중하부 경추손상에서 후방 경추 내고정술 및 골유합술의 분석결과 (Analysis of Posterior Cervical Fixation and Fusion in Subaxial Cervical Spine Injury)

  • Objective : In the retrospective analyzing 19 consecutive patients with subaxial cervical spine(C3~T1) injury treated by posterior cervical fixation and fusion, clinical manifestation, radiologic finding, operative technique, and postoperative results following 6 months were analyzed. Materials and Methods : Most common fracture level was C4-5, mean age 41, and male to female ratio 13 : 6. The most common cause of injury was motor vehicle accident(17 cases). In 19 cervical procedures, interspinous triple wiring was done in 14 cases, lateral mass plating in 5 cases, and additional anterior fusion in 2 cases. Results : Twelve weeks after operation, all cases were reviewed by plain cervical radiogram. In 17 cases that treated by posterior fusion only, 14 cases(81%) had kyphotic angle change less than $5^{\circ}$, 2 cases(12%) $5-20^{\circ}$, and 1 case(6%) more than $20^{\circ}$. Overall fusion rate was 88%, and there was no significant difference of bone fusion rate between autogenous bone graft and allogenous bone graft. Conclusion : In the case of severe posterior column injury or displacement, posterior approach seems superior to anterior approach, but in the case of combined anterior column injury, anterior approach is considered necessary. In this study, posterior fixation and fusion might be acceptable procedure for subaxial cervical fracture and dislocation, owing to its high fusion rate, low kyphotic angulation and low operation related complication rate.

상악 분절골 후퇴술 후의 상순위치 변화 연구 (A STUDY OF UPPER LIP PROFILE CHANGE AFTER ANTERIOR SEGMENTAL SETBACK OSTEOTOMY)

  • Purpose : Prediction for soft tissue change after orthognathic surgery is very important for the final esthetics. In this study, we have tried to get the amount of upper lip movement relative to bony segment movement after anterior segmental osteotomy by cephalmetric analysis to predict final upper lip position after surgery. Material and Methods : 20 patients was studied on whom anterior segmental osteotmy as performed by Cupar method during the years 2002 to 2003. Cephalometric radiograph were taken at 1month before surgery and 6 month after surgery. Change of upper lip was measured on landmark Ls and Sto relative to hard tissue (landmark Ia) setback on these X-rays and analyzed. Results : 1. Upper lip setback movement. Setback of upper lip showed proportional relation to the hard tissue setback and the ratio was about 84%(p=0.001). 2. Upper lip downward movement. Downward movement of upper lip showed no proportional relation to hard tissue setback And the amount was mean 1.38 mm and SD 1.21mm (p=0.922). Conclusion : The posterior movement of upper lip is affected by hard tissue movement and shows good proportional change whereas downward movement is not so much influenced by hard tissue movement. And we think slight downward movement shown in this study could be explained by the V-Y closure performed during surgery.

어깨뼈 내밈 운동 시 어깨관절 수평 벌림과 수평모음 저항과 외부 무게에 따른 앞톱니근, 위등세모근, 그리고 큰가슴근 근활성도 비교 (Comparison of the Serratus Anterior, Upper Trapezius, and Pectoralis Major Muscle Activity According to Horizontal Abduction and Horizontal Adduction Resistance and External Weight During Protraction Exercise)

  • Background: The increase in the activity of the upper trapezius (UT) muscle and decrease in the activity of the serratus anterior (SA) and lower trapezius (LT) muscle produce an abnormal scapulohumeral rhythem. Therefore, the SA is the focus of therapeutic exercise protocols for the rehabilitation. This study aimed to compare the activity of the muscles according to resistance levels during protraction exercise. Methods: Thirteen healthy 20 to 30 years old male and female subjected participated in the study. All subjects performed the scapular protraction exercise with the weights of 0, 5, and 10kg, combined with horizontal shoulder abduction and adduction. The electromyography data of the SA, UT, and pectoralis major (PM) during the exercise were recorded using surface electrodes. The ratios of PM/SA and UT/SA were calculated. Repeated one-way ANOVA was used to determine the statistical significance. Results: There was a significant difference in the muscle activity of the SA and PM (p<.05). Specifically, SA had the highest activity and PM had the lowest activity at an abduction resistance 10kg (p<.01). There was a significant difference in the ratios of UT/SA and PM/SA (p<.05). The ratio of PM/SA was the lowest at the abduction resistance of 10kg (p<.01). Conclusion: According to this study, for the selective activation of the serratus anterior, protraction exercises should be applied as a method of adding resistance to protraction and horizontal abduction.

Predictors of Catastrophic Outcome after Endovascular Thrombectomy in Elderly Patients with Acute Anterior Circulation Stroke

  • Objective: Avoiding a catastrophic outcome may be a more realistic goal than achieving functional independence in the treatment of acute stroke in octogenarians. This study aimed to investigate predictors of catastrophic outcome in elderly patients after an endovascular thrombectomy with an acute anterior circulation large vessel occlusion (LVO). Materials and Methods: Data from 82 patients aged ≥ 80 years, who were treated with thrombectomy for acute anterior circulation LVO, were analyzed. The association between clinical/imaging variables and catastrophic outcomes was assessed. A catastrophic outcome was defined as a modified Rankin Scale score of 4-6 at 90 days. Results: Successful reperfusion was achieved in 61 patients (74.4%), while 47 patients (57.3%) had a catastrophic outcome. The 90-day mortality rate of the treated patients was 15.9% (13/82). The catastrophic outcome group had a significantly lower baseline diffusion-weighted imaging-Alberta stroke program early CT score (DWI-ASPECTS) (7 vs. 8, p = 0.014) and a longer procedure time (42 minutes vs. 29 minutes, p = 0.031) compared to the non-catastrophic outcome group. Successful reperfusion was significantly less frequent in the catastrophic outcome group (63.8% vs. 88.6%, p = 0.011) compared to the non-catastrophic outcome group. In a binary logistic regression analysis, DWI-ASPECTS (odds ratio [OR], 0.709; 95% confidence interval [CI], 0.524-0.960; p = 0.026) and successful reperfusion (OR, 0.242; 95% CI, 0.071-0.822; p = 0.023) were independent predictors of a catastrophic outcome. Conclusion: Baseline infarct size and reperfusion status were independently associated with a catastrophic outcome after endovascular thrombectomy in elderly patients aged ≥ 80 years with acute anterior circulation LVO.

Monitoring Posterior Cerebral Perfusion Changes With Dynamic Susceptibility Contrast-Enhanced Perfusion MRI After Anterior Revascularization Surgery in Pediatric Moyamoya Disease

  • Objective: To determine whether dynamic susceptibility contrast-enhanced (DSC) perfusion magnetic resonance imaging (MRI) can be used to evaluate posterior cerebral circulation in pediatric patients with moyamoya disease (MMD) who underwent anterior revascularization. Materials and Methods: This study retrospectively included 73 patients with MMD who underwent DSC perfusion MRI (age, 12.2 ± 6.1 years) between January 2016 and December 2020, owing to recent-onset clinical symptoms during the follow-up period after completion of anterior revascularization. DSC perfusion images were analyzed using a dedicated software package (NordicICE; Nordic NeuroLab) for the middle cerebral artery (MCA), posterior cerebral artery (PCA), and posterior border zone between the two regions (PCA-MCA). Patients were divided into two groups; the PCA stenosis group included 30 patients with newly confirmed PCA involvement, while the no PCA stenosis group included 43 patients without PCA involvement. The relationship between DSC perfusion parameters and PCA stenosis, as well as the performance of the parameters in discriminating between groups, were analyzed. Results: In the PCA stenosis group, the mean follow-up duration was 5.3 years after anterior revascularization, and visual disturbances were a common symptom. Normalized cerebral blood volume was increased, and both the normalized time-topeak (nTTP) and mean transit time values were significantly delayed in the PCA stenosis group compared with those in the no PCA stenosis group in the PCA and PCA-MCA border zones. TTPPCA (odds ratio [OR] = 6.745; 95% confidence interval [CI] = 2.665-17.074; P < 0.001) and CBVPCA-MCA (OR = 1.567; 95% CI = 1.021-2.406; P = 0.040) were independently associated with PCA stenosis. TTPPCA showed the highest receiver operating characteristic curve area in discriminating for PCA stenosis (0.895; 95% CI = 0.803-0.986). Conclusion: nTTP can be used to effectively diagnose PCA stenosis. Therefore, DSC perfusion MRI may be a valuable tool for monitoring PCA stenosis in patients with MMD.

안모의 성장유형에 따른 경부견인 헤드기어의 효과에 대한 두부방사선계측학적 연구 (Cephalometric study of the effect of cervical pull headgear based on facial growth patterns)

  • 이 연구의 목적은 II급 부정교합의 치료를 위해 경부견인 헤드기어(cervical pull headgear)를 사용할 때, 치료에 불리한 요인으로 작용한다고 언급되는 효과들이 실제로 발현되는지의 여부와 안모의 성장유형에 따라 그 차이가 존재하는 지를 알아 보고자 함에 있다. 이를 위해서 삼성서울병원 치과교정과에서 II급 부정교합으로 진단되어 경부견인 헤드기어로 치료받은 환자중, 성장기에 있으면서 비발치 치료를 받은 환자를 연구대상으로 선별하였다. 그리고 연구대상을 "후안면/전안면 고경비" 에 따라 수직성장군과 수평성장군으로 분류하였다. 수직성장군 15명(남자 5명, 여자 10명)과 수평성장군 11명(남자 4명, 여자 7명)의 치료전, 후의 두부방사선규격사진상에서 SN-PP angle, SN-GoGn angle, SN-MP angle, PFH/AFH ratio, SN to 6, PP to 6 등을 계측하고, 비교분석하여 다음의 결과를 얻었다. 1)수직성장군에서 구개평면이 전하방으로 경사되었다. 2)수평성장군에서 후안면/전안면 고경비가 증가하였다. 3)수직성장군과 수평성장군 모두에서 하악평면각은 변화하지 않았다. 4)상악대구치의 정출양에 있어서 수직성장군과 수평성장군의 차이는 없었다.

Compensatory change of opposite hippocampus after temporal lobe surgery in patients with temporal lobe epilepsy Evidence from single-voxel proton MR spectroscopy

  • Purpose: To evaluate compensatory change of opposite hippocampus after temporal lobe surgery in th patient with temporal lobe epilepsy by using single-voxel proton MR spectroscopy. Method: Eighteen patients with intractable temporal lobe epilepsy (TLE) whose MR diagnos was unilateral hippocampal sclerosis (n=11) or localized unilateral anterior temporal lobe lesio (n=7) and who underwent anterior temporal lobectomy were included in the study. Singl proton MRS of opposite hippocampus was carried out on the same day or within 1 week af MR imaging before temporal lobe surgery and after over 1-year post-surgical follow-u Single voxel proton MRS were acquired using GE signa 1.5T scanner and spectrosco system (TR, 1500-2, 000: TE, 136-144). Region of interest (ROI) was placed in a simitar position for all examination to cover the medial temporal lobes including most of the head an body of hippocampus and a part of amygdala, the parahippocampal gyrus. The MR spectr were evaluated with a focus on the metabolite ratio of N-acetylaspartate (NAA choline-containing phospholipids (Cho), creatine (Cr). The metabolite ratios of NAA/ Cr were calculated from the relative peak height measurement. We evaluated change of th intensity ratio NAA/Cr between before and after surgery, to simplify quantification acro patients, because observed decreases in the ratio of NAA/Cr can be interpreted in terms o neuronal or axonal damage.

초등학생의 근시성 굴절이상과 안광학 성분들의 관련성 (The Correlation of Myopic Refractive Error and Ocular Components in Primary School Students)

  • 목적: 초등학생을 대상으로 근시성 굴절이상과 안광학 성분들 사이의 관련성을 알아보고자 하였다. 방법: 안질환이 없는 근시성 굴절이상 62명을 대상으로 굴절검사, 각막곡률반경, 전방깊이, 수정체두께, 안축장을 측정하고 분석하였다. 결과: 초등학생의 근시성 굴절이상은 안축장(저학년 r=-0.653, p=0.000/고학년 r =-0.742, p=0.000), AL/CR비(저학년 r=-0.571, p=0.000/고학년 r=-0.852, p=0.000)와 음의 상관성을 보였다. 결론: 초등학생의 근시성 굴절이상은 안축장과 및 AL/CR비와 상관성이 매우 높음을 알 수 있었다.

Effects of Reciprocal Inhibition Using Thera-band on Scapular Muscle Activities During Arm-lifting Exercises in Subjects with Rounded Shoulder Posture

  • PURPOSE: Excessive activity of the upper trapezius (UT) muscle contributes to a rounded shoulder posture (RSP) through abnormal rotation of the scapula that weakens the lower trapezius (LT) and serratus anterior (SA) muscles. This study compared the effects of two arm-lifting exercises with and without the use of a Thera-band on the activities of LT, SA, and UT muscles, and the LT/UT and SA/UT activity ratio in subjects with a rounded shoulder posture. METHODS: Sixteen subjects with RSP participated in this study. All subjects performed arm-lifting (AL), diagonal arm-lifting (DAL), arm lifting with isometric adduction (ALIA), and diagonal arm-lifting with isometric adduction (DALIA) exercises. The surface electromyography data, LT, SA, UT, and the LT/UT and SA/UT activity ratios were measured. A paired t-test was used to compare the differences between two arm-lifting exercises and two arm lifting with isometric adduction exercises. RESULTS: In ALIA and DALIA exercises, the UT muscle activity decreased significantly, whereas the LT/UT, SA/UT activity ratio increased significantly. The activity of SA muscle increased significantly more with DALIA than that with DAL. CONCLUSION: Arm-lifting exercises using a Thera-band can be implemented as an effective way to reduce the UT overactivity and increase the SA activity and LT/UT, SA/UT activity ratio in subjects with RSP.

한국산 및 일본산 진주담치의 패각형태에서 본 분류학적 고찰 (A Taxonomical Study on the Shell Morphology of Blue Mussel, Mytilus edulis galloprovincialis Lamarck in Korea and Japan)

  • 1. 우리나라의 강원도 죽변, 경북 영덕, 경남 일광면, 의창군, 충무, 삼천포와 일본의 암수현 대퇴만, 복정현 포저만, 신내천현 횡수하만의 호하대에 서식하는 진주담치를 채집하고, 패각의 내부형태에 관해 조사하였다. 2. 각고에 대한 전폐각근흔 길이의 비율을 조사한 결과 평균치가 가장 큰 것은 경북 영덕산으로 62.47이었으며, 가장 작은 것은 의창산으로 54.17이었다. 이 값은 유럽의 남방종(Mytilus galloprovincialis)의 값 66.06과 유사하며, 유럽 북방종(Mytilus edulis)의 값인 94.79와는 큰 차이가 있음을 알 수 있었다. 3. 각고에 대한 교판길이(hinge plate length)의 비율을 조사한 결과 평균치가 가장 큰 것은 죽변 산으로 61.31이었으며, 가장 작은 것은 일본 대퇴산으로 56.15였다. 이 값은 유럽 남방종(M. galloprovincialis)의 값 59.39와 유사하며, 유럽 북방종(M. edulis)의 값 84.80과는 큰 차이가 있음을 알 수 있었다. 4. 이상의 연구에서 우리나라와 일본 난류역연안에 서식하는 진주담치는 그 패각의 내부형태적 특징에서 지중해 연안, 즉 유럽 남방종인 Mytilus galloprovincialis와 동일한 사실이 판명되었다.
