트는 0.35mm 굵기의 스테인레스 강철사를 지그재그 형태로 구부려 만든 스텐트를 서로 연결하고 인조혈과(Dacton)을 입힌 후 봉합한 것으로 제조회사에 의뢰하여 자체제작 하였다. 결과: 시술받은 환자의 전례에서 스텐트-그라프트 삽입시 기술적 어려움은 없었으며, 스텐트-그라프트 삽입후 4일에서 7일 사이에 시행했던 추적검사(4례)에서 스텐트-그라프트 주위부 누출이나 당초 목표했던 위이에서의 이탈은 없었다. 1례의 외사성 대동맥 파열이 었었던 환자가 시술 후 13일째 급성호흡곤란증후군 및 다장기부전으로 사망하였고, 매독성 대동맥염이 있었던 흉복부 대동맥류 환자가 시술 후 6일째 상행대동맥 및 대동맥궁에 대한 수술을 시행받고 술후 22일째 문합부위 파열로 사망하였다. 결론: 대동맥내 스텐트-그라프트를 이용한 흉복부 대동맥 질환의 치료는 수술에 따른 이환률 및 사망률을 고려할 때 특히 다장기부전으로 수술이 어려운 경우 또는 수술 후 합병증의 발생이 강력히 우려되는 경우에 안전하게 시행할 수 있는 치료의 한 방편으로서 의미를 가진다고 사료된다.행성 이하선염과 파라인플루엔자 바이러스는 외피가 있는 170-180nm 이었다.TEX>$\pm$0.10(100분), 0.46$\pm$0.11(120분), 0.52$\pm$0.15(심폐기 재가동 30분), 0.62$\pm$0.15(60분), 0.76$\pm$0.17(심폐기이탈 30분), 0.81$\pm$0.20(60분), 0.84$\pm$0.23(90분) and 0.94$\pm$0.33(120분)를 보였고 이는 역행성 뇌관류 전에 비해 유의하게 증가된 소견이었다(p<0.05). 뇌신피질, 기저핵, 해마에서 전자현미경 조직 소견을 관찰하였으며 마이토콘드리아의 부종을 관찰할 수 있었다. 결론 : 역행성 뇌관류 120분 후에 S100 베타 단백의 유의한 증가를 관찰할 수 있었으며 뇌조직 손상과의 관련성은 좀 더 연구되어야 할 부분으로 생각된다. 장기 생존 모델을 통한 재평가가 필요하다고 사료되며 심폐바이패스 시행 등의 교란 인자도 고려해야 할 것이다.비해 Progenitor II의 형질전환 효율이 현저히 떨어졌다. L. acidophilus HY7008과 HY7001은 두 기기 모두 형질전환이 이루어졌으나, L. acidophilus WEISBY와 NCFM은 Progeni-tor II에서 전이가 일어나지 않았으며, Gene Pulser에서 전이균주를 얻어 두 electroporator간에 형질전환 효율의 차이를 보였다. 11. 제 4 장 서비스의 이용
제 12 조 (서비스 이용 시간)
① 서비스 이용은 당 사이트의 업무상 또는 기술상 특별한 지장이 없는 한 연중무휴, 1일 24시간 운영을
원칙으로 합니다. 단, 당 사이트는 시스템 정기점검, 증설 및 교체를 위해 당 사이트가 정한 날이나 시간에
서비스를 일시 중단할 수 있으며, 예정되어 있는 작업으로 인한 서비스 일시중단은 당 사이트 홈페이지를
통해 사전에 공지합니다.
② 당 사이트는 서비스를 특정범위로 분할하여 각 범위별로 이용가능시간을 별도로 지정할 수 있습니다. 다만
이 경우 그 내용을 공지합니다.
제 13 조 (홈페이지 저작권)
① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
