수술 후 인후 합병증은 통증이나 구토에 비하면 사소한 합병증이지만 흔히 환자들이 불편을 호소한다. 양 측 내시경적 부비동 수술 후에 양 코를 막아 입으로 숨 쉬게 되어 구강 건조가 더 심할 것으로 보고 환자들을 glycopyrrolate를 전투약 하고 반전시에 쓴 경우, 전투약 하지 않고 반전에만 쓴 경우, 전투약에만 쓰고 반전은 atropine을 쓴 경우, 전투약 하지 않고 반전은 atropine으로 한 경우의 네 군으로 나누었다. 마취 종료 1시간, 6시간, 12시간, 24시간, 48시간 후의 인후통과 쉰 소리 여부를 묻고, 없는 경우 0, 있는 경우에는 감기 시 목 아픈 것 보다 약하면 1, 비슷하면 2, 더 심하면 3으로 하여 심각도를 나누어 물었다. 각 군 간의 인후통 발생률은 1군에서 조금 높았으나 유의한 차이가 없었다. 심각도는 모두 1이하로 낮았다. 쉰 소리는 모든 환자에서 발생하지 않았다. 양측 내시경적 부비동 수술 후 인후 합병증은 심각도가 낮고 24시간 내에 사라지는 것으로 항콜린성 제제 사용을 피해야 할 필요성은 없을 것으로 보인다.
Dizziness can be classified mainly into 4 types: vertigo, disequilibrium, presyncope, and lightheadedness. Among these types, vertigo is a sensation of movement or motion due to various causes. The main causes of peripheral vertigo are benign paroxysmal positional vertigo (BPPV), acute vestibular neuritis (AVN), and Meniere's disease. BPPV is one of the most common causes of peripheral vertigo. It is characterized by brief episodes of mild to intense vertigo, which are triggered by specific changes in the position of the head. BPPV is diagnosed from the characteristic symptoms and by observing the nystagmus such as in the Dix-Hallpike test. BPPV is treated with several canalith repositioning procedures. AVN is the second most common cause of peripheral vertigo. Its key symptom is the acute onset of sustained rotatory vertigo without hearing loss. It is treated with symptomatic therapy with antihistamines, anticholinergic agents, anti-dopaminergic agents, and gamma-aminobutyric acid-enhancing agents that are used for symptoms of acute vertigo. Meniere's disease is characterized by episodic vertigo, fluctuating hearing loss, and tinnitus. It is traditionally relieved with life-style modification, a low-salt diet, and prescription of diuretics. However, diagnosis and treatment of the peripheral vertigo can be difficult without knowledge of BPPV, AVN, and Meniere's disease. This article provides information on the differential diagnosis of peripheral vertigo in BPPV, AVN, and Meniere's disease.
Background: Primary hyperhidrosis is a disorder of excessive sweating, which shares several features with anxiety disorders and has a negative impact on a patient's quality of life. Oral glycopyrrolate is one of the treatments available. There are a few published studies on the use of glycopyrrolate given orally in the treatment of hyperhidrosis. Methods: Thies is study was a review of case notes in a series of 36 patients with primary hyperhidrosis. We made a comparison between the Keller's scale score of a pre-glycopyrrolate medication group and the Keller's scale score f a post-glycopyrrolate medication group. The Milanez de Campos score, Short Form_36 (SF-36) score, Beck Depression Inventory (BDI) score, Beck Anxiety Inventory (BAI) score, and autonomic nervous system (ANS) scale score were also compared between the two groups. Results: In the post-glycopyrrolate medication group, there were declines in Keller's scale, and Milanez de Campos scale score and BAI score (P < 0.001). In addition, there were increases in SF_36 score in the post-glycopyrrolate medication group (P = 0.03) However, no changes were seen in, BDI score and ANS score in the post-glycopyrrolate medication group (P < 0.001). Conclusions: Glycopyrrolate is an effective initial method of treating primary hyperhidrosis that, reduces anxiety and improve patients' quality of life.
Compensatory hyperhidrosis or reflex hyperhidrosis is the increase in sweating in the postoperative stage of thoracic sympathectomy or lumbar sympathectomy. It shares several features with anxiety disorders and has a negative impact on a patient's quality of life. Oralglycopyrrolate is one of the treatment options available. This study reviewed case notes in a series of 19 patients with compensatory hyperhidrosis. We made a comparison between the Milanez de Campos score of a pre-glycopyrrolate medication group and the Milanez de Campos score of a post-glycopyrrolate medication group. The Beck Depression Inventory (BDI) score, Beck Anxiety Inventory (BAI) score, and autonomic nervous system (ANS) scale score were also compared between the pre-medication and post-medication groups. In the post-glycopyrrolate medication group, there was decrease in the Milanez de Campos score, BAI score, and BDI score (P < 0.05). But no meaningful change was seen in the ANS score in the post-glycopyrrolate medication group (P > 0.05). Glycopyrrolate is an effective medication in the treatment of compensatory hyperhidrosis that, can alleviate anxiety and improve patients' quality of life.
The tropane alkaloids hyoscyamine (its racemic form being atropine) and scopolamine are used medicinally as anticholinergic agents that act on the parasympathetic nerve system. Because they differ in their actions on the central nervous system, currently there is a 10-fold higher commercial demand for scopolamine, in the N-butylbromide form, than there is for hyoscyamine and atropine combined. Several solanaceous species have been used as the commercial sources of these alkaloids, but the scopolamine contents in these plants often are much lower than those of hyoscyamine. For this reason there has been long-standing interest in increasing the scopolamine contents of cultivated medicinal plants. Naturally occurring and artificial interspecific hybrids of Duboisia have high scopolamine contents and are cultivated as a commercial source of scopolamine in Australia and other countries. Anther culture combined with conventional interspecific hybridization also has been used to breed high scopolamine-containing plants in the genera Datura and Hyoscyamus, but without much success. The use of recombinant DNA technology for the manipulation of metabolic processes in cells promises to provide important contributions to basic science, agriculture, and medicine. In this review, I introduce on the enzymes and genes involved in tropane alkaloid biosynthesis and current progress in metabolic engineering approaches for tropane alkaloid, especially scopolamine, production.
Nocturnal enuresis is a heterogeneous disorder with various underlying pathophysiological mechanisms and causes a mismatch between the nocturnal bladder capacity and the amount of urine produced during sleep at night. It is associated with a simultaneous failure of conscious arousal in response to the sensation of bladder fullness. Generally, a complete history and physical examination, with a specific focus on the genitourinary, gastrointestinal, and neurologic systems, is sufficient to evaluate a patient with enuresis. The therapeutic focus is directed toward a differential approach based on the underlying mechanism and toward combination therapies such as alarm devices and desmopressin as well as anticholinergic agents and desmopressin. Children with increased nocturnal urine production usually have a good response to desmopressin therapy. Patients with a small bladder generally show a poor response to desmopressin treatment, but they would benefit more from combination therapy with enuretic alarm, urotherapy, and antimuscarinic agents in addition to desmopressin. Different types of bladder dysfunction, which result in a small nocturnal bladder capacity, probably contribute significantly to the pathogenesis of nocturnal enuresis, particularly in those with treatment failure and refractory symptoms. Because different clinical subgroups may show different responses to treatment, it is necessary to distinguish these subgroups before a decision on the specific treatment protocol can be made.
Background : Although chronic non-bacterial prostatitis is increasing, it is hard to treat effectively. In western medicine, antimicrobials drug, ${\alpha}$-adreno-ceptor antagonists, anti-inflammatory drugs, tricyclic antidepressants and anticholinergic agents are used commonly, but chronic prostatitis/chronic pelvic pain syndromes is confusing and frustrating for urologist. IDS(Indongsoyeom-bang) is used in treatment of chronic prostatitis/chronic pelvic pain syndromes. And it is reported that GLS(Gleditsiae spina) and TOF(Toosendan fructus) components of IDS have significant effect on protection of the glandular epithelial cells. Objective : In this study was conducted to investigate the therapeutic effects and action machanism of IDS in the rat model of non-bacterial prostatitis induced by castration and testosterone treatment. Methods : We observed six experimental objects of normal group, control group, testosterone group, and IDS 50 mg/kg, 200mg/kg, 400mg/kg group. Rats were treated with 17 ${\beta}$-estradiol after castration for induction of experimental non-bacterial prostatitis, which is similar to human chronic prostatitis in histophatological profiles. IDS and testosterone were administered as an experimental specimen and a positive control, respectively. The prostates were evaluated by histological parameters including the epithelial score and epithelio-stromal ratio for glandular damage. Also, the prostates were observed by Hematological alterations of WBC, RBC, hemoglobin and platelet. Results : While prostates of control rats revealed severe acinar gland atrophy and stromal proliferation, the rats treated with IDS-50 showed a diminished range of the tissue damage. Epithelial score was improved in IDS than that of the control. The epithelio-stromal ratio was lower in IDS when compared to that of the control. Also, the examination of bloods were not observed hematological change. Conclusion : These finding suggests that IDS may protects the glandular epithelial cells. We concluded that IDS could be a useful remedy agent for treating chronic non-bacterial prostatitis.
유뇨증의 치료제로서 널리 사용되고 있는 imipramine의 작용기전에 관한 학설이 여러가지가 있으나 그 중 콜린성수용체 봉쇄작용을 관찰하기 위한 방법으로 평활근 세포를 분리배양하여 단일세포에 대한 acetylcholine의 수축작용과 이에 대한 imipramine의 길항작용을 atropine의 그것과 비교해 보기로 하였다. 개의 방광을 적출하여 $0{\sim}4^{\circ}C$의 K-H 용액내에서 $2{\times}2mm$크기의 평활근 절편을 얻어 $36^{\circ}C$의 collagenase 용액에 넣고 95%/5% $O_2/CO_2$, 혼합산소 공급하에서 17~20분동안 배양하여 분리된 부유세포군을 5 ml test tubes에 나누어 담고 acetylcholine을 $10^{-14}M{\sim}10^{-9}M$의 농도로 첨가하였다. Acrolein 1%를 가하여 수축한 세포를 고정시킨 후, 위상차 현미경에 장착한 CCTV camera로 채취한 영상을 microscaler로 전송하고 monitor상에서 세포의 길이를 측정하였다. 분리된 세포들은 acetylcholine에 의해 5초 이내에 최대 수축을 보였으며 이후 120초까지 수축상태를 지속하였다. Atropine은 acetylcholine 유발 수축을 atropine $10^{-7}M$ 에서부터 농도의존적으로 억제하였으며, imipramine도 acetylcholine 유발수축을 atropine과 같은 경향으로 농도의존적으로 억제하였는데, imipramine $10^{-9}M$의 저농도에서도 유의한 억제를 보였다. 이상의 결과를 종합하면 본 실험에 사용된 조건하에서의 collagense를 이용한 세포 분리법은 단일세포를 대상으로 하는 실험을 위하여 가용한 방법이며, imipramine은 개의 방광평활근 단일세포에서 atropine보다 더 강력한 콜린성 수용체 봉쇄작용을 나타낸다고 생각되었다.
