Comparisons of occlusal force according to occlusal relationship, skeletal pattern, age and gender in Koreans (한국인에서의 부정교합 여부와 골격형태, 연령, 성별에 따른 교합력의 비교)

  • Objective: The aim of this study was to evaluate the occlusal force and contact area and to find its associating factors in Koreans. Methods: Occlusal force and contact area in maximum intercuspation were measured using the Dental $Prescale^{(R)}$ system in 651 subjects (15 with normal occlusion, 636 with various malocclusions divided into subgroups according to the skeletal pattern, Angle's molar relationship, age and gender). Results: Occlusal force of the normal occlusion group ($744.5{\pm}262.6N$) was significantly higher than those of the malocclusion group ($439.0{\pm}229.9N$, $p$ < 0.05). Occlusal force was similar regardless of differences in ANB angle or Angle's molar classification, however the increase in vertical dimension significantly reduced occlusal force ($p$ < 0.05). Conclusions: Occlusal force was significantly lower in the malocclusion group compared to the normal occlusion group, and in females compared to males, but it was not affected by age, antero-posterior skeletal pattern or molar classification. Although a hyperdivergent facial pattern indicated lower occlusal force compared to a hypodivergent facial pattern, the differences in skeletal pattern were not the primary cause of its decrease, but a secondary result induced by the differences in occlusal contact area according to the facial pattern.

Relationship between the Malocclusion and Caries Prevalence of Adolescents in Iksan (익산시 청소년의 부정교합과 치아우식 유병률 사이의 연관성)

  • The purpose of the present study was to evaluate the relationship between the malocclusion and dental caries in adolescents with permanent dentition. The subjects of the study were 385 adolescents aged 12 to 15 years. Dental Aesthetic Index (DAI) and DMFT index were recorded as clinical indicators of malocclusion and dental caries. The mean Dental Aesthetic Index (DAI) score of the subjects was 12.42 and the mean DMFT index of the subjects was 2.89. Four students (1.04%) were included in the group of orthodontic treatment mandatory, which signifies the handicapping malocclusion. Moreover, the result of gender-specific analysis of the DAI components observed that the prevalence of midline diastema and mandibular overjet were significantly higher among boys (p < 0.05). In 7 components (missing teeth, incisal segment crowding, maxillary anterior irregularity, mandibular anterior irregularity, mandibular overjet, anterior openbite, antero-posterior molar relationship) among the 10 DAI components, abnormal groups showed significantly higher DMFT index than normal groups (p < 0.05). The subjects in the group of definite to handicapping malocclusion (DAI ${\geq}$ 26) showed significantly higher DMFT index than the subjects in the group of minor or no malocclusion (p < 0.01). In addition, the DAI score had significant positive linear correlation with the DMFT index (r = 0.584, p < 0.01). Consequently, the current findings suggested a positive relationship between the malocclusion and caries prevalence. And several specific types of malocclusion were supposed to be significantly correlated with dental caries.

The variability of 6-D Skull Tracking(6DST) in Cyberknife for Bone metastasis patients (사이버나이프 6-D Skull Tracking의 유용성 평가)

  • Purpose : The purpose of this study is to evaluate the usefulness of 6 Dimensional Skull Tracking(6DST) in Cyberknife Stereotactic Body Radiation Therapy(SBRT) for the first and second cervical vertebrae(C1 and C2) metastasis. Methode and material : The Computed Tomography (Lightspeed VCT 64, General Electric Co. Waukesha, WI, USA) was used to acquire the CT images of the 9 patients with cervical vertebrae(C1 and C2) metastasis. Treatment plans for Xsight spine tracking and 6 Dimensional skull tracking were established with planning system (Multiplan system Version 4.6, Accuray, US). The results of XST and 6DST for each patient were analyzed with Microsoft Excel 2010. Result : The Maximum offsets of XST for C1 were 0.9 mm in Y(supero-inferior), 0.9 mm in Z(antero-posterior), 0.7 mm in X(left-right) direction, and rotations were and 1.0 degrees roll, 1.0 degrees pitch and 1.2 degrees yaw. The Maximum offsets of 6DST for C1 were 0.7 mm, 0.7 mm, 0.9 mm and $1.0^{\circ}$, $1.0^{\circ}$, $1.2^{\circ}$ for Y, Z, X and Roll, Pitch, Yaw. The Maximum offsets of XST and 6DST for C2 were 0.7 mm, 0.7 mm, 0.8 mm and $0.9^{\circ}$, $1.0^{\circ}$, $1.8^{\circ}$, and 0.9 mm, 0.7 mm, 0.9 mm and $0.9^{\circ}$, $0.9^{\circ}$, $1.0^{\circ}$ for Y, Z, X and Roll, Pitch, Yaw, respectively. Conclusion : XST and 6DST showed identical results for translations and rotations within the tolerance. It is possible to simplify the treatment time and procedure by using the 6DST. Therefore, 6DST is very useful methode with XST among the various tracking methods in Cyberknife for the patients with C1, C2 vertebral metastasis.

Mid-Term Results of Fixed Bearing Unicompartmental Knee Arthroplasty: Minimum 5-Year Follow-Up (고정형 슬관절 단일 구획 치환술의 중기 추시 결과: 최소 5년 추시)

  • Purpose: To evaluate the clinical and radiological outcomes, and the complications of unicompartmental knee arthroplasty (UKA) using a fixed bearing prosthesis after 5-year follow-up. Materials and Methods: Twenty-six knees (25 patients) that underwent fixed bearing UKA between May 2003 and August 2011 were included. The subjects were 3 males (3 knees) and 22 females (23 knees), and the average age was 63.5 years. The preoperative diagnosis was osteoarthritis (23 knees) and osteonecrosis (3 knees). The mean follow-up duration was 67 months (from 60 to 149 months). The clinical evaluation included pre- and postoperative American knee society knee and function score, and range of motion. The radiology evaluation included standing antero-posterior, lateral view, and fluoroscopic film to analyze the postoperative alignment and osteolysis. Results: The mean American Knee Society knee score and function score were improved from 42.0 and 57.5 to 87.9 and 85.0, respectively (p<0.001). The mean preoperative and postoperative range of motion was $132.9^{\circ}$ and $132.5^{\circ}$, respectively. The mean femorotibial angle were varus $0.5^{\circ}$ preoperatively and valgus $2.2^{\circ}$ postoperatively. A radiolucent line was observed in 2 knees; one knee had a stable implant, while in the other knee, patellofemoral arthritis was identified during UKA. Diffuse pain of the knee joint with tenderness of the medial joint line was identified at the follow-up, so conversion to total knee arthroplasty was recommended. No other complications, such as osteolysis, infections, postoperative stiffness, and dislocation, were encountered. Conclusion: The midterm results of fixed bearing UKA were clinically and radiologically satisfactory.

Effect of Nurida-Ball exercise on muscle function, spinal alignment, and dynamic balance capacity in Middle-Aged Men (누리다 볼 운동이 중년 남성의 근기능, 척추정렬 및 동적 균형능력에 미치는 영향)

  • The purpose of this study was to investigate the effect of Nurida-Ball exercise on isokinetic muscle function, spinal alignment, and dynamic balance capacity in middle-aged men. All middle-aged men(n=16) were divided into 2 groups: Ball exercise(BE, n=8) and control(CON, n=8) group. BE group performed the Nurida-Ball exercise(30 min/day, 3 days/week, 8 weeks) and isokinetic knee and trunk muscle function, spinal alignment, and dynamic balance capacity were measured. All of the measured variables calculated the mean and standard deviation and verified normality using the Shapiro-Wilk test. The independent t-test method and the Paired t-test method were then analyzed to identify differences between groups. This study found that isokinetic knee and trunk muscle function was significantly strengthened in the BE compared with CON group by increasing peak torque(PT) of right and left knee extension(60°/sec, p<0.01, respectively), average power(AP) of right and left knee extension(60°/sec, p<0.05, p<0.01, respectively), and PT of right knee flexion(180°/sec, p<0.05) and AP of right knee extension(180°/sec, p<0.05). In the change of isokinetic trunk muscle function, only PT of trunk extension(180°/sec) was increased in the BE compared with the CON group(p<0.05). In addition, Nurida-ball exercise can improve the spinal alignment by reducing the trunk inclination(p<0.05) in the BE compared with the CON group. Finally, dynamic balance capacity was also enhanced in the BE compared with the CON group by decreasing the score of overall balance index(OBI, p<0.01) and Antero-posterior balance index(p<0.05) in the Stage-6, and OBI(p<0.05) in the Stage-1. This result demonstrated that Nurida-ball exercise may improve spinal alignment, dynamic balance capacity, and isokinetic muscle function, which might be an effective way for the improvement of health-related fitness in middle-aged men.

Study on the Differences in the Results of Body Shape Test According to the Position of the Two Feet and the Usefulness of the Neck and Body Motion Image Test (두 발의 위치에 따른 체형검사 결과 차이와 체간신전 동작 이미지 검사의 유용성 연구)

  • Purposes: The purposes of this study were to investigate the relationship between the standing position of the subject and the normal standing position(NSP) and the straight standing position(SSP) and to investigate the possibility of different body shape test results depending on the status of the image inspection apparatus. Methods: The images of the NSP and SSP were compared with each other by body line BLS system. Results: At the time of examination, the position of the camera was captured at a position 2.3 m vertically from the posterior position 45 cm behind the subject. This is a privacy protection method for covering the breast of the subject. Results: The physiological characteristics of the anatomical position of the body align image test are the living body. NSP and SSP tests showed different shapes of the pelvis AS(antero-supero) and pelvis rotation in the transverse plane. Shoulder and arm displacement was observed in the trunk extension image capture. Conclusions: In the body alignment test, the pelvis position test images of NSP and SSP are evaluated differently for pelvis rotation, AS, and PS. At the extension position of the trunk, a test of the maximal extension range showed that the left and right shortening of the shoulder anterior muscles could be observed. Inducing and testing the trunk extension is also useful.
