주요우울증에서 Fluoxetine과 Amitriptyline의 치료효과에 대한 이중맹검법 비교연구 (Double-blind Comparative Trial of Fluoxetine and Amitriptyline in Major Depression)

  • The efficacy of fluoxetine was evaluated in 32 patients with major depression in double-blind amitriptyline-controlled clinical trials. Patients were randomly assigned to 6 weeks of treatment with 20mg/day of fluoxetine and 25-100mg/day of amitriptyline. We used the Hamilton rating scale for depression(HAM-D) and the Clinical Global Improvement(CGI) to evaluate the improvement of depression. In addition, we also used the Covi Anxiety Scale and the Anxiety/Somatization subscale of HAM-D to investigate the relieving effect of anxiety. The improvement by fluoxetine in mean total score of HAM-D and CGI was comparable to amitriptyline. Fluoxetine also reduced anxiety significantly, but there was no difference between fluoxetine and amitriptyline in relieving anxiety symptoms. Fluoxetine showed considerably less adverse effects, especially anticholinergic effect and weight gain than amitriptyline. In conclusion, 20mg/day of fluoxetine was sufficiently effective in the treatment of depressive patients and was better tolerated than amitriptyline.

Factors Affecting Depressive Symptoms in Children and Adolescents With Epilepsy

  • Objectives: We aimed to evaluate the clinical and psychological factors influencing depressive symptoms in children and adolescents with epilepsy. Methods: We administered self-reported questionnaires assessing children's depressive symptoms (Children's Depression Inventory, CDI) and anxiety (Revised Children's Manifest Anxiety Scale, RCMAS) to children and adolescents with epilepsy (n=87, age range=6-17 years). We asked their parents to complete questionnaires on epilepsy-related variables, parental stress (Questionnaire on Resources and Stress, QRS), parental anxiety (State-Trait Anxiety Inventory, STAI), family functioning (Family Adaptability and Cohesion Evaluation Scale, FACES), children's attention problems (Abbreviated Conners Parent Rating Scale Revised, CPRS), and children's behavioral problems (Korean Child Behavior Checklist, K-CBCL). Stepwise multiple regression analysis was performed to identify predictive variables affecting depressive symptoms. Results: Family adaptability (r=-0.240, p=0.026), family cohesion (r=-0.381, p<0.001), children's attention problems (r=0.290, p=0.006), children's anxiety (r=0.714, p<0.001), children's behavioral problems (r=0.371, p<0.001), parental anxiety (r=0.320, p=0.003), and parental stress (r=0.335, p=0.002) were significantly correlated with children's depressive symptoms. Children's anxiety (β=0.655, p<0.001) and parental stress (β=0.198, p=0.013) were significantly related to their depressive symptoms (adjusted R2=0.539). Conclusion: Clinicians should detect and manage children's anxiety and parental stress, which may affect depressive symptoms in children and adolescents with epilepsy.

A novel brief questionnaire using a face rating scale to assess dental anxiety and fear

  • PURPOSE. This study aimed to evaluate the reliability and validity of a four-item questionnaire using a face rating scale to measure dental trait anxiety (DTA), dental trait fear (DTF), dental state anxiety (DSA), and dental state fear (DSF). MATERIALS AND METHODS. Participants were consecutively selected from patients undergoing scaling (S-group; n = 47) and implant placement (I-group; n = 25). The S-group completed the questionnaire both before initial and second scaling, whereas the I-group responded on the pre-surgery day (Pre-day), the day of implant placement (Imp-day), and the day of suture removal (Post-day). RESULTS. The reliability in the S-group was evaluated using the test-retest method, showing a weighted kappa value of DTA, 0.61; DTF, 0.46; DSA, 0.67; DSF, 0.52. Criterion-related validity, assessed using the State-Trait Anxiety Inventory's trait anxiety and state anxiety, revealed positive correlations between trait anxiety and DTA/DTF (DTA, ρ = 0.30; DTF, ρ = 0.27, ρ: correlation coefficient) and between state anxiety and all four items (DTA, ρ = 0.41; DTF, ρ = 0.32; DSA, ρ = 0.25; DSF, ρ = 0.25). Known-group validity was assessed using the initial data and Imp-day data from the S-group and I-group, respectively, revealing significantly higher DSA and DSF scores in the I-group than in the S-group. Responsiveness was gauged using I-group data, showing significantly lower DSA and DSF scores on post-day compared to other days. CONCLUSION. The newly developed questionnaire has acceptable reliability and validity for clinical use, suggesting its usefulness for research on dental anxiety and fear and for providing patient-specific dental care.

연천 사고 피해 PTSD 환자의 해마 용적 : 8개월 추적 연구 (Hippocampal Volume of Patients who got Posttraumatic Stress Disorder from Yun-cheon Accident : 8-month Follow Up Study)

  • Objective : This study was conducted to evaluate the relation between symptoms and hippocampal volume in patients with posttraumatic stress disorder (PTSD). Methods : Eight patients with PTSD and five normal controls were enrolled in the present study. The severity of PTSD symptoms was measured using the clinician administered PTSD scale (CAPS), Hamilton Rating Scale for Depression (HAMD), Hamilton Anxiety Rating Scale (HAMA). Magnetic resonance imaging (MRI) was used to measure hippocampal volume. Results : There were significant differences in the CAPS, HAMD, and HAMA scores between the PTSD patients and normal controls. The left and right hippocampal volumes were smaller in the PTSD patients than in the normal controls, but this difference was not statistically significant. The results showed that PTSD symptoms were not related to hippocampal volume. Conclusion : In this study, we could not identify a difference in hippocampal volume between normal controls and patients with PTSD at eight months after trauma. Future research with different subjects and a larger sample size is needed in order to confirm the difference in hippocampal volume.

향 요법 손 마사지가 국소마취 부인과 수술 환자의 불안과 면역기능에 미치는 효과 (Effect of Aroma Hand Massage on Anxiety and Immune Function in Patients with Gynecology Surgery under Local Anesthesia)

  • Purpose: The purpose of this study was to examine the effects of aroma hand massage on anxiety and immune function in patients who had gynecology surgery under local anesthesia. Methods: The research design was a nonequivalent control group with pre-and posttest design. Data were collected from June 5 to October 6, 2010. Participants included 20 patients in the aroma hand massage group, 20 patients in a hand massage group, and 20 in a control group. As an experimental treatment, hand massage was carried out following the hand massage protocol. Measures consisted of the State Trait Anxiety, Numeric Rating Scale (NRS) for anxiety, vital signs (systolic and diastolic blood pressure, pulse rate), and salivary cortisol for anxiety, and immunoglobulin A for immune function. Results: Aroma hand massage and hand massage group showed lower levels in NRS for anxiety, systolic and diastolic blood pressure, and pulse rate (p<.001) compared to controls. No group differences were found for state anxiety, salivary cortisol and immunoglobulin A. Conclusion: The results indicate that aroma hand massage and hand massage are effective in reducing anxiety and can be complementary alternative interventions for women having gynecology surgery under local anesthesia.

개심술 환자 가족의 불안에 관한 연구 (A Study on the Anxiety of Open Heart Surgery Patient's Family)

  • The purpose of this is to find the anxiety score of OHS pt's family and general surgery pt's family and to offer basic data. The subjects of this study were 80 : 40 were OHS pt's family in Seoul Hospital and remaining 40 were OS pt's family in Capital Hospital. The data was collected between March 1, through May 30, 1992. The instruments used for this study were Zung's The Self-Rating Anxiety Scale. The collected daLa were analyzed using percentage, descriptive statistics, $X^2-test$, t-test and Two-way Anova. The finding's were as followed: 1. Test of hypothesis "The anxiety score of the Ol-lS pt's family were similar to as pt's family" was rejected (L=0.772 P>.05) 2. The mean score of anxiety of the OHS pt's family was 1.772, The mean score of anxiety of the OS pt's family was 1,790. 3. The relationship between anxiety of the OHS pt's family and GS pt's family and family's age was significant (P<.Ol) and the relationship between anxiety and age interaction effect. (F=242.0 P<.01) In conclusion, the anxiety score of the OI-IS pt's finding and GS pt's family was all high.

재활치료 환자 보호자의 불안감과 우울감에 대한 향기치료의 효과 (Effects of Aromatherapy on Anxiety and Depression of Caregivers for Patients in Rehabilitation Therapy)

  • 연구목적: 본 연구는 재활치료 환자를 간병하며 불안감과 우울감을 보이는 보호자에게 향기치료를 적용하여 향기치료 효과에 대한 객관적인 자료를 제공하고자 하였다. 방법: 충북대학교병원 재활의학과에서 물리치료중인 환자 보호자 70명에게 Beck 불안척도와 Beck 우울 척도를 시행하였다. Beck우울 척도상 10점 이상을 보이는 42명을 대상군으로 오렌지 치료군, 라벤더 치료군과 대조군 세군으로 나누어 램프 확산법으로 4주간 향기치료를 하였다. 최종적으로 40명에 대한 향기치료 전후의 Beck불안 척도, Hamilton 불안 평정 척도, Beck 우울 척도, Hamilton 우울 평정 척도, 행복지표 그리고 자아존중감 척도를 평가하였다. 결과: 오렌지 치료군은 대조군에 비해 Beck 우울 척도의 점수 변화에서 유의한 차이를 보였다. 라벤더 치료군은 대조군에 비해 Beck 불안 척도, Beck우울 척도와 행복지표의 점수 변화에서 유의한 차이를 보였다. 결론: 본 연구 결과는 오렌지향유 향기치료는 우울감에, 라벤더향유 향기치료는 불안감과 우울감에 효과가 있음을 시사한다.

틱 장애에서 습관뒤집기 훈련의 임상효과 (Habit Reversal Training in Tic Disorder)

  • Objectives : Tic disorder is known to be a chronic neuro-behavioral disease, cognitive behavioral treatment (CBT) strategies, like habit reversal training (HRT), are introducing recently. We report the effectiveness of HRT in Tourette disorder, which are very common in clinical settings. Methods : The DSM-IV clinical diagnosis applied by child psychiatrist. YGTSS, Kovac's children's depression inventory, Spielberger State-Trait Anxiety Inventory, Abbreviated Conners' Rating Scales, Dupaul ADHD Rating Scales are used. Ultimately, totally 10 children were evaluated. Subject group are consist of 6 boys and 4 girls, and the mean age was $10.90{\pm}1.73$ years old. This study is treatment-refractory 10 patients (from 9-14 years old) though 1 years drug treatment and psychiatric consultation were taken. We administered 5 times of HRT for 4 weeks. Results and Conclusion : There were improvement of scores in Yale Global Tic Severity Scale, Clinical Global Improvement. Our observations indicate that HRT might be effective in the treatment of Tourette disorder.

기공체조프로그램이 초산부의 불안 및 분만통증 완화에 미치는 효과 (Effects of A Qigong Training Program on the Anxiety and Labor Pain of Primipara)

  • Purpose: The purpose of this study is to verify the effects of the Qigong training program on the anxiety and labor pains of primipara. Method: The research subjects were a total of 60 primipara who consulted a doctor regularly concerning their antenatal care. Among them, 30 people were the experimental group, and the other 30 people were the control group, and were selected as homogeneous with the experimental group. The degree of anxiety and labor pains were measured by State-Trait Anxiety Inventory(STAI) and Graphic Rating Scale(GRS). SPSS WIN 11.0 was used for data analysis. Obstetric and general characteristics between experimental and control groups, and a homogeneity test of state and trait anxiety were done by both $X^2$ test and t-test. The hypothesis testing was analyzed by ANCOVA with a covariate of pretest value. Result: The first hypothesis, 'Primipara who join the Qigong training program have lower anxiety than those who do not join' was supported (F=28.8, p<.000). The second hypothesis, 'Primipara who join the Qigong training program have lower labor pain than those who do not join' was unsupported. Conclusion: It was verified that the Qigong training program was effective in alleviating anxiety; however it did not have any effect on relieving labor pain, so more in-depth research is needed later on.

Spiritual Care Training for Mothers of Children with Cancer: Effects on Quality of Care and Mental Health of Caregivers

  • Background: The purpose of this study was to explore the effectiveness of a spiritual care training package in maternal caregivers of children with cancer. Materials and Methods: This study was a quasi-experimental study with pretest and posttest design consisting of a sample of 42 mothers of children diagnosed as having cancer. Participants were randomly assigned to either an experimental or a control group. The training package consisted of seven group training sessions offered in a children's hospital in Tehran. All mothers completed the Spirituality & Spiritual Care Rating Scale (SSCRS) and the Depression, Anxiety and Stress Scale (DASS-21) at pre and post test and after a three month follow up. Results: There was significant difference between anxiety and spiritual, religious, Personalized care and total scores spiritual care between the intervention and control groups at follow-up (P<0.001).There was no statistically significant difference in stress and depression scores between the intervention and the control groups at follow-up. Conclusions: Findings show that spiritual care training program promotes spirituality, personalized care, religiosity and spiritual care as well as decreasing anxiety in mothers of children with cancer and decreases anxiety. It may be concluded that spiritual care training could be used effectively in reducing distressful spiritual challenges in mothers of children with cancer.
