The Conditions and Problems of Anti-Smoking Education Programs of Governmental Offices (전국 관청의 금연프로그램 실시 현황과 문제점)

  • The purpose of this study is to investigate the conditions and problems of anti-smoking education programs in government office for civil servants. A mail-in survey was carried out for the chief of the general affairs of 248 government offices. 1. There were 154 responses to the surveys. Only 34 governmental offices have carried out anti-smoking education programs for civil servants. 2. Only one office has a department to supervise and anti-smoking program. 17 offices have 1 worker and thirteen offices have no manpower to supervise anti-smoking program. 3. Only eight offices have budgeted for anti-smoking programs. The average budget was 3,750,000 Won. Expected budget for smoking cessation program was 7,500,000 Won. 4. 25 offices have an anti-smoking lecture program for civil servants. Only five offices have an anti-smoking counseling for smokers. 5. Only seven surveys responded that civil servants had a positive response to anti-smoking programs. 6. The most important problem of anti-smoking programs in governmental offices was the lack of concern of civil servants. Governmental offices have to provide the greatest administrative services and services concerning the quality of life for citizens. Anti-smoking education programs in governmental offices for civil servants was the beginning of an activity of the services concerning the quality of life for citizens. Governmental offices have to actively implement anti-smoking programs for civil servants.

Effects of an Anti-Smoking Program to Prevent Lung Cancer among Urban Aboriginals in Taiwan

  • Background and Purpose: Indigenous people who leave their hometowns and move to the city to earn a living became urban aboriginals. During the process of adapting to urban living situations, they may use various coping strategies such as smoking to overcome their stress. Therefore, it is crucial to provide health education including smoking prevention, increasing knowledge regarding of tobacco hazard, self-efficacy of anti-smoking, and adjusting smoking behavior so as to empower their anti-smoking motivation to prevent lung cancer. The purpose of this study was to explore the effectiveness of an anti-smoking program on urban aboriginals in Taiwan. Methods: A quasi-experimental study design with purposeful sampling was employed. A total of 125 aboriginal subjects were recruited from two local churches at Shu Lin area in northern Taiwan. Subjects were divided into an experimental group (n =64 ) and a control group (n = 61). Both took pre-tests in order to set baseline values, and only the experimental group participated for 3-weeks in the anti-smoking program classes. Both groups took post-tests immediately after the intervention in order to evaluate the immediate effects of the teaching program, and a follow-up test was conducted four weeks after the intervention. Data were analyzed using descriptive statistics, one-way ANCOVA, and repeat measure ANCOVA. Results: After controlling for confounding variables, the results showed that there were statistically significant differences in the self-efficacy of anti-smoking and smoking behavior between experimental and control groups in the immediately post-test and the follow-up test (p < 0.05). However, there was no significant differences in the recognition of hazards of smoking at eiter time point. Conclusions and Implications for Practice: The findings of this study revealed that the anti-smoking program effectively improved self-efficacy of anti-smoking, and decreased the smoking behavior in urban aboriginals. They provide useful information as a reference regarding of aboriginal health promotion to health providers. It is imperative that anti-smoking be reinforced for those regular smokers to prevent induction of lung cancer.

  • This study had two objectives. The first was to assess the frequency and content of school-based anti-smoking education received by Romanian adolescents aged 14-15. Secondly, the study aimed to evaluate to what extent the implementation of a specific 5 lessons smoking prevention program influences the quality of anti-smoking school education among Romanian adolescents. The investigation was performed in twenty schools from Cluj-Napoca, Romania, which were randomly assigned to the control and experimental conditions, resulting in 55 participating classes from the seventh grade (28 in the control group and 27 in the experimental group). The experimental group participated in a school-based smoking prevention program consisting of 5 lessons. The control group beneficiated only in the standard anti-smoking education offered by their schools. Six months after the program implementation, students from both experimental and control groups filled in a questionnaire, assessing several issues regarding their exposure to anti-smoking school education in the last year. The results showed a low exposure to anti-smoking school education among the Romanian adolescents. The implementation of the specific school-based smoking prevention program increased the exposure of Romanian adolescents to a higher number of lessons of smoking prevention and influenced positively the quality of these lessons. The study identified several gaps with respect to anti-smoking education in Romanian schools. It underlines the benefits of the implementation of a school based smoking prevention program with a clear structure, which contains appropriate educational messages and it is easy to implement.

The Effects of Follow-up Support in the Adolescent Smoking Cessation Program (청소년 금연 프로그램에서 추후관리의 효과)

  • The purpose of this study is to investigate the effects of follow-up support in the adolescent smoking cessation program. In this study, a model for an effective adolescent anti-smoking program, which includes follow-up support, is based on the method of cognitive-behavioral therapy. The study lasted for 7 weeks, while information on smoking including materials to be analyzed, were provided to the subjects. Meanwhile, the state of the smokers was studied while consultations on anti-smoking was also carried out in the 7th week. At first, the quantity of cigarettes smoked between groups made no difference, but there was an obvious difference in the rate of people who stopped smoking and in the quantity of cigarettes smoked between group I(managed for 7 weeks) ,group II (managed for only 4 weeks) and group III (not managed at all after anti-smoking class). According to the results of the study, we can conclude that the follow-up support plays a big role not only in raising the rate of success in the level of reformed smokers, but also in lowering the quantity of cigarettes smoked.

A Study on the Actual Conditions of Smoking in Middle and High School Students in One Region (일 지역 중·고등학생의 흡연실태)

  • To investigate the actual conditions of smoking in middle and high school students in Chinan County, I used a sturctured questionnaire for 1,579 students attending twelve middle-high schools from December 1, 1998 to December 20, 1998. I collected and data correlated the using an $SPSS-PC^+$ 1. The smoking rate of middle-high schoo1 students in Chinan County was 17.9%, relatively high. This smoking rate was different according to the gender, grade, religion, and economic situation. In mals, high school students, non-religious, students low income family students, the smoking rate was higher. The smoking rate of high school students was almost the same as the smoking rate of adults, generally higher than that of foreign teenagers. Because the smoking rat of studinets in the third grade of middle school and in the first grade of high School was six times higher, increased education should be conducted during this time in an attempt to curb the sudden increase of the smoking rate. The smoking rate of girl students was 5.0%; this has increased mor than three times from ten years ago. Consequently, counter measures should be taken against the smoking of female students as well as juvenile smoking in general. In addition, the smoking rate of middle-high school students showed interesting differences when correnated with enviornmental factors. Students with low grades, who are not satisfied with school life, who don't have both parents, who have uncaring parents who nare too strict or too arbitary, who have smoking parents, or who have experienced smoking commonly smoked. Therefore, to lower the smoking rate we should improve the school environment, improve a student's interest in school life. And parents or siblings should lead by example and quit smoking at home. Schools should educate students more effectively concerning the harmful effects of smoking and create an accurate understanding of its dancers. From the beginning, we should teach students never ever to touch cigaretts. 2. The surve discovered that most students started out of curiosity, or solicitantion from friends or elders at middle school, and had been smoking one to five cigarettes for more than a year. They obtained cigarettes at stores and most of them have friends who smoke. As a result anti-smoking education should be conducted at elementary schools prior to middle school. More than 95% of the teenagers who smoke had friends who smote and smoked out of curiosity or the recommendation of elders. Thus, we must focus on teenagers who smoke in group, rather than individually. Fuyrthermore, the strict application of the regulation of tobacco sales as well as tobacco cooperation from retailers are needed. While students did not show any mood or academic achievement difference after beginning smoking, 58.1% of the students a health situdation that was worse. Juvenile smoking is more harmful to the juvenile than adult smoking is to the adult. This should be focused on in an anti-smoking campaign. 3. Students who smoke hada more positive attitude toward smoking than students who don't smoke. Students who smoke had a tendency to have a nuetral position and are not concerned about smoking compared to non-smoking students. The survey showed that the great number of students had a nuetral position. Because this nuetrality may increase Juvenile smoking, education that provides an exact understanding of smoking should be performed to build the correct attidude toward smoking. 4. Middle school students smoke when angry, gloomy, anxious, a lone and when they have some problems to solve, on when they feel inconveniened in other wores, they smoke to reliver stress. They also smok due to addiction. Because smoking is not a praetical method to relieve stress, a program which helps to acquire positive relief stress should be provided to help reduce smoking. 5. About 65% of students who smoke want to quit smoking because of health problem, 78% of them have tried mor than once to quit but failed due to weak will power and peer pressure from friends who smoke. Juvenile smoking is group, oriented. Thus, the program that advances less smoking will be the one that focuseds on groups. 6. As for advice to students who want to quit smoking, "persuasion" was used most commonly, followed by a "presentation on how to quit smoking". Another method were severe punishment. About 70% of the students wanted the anti-smoking guide at school. 7. Most students (73.5%) had a position that more anti-smoking education at school is needed. Obriously, then, anti-smoking education at middle-high schools should be reinfoced. Although the education which explains the harmful influence of tobacco is known as an efficient way prevent smoking; it does not influence students who already smoke. Therefore, for students who smoke, multi-dimensional approaches must be attempted that include physical training, phychokogical approache, consultation and discussion, medical chek-ups, audio-visual education technigues, and professonal instructors, in addition, because smoking students have more negative on lukewarm attitude to anti-smoking education anti-smoking education should be conducted through a communicative style by dedicated teachers who care about students. A Study on the Factors Affecting Smoking Cessation Behavior of University Student Smokers (일 지역 금연희망 대학생의 금연행위 관련 요인 연구)

  • This study was conducted to identify the factors affecting smoking cessation behavior of the university student smokers for developing the effective smoking cessation program. For this purpose, data were collected from 64 students registrated at the university health center for anti-smoking acupuncture through May, 2003. Data were analyzed with percentile, mean, t-test, ANOVA, pearson correlation by SPSS/PC program. The results were as follows ; There was significant relationship in the scores on perceived benefits(t=-2.05, p=.045), chance-health locus of control(t=-2.28, p=.026) and nicotine dependence (t=-1.96, p=.056) according to the grade. And also there was significant increase in the nicotine dependence in the higher grades. There were significant positive relationship between external locus of control and perceived benefits(r=.389, p=.001), perceived barriers and benefits(r=.457, p=.000), and significant negative relationship between the daily life activities and perceived barriers(r=-.350, p=.005). From these results, it can be concluded that the effective smoking cessation program could be considered of perceived benefits & barriers, and nicotine dependence to promote anti-smoking behavior for university student smokers.

A Study on Anti - Smoking Education of Middle and High Schools in Seoul (서울시내 중 . 고등학교에서의 금연교육실태)

  • This study was performed during the period between March 22 and July 23, 1989 in other to examine status of aniti - smoking eduation in middle and high school in Seoul. The results were as follows; 1. The respondents of this study were 403 nurse teachers of whom 43.7% had implemented anti - smoking education and 56.3% had not implemented anti - smoking education. Classified by school, those schools implementing anti - smoking education were in order of frequency; boys' high schools (32.4%), coeducational high schools (18.3%) and boys' middle schools (13/6%) and those schools not implementing anti-smoking education were in order of frequency: girls' high schools (23.8%), girls' middle schools (22.5%) and coeducation middle schools(7.6%). 2. The number of times that anti - smoking education was done was once a year (60.8%) or twice a year (38.2%). For teaching materials, 49.4% of the schools reported possessing teaching materials and 50.6% reported not possessing teaching matenials. The content of the anti - smoking education material was classified by grade for 27.8% of the schools and not classified by grade for 72.2%. 3. In schools where anti - smoking education directed at giving up smoking was being used, several problems were presented; lack of reference data and education of data(38.1%) lack of equipment(29.0%), smoking by teachers(13.6%). On the other hand, in schools where anti - smoking education was not being used, the ploblems identified were, in the following order, lack of reference data and education of data(38.1%), lack of equipment(29.0%) lack of time (15.0%) and lack of information (15.4%). There was statistically significant difference the two types of schools. 4. In discussing the proper time for education on giving up smoking, the teachers in school with indicated anti - smoking education the following, in order of frequency, the first year of middle school(30.5%), the second year(27.6%), the last year of middle school (18.4%), primary school (11.9%). For those who did not have anti - smoking education, the result were similar, the first year of middle school (31.9%), the second year (23.5%), primary school (17.6%), and the last year of middle school (15.5%). There results were not statisically significant but they supports the idea of eaely education directed at giving up smoking. 5. The actual frequency of students smoking was as follows. In schools with anti - smoking education 33.5% of the students smoked a lot and seriously 33.0% smoked a little and not seriously. In schools without anti - smoking education, the majority of the teachers (50.4%) indicated that the students were 'few and not serious' followed by 'I don't know well'(19.8%), 'many but not serious'(15.6%), 'many and serious'(15.2%). This implies that the teachers in schools which have anti - smoking education think smoking is more seriously than those who are in schools which do not have anti - smoking education though it is influenced by the members of the school. The opinions on penalities for smoking were as follows; in schools with anti - smoking education, 'punish'(52.8%) and 'give advice'(27.8%). In schools with no anti-smoking education 'punish'(41.9%), 'give advice'(24.5%) and 'I don't know what the rules for punishment should be'(18.5%), and 'do not punish'(16.4). 6. For knowledge about smoking by nurse - teachers, in schools having anti - smoking education the average score was 30.40. There was not statistically significant differences in these scores. But, there is an indication of a need for a deeper and a moer systematic knowledge of smoking as shown by the problem points; lack of reference data and educational data. The reason for this is that education to give up smoking is not considered a part of routine life. But the majority(95.2%) of the respondents indicated that a systematic program in the schools would meet that need.

An Attitudinal Study of Smoking Behavior among Korean Adolescents (청소년의 흡연행위에 관한 태도 연구)

  • This study. aimed to categorize attitude of smoking behaviors among adolescents and to provide useful data for development of anti-smoking programs for students was conducted from February 1 to August 15. 1999. using the Q method. After reviewing the literature and preceding studies. the researcher proceeded to interview 15 middle and high school students and thereupon. developed a statement form consisting of 34 items. Then. 33 students were sampled. The collected data were coded after having been scaled from '1' to '9'. and then. was analyzed using the PC QUANL program. After all. three types of adolescents were identified. The first type or 'negation' type students perceive smoking as bad habit. and therefore. do not admit of smoking. The second type or 'self-choice' type students conceive that anyone can smoke depending on their judgement. and therefore. admit of other students' smoking. The third type or 'self-perceived refusal' type admit of other students' smoking. but refuse smoking for themselves. Lastly. it is believed that the results of this study would be useful for designing anti-smoking health programs for our adolescents. It is also hoped that this study would be followed up by such studies suggested by the data for the development of anti-smoking programs for each category of students.

Status of Smoking Prevention Education in Elementary Schools (초등학교의 흡연교육 실태)

  • A survey was conducted from September 15 to December 15 2001. Structured questionnaires were mailed to school nurses in 607 elementary schools around the country in order to determine the current status of anti-smoking education in elementary schools. The result were as followers 1. Of the 607 schools, 535 $(88.1\%)$ offered smoking-prevention education. By region, Jeju-do $(100\%)$ topped the list, followed by Seoul $(90.3\%)$, Gyeongsang-do$(90.2\%)$, Jeolla-do$(88.9\%)$, Gangwon-do $(87.8\%)$, Chungcheong-do $(84.6\%)$, and Gyeonggi-do $(81.4\%)$. 'Recognition of the need for anti-smoking program $(86\%)$' was a major motivation for initiating the program, while 'too much workload $(46.4\%)$' was cited as a main reason for the failure to do so. 2. The classes were offered mostly for 6th-grade students $(87.8\%)$, while $9.0\%$ and $2.0\%$ were implemented at 5th- and 4th- grades, respectively. 3. $49.1\%$ of the classes offered lasted one hour, while $31.8\%$ involved a two-hour program. 4. Programs were mainly about smoking-related diseases, habitual nature of smoking, impediment to growth and development, etc. 5. Audio-visual lecture $(46.5\%)$ was most frequently used as a method of education, followed by lecture. 6. $72.7\%$ of the programs used classroom as a unit of education, while collective education by sex or by grade accounted for $22.6\%$. 7. Video $(51.0\%)$ was the most popular medium for education, while computer ranked second with $26.5\%$. 8. $92.5\%$ of the education was offered by school nurses. 9. $99.2\%$ of school nurses responded in favor of anti-smoking programs. with $60.1\%$ of them answering that such education is a must. 5th grade was the most commonly cited grade for the initiation of the programs, followed by 4th grade and 6th grade. $33.2\%$ picked two hours as the most appropriate length of the program at the elementary school level. while $25.1\%$ chose 3 hours out of the range of 1-11 hour(s). 10. With regard to the evaluation by school nurses on smoking-prevention program, more than $30\%$ felt that hours of education, education materials, medium of education, interests of other teachers, interests of school authorities, etc. were inadequate or insufficient.

A Study on the Actual Condition of the Adult-smoking in a Region (일 지역 성인의 흡연실태)

  • To investigate the actual condition of the adult-smoking in Chinan County. I used self-reporting questionnaires among 923 residents living in nine districts selected at random among the sites of eleven eup-myons from December 28. 1998 to January 6. 1999. collected data and analyized using SPSS. The smoking rate of adults in Chinan County was $36.9\%$. There was a significant difference in smoking rate according to the age. gender, education and job among general characteristics. The smoking rate in group of above 40s was about $40\%$ and that of the male took $55.5\%$ which was higher than that of female and the smoking rate of the people having high-school education was the highest$(46.2\%)$. With regard to the career. the smoking rate of the farmers ranked first$(46.1\%)$. Therefore anti-smoking business for smokers should be focused on above 40 years old, males. people having high school education and farmers. As the result of the survey of smoking habits of 340 people who smoke currently, the average smoking begining age was 21.8 and $81.5\%$ among them was between 16-25 years old. $4.4\%$ was under 15. during around the elementary school. Most motives to smoke were as followed; curiosity or taste$(33.6\%)$, relief of stress $(31.2\%)$, peer presure$(26.5\%)$ And considering the amount of the cigarette which is smoked per day, the case which is less than a pack of cigarettes was highest as $75.5\%$ and the people who smoke over one pack of cigarettes took $24.5\%$. As for the kind of tobacco which is smoked, how to smoke and the desire for the smoking, most case was toxin was moderate$(47.8\%)$ or mild$(46.9\%)$. shallowly$(49.0\%)$ or deeply$(46.3\%)$ and under stress$(33.4\%)$. after meals$(27.8\%)$, during drink$(15.7\%)$ and so on. The highest point marked among the factors of smoking motives was 'the reduce of negative emotion' $(3.27\pm1.00)$. followed by 'uncomfortable habits' $(2.87\pm1.02)$, 'addiction' $(2.84\pm1.06)$. 'habit' $(2.74\pm1.12)$. 'pleasure' $(2.70\pm1.04)$. 'stimulus' $(2.59\pm.90)$, 'sensation-exercise satisfaction' $(2.42\pm.97)$. Smokers smoke to reduce the negative emotions when angry in most common case. depressed. anxious. uncomfortable. lone. ashamed or embarrased. and intend to solve the certain problem. etc. Other motives are uncomfortable habit. addiction. habit. pleasure and the pursuit of stimulus. The level of nicotine dependence of adults m Chinan County was 10.57 which amount to 'high' wholly. As the resulf of the level of nicotine dependence score. the people who are low in the level of nicotine was $33.5\%$. the people who are high was $48.2\%$. very high was $18.4\%$. The approach for anti-smoking for smokers should be conducted differently according to the level of the nicotine. For the people who are in low level of nicotin dependence the prohibition of the smoking should be guided through the approach to foster strong will. for those who are in 'high' by acquiring proper method for the prohibition of smoking. and for those who are 'very high' the anti-smoking should be induced by providing proper program because of the possibility of the suffer from abstinence syndrome. The difference of the level of nicotine with the general characteristics of the objects had not statistically significant difference. The difference of the level of nicotine dependence accompanied by smoking habit had statistically significant difference according to the amount of smoke, the kind of tobacos. smoke inhale habit. In other words, the group of heavy smokers had higher level of the nicotine dependence than that of the light smokers relatively and the group which smoke strong taboaco has higher level of nicotine than that of which smoke mild or moderate. And the group of smokers who smoke deeply has higher level of nicotine than that who smoke shallowly or nonswallow. Aa a result of the analysis of the correlation between smoking motive factors and the level of nicotine, there was the indication that people who smoke for the decrease of the negative emotion. habit, pleasure. stimulus. sensation-exercise satisfaction had high level of the nicotine dependence. As the result of the anti-smoking will of smokers. $65.0\%$ of them had prohibition of smoking will. $29.3\%$ had no will to quit smoke. The most important reason for anti-smoking was health. $67.9\%$ had experience to try to quit smoke and the biggest reason to fail to quit smoking was the lack of the will power to keep anti-smoking. $52.8\%$ of them were advised to stop smoking from their spouses or children. only $2.8\%$ were by medical. The people who have the opinion to need anti-smoking education were $69.6\%$. Therefore when the business for the hygine of the mouth for adult is set. it should be centered on the people who have intention of prohibition of the smoking and help to quit smoking by way of other affirmative counter-program not smoking under stress.
