Persistent Fifth Aortic Arch with Coarctation

  • Persistent fifth aortic arch (PFAA) is a rare congenital anomaly of the aortic arch frequently associated with other cardiovascular anomalies, such as tetralogy of Fallot and aortic arch coarctation or interruption. We report the case of a neonate with PFAA with coarctation who successfully underwent surgical repair.

  • Idiopathic pulmonary arterial hypertension eventually leads to right-sided heart failure and sudden death. Its mortality rate in children is still high, despite improvements in pharmacological therapy, and therefore novel treatments are necessary. The Potts shunt, which creates an anastomosis between the left pulmonary artery and the descending aorta, has been proposed as a theoretically promising palliative surgical technique to decompress the right ventricle. We report the case of a 12-year-old girl with suprasystemic idiopathic pulmonary hypertension and right ventricular failure who underwent a Potts shunt for palliation with good short-term results.

Emergency Pulmonary Artery-to-Systemic Artery Shunt to Break the Positive Feedback Loop of a Pulmonary Hypertensive Crisis after Neonatal Coarctation Repair

  • A 2.5-kg neonate with coarctation of the aorta and a small left ventricle experienced a severe pulmonary hypertensive crisis. An emergency pulmonary artery-to-systemic artery shunt was placed to break the positive feedback loop caused by pulmonary hypertension and functional mitral stenosis. This shunt provided immediate relief of suprasystemic pulmonary hypertension and the resultant low cardiac output.

상행대동맥을 포함한 대동맥박리에 대한 외과적 치료 (Surgical Treatment of Aortic Dissection Involving Ascending Aorta)

  • 1989년 1월부터 1995년 7월까지 상행대동맥 박리환자 18례에 대한 교정수술을 시 행하였다. 남여비는 9 : 9였으며 연령분포는 41세부터 68세까지로 53.8세 였다. 급성박리가 13례, 만성박리가 5례 였다. 수술방법은 급성 13례에서 상행대동맥만 치환한 경우가 6례, 상행대동맥과 대동맥궁 일부를 치환 한 례가 3례, 상행대동맥과 대동맥궁 전부를 치환한 례가 1례, Bentall 술식이 1례, Bentall 술식 에 대동맥 궁 전부를 치환한 례가 1례 있었다. 만성 5례중 Bentall 술식을 시행한 경우가 3례, 상행대동맥을 치환한 경우가 1례, 상행대동맥 에 대동맥궁 일부를 치환한 례가 1례 있었다. 대동맥궁 교정수술을 시행한 환자 7례중 4례에서 선택적 뇌관류법을 시행하였고 3례에서 완전 순환차단법을 시행하였다. 18례중 급성에 서만 4례 사망하여 수술사망율은 22.2%였다. 사망원인은 출혈이 2례, 좌심실부전이 2례였다. 생존환자 14례를 술후 평균 17개월 외래추적하였으며 이중 1례에서 재수술후 2개월째 패혈증으로 사망하였고 나머지 환자는 양호한 편이다.

  • Confusion of a patent ductus arteriosus (PDA) for the descending thoracic aorta is a fatal error occurring occasionally in infants or neonates. As a result, the left pulmonary artery (LPA) may be misconceived as the PDA, and ligated. This surgical mishap of other hospital leads to serious congestive heart failure and loss of left lung function due to the underdevelopment in the peripheral vascular and alveolar structures in neonates and premature infants. In this report, 3 cases of LPA ligation and subsequent treatment are presented.

기관지 대동맥루 (Bronchoaortic Fistula)

  • Bronchoartic Fistula Secondary to Pulmoanry Tuberculosis Bronch-aortic fistula is a exceptionally rare complication of pulmonary tuberculosis. We report herein, a case of 39 years woman who underwent successful repair of aor-tobronchofistula. She was admitted because of massive hemoptysis via emergency room, she had several bouts of massive hemoptysis prior to hospitalization. Thoracic-aortic pseudoaneurysm had detected by chest CT by chance. The eroded, perforated descending aorta was repaired with patch aortoplasty during temporarily clamping, followed by Left lower lobectomy and omentopexy. Pathological examination revealed pulmonary tuberculosis of superiror seg. of lerg lower lobe and aortitis. The patient had uneventful recovery was well at OPD follow-up check.

  • The renovascular hypertension is a rare disease in which obstructive lesion of renal artery results in hypertension and it is the most common surgically curable form of hypertension. Recently, we experienced two cases of renovascular hypertension treated successfully with resection of the stenotic segment and end-to-side reanastomosis of the renal artery to the aorta in one case, and with resection of the stenotic segment and saphenous vein bypass graft in the other case at the Thoracic & Cardiovascular Surgery, Chungnam National University Hospital. Pathologic examinations revealed fibromuscular dysplasia of the medial layer of the renal artery in both cases

  • Infection after reconstructive surgery is one of the most catastrophic postoperative complication in vascular surgery. Mortality rates reported from a world-wide experience range between 25 and 88 percent. The surgeon faced with such a complication must choose among many diagnostic and management options to maximize limb salvage and survival based on the presentation and site of the infectiota the degree of ischemia of the lower extremities, and the overall medical condition of the patient. We successfully managed with descending thoracic aorta-to-bifemoral arteries bypass after the entire removal of the infected axillo-bifemoral graft because of bypass graft infection.

  • Anomalous aortic origin of the left subclavian artery (LSCA) from the left pulmonary artery (LPA) is a rare congenital cardiac malformation. We describe a case of LSCA from the LPA via ductus arteriosus in association with a double-outlet right ventricle, which never has been reported previously in Korea.

폐 내엽형 격절부:1례 보고 (Intralobar Puhnonary Sequestration - Report of one case -)

  • We experienced a case of intralobar pulmonary sequestration preoperatively confirmed. The 10 years old male patient was admitted beacuse of recurrent episode of coughing and production of purulent sputum. the chest X-ray showed a dense mass containing a large cyst with air-fiuid level in right lower lung field. An aortogram was performed and revealed that the sequestrated portion of the RLL was supplied by an aberrant large artery arising directly from the lower thoracic aorta. After division and ligation of the aberrant artery in pulmonary ligament, the right lower lobectomy was performed and the patient`s postoperative course was uneventful.
