Hypersensitivity Reaction to Perioperative Drug Mistaken for Local Anesthetic Systemic Toxicity in a Patient under Brachial Plexus Block

  • Perioperative anaphylaxis, although rare, is a severe, life-threatening unexpected systemic hypersensitivity reaction. Simultaneous administration of various drugs during anesthesia, the difficulty of communicate with patients in sedation and anesthesia, and coverage of the patient with surgical drapes are considered to be factors that impede early recognition of anaphylactic reactions. It is very important to perform an intradermal skin test because antibiotics are the most common cause of perioperative anaphylaxis. We report a case of negative-intradermal skin test antibiotic anaphylaxis mistaken for local aesthetic systemic toxicity without increase of serum tryptase for confirmative diagnostic biomaker during surgery under brachial plexus block. It is not possible to exclude the danger of anaphylaxis completely, even if it is negative-intradermal skin test and normal tryptase level. Therefore, anesthesiologists should be closely monitored and treated early for antibiotics related hypersensitive reaction, like other medicines during anesthesia.

Scrotal pyocele secondary to gastrointestinal perforation in infants: a case series

  • Pyocele in infants is rarely described in the literature, but it is an emergent condition that requires rapid recognition and treatment to prevent testicular loss. If peritonitis due to gastrointestinal perforation occurs, abdominal contamination may spread through a patent processus vaginalis in an infant, which may lead to pyocele. We report the cases of three infants with scrotal pyocele due to the spread of infection or inflammatory material from the intraperitoneal cavity through a patent processus vaginalis. Two infants were surgically treated, while the other was treated with percutaneous aspiration and intravenous antibiotic administration. Although rare, pyocele should be considered in the differential diagnosis of acute scrotum in infants, especially in infants who previously had peritonitis due to gastrointestinal perforation.

  • Clinical and molecular phenotypes of asthma are complex. The main phenotypes of adult asthma are characterized by eosinophil and/or neutrophil cell dominant airway inflammation that represent distinct clinical features. Upper and lower airways constitute a unique system and their interaction shows functional complementarity. Although human upper airway contains various indigenous commensals and opportunistic pathogenic microbiome, imbalance of this interactions lead to pathogen overgrowth and increased inflammation and airway remodeling. Competition for epithelial cell attachment, different susceptibilities to host defense molecules and antimicrobial peptides, and the production of proinflammatory cytokine and pattern recognition receptors possibly determine the pattern of this inflammation. Exposure to environmental factors, including infection, air pollution, smoking is commonly associated with asthma comorbidity, severity, exacerbation and resistance to anti-microbial and steroid treatment, and these effects may also be modulated by host and microbial genetics. Administration of probiotic, antibiotic and corticosteroid treatment for asthma may modify the composition of resident microbiota and clinical features. This review summarizes the effect of some environmental factors on the upper respiratory microbiome, the interaction between host-microbiome, and potential impact of asthma treatment on the composition of the upper airway microbiome.

Ceftizoxime의 장내세균에 대한 시험관내 및 생체내 항균효과 (Antibacterial Activity of Ceftizoxime Against Gram Negative Enteric Bacteria in vitro and in vivo)

  • 반합성 ${\beta}$-lactam 항생제로서 여러 세균에 ${\beta}$-lactamase에 대해 높은 안정성을 나타내는 ceftizoxime의 임상 분리 장내 세균 18주에 대한 시험관대 및 생체내 항균효과와 내성균의 출현여부를 조사하고 ampicillin, tetracycline, gentamicin 및 cefadroxyl와의 항균력을 시험관내 설험으로 비교하였으며 또한 마우스 장내 정상균총에 미치는 영향을 관찰하여 다음과 같은 결론을 얻었다. 1. Ceftizoxime은 그람음성 장내 세균에 대하여 시험관내에서 높은 항균력을 나타내었으며 이러한 항균력은 ampicillin, tetracycline, gentamicin보다 높았으며 ${\beta}$-lactamase 산생 내성균주의 치료제로 알려진 cefadroxyl 보다도 높았다. 2. 시험관내 다체 내성균주는 ampicillin, cefadroxyl, gentamicin 및 tetracycline에 대한 내성주는 E. coli 1주, ampicillin, cefadroxyl 및 tetracycline에 대한 내성주는 E. coli 2주, ampicillin, gentamicin 및 tetracycline에 대한 내성주는 Shigella dysenteriae 이였으며, ampicillin 및 tetracycline에 대한 내성주는 Enterobacter cloacae였고 ampicillin 및 tetracycline에 대한 내성주는 E. coli 5주이었다. 3. Ceftizoxime에 대한 내성균주의 출현율은 매우 낮았다. 4. 서로 다른 내성을 보인 9균주 E. coli (6 strains, Shigella dysenteriae, Enterobacter cloacae, and Samonella typi)중 plasmid 분리는 6균주에서 관찰되었으며 이는 E. coli 5주와 Shigella dysenteriae 1주 있었다. 그러나 항생제에 대한 저항의 marker로서는 결정할 수 없으므로 이를 위해서는 curing experiment를 사용하여야 한다. 5. Ceftizoxime은 마우스 장내 정상균총에는 현저한 영향을 미치지 않았다. 이상의 결과에서 ceftizoxime은 그람음성 장내 세균에 대하여 높은 항균력을 나타내며 장내세균 감염치료에 선택적 항생제로 이용될 수 있을 것으로 사료된다.

호랑나비 유래 항균 펩타이드 파필리오신 3의 항염증 활성 (Anti-inflammatory Activity of Antimicrobial Peptide Papiliocin 3 Derived from the Swallowtail Butterfly, Papilio xuthus)

  • 본 연구에서는 호랑나비 유충의 유전체 분석을 통해 선별된 파필리오신 3의 항균 및 항염증 활성을 확인하였다. 선행연구에서 RNA 시퀀싱 분석을 통해 호랑나비의 전사체를 분석하였으며, 결과를 바탕으로 인실리코(in silico) 분석을 진행하여 전사체 유래 항균 펩타이드를 스크리닝하고 선발하였다. 수행된 항균 활성 및 용혈 활성 테스트에서 파필리오신 3은 그람음성균인 E. coli와 그람양성균인 S. aureus에 대해 강력한 항균활성을 나타낸 반면 마우스 적혈구에 대한 용혈 활성은 전혀 없었다. 다음으로 마우스 대식세포주 Raw264.7 세포를 이용하여 파필리오신 3의 항염증 활성을 확인하였다. 그 결과 파필리오신 3은 LPS로부터 유도된 Raw264.7 세포들의 산화질소 생성을 감소시키는 결과를 보여주었다. 뿐만 아니라 실시간 역전사 중합효소 연쇄반응(qRT-PCR) 방법과 효소결합면역흡착측정법(ELISA)을 통해 파필리오신 3이 Raw264.7 세포에서 전염증성 사이토카인(IL-6, IL-1β)의 발현을 감소시킨다는 것을 확인할 수 있었다. 또한, 염증반응의 신호전달인자들(MAPKs, NF-κB)의 인산화를 억제하는 것을 확인하였는데, 이는 파필리오신 3이 LPS와의 상호작용을 통해 결합하여 효과를 나타낸다는 것을 확인할 수 있었다. 이러한 결과들은 호랑나비 유전체 분석을 통해 확인된 파필리오신 3이 새로운 항균 및 항염증 치료제로서 개발하는데 가능성 있는 물질로 사료된다.

Smith-Magenis Syndrome (SMS) 환아의 증례 보고 (SMITH-MAGENS SYNDROME (SMS) : A CASE REPORT)

  • Smith-Magenis syndrome(이하 SMS)은 제 17번 상염색체의 부분적 결손으로 발생하는 다발성 선천적 기형 및 정신지체를 특징으로 하는 증후군으로 1 : 25,000의 빈도로 발생하는 것으로 알려져 있다. 이 증후군은 작은 키와 단두증(brachycephaly)을 동반한 편평한 중안면, 처진 입, 종종 두드러지게 붉은 뺨, 성인에서 돌출된 턱 등의 특징적인 얼굴 생김새, 만성이염, 청각 손상, 사시와 근시를 포함한 눈의 이상, 목 쉰 소리, 짧은 손가락과 발가락, 심장 질환, 비뇨기 질환, 척추 만곡, 비정상적 걸음걸이, 통각에 둔감함 등의 신체적 특징을 지닌다. 특히 유아기에는 토실토실한 아기 얼굴, 행복한 기분, 드문 울음소리, 낮은 근 긴장도, 섭식 장애 등의 특징이 있다. 수면 장애를 포함하여 언어 지체 및 발음 장애, 발달지체, 학습 장애, 정신지체, 활동항진과 자해, 폭발적 분노, 지속되는 짜증, 파괴적이고 공격적인 행동, 흥분성, 흥분시에 팔로 안거나 손을 끼우는 행동 등의 행동 발달적 특징을 나타낸다. 본 증례는 3세 3개월된 남아로 생후 10개월에 SMS를 진단받은 환자로 상악 좌측 제 1유구치의 통증을 주소로 본과에 내원하였으며, 임상 및 방사선학적으로 다발성 우식증을 보였으며, 소아과 주치의에게 의뢰한 결과 심내막염의 예방을 위한 항생제의 처치를 처방 받고, 환아의 행동 발달적 문제로 인해 물리적 속박의 방법 하에 외래에서 치료되었다. 이 증례를 통해 SMS라는 희귀한 증후군에 관하여 보고하고자 한다.

육계에서 조직과 혈액의 enrofloxacin 및 대사성 ciprofloxacin 잔류 상관관계 조사 및 검사방법간의 비교 (Comparison of detecting methods and the relationship between tissue and blood for enrofloxacin and metabolic ciprofloxacin residues in broiler chickens)

  • The purpose of this study was to evaluate detecting methods and the relationship between tissues and blood for enrofloxacin and metabolic ciprofloxacin residues in broiler chickens. Two groups of broiler chickens were administrated via the drinking water with $50{\mu}g/mL$ and $100{\mu}g/mL$ of enrofloxacin for 5 days, respectively. The concentration of enrofloxacin and metabolic ciprofloxacin in tissues (muscle and kidney) and blood were measured during administration period (for 5 days) and withdrawal period (for 12 days) by high performance liquid chromatography (HPLC) method. Also, all samples were conducted for screening of residues by microbial method using E. coli for quinolone detection and immuno-chromatography method using Smart kit. The relationship between tissues (muscle and kidney) and blood for enrofloxacin and metabolic ciprofloxacin residues in broiler chickens was followed : The levels of enrofloxacin and metabolic ciprofloxacin residues in muscle and kidney were higher 2.9~3.2 folds, 3.6~3.8 folds more than the residues levels in blood, respectively. These results support we can predict the residues in muscle and kidney from the residues in blood. In comparison of detecting methods for antibiotic residues, microbial method using E. coli for quinolone detection and immuno-chromatography method using Smart kit could detect positive reaction at similar or lower concentration than violative concentration of enrofloxacin and metabolic ciprofloxacin in chicken tissues. These results support what two screening methods are useful for screening of quinolone detection in chickens.

새로운 Anthracycline계 항암성 항생물질 DA-125의 랫드에 대한 4주 용량설정시험(DRF)과 13주 아급성 독성시험 (Four-Week Dose-Range Finding and 13-Week Repeated Dose Intravenous Toxicity Studies in Rats with DA-125, a New Anthracycline Antitumor Antibiotic)

  • This study was conducted to investigate the repeated dose toxicity of DA-125, a new anthracycline antitumor antibiotic, in rats. Before the 13-week main study, a 4-week dose-range finding (DRF) study was carried out. The administration of DA-125 intravenously at dosage levels of 0, 0.125, 0.5, 2.0, and 8.0 mg/kg/day to rats for 4 weeks resulted in premature deaths of all animals in the 8.0 mg/kg/day group and in the deaths of 4 males and 4 females at 2.0 mg/kg/day. Body weights were markedly reduced in the 8.0 mg/kg/day group and showed dose-related decreases in all treatment groups when compared with the control group. Reductions in weight gain were slight and not significantly different at 0.125 mg/kg/day but animals receiving 0.5 mg/kg/day showed more marked decreases in gain in a clear dose-related manner Based On the results of the above DRF study, a 13-week repeated dose intravenous toxicity study in rats with DA-125 was performed at a dose level of 0, 0.012, 0.08 and 0.3 mg/kg/day. No treatment related effects were noted in behavior or body weight in all treatment groups. One male at the highest dose level died on study day 26, but the death could not be related to test article toxicity. Swelling and scabbing of the ears was present in all of the groups, including the control group. There were no treatment related changes in the hematological, biochemical or urinalysis values in all treatment groups. Thymus weights were significantly reduced ill males receiving 0.3 mg/kg/day and they were sligltly, and not significantly, reduced in females of the same group. While there were no associated histological changes. Treatment related necrosis was found in the tail vein (injection site) at 0.08 and 0.3 mg/kg/day. On the basis of these results, the no observed effect level (NOEL) was 0.012 mg/kg/day and the maximum tolerated dose (MTD) was estimated to be more than 0.3 mg/kg/day under the conditions tested.

Comparison of Inflammatory Markers Changes in Patients Who Used Postoperative Prophylactic Antibiotics within 24 Hours after Spine Surgery and 5 Days after Spine Surgery

  • Objective : C-reactive protein (CRP) level, erythrocyte sedimentation rate (ESR), and white blood cell (WBC) count are inflammatory markers used to evaluate postoperative infections. Although these markers are non-specific, understanding their normal kinetics after surgery may be helpful in the early detection of postoperative infections. To compliment the recent trend of reducing the duration of antibiotic use, this retrospective study investigated the inflammatory markers of patients who had received antibiotics within 24 hours after surgery according to the Health Insurance Review & Assessment Service guidelines and compared them with those of patients who had received antibiotics for 5 days, which was proven to be non-infectious. Methods : We enrolled 74 patients, divided into two groups. Patients underwent posterior lumbar interbody fusion (PLIF) at a single institution between 2019 and 2020. Group A included 37 patients who received antibiotics within 24 hours after the PLIF procedure, and group B comprised 37 patients who had used antibiotics for 5 days. A 1 : 1 nearest-neighbor propensity-matched analysis was used. The clinical variables included age, sex, medical history, body mass index, estimated blood loss, and operation time. Laboratory data included CRP, ESR, and WBC, which were measured preoperatively and on postoperative days (POD) 1, 3, 5, and 7. Results : CRP dynamics tended to decrease after peaking on POD 3, with a similar trend in both groups. The average CRP level in group B was slightly higher than that in group A; however, the difference was not statistically significant. Multiple linear regression analysis revealed operation time, number of fused levels, and estimated blood loss as significant predictors of a greater CRP peak value (r2=0.473, p<0.001) in patients. No trend (a tendency to decrease from the peak value) could be determined for ESR and WBC count on POD 7. Conclusion : Although slight differences were observed in numerical values and kinetics, sequential changes in inflammatory markers according to the duration of antibiotic administration showed similar patterns. Knowledge of CRP kinetics allows the assessment of the degree of difference between the clinical and expected values.

Specific Alternation of Gut Microbiota and the Role of Ruminococcus gnavus in the Development of Diabetic Nephropathy

  • In this study, we aim to investigate the precise alterations in the gut microbiota during the onset and advancement of diabetic nephropathy (DN) and examine the impact of Ruminococcus gnavus (R. gnavus) on DN. Eight-week-old male KK-Ay mice were administered antibiotic cocktails for a duration of two weeks, followed by oral administration of R. gnavus for an additional eight weeks. Our study revealed significant changes in the gut microbiota during both the initiation and progression of DN. Specifically, we observed a notable increase in the abundance of Clostridia at the class level, higher levels of Lachnospirales and Oscillospirales at the order level, and a marked decrease in Clostridia_UCG-014 in DN group. Additionally, there was a significant increase in the abundance of Lachnospiraceae, Oscillospiraceae, and Ruminococcaceae at the family level. Moreover, oral administration of R. gnavus effectively aggravated kidney pathology in DN mice, accompanied by elevated levels of urea nitrogen (UN), creatinine (Cr), and urine protein. Furthermore, R. gnavus administration resulted in down-regulation of tight junction proteins such as Claudin-1, Occludin, and ZO-1, as well as increased levels of uremic toxins in urine and serum samples. Additionally, our study demonstrated that orally administered R. gnavus up-regulated the expression of inflammatory factors, including nucleotide-binding oligomerization domain-like receptor pyrin domain-containing protein 3 (NLRP3) and Interleukin (IL)-6. These changes indicated the involvement of the gut-kidney axis in DN, and R. gnavus may worsen diabetic nephropathy by affecting uremic toxin levels and promoting inflammation in DN.
