The Statistical Analysis for the fate of Antibiotic Resistance according to the Spatial and Operational Wastewater Treatment Factors (하수 처리시설의 공간 및 운전인자에 따른 항생제 내성의 통계학적 분석)

  • The aim of this study was to examine the fate of tetracycline resistant bacteria (TRB) and tetracycline resistant genes (TRG) according to the spatial and operational wastewater treatment factors. As part of the effort, TRB and TRG of water samples at each unit processes of three different wastewater treatment plants (WWTPs) were analyzed over seven month study periods. With the data about different spatial and operating conditions of these WWTPs, TRB and TRGs, principal component analysis (PCA) was performed to find out any general correlation trend. Based on the statistic analysis results, the extent of TRB concentration in the activated sludge (TRBAS) is much related to the TRB concentration in primary clarifier effluent (TRBPE). Also, the study results indicated that the fate of TRB and TRG are significantly affected by the SRT variations.

Comparison Between Antimicrobial Susceptibility Test and mecA PCR Method for Reading of Methicillin-Resistant Staphylococcus aureus (메티실린 내성 황색포도알균 판독에 있어 항균제 감수성 검사와 mecA PCR법의 비교)

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of major pathogen causing hospital infection and several diseases such as purulent infection, bacteremia. The isolation ratio of MRSA is gradually increased up to 80% in the hospital, which makes a limitation for treatment of antibiotics because the isolated MRSA show resistance to methicillin as well as other antibiotics. This study proposes that mecA detecting methods which are not commonly used because of cost in the hospital is a more accurate method than Susceptibility Testing to detect a MRSA. We compared Staphylococcus aureus ATCC 29213 as a negative control and 20 MRSA strains isolated from patients by these two methods. We amplified mecA gene by polymerase chain reaction (PCR) and confirmed the PCR products by sequencing. All of the MRSA showed oxacillin and cefoxitin resistance whereas 85% (16/19) of the strains had mecA wildtype. These results suggest that some of the MRSA are mecA mutants therefore mecA genotyping reinforces the MRSA detection by antibiotic susceptibility test.

Optimal Modified Extended Antibiotic Prophylaxis for Prostate Biopsy: The Addition of Two Intravenous Doses of Amikacin to Ciprofloxacin

  • Purpose: This retrospective study was undertaken to investigate whether increasing amikacin dosage for ciprofloxacin prophylaxis in patients with fluoroquinolone (FQ)-resistant rectal flora reduce infectious complications after transrectal ultrasound-guided prostate biopsy (TRUSPB). Materials and Methods: A total of 430 patients with FQ-resistant rectal flora based on rectal swab cultures were divided into two groups. Patients in both groups were administered ciprofloxacin (400 mg, intravenous [IV], twice daily) on the same day as TRUSPB and one day after biopsy. However, whereas group 1 patients (n=202) were administered a single injection of amikacin (1 g, IV) one hour before TRUSPB, patients in group 2 (n=228) were administered two injections of amikacin (1 g, IV) before one hour TRUSPB and again on the day after TRUSPB. Results: Of the 430 study subjects, 129 (30.0%) showed extended-spectrum beta-lactamase (ESBL) positivity. The overall incidence rate of infectious complications was 2.8% (12/430). Infectious complication rates were 4.0% (8/202) in group 1 and 1.3% (3/228) in group 2 (p=0.075). Urinary tract infection and acute prostatitis were more frequent in group 1 (3.5% vs. 0.4%, p=0.029). Infectious complication rates in ESBL negative patients were 3.4% (5/145) in group 1 and 1.3% (2/156) in group 2, whereas those in ESBL positive patients were 7.0% (4/57) in group 1 and 1.4% (1/72) in group 2. Conclusions: Increasing the dosage of amikacin for ciprofloxacin prophylaxis reduce infectious complications in patients with FQ-resistant rectal flora and to be more effective in ESBL positive patients with FQ-resistant rectal flora.

Effects of Coinoculation by Bradyrhizobium japonicum with other Rhizobacteria and of Antibiotic Treatment on Soybean Growth and Nodule Formation (근류균(根瘤菌)과 타(他) 근권미생물(根圈微生物)과의 혼합접종(混合接種) 및 항생제처리(抗生劑處理)가 대두(大豆)의 생장(生長)과 근류형성(根瘤形成)에 미치는 영향(影響))

  • The Neomycin resistant gene in transposon 5 (Tn5) was introduced into a mutant of Bradyrhizobium japonicum RJB 6 $str^rnal^r$ by conjugation. This marked strain was used in coinoculation of soybean with Pseudomonas fluorescens(Ps-3) and Bacillus subtilis(BCAC-4) and in antibiotic treatment of soybean for studying rate of nodule formation and growth promoting effect on soybean plant. When the marked strain RJB 6 $str^rnal^rneo^r$ with Ps-3, BCAC-4, and Ps-3 plus BCAC-4 was coinoculated into two soils, the recovery rate of the marked strain was 8.5, 4.3 and 2.9 percent in soybean cultivated soil, and 10.3. 6.0 and 5.0 percent in soybean uncultivated soil. The best growth promoting effect of coinoculation on soybean plant was found with the marked strain plus Ps-3. When the marked strain was inoculated into soybean with antibiotic treatment, the rate of nodule formation in soybean cultivated soil was between 12.5 and 25.4 percent, while that in soybean uncultivated soil was between 23.7 and 43.2 percent. The highest rate of nodule formation with the marked strain was found in streptomycin 1000 ppm treatment plot.

A Fifteen-year Epidemiological Study of Ventriculoperitoneal Shunt Infections in Pediatric Patients: A Single Center Experience (소아 환자에서의 뇌실-복강 단락 감염의 역학적 고찰: 15년 간의 단일 기관 연구)

  • Purpose : Ventriculoperitoneal (VP) shunt insertion is an important treatment modality in children with hydrocephalus. VP shunt infection is a major complication and an important factor that determines the surgery outcome. This 15-year study was performed to evaluate the epidemiology of VP shunt infections in pediatric patients treated at our center. Methods : A retrospective review of medical records was performed in patients 18 years old or younger who underwent VP shunt insertion surgery from April 1995 to June 2010. Results : Three hundred twenty-seven VP shunt surgeries were performed in a total of 190 pediatric patients (83 females, 107 males). The median age of the patients was 2.4 years (range, 0.02-17.9 years). Having a malignant brain tumor was the most frequent cause for VP shunt insertion. The shunt infection rate was 6.7% (22/327) per 100 operations and 9.5% (18/190) per 100 patients, and the incidence rate was 0.45 infection cases per 100 shunt operations-year. The most common pathogen was coagulase-negative staphylococcus (n=7) followed by methicillin resistant Staphylococcus aureus (n=1). Ten cases were treated with vancomycin and beta-lactam antibiotic (cephalosporin or carbapenem) combination therapy and 7 cases were treated with vancomycin monotherapy. The median duration of antibiotic treatment was 26 days (range, 7 to 58 days). Surgical intervention was performed in 18 cases (18/22, 81.8%). Conclusion : Epidemiologic information regarding VP shunt infections in pediatric patients is valuable that will help guide proper antibiotic management. Additional studies on the risk factors for developing VP shunt infections are also warranted.

Studies on Antibiotic Resistance and Growth Characteristics of Shigella Sonnei Isolated from Patients of Shigellosis (이질환자에서 분리한 Shigella Spp.의 항생제 내성과 성장특성)

  • Antibiotic resistance of thirty strains of Shigella sonnei isolatedfrom patient of Shigellosis outbreke at Young Cheon area in 1998 was tested. Twenty-seven strains were resistant to Tr(Trimethoprim-Sulfamethoxazol) and Shigella sonnei SG-48 was resistant to Tr(Trimethopirm-Sulfamethoxazol), Ap(Ampicillin), Cp(Cephalothin) and Pi(Piperacillin). Shigella sonnei SG-49, SG-66, and SG-73 were senstive to all tested antibiotics. Physiological charactristics of isolated Shigella sonnei SG-48, SG-49, SG-57, and SG-73 such as effect of pH, NaCl concentration and temperature on the growth, survival in adverse condition and heat resistance were investigated Growth of the strains were inhibited at pH 4 and pH 9. All strains were grown in Tryptic soy broth containing 6% of NaCl but inhibited in TSB containing 9% of NaCl except Shigella sonnei SG-73 after incubation for 18hrs at 37$^{\circ}C$. Selected strains grew during storage at 10 but did not grow at 4. The strains were survived in 1% pepton solution for 15 days at 37$^{\circ}C$. Viable cell of selected strains were decreased 45 log cycle after heat treatment for 30 mins at 6$0^{\circ}C$ but did not detect by heat treatment for 5 mins at 7$0^{\circ}C$.

Antibiotics Resistance and Molecular Analysis of Enterococcus Isolated from the Han-river in Korea (한강에서 분리한 장구균의 항생제 내성과 분자생물학적 분석)

  • Identification was performed in March 2008 for the 76 Enterococcus strains isolated from the Han-river, which is used as water supply for Seoul citizens. The antibiotic susceptibility, antibiotic resistant structural analysis, trans-conjugation, pulsed-field gel electrophoresis (PFGE) and multi-locus sequence typing (MLST) were also carried out for the isolated strains. Among the isolated strains, 25 strains were E. casseliflavus, 4 strains were E. faecalis and 1 strain was E. hirae. Investigation of antibiotic susceptibility indicated that 15 strains demonstrated tolerance against vancomycin, and that 11 strains of E. faecium and 4 strains of E. casseliflavus were VRE. The vanA gene detection of the VRE strains revealed that 6 E. faecium strains were vancomycin-resistant Enterococcus faecium (VREF) possessing vanA. Analyses of transposon Tn1546 structure containing vanA demonstrated that Km36 and Km37 belonged to Tn1 type, Km20 and Km38 was Tn2 type, and Km39 and Km40 was Tn3 type. PFGE disclosed that among the 6 VREF strains, Km36 and Km37 exhibited equivalent subtype, while the rest 4 strains showed subtypes different to each other. MLST for the 6 VREF strains disclosed that 3 strains were ST78, while the rest 3 strains were ST18, ST192 and ST230, respectively. All these clonal complexes were derived from CC17 which has been isolated from clinical sources. 4 strains belonged to CC78, while the rest 2 strains were CC18 and CC192, respectively.

Antibiotic Resistance Patterns of Enterococcus spp. Isolated From Commercial Frozen Foods (시판 냉동식품에서 분리한 장구균의 항생제 내성 양상)

  • From April to December in 2009, microbial investigation is accomplished for 100 frozen foods asked to microbial control team that corresponds with total aerobic viable bacteria, coliform group, Escherichia coli, Enterococcus spp. and antibiotic resistance patterns of Enterococcus spp. isolates are investigated. Average of total erobic viable bacteria numbers is $4.3{\times}10^4CFU/g$. Average of coliform group numbers is $4.3{\times}10^3CFU/g$. Average f Enterococcus spp. numbers is $1.8{\times}10^3CFU/g$. Escherichia coli from 100 frozen foods is not detected and detection ate is 0.0%. 22 Enterococcus spp. are isolated from 100 frozen foods. 12 of 22 Enterococcus spp. strains are identified as E. faecium. 7 of 22 Enterococcus spp. strains are identified as E. faecalis. 2 of 22 Enterococcus spp. trains are identified as E. gallinarum. 1 of 22 Enterococcus spp. strains is identified as E. hirae. Enterococcus spp. solates show a high resistance to erythromycin, rifampin, tetracycline, ciprofloxacin, chlorampenicol, penicillin and susceptibility to vancomycin, ampicillin, gentamicin, strepomycin, linezolid. 15 of 22 Enterococcus spp. strains are multi-resistant and the most frequent multi-resistant pattern is erythromycin-rifampin for 6 Enterococcus spp. strains.

Plasmid profiling of multi-drug resistant Vibrio sp. isolated from influent and effluent water samples of fish farms in Jeju, South Korea (제주 양식장 유입수과 방출수에서 분리한 다제내성 Vibrio균 플라스미드 프로파일링)

  • The objective of this study was to investigate the plasmid profiling of multi-drug resistant (MDR) Vibrio in influent (inflow) and effluent (discharged) water samples of fish farms in Jeju, South Korea. MDR isolates identified through disc diffusion susceptibility tests, were subjected to plasmid profiling. One hundred fifty Vibrio isolates were obtained from each influent and effluent water sample. All MDR isolates were subjected to plasmid profiling. Greater number of bacteria were enumerated from effluents (61%) comparing to influents (39%). High incidence of neomycin, sulfamethoxazole, amoxicillin and oxytetracycline resistance was observed among the isolates, which was higher in effluent samples. In contrast, Vibrio isolates were more susceptible to florfenicol, chloramphenicol, ciprofloxacin, and nalidixic acid. Among 99 (influent 39 and effluent 60) MDR isolates, a total of 58 (influent 38 and effluent 20) were found to bear plasmids ranging from 1.7 kb to >10 kb and showed 19 different antibiograms according to the size of plasmids. MDR isolates showed six and four distinct plasmid profiles in influent and effluent, respectively. Effluent samples contained more plasmid-carrying MDR Vibrio isolates with more diverse plasmid profiles and antibiograms, suggesting that fish farm tanks may serve as a reservoir of antibiotic resistance genes. The presence of plasmid-carrying MDR Vibrio isolates in fish farm effluent water may contribute to the dissemination of antibiotic resistance genes to the environments, which ultimately poses threat to human health.

Antibiotic Resistance of Salmonella spp. After Exposure to Mild Heat Treatment (살모넬라 균주들에서 열처리에 의한 항생제 내성 연구)

  • Salmonella is widely prevalent in various environments and often detected in poultry. In this study, we investigated the effect of heat treatment on heat resistance via measuring the minimum inhibitory concentration (MIC) values of antibiotics after 3, 6, and 9 min of acclimatization to mild heat treatment (50℃) against 11 strains of Salmonella spp. Most strains were susceptible to chloramphenicol and their MIC values were maintained or decreased after heat treatment compared to the control. Most control and heat-treated strains showed susceptibility or intermediate resistance to ciprofloxacin. All isolates were susceptible to tetracycline, with the MIC increasing after heat treatment for S. Gaminara BAA 711. In the control, three, two, and six strains were susceptible, intermediate resistance, and resistant to gentamicin, respectively. Among them, S. Heidelberg ATCC 8326 had an intermediate MIC breakpoint of 8 ㎍/mL in the control; however, after 3 and 9 min of heat treatment, the MIC value increased to 16 ㎍/mL, indicating it to be resistant. The results of this study revealed the changes in antibiotic resistance in some of the 11 strains after heat treatment. MIC values of ciprofloxacin increased when S. Montevideo BAA 710 was heat treated for 3 and 6 min. MIC values of gentamicin increased after 3 min of heat treatment for S. Enteritidis 109 D1 and after 3 and 9 minutes of heat treatment for S. Heidelberg ATCC 8326. The MIC value of tetracycline increased when S. Gaminara BAA 711 was heat treated for 6 and 9 min.
