A comparative study of the psychological impacts of tasks related and unrelated to COVID-19 on nurses: a cross-sectional study

  • Background: This study assessed the psychological impact of the outbreak of coronavirus disease 2019 (COVID-19) on university hospital nurses. It provides an assessment of mental health issues, including depression, anxiety, distress, and burnout of nurses dealing directly and indirectly with COVID-19. Methods: In a web-based, cross-sectional study, 111 nurses from Daegu Catholic University Hospital in Korea were enrolled from August 4 to August 9, 2020. Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Impact of Event Scale-Revised, and Maslach Burnout Inventory were used to assess the psychological symptoms of depression, anxiety, psychological distress, and burnout among the study participants. Results: Of 111 nurses, 35 (31.5%), nine (8.1%), 26 (23.4%), and 49 (44.1%) experienced depression, anxiety, distress, and burnout, respectively. Nurses who performed COVID-19-related tasks were more likely to have moderate depression (related vs. unrelated, 52.0% vs. 25.6%; p=0.037). There were no differences in anxiety, distress, and burnout between nurses with and without COVID-19-related tasks. More than 50% of the participants showed receptive and positive attitudes toward caring for COVID-19 patients. Conclusion: Nurses who performed COVID-19-related tasks had a higher risk of depression. There were no significant differences in anxiety, distress, and burnout between the two groups. Since nurses who perform COVID-19-related tasks are more prone to psychological distress, continued psychiatric interventions are required for infectious disease outbreaks with a high mortality rate for healthcare workers who are emotionally vulnerable.

고등학생의 정신건강 상태에 관한 연구 -SCL-90을 이용, 서울시 인문계 1 . 3학년을 중심으로- (A STUDY ON MENTAL HEALTH STATE OF HIGH SCHOOL STUDENTS)

  • This study was conducted to find out the mental health state of high school students. First-year students and third-year students af liberal high schools is Seoul were subject to this study. Questionnaire of Symptom Checklist-90 and various factors was adopted for the survey. A total of 916 questionnaires was sellected for the analysis, and the results are obtained as follows; 1) The characteristics in responses to mental health scale showed that obsessive-compulsive scale score was the highest, followed by interpersonal-sensitivity, depression, hostility, and anxiety. The subject group of the students showed higher scores in nine symptom dimensions except somatization than other normal group. 2) Girl-students showed higher scores than boy-students in somatization, depression, and anxiety, whereas the opposite was true in hostility. 3) Third-year students got high scores, in somatization, anxiety and Depression. 4) Parental marriage state of the repondents showed significant differences in nine symptom dimensions of mental health. Scores of the students with parents was the lowest, followed by those of students with only mother, only father and the rest(no parents, divorced, sepaerated, step-parent) in increasing order. 5) Smoking students showed high score in obsessive-compulsive, depression, hostility, paranoid ideation, and psychoticism. Especially in hostility, they got much higher score. 6) Students with poor record at school got higher scores in every symptom dimension than those with good record at school, especially in obsessive-compulsive and depression scale. 7) Parents' attitude toward student showed significant effect on every scale. Students under over-expectation or indifference from parents were in bad mental health state. 8) Students who have advisor proved to be in better mental health state than those who never consult their personal problems with others. 9) He who has family history got higher scores in some scales. 10) Respondents who looked upon what they have learned in high school as being rather an obstacle to sound social life got high scores in all the symptom dimensions and next came those of the students who answered that there were a lot of unnecessary things in their learning. 11) Those for whom it would not quite necessary to enter college if there were little formal schooling discrimination in society got high scores in obsessive-compulsive, interpersonal sensitivity, depression, hostility, and in psychoticism, especially higher in obsessive-compulsive scale. 12) Mental health state of the students who are influenced by the social surroundings, mass media, and the home environments showed high score in 8 symptom dimensions. 13) Abnormal response frequency of this sample is as follows; 24.0% of boys, 23.8% of girls, 22.5% of the first-year students, and 26.9% of the third-year students. There were significant difference among the grades. 14) The factors of distinctive correlation between the dimensions of SCL-90 and 16 factors were the father's negative attitude and depression, negative responses on teaching contents and anxiety, and smoking and hostility. In conclusion, mental health state of liberal highschool students on the whole showed worse than other normal groups. It had close terms with relation with their parents, schoolwork, smoking, teaching contents, the social surrounding, mass media, and the home environments. Thus I believe there need not only mental health education of students, training of teachers, counceling of parents, but also changes in teaching contents, and the improvement of educational system and the social surroundings under the national support.

한글판 행동활성화 척도의 신뢰도와 타당도 (The Reliability and Validity of the Korean Version of Behavioral Activation for Depression Scale)

  • Objectives The Behavioral Activation of Depression Scale (BADS) has been reported to be a valid tool for assessing the different behavioral aspects of depression, such as activation, rumination or avoidance, and functional impairment. The aim of this study was to assess the reliability and validity of the Korean version of BADS (K-BADS). Methods A sample of 196 outpatients completed the K-BADS and the data were analyzed for internal consistency and factor structures. An additional 51 outpatients re-filled the K-BADS after two weeks for the test-retest reliability. To test for the validity, the Hospital Anxiety and Depression Scale (HADS), Working Alliance Inventory (WAI), Drug Attitude Inventory-10 (DAI-10), and Mindfulness Attention Awareness Scale (MAAS) were administered. Results Internal consistency of K-BADS was good (Cronbach's alpha=0.843) and principal component factor analysis revealed the four-factor structure. The K-BADS showed a reasonable test-retest reliability (r=0.863, p<0.001). The total score of K-BADS correlated significantly with the total scores of the HADS depression (r=-0.694) and HADS anxiety (r=-0.681). No correlations were found between the K-BADS and the K-WAI (r=0.170) and between the K-BADS and the K-DAI-10 (r=0.311). Conclusion The K-BADS is a reliable and valid instrument for measuring the behavioral activation for depression in Korean patients with depressive symptoms.

산후우울증 환자에서 혈중 Nitric Oxide Metabolites의 혈장 농도의 변화 (Plasma Levels of Nitric Oxide Metabolites in Patients with Postpartum Depression)

  • Background : Some reports have suggested that decreased nitric oxide metabolites($NO_x$) and activity of nitric oxide synthase could be related to the pathophysiology of depression. We evaluated plasma levels of $NO_x$ in pregnant women with and without postpartum depression at prenatal and postnatal period. Methods : The plasma concentrations of $NO_x$ were measured in 104 pregnant women in the third trimester and at 6 weeks postpartum and in 64 normal controls. The severity of depression and anxiety was measured with the Edinburgh Postnatal Depression Scale(EPDS), Beck Depression Inventory(BDI), and Beck Anxiety Scale(BAI). Results : Plasma $NO_x$ levels at 6 weeks postpartum were significantly lower in cases of postpartum major depression(EDPS scores${\geq}$13 points) than in cases without depression(EDPS scores${\leq}$9 points). Plasma $NO_x$ levels had significantly negative correlation with EPDS scores at 6 weeks postpartum. Conclusion : We demonstrate that decreased plasma $NO_x$ is associated with postpartum depression. Further studies are required to determine whether individual serum concentration of plasma $NO_x$ alone could predict maternal depression.

초등학교 고학년 아동의 컴퓨터 게임 양상과 정서 및 행동 특성 (Pattern of Computer Game Play and Emotional and Behavioral Characteristics in Upper Grade Elementary School Children)

  • Objective : The purpose of this study was to investigate the relationship between the pattern of computer game play and emotional and behavioral characteristics of 5th and 6th grade elementary school children in Jeonju city. Methods : Fifth and sixth graders from two elementary schools (N=413, M=214, F=199) were chosen to participate in this study. The participants completed self-report questionnaires designed by the authors, and were also evaluated based on a computer game addiction scale, state-trait anxiety inventory for children, children's depression inventory and Piers-Harris children's self-esteem scale. The parents of the participants determined the disruptive behavior rating scale of the participants. Results : Positive links were reported between the pattern of computer game play (years of computer game experience, average length of each computer game play session, average time spent on computer game play per day, and average frequency of computer game play per week) and scores of computer game addiction scale. In addition, significant correlation was found between the internalized problems of children (trait anxiety, state anxiety, depression, and self-esteem), and computer game addiction scale score, among both sexes (p<0.01). There were significant correlations between game addiction scale score and inattention (p<0.01) as well as total attention deficit hyperactivity disorder (ADHD) score (p<0.05) in girls. Conclusion : Our findings suggest that computer game addiction is related to symptoms of internalizing, particularly to the trait anxiety. Significant positive correlations between game addiction scale score and externalized behavioral problems were found only in girls.

조기진통 임부의 피로, 우울과 불안이 삶의 질에 미치는 영향 (Influence of Fatigue, Depression and Anxiety on Quality of Life in Pregnant Women with Preterm Labor)

  • Purpose: The purpose of this study was to identify effects of fatigue, depression and anxiety on quality of life in pregnant women with preterm labor. Methods: With a survey design, data were collected from 138 mothers who were admitted at a hospital in Seoul, between June 2014 and September 2015. Instruments used to collect the data for the study were: Fatigue Continuum Form, Depression anxiety stress scale (DASS-21) and maternal postpartum quality of life (MAPP-QOL). Results: The mean fatigue score was 68.30 with 50.7% of women being depressed and 79.7% of the 138 women being anxious. The mean quality of life was 18.92 with quality of life being associated with fatigue, depression and anxiety. Depression and fatigue explained 26% of the variance in quality of life. Conclusion: Depression and fatigue adversely affected women's quality of life. It is important to address appropriate management of depression and fatigue in order to improve quality of life in pregnant women with preterm labor.

Prevalence of Anxiety May Not be Elevated in Thai Ovarian Cancer Patients Following Treatment

  • Background: To compare prevalence of anxiety in ovarian cancer patients following primary treatment to that of normal women and to examine predicting factor. Materials and Methods: In this cross-sectional study, 56 ovarian cancer patients who had primary surgical treatment within the past five years (cancer group) and 56 age-matched women who attended an outpatient clinic for check-ups (non-cancer group) were recruited from June 2013 to January 2014. The hospital anxiety and depression scale (HADS), was used to determine anxiety level of the participants with the score of ${\geq}11$ suggestive of anxiety. The prevalence of anxiety symptoms and mean HADS scores for anxiety were compared between the study groups. For those with ovarian cancer, associations of demographic and clinical factors with anxiety was examined. A p-value of <0.05 was considered significant. Results: Participants in the non-cancer group had higher rate of medical comorbidity, higher salary, and more frequent university education. The prevalence of anxiety was not different between the groups, at 7.1% each. The mean HADS scores for anxiety subscale were not significantly different between the groups, 5.0 in the cancer group vs 6.1 in the non-cancer group (p=0.09). On multivariable analysis, no demographic or clinical factors significantly associated with anxiety were identified. For the cancer group, no association between any particular factors and anxiety was demonstrated. Conclusions: The prevalence of anxiety in women with ovarian cancer following primary treatment was comparable to that of normal women seeking routine check-up.

Effects of Mindfulness Based Stress Reduction Program on Depression, Anxiety and Stress in Patients with Aneurysmal Subarachnoid Hemorrhage

  • Objective : In this study, the Mindfulness Based Stress Reduction (MBSR) program was applied to patients presenting with depression and anxiety after surgery from spontaneous subarachnoid hemorrhage (SAH) and the effects were assessed. Methods : The subjects were patients admitted for cerebral aneurysm rupture and treated by means of surgery from March to December, 2007. More than 6 months had passed after surgery, without any special lesions showing up on computed tomography (CT), and the Glasgow outcome scale (GOS) was 5 points. Among patients with anxiety and depression symptoms, 11 patients completed the program. The MBSR program was conducted once a week, 2.5 hours each, for 8 weeks. The evaluation criteria were : 1) the Beck Depression Inventory (BDI): it measures the type and level of depression, 2) the State-Trait Anxiety Inventory : the anxiety state of normal adults without mental disorder, and 3) Heart Rate Variability (HRV) : the influence of the autonomous nervous system on the sinoarterial node varies continuously in response to the change of the internal/external environment. Results : The BDI value was decreased from 18.5 ${\pm}$ 10.9 to 9.5 ${\pm}$ 7.1 (p = 0.013) : it was statistically significant, and the depression level of patients was lowered. The state anxiety was decreased from 51.3 ${\pm}$ 13.9 to 42.3 ${\pm}$ 15.2; the trait anxiety was reduced from 50.9 ${\pm}$ 12.3 to 41.3 ${\pm}$ 12.8, and a borderline significant difference was shown (p = 0.091, p = 0.056). In other words, after the treatment, although it was not statistically significant, a decreased tendency in anxiety was shown. In the HRV measurement, standard deviation normal to normal (SDNN), square root of the square root of the mean sum of squared differences between adjacent normal to normal intervals (RMSSD), and total power (TP) showed significant increase, Physical Stress Index (PSI) showed a significant reduction, and thus an improvement in the homeostatic control mechanism of the autonomic nervous system was ween. Conclusion : The MBSR program was applied to the patients showing anxiety and depression reaction after SAH treatment, and a reduction in depression symptoms and physiological reactions were observed. The application of the MBSR program may be considered as a new tool in improving the quality of life for patients after surgery.

돌봄 종사자들의 감정부조화와 우울 및 불안 간의 관계에 대한 융합연구 -정서지능의 조절효과를 중심으로- (Convergence Study about the Relationship among Emotional Dissonance, Depression and Anxiety in Care Service Workers -Focused on the Moderating Effects of Emotional Intelligence-)

  • 본 연구의 목적은 돌봄 종사자의 감정부조화와 우울, 불안 간의 관계를 조사하고, 감정부조화와 우울 및 불안 간의 관계에서 정서지능의 조절효과를 알아보는 것이다. 이를 위하여 돌봄 종사자 142명을 대상으로 감정부조화 평가척도, 정서지능 척도, PHQ-9, GAD-7을 실시하였다. 수집된 자료는 SPSS 25.0 프로그램으로 분석하였다. 분석 결과, 감정부조화와 타인 정서인식이 우울 및 불안에 유의한 영향을 미치는 것으로 나타났다. 또한, 감정부조화와 우울 간의 관계에서 타인 정서인식의 조절효과가 나타났다. 마지막으로, 이러한 결과를 바탕으로 돌봄 종사자의 감정노동 및 정신 건강 문제에 대한 개입 방안을 제안하고 논의하였다.

  • Objectives: The purpose of this study was 1. To investigate correlation of subjective stress with patient's prognosis 2. To observe the effect of Kami-shoyo-san on anxiety and depressive disorder. Methods: We investigated 3 cases of in-patients from traffic accidents. Patients were treated with Kami-shoyo-san. The Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were used to assess anxiety and depression, respectively. We also used Qi stagnation test and Qi counterflow test according to Terasawa's criteria for diagnosis of anxiety and depression. Pain was evaluated by Numeric Rating Scale (NRS). Results: After treatment, the patient's symptoms improved considerably. Conclusions: 1. There were significant correlations between subjective stress and prognosis. 2. Kami-shoyo-san has good effect on anxiety and depressive disorder.
