Posterior Vertical Approach for Sacrococcygeal Teratomas in Children (소아의 천미추부 기형종 수술시 후방종단식 술식의 이용)

  • Pediatric surgeons are familiar with the posterior sagittal approach to the rectum at sacrococcygeal area and well oriented with the anatomy because of the Penal procedure for imperforate anus. The author utilized the posterior vertical elliptical incisions in 12 cases of sacrococcygeal teratoma since 1987. For presacral tumor(type IV)$^2$, the incision was exactly same as the posterior sagittal procedure for imperforate anus. But the out-growing(type I) or dumbbell-shaped(type II & III) tumors, a vertical elliptical incision was required. For the laterally deviated tumors, a vertical and half-chevron incision was utilized in one case, but an unbalanced vertical elliptical incision was acceptable for the remaining two cases, with shrinkage of the overlying skin. In dumbbell-shaped tumors(type II & III), the narrow waist of the tumor was at the level of the levator muscle, which formed a muscle-belt on the tumor waist. A careful dissection to save the muscle-belt seemed to be the most important point during this procedure, utilizing the nerve-stimulator. After complete removal of the tumor and the coccyx, the levator muscles and the skin were closed in vertical fashion along the midline. For the caudally extending tumors in 3 cases, the muscle complex was divided in midline. Nothing by mouth and total parenteral nutrition was maintained for 1 week and then laxatives were given for 2 weeks in order to give the sphincters rest. Operative scars were acceptable resembling natural vertical midline folds, and the sphincter function was continent in all cases. In conclusion, vertical elliptical incision in sacrococcygeal teratoma is recommended because of the acceptable scar, functional restoration, and because it is a familiar procedure particularly for the pediatric surgeons who are accustomed performing posterior sagittal approach for imperforate anus.

  • Morphology and histochemical characteristics of the alimentary tract in surfperch, Ditrema temmincki were investigated by histological observation. The relative length of gut(RLG) in surfperch, that is the postpharyngeal portion of the alimentary tract, is about 0.89 to standard length. The absence of a stomach, which is the part of expantion of the alimentary tract between esophageal end and the entrance of the bile duct into the intestine, was observed. The alimentary tract is divided into the esophagus, esophageal-intestine part, anterior intestine, mid intestine, posterior intestine, intestinorectum part, rectum, rectal-anus part, and anus by morphology and histochemical features. Morphology of mucosal folds is the most complexity in the rectum and the tunica muscularis of the esophagus and anus is more advanced than those of other parts. The epithelial layers of mucosal folds consist of columnar epithelium except for cuboidal cells of the anterior part of the esophagus. The goblet cells and polysaccharide absorptive cells were observed in the alimentary tract. Nutritive polysaccharide is mainly absorbed by the absorptive cells in the posterior intestine of the surfperch.

The Experience of the VATER Association in One Hospital (한 병원에서 경험한 VATER 연관기형)

  • VATER association is defined as a combination of 3 or more anomalies- vertebra (V), imperforate anus (A), esophageal atresia with or without tracheoesophageal fistula (TE), renal and radial anomaly(R). We reviewed our experiences in one center to determine etiology, prevalence, clinical manifestation, other associated anomaly and prognosis. Two hundred and twenty-three cases that underwent operations for imperforate anus or esophageal atresia were analyzed retrospectively through medical records at Department of Pediatric Surgery, Asan Medical Center from June, 1989 to July, 2005. The total number of neonates who had been admitted during period of study were 46,773 and VATER association was 9 (0.019 %, 1.92 persons per 10,000 neonates). Median gestational age and birth weight were $37^{+4}wk$ ($35^{+1}$ - $41^{+4}$) and 2,594 g (1,671-3,660), respectively and median age of mother was 32 years (23-38). There was no family history. Three patients were twins but their counterparts had no anomalies. Patients who have 3 anomalies were 6, 4 anomalies in two and 5 anomalies in one patient. Vertebra anomalies were detected in 7(77.7 %), imperforate anus in 8(88.9 %), esophageal atresia in 5 patients (55.6 %), renal anomaly in 6(66.7 %), and radial anomaly in 5(55.6 %), respectively. Four patients are alive, 2 patients were lost during follow up period. Three patients died due to neonatal sepsis, respiratory dysfunction and cardiac failure. VATER association did not appear to be a definite risk factor, but merely a randomized combination of 5 anomalies. The prognosis was dependent on the other associated anomalies, appropriateness of management and operation. Careful follow-up and aggressive treatmentare required for improving survival and quality of life.

  • The food habits and health complaints were studied for 459 persons, 60 years and over, in the rural area, JKurye-Gun Chonnam, from April 28 to May 1, 1986. by the questionnaire sheets ; one was for the food habits, and the other was for health complaints(the standarized questionnair designed in Todai Health Index). The results obtained were as follows. 1) Mean score of the food habit was 11.2$\pm$3.5 in male and 10.8$\pm$2.9 in female. The score in both sexes showed a falling tendency as their ages increased. 2) THI scores in surfferings eye & skin, mouth & anus were higher (p<0.01, p<0.05, p<0.01) in female than in male. As age increased, the THI score showed a rising tendency. In case of male, THI socre in digestive organs was higher (p<0.05) in sixties than in other age. 3) The rate of living together with their family or spouse in the male aged was 77% higher than in the female aged. whilte the rate of celibacy in the female aged was 28%, higher than 3% in the male aged. DMF value was higher in female than male. The amount of alcohol ingestion were 2.1 and 0.6 Cup in male and female, respectively and the amount of smoking were 10.5 and 3.1 cigarettes in male and female respectively. Both alcohol intake and smoking were higher in male(p<0.01) 4) In case of male, the correlation coefficient between the score of food habit and THI score was negative in suffering eye & skin, mouth & anus, respectively, and in female it was negative in digestive, mouth & anus. As the score of food habit become low, the THI score become high. 5) Those living alone in both sexes showed a low score in food habit.

  • To study on the assignment of the eight trigrams (八卦) in human body being based on based the number of HADO(河圖)-NAGSEO(洛書). Being based on the number of HADO(河圖)-NAGSEO(洛書) and the BEOK-HEE's diagram of eight trigrams order(伏羲次序圖), human being organ (five-Jang organs (liver, heart, spleen, lung and kidney), five organs (ear, eye, mouth and nose), provisional namely six entera for the process on intake, digestion, excretion of food (mouth, stomach, small intestine, large intestine, rectum, anus) are assigned eight trigrams (八卦). Eight trigrams(八卦) are attached to the based on the number HADO(河圖)-NAGSEO(洛書): 1-GEON(乾), 2-TAE(兌), 3-LEE(離), 4-JIN(震), 9-SON(巽), 6-GAM(坎), 7-GAN(艮), 8-GON(坤). And (八卦) are attached to the human being organs (five-Jang organs (live, heart, spleen, lung and kidney), five organs (ear, eye, mouth, nose, anus) : 1-GEON(乾)-left lung-left nose, 6-GAM(坎)-right kidney-right ear, 7-GAN(艮)-left heart-anus, 8-GON(坤)-right liver-right ear. Eight trigrams(八卦) are attached to the provisional namely six entera for the process on intake, digestion, excretion of food : the air(GEON 乾), the month(TAE 兌), the stomach(LEE 離), the small intestine(JIN 震), the large intestine(Son 巽), the rectum(GAM 坎), the anus(GAN 艮), the excrements(GON 坤). The BEOK-HEE's diagram of eight trigrams order(伏羲次序圖) means immutable order naturally. The process of the human being organs( five-Jang organs, five organs and six entera's digestion) is also unchangeable in region and program. Therefore we can set up the five-Jang organs, five organs and the process of six entera's digestion as the category of observation on the basis of BEOK-HEE's diagram of eight trigrams order(伏羲次序圖), and then we can arrange eight trigrams (八卦) on the five-Jang organs, five organs and the process of six entera's digestion.

First detailed morphological description of the loggerhead sea turtle (Caretta caretta) caught from the Yellow Sea of Korea

  • To date, no study has reported detailed morphological characteristics of Korean sea turtles. Due to the lack of such basic information on Korean sea turtles, further related studies have been difficult in South Korea. In this report, we determined the species and the sex of the one sea turtle caught from the Yellow Sea of Korea (Taean-gun, Chungcheongnamdo) on July 17, 2013, and described its detailed morphological characteristics. The sea turtle was identified as a loggerhead sea turtle (Caretta caretta) by the presence of an interprefrontal scale on the head. The turtle had three times longer length between the edge of anal scute to the anus than that between the anus to tip of the tail, and the size of a pair of claws on the flippers were distinctively different, suggesting that the turtle was a male. Finally, the assumption that the sea turtle might be sexually mature is based on its body weight (59.95 kg), the maximum straight length of the carapace (72.5 cm), and the worn serrated parts at the edge of supracaudal scutes. The loggerhead sea turtle described in this study is the first record from the Yellow Sea of Korea.

Current Status of Taeniasis and Cysticercosis in Vietnam

  • Several reports on taeniasis and cysticercosis in Vietnam show that they are distributed in over 50 of 63 provinces. In some endemic areas, the prevalence of taeniasis was 0.2-12.0% and that of cysticercosis was 1.0-7.2%. The major symptoms of taeniasis included fidgeted anus, proglottids moving out of the anus, and proglottids in the feces. Clinical manifestations of cysticercosis in humans included subcutaneous nodules, epileptic seizures, severe headach, impaired vision, and memory loss. The species identification of Taenia in Vietnam included Taenia asiatica, Taenia saginata, and Taenia solium based on combined morphology and molecular methods. Only T. solium caused cysticercosis in humans. Praziquantel was chosen for treatment of taeniasis and albendazole for treatment of cysticercosis. The infection rate of cysticercus cellulosae in pigs was 0.04% at Hanoi slaughterhouses, 0.03-0.31% at provincial slaughterhouses in the north, and 0.9% in provincial slaughterhouses in the southern region of Vietnam. The infection rate of cysticercus bovis in cattle was 0.03-2.17% at Hanoi slaughterhouses. Risk factors investigated with regard to transmission of Taenia suggested that consumption of raw meat (eating raw meat 4.5-74.3%), inadequate or absent meat inspection and control, poor sanitation in some endemic areas, and use of untreated human waste as a fertilizer for crops may play important roles in Vietnam, although this remains to be validated.

A Study on Hygiene and Skin Pruritus in Rural Elderly People (농어촌 복합 섬 지역 노인의 위생습관과 피부 소양감)

  • Purpose: This study was done to develop data for a hygiene promotion program for elders living on rural islands. Method: Self-administered questionnaires were used to collect data. Results: Most of the elders reported brushed their teeth and washing their faces twice a day. For other types of washing, 98.8% of reported washing their hands after the toilet and 22.8% reported washing their anus after defecation. Most of the elders reported changing their underwear two to three times a month and cutting their nails more than four times a month. Itchiness was reported by 24%. There were significant differences according to age for teeth brushing, face washing, shampooing, bathing, washing hands after the toilet, and cutting nails. According to sex, there were significant differences in teeth brushing, face washing, washing hands after using the toilet, washing anus after defecation, changing underwear, cutting nails, and feeling itchy. The elders showed significant differences in bathing, changing underwear, and cutting nails according to subjective economic status. They showed significant differences in teeth brushing and changing underwear when they lived with their families. Conclusion: In order to develop a proper hygiene promotion program for the elderly, their age, sex, and physical condition should be considered as well as their environmental, economical, and geographical condition.

Short-Term Outcomes of Transanal One-Stage Pull-Through Procedure for Hirschsprung's Disease (Hirschsprung 병에서 Transanal One-Stage Pull-Through 술식의 단기 성적)

  • The aim of this study was to evaluate the short-term outcomes of the trans anal one-stage pull-through procedure(TOP) in Hirschsprung's disease. Eight patients aged 3 weeks to 8 months with Hirschsprung's disease underwent this procedure. A rectal mucosectomy was performed from just proximal to dentate line to the level of peritoneal reflexion, where muscle layer was incised circumferentially. Rectosigmoid was mobilized out through the anus, and full-thickness frozen biopsy was taken for confirmation of ganglionic cells. After the rectal muscular cuff was divided longitudinally in the posterior aspect, aganglionic bowel was removed and ganglionic colon was anastomozed to the anus. The mean operating time was 161 minutes, and the mean hospital stay after operation was 3.8 days. Five patients had three to four bowel movement per day without other therapy at mean postoperative 39.2 days. Although long-term follow-up will be required, the TOP might be the new alternative surgical procedure for Hirschsprung's disease.

Tubular Colonic Duplication Presenting as Rectovestibular Fistula

  • Complete colonic duplication is a very rare congenital anomaly that may have different presentations according to its location and size. Complete colonic duplication can occur in about 15% of all gastrointestinal duplications. Double termination of tubular colonic duplication in the perineum is even more uncommon. We present a case of a Y-shaped tubular colonic duplication which presented with a rectovestibular fistula and a normal anus. Radiological evaluation and initial exploration for sigmoidostomy revealed duplicated colons with a common vascular supply. Endorectal mucosal resection of theduplicated distal segment till the colostomy site with division of the septum of the proximal segment and colostomy closure proved curative without compromise of the continence mechanism. Tubular colonic duplication should always be ruled out when a diagnosis of perineal canal is considered in cases of vestibular fistula alongwith a normal anus.
