OCCLUSAL REPOSITIONING SPLINT에 의한 악관절음(顎關節音)의 치료(治療)에 관한 비교연구(比較硏究) (COMPARATIVE STUDY OF EFFECT OF STABILIZATION SPLINT AND ANTERIOR REPOSITIONING SPLINTS FOR CLICKING SOUND)

  • This study conducted 2 types of occlusal splint therapy to eliminate clicking sound. 15 patients who had clicking on their joints were selected, and divided, at random, into 3 groups. In the first group, 4 persons put on stabilization splint and in the second group, 6 persons put on anterior repositioning splint which had made condyle to protrude 3mm, and in the last group, 5 persons put on anterior repositioning splint which had made condyle to protrude 6mm. Patients who wore anterior repositioning splint were instructed to use the splint for all days. The evaluation of clicking was measured by occlusal soundscope. The clicking was converted to aucostic signal by the attached microphone, instead of vibrating sensor The in-put aucostic signal on the occlusal soundscope made it possible to observe the experiment's result. Anterior repositioning splint was produced in the centric occlusion state, when the model was mounted to articulator and inserted two pieces of 3mm and 6mm resin blocks each into the rear wall of articulator fossa. The observation of the patients who wore three different splints for 3 weeks has reached the following conclusions: 1. Stabilization splint produced no effect in eliminating the clicking sound. 2. Anterior repositioning splint therapy with 3mm condylar protrusion produced significant improvement in eliminating the clicking sound. 3. The 6mm protruded anterior repositioning splint caused pain on affected TMJ area as well as the clicking on unaffected joint.

  • In recent years the relationship between occlusal stability, mandibular position and temporomandibular joint function has been greatly emphasized. Anterior repositiong splint has been used for the purpose of correcting a disk-condyle in coordination, so we have usually wed it in ease of the click of the temporpmandibular joint. We have used anterior repositioning splint in 28 patients who have chief-complain of click in symptoms of the temporomandibular joint arthrosis. At the patients who had long-lasting symptom and sign, late click or degenerative change of the temporomandibular joint, the anterior repositioning splint had less effect on than we had expected. So we are now to report that we must pay attention to use of anterior repositioning spint.

  • Temporomandibular joint(TMJ) disorders have characteristic clinical findings such as pain, joint sound, and abnormal jaw function. With the rapid progress in TMJ imaging techniques, magnetic resonance imaging(MRI) especially provides the clinician and researcher with the ability to evaluate TMJ disorders(effusion, anterior disk displacement) and the hard and soft tissue of the TMJ. In order to manage the internal derangement of TMJ such as anterior disk displacement, the so-called appliance therapy is commonly used. This technique became the treatment of choice before surgical intervention by many practitioners. The purpose of the present study was to evaluate and determine the efficacy of anterior repositioning splint by means of the change of the position of the disk through pre- and post-treatment MRI. MRI and clinical symptom were carefully recorded in TMJ disorder patients. The relationship between TMJ symptoms such as pain, clicking sound, and disk displacement and effusion in MRI were analyzed. Then, splint therapy was applied for 3 months according to our protocol; at day 1, patients should wear 24 hours, in order to accomodate the splint and then, wearing time is reduced to 12 hours per day for one month. During next 2 months, it is tapered to 8 hours per day according to the evaluation of the clinical symptoms. Post-treatment MRI was obtained after 3 month protocol. This treatment strategy improved the clinical symptoms of TMJ disorder, and effusion and articular disk position showed a significant change. In conclusion, a treatment modality using an anterior repositioning splint therapy is an appropriate method for temporomandibular disorder patients.

전방재위치 교합장치를 이용한 비정복성 관절원판변위 환자의 치험례 (Case Report : Treatment of Disc Displacement without Reduction Patient Using ARS)

  • The treatment of anterior disc displacement without reduction patients needs speicific treatment methods different from other Craniomandibular Disorders. Those are manual manipulation, anterior repositioning splint; ARS and step-back. It is well known that the use of Anterior Repositioning Splint is effective on relief of TMJ symptom. But the side effect of long- term ARS wearing, which is irreversible posterior open bite induction, has made many clinicians avoid ARS treatment. This report introduces a clinical case recently proven part- time wearing ARS method for treatment of Disc Displacement without reduction patient' that can reduce side effects with good efficacy. But now clinical statistical studies and basical histophysiological studies are more needed.

교합장치요법으로 치료한 측두하악장애 환자의 임상적 연구 (Clinical Features of the Temporomandibular Disorder Patients with Occlusal Splint Therapy)

  • This study was performed to evaluate the clinical features and treatment results on 55 temporomandibular disorder patients who had treated by conservative treatment using occlusal splint. The results were as follows; 1. The ratio of men to women was about 1:2.2 and most of the patients were second and third decades. 2. Pain was the most frequent symptom, followed by clicking and mouth opening limitation. 3. The number of acute and chronic groups on the basis of 6 months duration of symptoms were similar. 4. Most of patients had Angle's Class 1 molar relationships(78.2%), followed by Class 3 and Class 2. 5. Centric Relation splints were used alone for treatment of 34 patients and 21 patients were treated with Centric Relation splints and Anterior Repositioning splints. The treatment duration of the patients who had complained pain was average 9.8 weeks and 6 patients of them had slight pain continuously during follow-up. 6. Eleven patient's maximum mouth opening who had mouth opening limitation was improved from 30. 7mm to 43.0mm during procedures.

  • Patient with skeletal class II relationship was treated with L-ARS. L-ARS is fixed functional appliance that could be effective in children and adolescent patient who don't wear activator. The following results were obtained ; 1) Growth of Mandible was stimulated and overjet was decreased, therefore coupling of anterior teeth was established with L-ARS which is fixed functional appliance. 2) These changes were accomplished with Mandibular skeletal growth with no effect on the Maxilla. 3) L-ARS was especially effective on patient who deny to wear the removable functional appliance.

하악위 변화에 따른 저작근의 근전도학적 연구 (An Electromyographic Study on Changes of Mandibular Position)

  • 저자는 하악의 위치변화가 저작근에 미치는 영향을 근전도학적으로 평가하고자, 측두하악장애의 징후와 증상이 없는 정상성인 28명을 대상으로 중심위 교합장치(centric relation splint), 중심교합위 교합장치(centric occlusion splint), 전방 재위치 교합장치(anterior repositioning splint)를 장착시킨 상태에서 하악의 중심의, 중심교합위, 전방위률 유도한 후 최대 이악물기 상태의 좌우측 교근 및 전측두근의 근전도를 채득, 분석, 평가한 결과 다음과 같은 결론을 얻었다. 1. 전측두근에서 하악의 위치변화에 따른 최대 근활성도의 차이가 나타났으나(p<0.05), 교근에서는 나타나지 않았다. 2. 전측두근의 경우 중심교합위에서 최대 이악물기 상태의 최대 근활성도는 교합 장치물을 장착하지 않은 경우 및 중심위에서와 비교하여 감소하였다(p<0.05). 3. 전측두근의 경우 전방위에서 최대 이악물기 상태의 최대 근활성도는 교합 장치물을 장착하지 않은 경우 및 중심위에서와 비교하여 감소하였다(p<0.001). 4. 교근과 전측두근의 좌우 비대칭지수(asymmetry index)는 하악의 위치변화에 따른 차이가 나타나지 않았다.

CLOSED LOCK증례에 대한 악관절 세정술의 임상적 연구 (CLINICAL STUDY OF ARTHROCENTESIS-CASES OF CLOSED LOCK)

  • Purpose : To evaluate the efficacy of arthrocentesis for treatment of closed lock. Material : 42 patients were diagnosed to closed lock by physical, radiographic examriation and undergone arthrocentesis. All patients have a pain and mouth opening limitation on affected site. Method : Arthrocentesis was done under conscious sedation and local infiltration anesthesia, normal saline and some steroid was injected on upper compartment of tempormandibular joint. After pumping and lavage, manual reduction procedure of anterially displaced disc was done. All the patients wear an anterior repositioning splint just after arthrocentesis. The result of arthrocentesis was assessed by pain and difference of mouth opening. Result : Difference of mouth opening after arthrocentesis was improved to 18.85mm and pain was gradually decreased. All patients were worn stabilization type splint after mean 1.84 months. 6 patients had relapse of mouth opening limitation so done arthrocentesis again and delivered good results.

측두하악장애환자의 임상양태와 치료결과와의 관계 (Relations between Clinical Findings and Treatment Results in Patients with Temporomandibular Disorders)

  • This study was performed to evaluate and compare conservative treatment results by several parameters such as age, sex, symptom duration, type and timing of joint sound, parafunctional habits, splint type, and diagnostic classification. There have been too many articles reporting long term results of conservative treatment but articles related to comparison of treatment results by patients' self-evaluation have been rarely reported. For this study 258 patients with temporomandibular disorders(TMDs) were selected and examined by routine diagnostic procedure for TMDs. The subjects were classified into 5 TMDs subgroups ad treated with conservative treatments involving splints, physical modalities, jaw exercises, and counseling. Visual analogue scale(VAS) about pain, joint sound, and mouth opening limitation was recorded respectively during treatment period. From the VAS data and treatment duration, VAS treatment index(VAS Ti) was calculated. The obtained results were as follows : 1. Pain was the most frequent main symptom in subjects with temporomandibular disorders, and main symptom for mouth opening limitation was comparatively less than for pain or sound in disk displacement with reduction group or in degenerative joint disease group. 2. Degenerative joint disease group had the most poor treatment results and highest occlusal index of Helkim's index. 3. Good prognosis for conservative treatment was observed in acute group, under 6 months than chronic group, 6months over in symptom duration, and subjects with 40 years over in age showed the most poor prognosis. 4. Subjects treated with anterior repositioning splint had better treatment results than subjects treated with centric relation splint, but statistical significance in VAS Ti and treatment duration was not observed. 5. Treatment results according to affected side, types and point of joint sound did not show consistent statistical results. 6. The result for conservative treatment was observed poor in subjects with bruxism and clenching. 7. In studying coincidence between preferred chewing and affected side, frequency of preferred chewing side, in unilateral affection, was higher in ipsilateral than in contralateral side.

Effect of arthrocentesis on the clinical outcome of various treatment methods for temporomandibular joint disorders

  • Background: We evaluated the improvement of pain and the increase in mouth opening after temporomandibular joint arthrocentesis and the possible association with various factors such as previous splint treatment, medication, and diagnosis. Results: We studied 57 temporomandibular joint disorder patients who underwent arthrocentesis at Korea University Anam Hospital. These patients (24 males and 33 females, aged between 15 and 76 years) underwent arthrocentesis that was performed by one surgeon. The degree of mouth opening (assessed using the maximum mouth opening: MMO) and pain (assessed using the visual analog scale: VAS) were assessed pre- and post-arthrocentesis. The study also investigated whether treatment modalities other than arthrocentesis (medication and appliance therapy) were performed. Statistical analysis revealed that there was a significant difference in mouth opening and pain after temporomandibular joint arthrocentesis. Preoperative appliance therapy affected the results of arthrocentesis, but it was not statistically significant. With regard to pain relief, preoperative diagnosis did not show a significant difference. However, with regard to maximum mouth opening, patients with disc displacement without reduction with limited mouth opening (closed lock) showed the highest recovery (11.13 mm). Conclusion: The average of MMO increase after arthrocentesis was 9.10 mm, and patients with disc displacement without reduction with locking (closed lock) showed most recovery in maximum mouth opening and it was statistically significant. The average pain relief of patients after arthrocentesis was 3.03 in the VAS scale, and patients using anterior repositioning splint (ARS) preoperatively showed the most pain relief.
