Surgical Correction of Complete Vascular Ring Associated with Kommerell's Diverticulum (Kommerell 게실과 동반된 완전 혈관륜의 수술적 교정)

  • An 11-month old girl with a feeding difficulty and recurrent aspiration pneumonia received surgical correction of complete vascular ring, which was formed by right aortic arch, aberrant left subclavian artery(LSCA) originating from Kommerell's diverticulum(KD) and ligamentum arteriosum. Through left posterolateral thoracotomy, the ligamentum arteriosum was divided to relieve the tracheo-esophageal bundle. KD was separated from the right descending aorta, and the left subclavian artery was severed from KD at its origin and trasfered to the side wall of left common carotid artery. Postoperative course was uneventful, and the patient has been followed up with a clinical improvement.

Effects of Lipopolysaccharide-Induced Heme Oxygenase and Carbon Monoxide Production on the Aortic Contractility (Lipopolysaccharide에 의한 Heme Oxygenase Induction과 Carbon Monoxide생성이 혈관수축력에 미치는 영향)

  • Heme oxygenase is a rate-limiting enzyme in heme catabolism that cleaves heme to form biliverdin, iron, and carbon monoxide. Heme oxygenase-1 is expressed in many types of cells and tissues and is highly induced in response to oxidative stress. Carbon monoxide, one of the products of heme oxygenase, can stimulate soluble guanylate cyclase and dilate the vascular smooth muscle. So, the induction of heme oxygenase by lipopolysaccharide (LPS)-induced oxydative stress and the effect of the resultant carbon monoxide on aortic contractility were examined in this study. Zinc protoporphyrine IX (ZnPP), a inhibitor of heme oxygenase, elicited weak contraction of thoracic aortic ring, and this effect was more potent in aorta of LPS-treated rats than control and was blocked by methylene blue. The hyperreactivity to ZnPP in LPS-treated group was blocked by co-treatment with aminoguanidine. In the aortic ring of LPS-treated rats, ZnPP didn't change the vasoreactivity to phenylephrine or acetylcholine. ZnPP elicited hyper-tensive effect in concious rats, and pretreatment with LPS did not affect this effect. Prazosin significantly diminished the hypertensive effect of ZnPP. These results indicate that LPS induced heme oxygenase in aotra, and the resultant carbon monoxide diminished the aortic reactivity to vasoconstrictor.

Changes of Vascular Contraction and Relaxation of Rat aorta under Arsenic Stress (비소 스트레스에 의한 흰쥐 대동맥의 수축과 이완반응의 변화양상)

  • In order to examine whether arsenic, one of environmental stress, contribute to augumentation and relaxation of rat aorta, this study was performed in vivo and in vitro, using intacted or denuded rats aorta ring preparation, respectively. The carotid arterial pressure was recorded on an ink-writing physiograph(Grass Co. 79E) connected to strain gauge. The contractile response of vascular ring with or without endothelium preparation isolated from rat were determined in organ bath and was recorded on physiograph connected to isometric transducer. Vasopressin-,and phenylephrine- induced increase in arterial pressure significantly enhanced in arsenic-treated rats; increase of 19.1%, and 46.6%, respectively. Vascular contractile response was measured in vitro preparations exposed to 0, 0.5, 1, 2 and 4 mM of arsenic for 1, 3, 5 and 8 hours. The dose-vascular responses of phenylephrine augmented by increasing dose of arsenic in the strips exposed to arsenic for 8 hours, and did not augmented for 1, 3, 5 hours. The phenomenon was not affected by strips denuded endothelium. And the response of relaxation of rat aorta induced by nitroprusside was not influenced by arsenic stress, but acetylcholine was a little increased. compared to that of control. There were no significant difference in relaxation between control and arsenic treated rings with endothelium or denuded. All of the results, phenyleprine-induced vascular contraction was significantly enhanced in 4 mM arsenic-treated rat aortic rings compared with control, whether endothelium was present or denuded at 8 hours after arsenic treatment. It may be a mechanism by which long-term arsenic stresses play a role in development of hypertension.

Anomalous Systemic Arterial Supply to the Lung without Sequestration A Cases Report (폐분획없는 폐동맥의 하행대동맥 기시이상 1례 보고)

  • The anomalous systemic arterial supply to the lung without sequestration is an uncommon congenital anomaly characterized by the presence of a part of lung tissue which supplied by an aberrant artery from the aorta or its branch and normal communication with the normal bronchial tree, and it was reported by Huber in 1777 first. It differs from bronchopulmonary sequestration in having normal bronchial communication from the remainder lung and normal lung histology. We experienced a case of anomalous systemic arterial supply to the lung without sequestration, which was confirmed preoperatively. The patient was 16 year old male and came to us with complaints of mild fever and profuse sputum with coughing. Chest film showed a ring-like soft tissue shadow in Rt. middle lung field. Aortogram revealed an aberrant artery originated from abdominal aorta at 12th thoracic vertebral level and supplying a part of Rt. lower lobe of lung. At. the time of operation, an aberrant systemic artery which originated from the abdominal aorta supplying the Rt. lower lobe was noted, and the bronchial communications were normal. After division and ligation of the aberrant artery, Rt. lower lobectomy was performed The postoperative courses were uneventful and the patient was discharged with good condition.

  • Supravalvular aortic stenosis may be defined as an obstructive congenital deformity of the ascending aorta which originates just distal to the level of the origins of the coronary arteries It may be localized or diffuse. Enlargement of the aorta with a diamond-shaped patch of the noncoronary sinus of Valsalva was reported in 1961 by McGoon and associates But this reconstruction is asymmetric and the aortic obstruction may remain. In 1977, Dotty and associates reported the extended aortoplasty, the supravalvular ring was incised at two points in the noncoronary and in the right coronary sinuses of Valsalva closed with a tubular Dacron prosthesis of inverted Y-shape tailored to reconstruct the aorta We experienced three cases of the supravalvular aortic stenosis. The 11-year-old female and 4-year-old male with localized supravalvular aortic stenosis in William`s syndrome were operated with an inverted Y-shaped aortotomy toward the non-coronary sinus and the right coronary sinus and closed with "Hemashield`s collagen impregnated Dacron" tube graft, fashioned into "pantaloon" form patch. The 12-year-old male with localized supravalvular aortic stenosis and mitral insufficiency in William`s syndrome were operated with same procedure as two other patient above-mentioned for relief of supravalvular aortic stenosis and with mitral valve replacement. Postoperative course has been good.ourse has been good.

Surgical correction of congenital aortic stenosis - Report of 14 cases - (선천성 대동맥협착증 수술치험 14례 보)

  • Over the past 6 years, from July, 1981. through June, 1987., 14 consecutive patient with congenital aortic stenosis underwent corrective surgery in our department of Thoracic and Cardiovascular Surgery. The patient ranged in age from 1 to 20 years. There were 8 male and 6 female patients. According to the operative findings, stenotic site was valvular stenosis [5 cases], subvalvular stenosis [5 cases], supravalvular stenosis [2 cases], valvular and supra valvular stenosis [1 case]. We have performed valvotomy and commissurotomy [5 cases]. Resection of subvalvular membrane [3 cases], patch enlargement of Ascending aorta [2 cases], LV myotomy [2 cases], valvotomy and excision of membrane [1 case], patch enlargement of ascending aorta and valve ring [1 case]. There was one hospital mortality [7.1%]. He died of C-I bleeding and sepsis on the 25th postoperative day. All survivors showed improvement in NYHA functional class in the 160 patient/month follow up period.

  • A 73-year-old woman who underwent combined bioprosthetic mitral valve replacement, tricuspid ring annuloplasty, and coronary artery bypass grafting 12 years previously visited our clinic due to aggravated dyspnea caused by structural valve deterioration of the mitral prosthesis. Because aortic or femoral artery cannulation and cross-clamping would have a high risk of stroke owing to severe calcification of the ascending aorta and ilio-femoral vessels, and because there was a risk of redo sternotomy due to the patent bypass grafts, a comprehensive approach including axillary artery cannulation, a minimally invasive right thoracotomy approach, and a clampless hypothermic fibrillatory arrest technique was used during redo mitral valve replacement.

Enhanced Vasorelaxation Response to Cromakalim in Spontaneously Hypertensive Rats

  • To investigate the properties of cromakalim-opened $K^{+}\;channels$ in aorta of spontaneously hypertensive rats (SHR), the effect of cromakalim on tension was compared in endothelium-rubbed aortic rings from SHR and normotensive Wistar-Kyoto rats (WKY). 1. Cromakalim relaxed the aortic ring contracted by $10^{-7}$ M norepinephrine (NE) dose-dependently, and this relaxant response to cromakalim was blocked by $10^{-5}$ M glybenclamide. 2. Cromakalim also relaxed the contraction induced by high $K^{+}$-solution or 10 mM tetraethylammonium dose-dependently. However, the relaxant response to cromakalim was decreased by raising the $K^{+}$ concentration. 3. SHR aorta exhibited myogenic tone in resting state which was inhibited by cromakalim, verapamil or $Ca^{2+}-free\;PSS.$ Whereas, WKY aorta did not exhibit any myogenic tone in resting state. 4. When aortic rings from both strains were contracted by $20\;mM\;K^{+}\;or\;NE$, relaxant responses to low concentration of cromakalim $(below\;10^{-7}\;M)$ were not different between WKY and SHR, but maximum relaxant response to cromakalim $(above\;3{\times}10^{-7} \;M)$ was greater in SHR than in WKY. 5. When the relaxant response to cromakalim was expressed as percent of maximum relaxation induced by $Ca^{2+}-free\;PSS$, relaxant response to cromakalim in 20 mM $K^{+}-induced$ contraction was not different between WKY and SHR. From the above result, it is suggested that relaxant responses to cromakalim are greater in SHR than WKY, and this may be due to the myogenic tone of aortic rings from SHR.

Video-Assisted Thoracoscopic Division of Vascular Rings

  • This study reports our early experience with thoracoscopic division of vascular rings. Three patients were reviewed; their ages at surgery were 25 months, 4 years, and 57 years. All patients were suffering from complete vascular rings involving combinations of the right aortic arch, left ligamentum arteriosum, Kommerell's diverticulum, and retroesophageal left subclavian artery. The median surgical time was 180.5 minutes, and the patients showed immediate recovery. Three complications, namely chylothorax, transient supraventricular tachycardia, and left vocal cord palsy, were observed. Our early experience indicates that thoracoscopic division of a vascular ring may provide early recovery and could be a promising operative choice.

흰쥐 적출대동맥의 수축력에 미치는 열과 Nacl의 영향

  • In this study, in order to examine whether salt and heat shock stress would alter or not contraction and relaxation of isolated rat aorta. Under anesthesia with sodium pentobarbital(50 mg Kg$^{-1}$ i.p.), male Sprague Dawley rats weighing 300-330 g were subjected to 0, heat shock combined salt stress, where as the sham group was left at modified Krebs-bicarbonate solution. To measure contractile response of vascular ring preparation isolated from rat was determined in organ bath and was recorded on physiograph connected to isometric transducer. And the strip was checked for expression of heat shock protein(Hsps) by means of western blotting. The combination group of heat and 50 mM NaCl group increased vascular contractility, and the heat and 150 mM NaCl group decreased vascular contractility for 5 hours, and then recovered for 8 hours compared to that of control. Expressin of Hsp 70 of vascular muscle of rat aorta more increased by combination of heat and NaCl treatment than those of single treatment of heat or NaCl treatment, and vascular Hsp 70 showed a little decrease at 8 hours compared at 5 hours. These result indicate that mixed environmental stress either increased or decreased in vascular contractility by combination of heat and NaCl concentration.
